All the above-mentioned tariffs include the O2 Virtual Technician service, which provides proactive monitoring of the Service. If a

possible fault is identified on the Service, O2 contacts the Subscriber to ensure their cooperation in the fixing of this fault.

The Internet START HD and Internet START HD Plus tariffs can be activated only by changing from the Internet Start, Internet Start

PROfi, Internet Start Plus, Internet Start Plus PROfi tariffs.The tariffs in the PROfi variant include a guarantee of the fault being removed within one business day from the moment it was

reported, one add-on service “1 fixed address v4”. If the Subscriber uses a different variant of the add-on service fixed IP address, i.e.

1+4 fixed IP addresses v4, or 1+8 fixed IP addresses v4, or 1+16 fixed IP addresses v4, billing based on the current O2 Internet

Connection pricelist applies. Each Subscriber who newly sets up the O2 Internet Connection service provided via a fixed line with a

tariff in the PROfi variant can get the O2 Comfortable Installation for the price of CZK 1 with a VAT (CZK 0.83 without a VAT)

Loyalty price

In the case of the HD series without a commitment from the current offer for the consumer, its price is equal to the price of the

same tariff with a commitment (plus "Plus") from the moment (a) the charging of such a HD free- if no such arrangement is agreed,

the expiration of 24 months of its proper use and payment, if such a moment occurs from June 1, 2018.

The tariffs in PROfi variant including, a guarantee of removal of the defect within one working day from its reporting, an additional

service "Fix IP adress v4". If the Subscriber uses a different option of a supplementary service, fixed IP address, ie 1 + 4 fixed IP

addresses v4, or 1 + 8 fixed IP addresses v4, or 1 + 16 fixed IP addresses v4 are charged according to current O2 pricelist Internet

connection. Each Subscriber who opts for the O2 Internet connection provided via a fixed line network with a PROfi tariff can be

provided with a O2 pohodlná instace at a price of CZK 1.00 with VAT (CZK 0.83 without VAT)

Form August 1, 2017 it will not be offered to the newly established tariffs in the PROfi version, antispy/antimalware filtr. From

September 1, 2017 the provision of web-based services will be terminated at all PROfi tariffs, antispy/antimalware filtr.

B. SHORT-TERM MARKETING EVENTS

1. Special offer – modem at CZK 399

Conditions:Each subscriber to O2 Internet Connection provided via a fixed network, who migrates to VDSL technology and at the

same time uses any tariff with a maximum download speed of up to 100,000 kb/ps from the current O2 service offer

Internet connection provided over a fixed network, or change tariff on tariff PROFi Internet PREMIUM HD including the

version Plus, PROFi Internet Aktiv HD including the version Plus, PROFi Internet Optimal HD including teh verison Plus,

may get, from July 1 to 31, 2018, the following bonus specified in paragraph B:

The Subscriber may buy a Basic modem at the price of CZK 399 incl.VAT (CZK 329,75 excl.VAT).

2. Special offer - Internet OPTIMAL HD Plus for CZK 469 (VAT inclusive) with commitment

with

The offer subject to the terms and conditions cannot be delivered for Subscribers whose technical conditions disallow

the installation of the service at time of application.

One of the conditions for the awarding of benefits and discounts arising from the offer is that the request for service

installation must be submitted to O2 in time and in a correct manner meeting the deadline specified in point A. The “D”

date for the consumption of the benefit is the date of delivery of the correct completed O2 request form.

This special offer cannot be combined with other discounts offered by O2 for the O2 Internet service.

Each current Subscriber to O2 Internet connection provided via a fixed network (natural entity registered under the birth

number and business customer identified based on IC without a framework agreement signed with O2 on the provision

of beneficial sales conditions for the O2 services) with tariff Internet OPTIMAL HD in in the option with or without

commitment, not using the offer of a reduced price for O2 Internet having no contract commitment for this service and

who will give permission to be contacted by O2 with marketing offer will be allowed to use this Special offer in the period

between July 1 to 31, 2018. Subscribers can call O2 free line no. 800 02 02 02, to receive as a bonus, in return, a cheaper

Internet Optimal HD Plus tariff at CZK 469 VAT inclusive (CZK 387,60 VAT exclusive) per billing period for a period of 24

months in exchange for a new commitment to the proper use and payment of the service for 24 months.

When migrating to another tariff, the Subscriber loses their right to the special price of the tariff.

From the 25th month, O2 is entitled to request a standard price list from the subscriber for the selected O2 Internet

Service tariff.

