3. Narrative of Center’s Facilitation of Research, Education and Training
2011 – 2012 HVRC Annual Report Summary of Accomplishments:
Efforts continue toward our mission to foster collaborations in both clinical and basic science research in hypertension and vascular disease. However, service to the institution and professional societies is encouraged. In particular, senior faculty link other colleagues and junior investigators with professional society leadership opportunities. In accomplishing our goals, ~60% of Center faculty provide service as president, chair or committee member to major societies [APS, FASEB, ASPET, AHA, Consortium for Southeastern Hypertension Control (COSEHC)], in peer review for national agencies (NIH, CDC, CSR) and associations (NKF, AFAR, COSEHC, AHA), and volunteer service at the VA. Notable highlights include service on boards/external advisory committees (AHA Mid-Atlantic Affiliate, Leadership Committee for Council for High Blood Pressure Research, Winston-Salem Heart Ball, North Carolina American Indian Health Board, HBCU-UP NSF at Johnson C. Smith University, Minority Access for Research Careers (MARC) and RISE programs at Winston-Salem State University, COSEHC, Kern Aspen Lipid Conference, International PedCAM Network, International Freshwinds Institute of Integrated Medicine of Mitra Programme, UK, International Board of Advisors of NIKIM of the Dutch Society of Integrative Medicine, Planning Committee of the Forsyth County Health Disparities Town Hall Meeting, Forsyth Davie Stokes Medical Society). These activities are listed in the Faculty Achievement Table following the narrative. Center members continue to be well represented throughout WFSM on committees, task forces and working groups for administration, teaching and research, including as member of ATI teams.

Collaborative Research Activities Fostered by the Center
Center Core Faculty increased to fourteen for 2011-12 (Diz, Aileru, Brosnihan, Chappell, Fortunato, Gallagher, Groban, Hawfield, Lindsey, Petrovic, Shaltout, Tallant, Varagic, Yamaleyeva). Core faculty are defined as those who direct Center resources (see Organizational Charts for details) or provide key expertise for collaborative activities and represent 17 different departments and Institutes/Centers. This overlap affords good

communication between programmatic elements within the institution. Core faculty hold biweekly “Data Chats” to encourage shared ideas, feedback for plans for grants, strategic thinking for programmatic funding. Four Core Faculty are junior investigators whose submitted proposals in 2011-12 will be starting in FY13. Figures 4-6 illustrate the funding history of the Core Faculty, the collaborative nature of their funding, and the increase in clinical research grant activity by these investigators.

Major support for collaborative research activity includes Core faculty as integral to two program project grants, one continuing program ‘Vasodepressor Mechanisms in Hypertension (HL-51952)’ and one renewed this year entitled ‘Prenatal Events - Postnatal Consequences (HD-047584)’. Center members are involved in other programmatic funding in the Pepper Center (AG021332), Lipid Sciences “Lipoprotein Metabolism in Atherosclerosis (HL-49373), Comprehensive Cancer Center and in clinical trials (SPRINT, ACCORD, TRIP, CARDIA, CLIP,SEA, ACTION, HELP-PD).
Collaborative efforts led to new funding from intramural sources (TSI, Pepper Center, Center for Biomolecular Imaging, IRSC, Center for Integrative Medicine, WFSM Venture Fund and Lois Groskert Heart Fund) and extramural NIH and industry funding for competitive research grants using magnetic resonance spectroscopy for correlating brain transmitters with autonomic function and adiposity, applying vascular anti-growth agents for cancer therapy in children,

characterization of cellular growth properties of muscadine extracts that have been extended from vascular actions to anti-cancer effects, and application of vascular measures in non-human primates.
Center support for core facilities generated pilot data for research leading to extramural support, including data obtained with equipment purchased with capital funds for human subject studies leading to 4 new grants and additional planned clinical research grant submissions; two Center cores made substantial contributions to the data generated by TSI mini-grant funding and core faculty have obtained internal funds from other Centers for pilot studies. Examples of ongoing clinical/translational research topics facilitated or funded by HVRC core resources (Figures 7 – 9 illustrate core facilities and technologies): human urine proximal tubule cells used for expression of components of the renin-angiotensin system; urinary vitamin D and racial differences in adolescents born prematurely (Pediatrics, Ob/Gyn, Perinatal, Nephrology); non-invasive autonomic or vascular dysfunction assessment for dietary nitrite intervention, characterization of children with chronic nausea, as objective measures of behavioral interventions for stress and weight reduction, and coupled with magnetic resonance spectroscopy for brain transmitters in the elderly and as correlates with adiposity (Pediatrics, Complementary and Integrative Medicine, Reynolda faculty, Neurology, Anesthesiology, Pepper Center, CBI, Maya Angelou Center); cancer treatment Phase I/II trials (Pediatrics, Cancer Center); treatment modalities to reduce cognitive function decline following radiation treatment, to reduce fibrosis after radiation treatment, and to prevent cardiac pathology associated with cytotoxic therapies or drug abuse (Brain Tumor Center of Excellence, Radiation Biology, Orthopedic Surgery, Cancer Biology, Physiology & Pharmacology, Neuroscience); and sex differences in sub-clinical vascular disease in patients at risk for stroke (Neurology). Translational studies focus on non-invasive blood pressure and vascular measures to complement neurohumoral profiles in Vervet monkeys (Primate Center), angiogenesis and cell growth (Cancer Center), gender differences and salt sensitivity related to aging, heart function and renal injury in rats, pre-eclampsia in humans and rats, and fetal programming of cardiometabolic disease in humans and sheep (Anesthesiology, Perinatal, Pediatrics, Nephrology).

