Inmaten stated there were multiple ansactions ranging from $40.00 to $100.00. Inmaten sted al l of the money·ansacon numrs were provided to him by his mother. via the,rerded inmate llephone. Beginning June 14 2017 the above named indivldl(al a Corrections Officer with the Florida Department of Corrections became the subject of an Investigation concerning the unlawful Introduction of contraband and unlawful compensation Such information was derived from inmate and civilian sources

IOn June 14 2017 Inmatn provtded information to security s1aff at Suwannee Correctional Institution Annex (SWCI) indicati g Officer Mattie Hooker was introducingcigarettes cellular telephones and bullets- to an inmate t side the secure facility further provided what area of a dormitory could be searched w ere the contraband was being hidden Security staff conducted a search of thal location and discovered a posh style cellular phone the inmate previously described and stated Officer Hooker ttooght into the facility Several inmates who wee suspected middotOf being involved with the conn band were placed In administrative confinement rnOn June 14 2017 Inmate ovlded a sworn statementnto your afftant Indicating Inmate had been working with Inmate who was Involved in a sexual relationship with Officer Hooker Tf two Inmates were working together to

get a cell phone into the facility Inmaten stated via Money Gram for the introduction of cigarettes and

cellular phone was deflvered Inmate began makiqg threats ton and extorting Inmate out or more money Inmate iurthered saying the Money Grams were -sentfo Uve

Oak Florida in Officer Hookers name Inmaten stated there were multiple transactions ranging from $4000 to $10000 Inmaten stated all of the moneymiddottransaclion numbers were provided to him by his mother via therecorded inmate lElephone Inmatenwent on to say about a month prior to that date lnmatefn approached him In lhe dorm and had live (5) small caliber bullets in his hand 22 or 25 it appeared Inmate also had a touchscreen cellular phone When lnm-ate questioned Inmate about the contraband Inmate stated Officer Hooker broJght him the bullets and phone Inmate

mdlcated the phone was to research how lo OBke a zip gunmiddot or self-fashioned hand gun _

Inmate stated Officer Hooker was sexually iovdtved with Inmate as well Inmate

stated Officer Hooker had been having sex with Inmate in the officers station bathroom while on shift Officer Hooker also went as r as to jn

that Inmate is in of Inmatenstated this was done by Officer Hooker perfonnlng swlt lets on several of the members of the g _ang inside the prison al one time Inmate went pn to say Officer Hooker later brought tattoo Ink into the facility and allowed one of the memberstto put an tattoo on her leg Inmate

was not sure which one he only heard about the tittoo from Inmate

On June 15 2017 I reviewed all of Inmatentime All of those calls were placed to Inmate Of 1hose calls I found four instances when Inmaten instructs hrs mother to send money to Officer Hooker Shortly thereafter in a subsequent phone call would provfde Inmate

with an eight (8) digit money control number from Money Gram I took note of all of the provided numbers and checked them on the website mone ramcom utilizing the websites transaction tracker With his tool I was able to enter the our numbe rs and input Hooker as themiddot last name of the person picking up the money (this Is re uired Information tor the tracker) All of the transactions showed valid and complete wlthln the jltlme days of the phone calls In total I found Officer Hooker to have received $34000 from thf transactions Intercepted through the recorded calls On multiple phone calls Inmaten is heard tellingn that some of thengang members were standing beside him and he was bn g put down onbull or extorted out of the moneyn was sending to Officer Hooker Tn

Based on these findings and the proximity of Officer Hookers resfdence I made contact with the Wa1-Mart store in Live Oak and spoke to their loss Prevenlion -team 1 provided all of the MoneyGram numbers I found and asked they referenee hem to their records for pickups I was later confacted by an associate who had confirmed twolof the transactions were redeemed on June 2 2017 for $10000 Both transactions were sign by Officer Hooker Even further video footage was obtained of Officer Hooker in the store At my review of the video from previous unrelated contact with Officer Hooker I confinned the 1 erson in the video was in fact Officer Hooker

On June 15 2017 I made contact with Inmate agreedmothern to meet with Inspector Lee McGauley and your affiant in Jackson llowing day On June 16 2017 we met at the Krystals Restaurant al thentersection C)f 103n1 Street and Blanding Boulevard In Jacksonville provided a sworn statementnconlirming she sent MoneyGrams to Officer Hooker multiple times throughout the past fewn onths fndicated she received Officer Hookers name and personal infonnallon from a contraband cellular phone that she beUeved her son was using fell like Inmate that he was being extorted by other inmates and Officer Hooker was one ofihe inmates wiyes or girlfriend ll was not until June 4 2017 in Inmaten last phone call that he revealed Pfficer Hooker was in facla Correctional Officer could not recall how many wire trasfdrs she had sent to Officer Hooker but estimated the total monies to be over a $100000 Furttjer legal process has been submitted to MoneyGram to further investigate the alleged otherntransactionsn also provided photos of text messages between her and Officer Hooker as well as phone records shoWing the phone calls between Officer Hooker and also stafed after Inmate was placed In confinement she was contacted by someone aUeging 19 be Officer Hookers friend This person toldn if she did not want her son to be hurtn would throw away all of the MoneyGramnrecords she had stated she could -tell by the back ground noise the caller was in a prisondonn After that call received a call from Offirr Hooker Officer Hooker claimed the people that calledn were hertriends and they wer looking out for her Officer Hooker -then tried to reassuren ihat nothing would happen IQ 1 mate

On June 16 2017 l received a phone call from Walker mons the Warden at SWCI Warden Clemons siated the facility received a phone callirom a fo mer correctional officer Robert HenryJ Henry spoke to Assistant Warden James Taylor at the ti e and reported receiving a disturbingT phone call from Officer Hooker Robert Henry stated Officf r Hooker called him the previous night

Officer Hooker stated and she has fallen In love

and advised Henry she is In a relationship with irrmate the relationship began with her receiving a letter from lnmate with hlm Officer Hooker stated she is in love wlth 1nmale and is waiUng on him to get out so they can live together Henry advised that Officer Hcentollter stated other inmates are looking out for her

Count 2 On May 23 2017 May 26 2017 June 1 2017 and June 2 2017 Officer Mattie Hooker did in fact receive a MoneyGram wire transfer for $40 from the mother of P_inmatee This wire transfer wF sent to Officer Hooker al the behest oflnmatee Inmate requested his mother wire the monies to OfflcereHooker because Inmates who were documented gang mbers were extorting InmateefOfficer Hooker was known to be in a relationship withmiddotthe leader of these inmatee