Conditions:

One of the conditions for the awarding of benefits and discounts arising from the offer is that the request for service

installation be submitted to O2 in time and in a correct manner meeting the deadline specified in point A. The date of

delivery of a duly filled O2 request is decisive for claiming benefits.

This offer is available only for participant who used the service O2 Internet ADSL technology for at least three months.

Standard list price of Internet OPTIMAL HD Plus tariff is CZK 499 VAT per billing period.

One of the conditions for the awarding of benefits and discounts arising from the offer is that the request for service

The Subscriber, who, on the basis of this offer, newly activates O2TV digital television service, can arrange an add-on

service Multi for the price of CZK 99 incl.VAT (81.82 excl.VAT) / billing period.

The subscriber shall pay the price of CZK 499 incl. VAT (CZK 412.40 excl. VAT) / billing period for the use of the Internet

OPTIMAL HD Plus tariff, the price of CZK 549 incl. VAT (CZK 453.70 excl.VAT) / billing period for the use of the Internet

ACTIV HD Plus tariff, the price CZK 649 incl. VAT (CZK 536.36 excl. VAT) / billing period for the use of the Internet

PREMIUM HD Plus tariff, the price CZK 499 incl. VAT (CZK 412.40 excl.VAT) / billing period for the use of the Internet

OPTIMAL HD tariff.

The Subscriber (natural persons registered with their birth certificate number) can newly activates, on the same PBX, a

O2TV digital television service with a commitment to use the service and pay for it for 24 month at the same time, the

price of CZK 299 incl. VAT (247.12 excl.VAT) / billing period for the use of O2TV M tariff, the price of CZK 399 incl. VAT

(329.75 excl.VAT) / billing period for the use of O2TV L tariff, the price of CZK 749 incl. VAT (619.01 excl.VAT) / billing

period for the use of O2TV XL tariff.

The monthly tariff fee of O2TV M or O2TV L also includes the monthly fee for an add-on service O2TV provided by other

devices. If this offer is used, no additional add-on services are provided within the monthly fee.

This special offer cannot be combined with other discounts offered by O2 for the O2 Internet connection service.

4. Special offer – advantage of certain types of HW

Conditions:

Each Subscriber without a General Agreement about Providing of Subsidized Services who, between July 1 to 31, 2018,

newly activates O2 Internet Connection service provided via a fixed network with the Internet OPTIMAL HD Plus tariff,

Internet Activ HD Plus tariff, Internet PREMIUM HD Plus tariff or O2 Internet Connection provided via a mobile network

with O2 Internet OPTIMAL HD tariff with a commitment to use the service continuously and to pay for it for 12 months,

and who will provide O2 with five contacts for people potentially interested in O2 services and who will guarantee that

these interested parties agree to be approached by O2 at these contacts, this Subscriber gets the bonuses specified in

this offer:

The offer subject to the terms and conditions cannot be delivered for Subscribers whose technical conditions disallow

the installation of the service at time of application.

3. Special offer – Internet OPTIMAL HD Plus for CZK 439 (VAT inclusive)

plus commitment Conditions:

From the 25th month, O2 is entitled to request a standard price list from the subscriber for the selected O2 Internet

Service tariff.

Each current subscriber to O2 Internet service provided via fixed network (a natural entity registered by birth number

and business entity identified based on identification code having no framework contract with O2 for the provision of

beneficial sales conditions for the consumption of services), with tariff Internet OPTIMAL HD in in the option with or

without commitment, not benefiting from a lower rate of O2 Internet and who will provide O2 with one contact for

people potentially interested in O2 services and who will guarantee that this interested party agree to be approached by

O2 at this contact, can between July 1 to 31, 2018, can call O2 free line no. 800 02 02 02, to receive as a bonus, in return,

a cheaper Internet OPTIMAL HD Plus tariff at 439 VAT inclusive (CZK 362,81 VAT exlusive) per billing period, for a period

of 24 months in exchange for a new commitment to the proper use and payment of the service for 24 months..

The Subscriber can purchase a set-top box for CZK 999 incl. VAT (CZK 825.62 excl. VAT).

The Subscriber can purchase a Basic VDSL modem to use the service for the price of CZK 1 with VAT (CZK 0.83 excl. VAT).

When migrating to another tariff, the Subscriber loses their right to the special price of the tariff.

Standard list price of Internet OPTIMAL HD Plus tariff is CZK 499 incl. VAT per billing period.

The offer based on these conditions cannot be used by subscribers if the service activation cannot be carried out at their

location for technical reasons at the time when they submit the request for service activation.