Collaborative efforts led to acquisition of the Vevo LAZR small animal imaging equipment (~$1 million), funded in July FY13 by a large equipment grant to Hypertension Center faculty (Figure 9) in collaboration with the Center for Biomolecular Imaging. This equipment will provide state-of-the-art small animal imaging to investigators in the Hypertension Center as well as throughout the medical campus, to provide non-invasive imaging of cardiovascular function, blood flow and angiogenesis, to generate pilot data for research leading to extramural support. Initial costs for pilot projects will be offset by HVRC faculty effort and technical and computer support from CBI.
Center membership increased from 48 to 49 with ~12 new members in process. Collaborations are the basis for both grants and publications with Anesthesiology, Biochemistry, Cancer Biology, Cardiology, Comparative Medicine, Emergency Medicine, Gerontology & Geriatrics, Health & Exercise Science, Hematology/Oncology, Internal Medicine, Molecular Genetics & Genomics, Molecular Medicine, Nephrology, Neurobiology & Anatomy, Neurology, Obstetrics & Gynecology, Orthopedic Surgery, Pathology-Lipids, Pediatrics, Physics, Physiology & Pharmacology, Public Health Sciences, Radiation Oncology, Center for Biomolecular Imaging, Complementary & Integrative Medicine, MACHE, Sticht and Pepper Centers, Primate Center, Translational Science Center, Translational Sciences Institute, Women’s Health Center, and WSSU, NCCU, VaTech, and Livingstone.
Center faculty maintained outstanding productivity with over 203 journal articles, 23 books or book chapters as shown in Figure 10. Figure 11 shows that the 14 core faculty members in FY11 had 31 full length publications, 72% of which are with another Center Faculty member, and have ~100 different internal and external co-authors as a measure of their collaborative approach to research.

Figures 12 and 13 highlight historical funding for Center Faculty and the funding sources for FY12. There are ~83 projects for $14.8 million in total funding reported by the 38 Center faculty members whose data are reported through WFSM (does not include the faculty from other institutions). The 14 core faculty held ~$4.2 million in annual direct costs funding in FY2011, with over 86% of their grants held with 1 or more co-investigators within the Center. Another $1.5 million in annual direct costs in new funding starts in July 2012 for Core Faculty. All core faculty have clinical research collaborations with active or pending grants.

Community Engagement and Research
The Consortium for Southeastern Hypertension Control (COSEHC), headquartered within the Center, maintains community outreach activities including screening and educational sessions that involve Center faculty, students and fellows.
Collaborative research efforts by center faculty with local pharmaceutical/nutraceutical companies focused on a novel drug for the treatment of hypertension (Targacept) and mucadine grape extracts to reduce angiogenesis/tumor growth (Nature’s Pearl).
Long-standing pipeline programs continue including the 20th year of the Excellence in Cardiovascular Sciences Summer Program, the 5th year of the Translational Sciences Institute Scholars summer programs, and the 10th year of the Post-baccalaureate Research Education Program (PREP) to increase diversity in PhD graduate programs. Competing continuation applications were submitted and are pending for both the summer program and the PREP.

The Hypertension Core Laboratory maintained CLIA certification and provided service to basic science and clinical research projects and clinical practice conducting over 7907 assays for 19 different internal investigators in 2011-12 for North Carolina Baptist Hospital Pathology, Pediatrics, Radiation Biology, Cardiology, Anesthesiology, Nephrology, Cancer Biology, Health & Exercise Sciences, General Surgery, Sticht Center, and Neurology. In addition, extramural use includes 13 investigators from 9 different institutions (Vanderbilt, Iowa, Alberta, Wright State, Mississippi, Florida, Northwestern, Toronto, Delaware). Billings were ~$300,000.
Other cores provide support to basic science and clinical research projects including the Cellular & Molecular Biology Lab, Transgenic Animal Core and Peptide Biochemistry & Metabolism.
Clinical support includes: CDC-sponsored hypertensive registry for the emergency room; non-invasive hemodynamics lab for arterial stiffness, vascular resistance and central pressures; and ambulatory blood pressure. We implemented autonomic function assessments for research and as billable clinical procedures to be used independently or in combination with tilt table testing in 2011.
Research Education, Training, and Career Development
Faculty contributed to medical student, resident and fellow, and graduate and undergraduate teaching and research training. Center core faculty are involved as training faculty in 17 of the 21 funded T32 or R25 training programs at the institution and are program directors or on Internal Advisory Committees of 9 (Figure 14). Center faculty were leaders of 3 graduate programs and participate in all Biomedical Sciences Tracks.

A US-Brazil Consortium, funded by DOE in collaboration with Wright State University, WSSU, NCCU and 4 institutions in Brazil, hosted 12 Brazilian undergraduate students for 6 months.
The Excellence in Cardiovascular Sciences and TSI Summer Programs, along with partnerships with JC Smith University and WSSU, hosted 21 undergraduate students for research experiences.
The NIGMS Post-baccalaureate Research Education Program (PREP) to increase diversity in graduate programs sent 4 students into PhD programs, 1 into an MD program, with 2 continuing for a second year, and 7 new students entered the program in August 2012.
Faculty participated on the Advisory Committee, Minority Access for Research Careers (MARC), at Winston-Salem State University, and served as mentors.

Summary:
These efforts fulfill the Center’s mission to expand collaborative efforts in hypertension and vascular disease research in both clinical and basic science arenas throughout the institution, to develop stronger collaborative approaches for non-invasive detection of autonomic dysfunction and vascular disease and apply these to treatments for cardiometabolic disorders and in cancer control, and to expand activities of faculty in research training and education in conjunction with the activities of the Translational Science Institute.
