What you need to give us• Details of the certified course content from the university, college
or institution including the duration, fees, results sheet and/orcertificate.
• Result sheet is mandatory for reimbursement.• The course must be associated to the work you do In the event
an employee fails one or more subjects, the re-sit fee will not beaccomodated via this scheme.
• Unit Managers (UM) & above are eligible for reimbursement ofMBA, MSc or any Masters Programme.
• For Asst. Managers and Specialists who have completed fiveyears of service, Dialog will reimburse yourinvestment for MBA/MSc Programmes,subject to a bond of two years offurther service from the resultsreceived date. The bond isapplicable in the event youhaven’t already completed tenyears of service, and for those ofyou who have alreadycommenced your course; proofof course completion post 1stJanuary 2016 will make youeligible till 31st December 2017.

How to apply?
You make the request via HCM with supporting documents attached
Once approved, GHR will make the payments to you along with the salary (applications approved by the 5th of the respective month will be reimbursed through pay of the same month)
• This scheme is applicable for Universities and AcademicInstitutions ranked amongst the top 400 in the world,and/or are registered with the University GrantsCommission of Sri Lanka.
• 50% of the course fee and 100% of the examination feecan be reimbursed within the annual entitlement
• Registration and any other payments made to theinstitute cannot be accomodated via this scheme
• You must apply for reimbursement within 3 months uponreceiving results
• Subject wise reimbursment is not allowed, howeverstage or semester wise reimbursement is possible

How to get your vehicle loan?

If employee fails to settleat the time of resignation,to recover from guarantors orseek legal action to recover

You can obtain a salary loan upon completing 2 years of continuous service and have a clean disciplinary record.
What you need to fulfill• Have no more than 40% deductions of your pay (basic)• Have no outstanding loans for the past 6 months• If you are on contract basis, you need to have a further
6 or 12 months contract period based on installments
What you will be entitled for• Executive to Senior Executive – could obtain 01 month base
pay as a loan• Officer and below levels – could obtain 03 months base pay
as a loan• If you are on a contract basis you need to have a further
06/12 months contract depending on the re-paymentperiod

You need to have• Two guarantors (Internal)• Guarantors should have completed 2 years of

• Loan re-payment period - 06 months or 12 months• If the loan recovery period is 06 months - NO interest will
be charged• If the loan recovery period is 12 months, 1% per month,
interest would be applicable from the 7th to the 12th month

What you can claim• Medical claims (including ambulance service, scans, tests,
vaccinations, dental, spectacles) for yourself and/or animmediate family member must be supported byprescription/s and original receipt/s from a registeredmedical practitioner. The bills should not be older than30 days from the date of issue.

How can you apply?
You apply for the outpatient reimbursement via HCM and hand over the claim to GHR supported by the following documents:
• Printout of the HCM submission• Hard copy of the prescription (original or certified copy mandatory)• Orginal payment receipts
Upon receiving the documents, GHR will notify you with a received alert
Verification of reimbursement documentation by GHR if required
You must promptly clarify / act on queries if any
Once approved by GHR, the reimbursable amount will be remitted to the employee’s bank account.

How to obtain your hospitalisation cover?
Upon renewing the insurer every year, you will be informed of the name of the insurance company & hotline number
In the event of hospitalisation (except for the sole purpose of medical testing) which includes general personal accident and critical illness, you need to inform the admissions desk of the hospital / nursing home and insurer via the insurance hotline
Cashless routeOn admission, you should inform the insurer via the insurance hotline and obtain a reference number
On being discharged from the hospital, you inform the insurer via hotline, in order to settle bills
Costs incurred outside of the stipulated limits, must be borne by you
Reimbursement routeWhen being discharged from the hospital, you should inform the insurer via the insurance hotline
Upon being discharged from the hospital, you must obtain a doctor’s endorsement on the Hospitalisation Insurance Claim Form, pay the final bill and obtain the diagnosis card to be handed over to the Insurance Unit, along with the duly completed hospitalisation insurance claim form, for reimbursement purpose within 10 working days
You will be informed to collect the reimbursementpayment by cheque from the Insurance Unit/GHR