One of the conditions for the awarding of benefits and discounts arising from the offer is that the request for service

installation must be submitted to O2 in time and in a correct manner meeting the deadline specified in point A. The “D”

date for the consumption of the benefit is the date of delivery of the correct completed O2 request form.

The providing of benefits according to paragraphs E of this offer is conditioned by the fact that the digital O2TV service

activation will include the purchase of a set-top box.

If the Subscriber cancels the O2 Internet Connection service during the subsidized period, they lose their right to claim

benefits provided as part of the digital O2TV service based on this offer.

The O2 Multi service cannot be used to cover the price of the set-top-box.

Providing of the benefit is conditioned by the fact that the subscriber did not use the services for at least 3 months prior

to submitting the request to have them activated.

5. Special offer – tariff for two months with 100% discount

This Special Offer cannot be combined with other benefits and Special Offers offered with the O2 Internet Connection

service with the exception of the Special Offers – benefits on certain types of HW.

Conditions:

A. Every Subscriber who between July 1 to 31, 2018, newly activates the O2 Internet Connection service with any tariff

from the current offer with a commitment to use the service for 24 months at the service location address specified on

the “Special offer – modem for CZK1 and tariff for two months with a 100% discount” list available at www.o2.cz (link

path: Customer Care/Documents/Home Internet/Other Documents) is entitled to the following bonuses:

The subscriber is entitled to a 100% discount on the monthly tariff for the first two months from the date of setting the

tariff. For tariffs with a maximum download speed of up to 100,000 kb / s, it can also purchase a basic modem to use the

service at a price of CZK 1 with VAT (0,83 CZK) excluding VAT.

Once the advantageous period runs out the Subscriber pays a standard monthly fee for the tariff.

If the Subscriber changes or cancels the tariff, they lose their right to the beneficial tariff fee.

The offer under these terms and conditions cannot be used by Subscribers who cannot have the service set up for

technical reasons at the time they submit their request to have the service activated.

The provision of benefits based on this offer is conditioned by the fact that the request to have the service activated is

submitted to O2 properly and on time in the period specified in paragraph A. The date when a properly completed

request was submitted to O2 plays a decisive role in the claim of the benefit.

The providing of benefits based on these terms and conditions is conditioned by the fact that the Subscriber has not

used the service for at least three months prior to submitting the request to have the service activated.

This Special offer cannot be combined with currently Special Offers provided by O2 related with the O2 Internet

Connection service and O2TV digital television service except Internet HD - Special Offer. Special Offer - Free Internet

commitment, not using the offer of a reduced price for O2 Internet having no contract commitment for this service and

who will give permission to be contacted by O2 with marketing offer will be allowed to use this Special offer in the period

between July 1 to 31, 2018. Subscribers can call O2 free line no. 800 02 02 02, to receive as a bonus, in return, a cheaper

Internet PREMIUM HD Plus tariff at CZK 549 VAT inclusive (CZK 453,72 VAT exclusive) per billing period for a period of 24

months in exchange for a new commitment to the proper use and payment of the service for 24 months.

Conditions:

When migrating to another tariff, the Subscriber loses their right to the special price of the tariff.

From the 25th month, O2 is entitled to request a standard price list from the subscriber for the selected O2 Internet

Service tariff.

12. Special offer –DUO for O2 Internet

Each subsrciber (natural entity registered under the birth number) who requests the special offer during the period from

July 1 to 31, 2018 and activates a new or special offer already has an existing O2 Internet with OPTIMAL HD Internet

tariff, Internet OPTIMAL HD Plus, Internet AKTIV HD, Internet AKTIV HD Plus, Internet PREMIUM HD, Internet PREMIUM

HD Plus, Internet ULTRA HD, Internet ULTRA HD PLus incl. their PROfi variants and activates, or at the same time, the

special offer already used by O2 Mobile Internet connection with Internet Optimal Air in the variant with commitment is

entitled to the following special offer.

The subscriber mentioned in point A above is entitled to a discount of CZK 250 with VAT (CZK 206.62 excl. VAT) from the

monthly tariff of the Otimal Air tariff in a variant with a commitment for the entire duration of the use of the O2 Internet

Connection service.

The bid according to these conditions can not be used by the participants, for whom the establishment of services is not

technically feasible at the moment of requesting the establishment of these services. If a technical obstacle is detected

in the establishment of the O2 Internet Connection Service in the fixed network only after the application has been

submitted, this Subscriber shall be entitled to a CZK 250 VAT deduction from the Monthly Pricelist of the Internet

Optimal Air Tariff in a variant with a commitment for 24 months from the date a technical barrier has been identified.

