Consumers are eligible for this promotion only if they purchase a Tempur-Ergo Premier Adjustable Base with a Tempur-Pedic mattress.

What has to happen for the customer to get the Bose gift?

1. Customer makes purchase. Store provides Customer a flyer explaining how they will receive the their gift. (see flyer, page 2). Stores have been sent flyers to be given customers at time of sale. If, for any reason, flyers are not available, it is possible to go to the Current News page at www.fmgbuyinggroup.com/currentnews.aspx to download and print a copy of the flyer which can be given to the customer. (No password is required to access this page.)

2. After the sale has been processed, sales force/management will tag sale as being eligible for this promotion. The way this occurs is completely up to each individual company.

What has to happen for the customer to get the Bose gift?

3. Record of qualifying sale.

At the conclusion of the event, each participating retailer must provide to FMG a listing of all Customers that have made a qualifying purchase during the event.

This simple spreadsheet requires only a modest amount of information--name, address, phone, retailer, branch, date of purchase, items purchased, and delivery date.

Only customers with a DELIVERY COMPLETION DATE indicated are eligible to receive their Bose gift. The last date for the consumer to submit gift request is Jan. 1, 2016 due to Bose ever changing product lines. The sale must be completed and FMG must be notified prior to that date.

Smart Source LLC will contact the purchasing consumer by phone. It is extremely important that telephone information (more than one number where possible) be accurately listed on the worksheet.

4. Purchased items are delivered to the Customer. Store notifies FMG using the ”Tempur-Pedic Bose Giveaway Promotion Dealer Worksheet” (see prior page) that the Customer is now fully eligible to receive their gift. FMG notifies Smart Source LLC. that contact can be made with the Customer.

Phone contact will be made within 2-3 days of FMG’s submission of the consumers information to Smart Source LLC. (submission will not be made until after delivery date).

What has to happen for the customer to get the Bose gift?

massage.

What has to happen for the customer to get the Bose gift?

5. Once Smart Source LLC. has made phone contact with the customer the item that the customer has chosen will be drop shipped directly to the provided address within 7-10 business days (East Coast should be shipped in fewer than 7 days but the west coast could take the fully allotted time) .

SUMMARY

There is no charge to the consumer or to FMG member retailers for the Bose gifts.FMG member retailers do not have to handle the gifts. Stores are only required to provide the list of qualifying customers. Contact will be made by the clearinghouse, the customer will choose the gift they prefer and the gift will be drop shipped directly to the consumer.

Customers will receive a Bose gift upon completion of the delivery date. The customer will choose from among the offered gifts at the time of phone contact from Smart Source LLC.

Offer valid for purchases made between October 1, 2015 and October 19, 2015 only. Prior sales excluded. Only one gift may be selected per Ergo-Premier adjustable base and mattress purchase There is no limit on the number of gifts for which a customer may qualify. One gift per each complete Tempur-Pedic Ergo Premier base and mattress set purchased. Purchase of a king set entitles the customer to only one gift.

Upon submission of consumer information from FMG (FMG will submit purchaser names with completed deliveries to Smart Source LLC weekly beginning the week of October 19, 2016), Smart Source LLC. will process the request and contact the consumer by phone within 2-3 business days. If a consumer cannot be reached by Smart Source LLC, Smart Source LLC will notify FMG and the store where the purchase was made. Once shipped a gift can not be withdrawn. In the case of a return or cancellation where a gift has been issued, the value of the gift issued should be deducted from any refund by the retailer. Gift request must submitted on or before January 1, 2016. This is due to changes that could be made in the Bose product offering All taxes are the responsibility of the person receiving the redemption.

How is the event being supported?

Vinyl 24 x 62.5" banner will be available to stores for this event.

The banners have grommets so that they can be used on X-stands, mounted to a wall, or hung from the ceiling.

The banners are being provided to FMG member retailers at no charge.