Upon the expiration of the preferential period, the Subscriber is obliged to pay the monthly pricelist price for this tariff.

In case of breach of any of the conditions of the discount provided in this special offer, the right to further discounting is

automatically waived: If the service is canceled, the standard price list of the current tariff will continue to be charged

without discount unless expressly agreed otherwise with O2.

Conditions:

This Special Offer can be combined with the currently offered O2 Special Offers related to O2 Internet Connection.

14. Internet HD - Special offer

Each Subscriber (a natural person registered by their birth certificate number), who asks for this special offer between

July 1, and July 31, 2018, and who newly sets up the O2 Internet Connection service with any tariff (with the exception of

the PROfi line tariffs) from our current offer with a commitment to use the service for 24 months and each Subscriber

(business client identified based on their IČ – company identification number, who does not have a Framework

Agreement about Providing of Subsidized Business Terms and Conditions when Getting Services signed with O2), who at

the same time newly activates an O2 Internet Connection service with the PROfi line tariff from the current offer with a

commitment to use the service for 24 months is entitled to the following bonus:

Conditions:

The Subscriber is entitled to a discount of CZK 200 with VAT from the monthly pricelist price of the tariff for the period of

the first six months from setting up the tariff.

Once the subsidized period runs out, the Subscriber will pay the standard monthly pricelist price for the tariff.

When the tariff is changed or the commitment is cancelled, the Subscriber loses their right to the subsidized tariff price.

The offer based on these terms and conditions cannot be used by Subscribers who cannot have the service set up for

on their birth certificate number and a business customer identified based on their IČ, a company identification number,

who does not have a Framework Agreement about Providing of Subsidized Business Terms and Conditions when Getting

Services signed with O2) with the Internet ULTRA HD tariff in the option with a commitment or without one, who does

not use the offer of a lowered price for the O2 Internet Connection service, and who submits a marketing consent to be

addressed by O2, can get this Special Offer using a toll-free O2 line at 800 02 02 02 between July 1 and July 31, 2018, in

exchange for a bonus in the form of a lowered price of the Internet ULTRA HD Plus tariff to CZK 649 with VAT (CZK

536.36 without VAT) per billing period for the period of 24 months in exchange for arranging a new commitment to

properly use the service and to pay for it for the period of 24 months.

60, FREE O2 120, FREE Volání, FREE 200 MB, FREE 1.5 GB, FREE 6 GB, FREE 10 GB, FREE 20 GB, O2 Spolu 1.5 GB, O2 Spolu

3 GB, O2 Spolu 6 GB, O2 Spolu 10 GB, O2 DATA 2 GB, O2 DATA 5 GB, O2 DATA 10 GB tariffs with a commitment is

entitled to this special offer.

A subscriber specified above in paragraph A. is entitled to a discount of CZK 100 with VAT (CZK 82.65 without VAT) form

the pricelist monthly tariff price of the O2 Internet Connection service arranged in contract ad A(i) when it is signed for

as long as their contract ad A(i) and at the same time contract ad A(ii) last continuously with tariffs agreed on when both

contracts were signed.

The offer based on these terms and conditions cannot be used by subscribers at whose locations it is not possible to set

up these services for technical reasons at the time their request to set up these services is submitted.

If there is an infringement of any of the terms and conditions of providing the discount specified in this special offer, the

right for further claim of the discount is automatically terminated: if the service is not cancelled, the subscriber is further

charged the standard pricelist price of the current tariff without a discount unless it is specifically arranged otherwise

10. Unless it is provably agreed on otherwise between the subscriber and O2, by negotiating a new tariff with a commitment

from September 1, 2017, a new commitment is negotiated from the same moment; this does not apply to a change of tariff

within the HD line tariff, if the change is negotiated before June 1, 2018.

C. JOINT REGULATIONS FOR SERVICES AND COMMITMENTS

WITH THE O2 INTERNET CONNECTION SERVICE PROVIDED

VIA A FIXED LINE ACTIVATED PRIOR TO FEBRUARY 28, 2017

When a tariff with a commitment is swapped for a different tariff with a commitment, the period of the previous tariff with a

commitment is calculated into the future commitment for the customer, if the commitment was agreed till 18.2.2015. The

calculation of the commitment is done only when the tariff is changed, it does not apply to the cancellation and new

activation of a service.

If necessary for completing network, O2 is entitled to charge the subscriber for compensation of costs incurred to complete

the network based on subsriber’s requirements.

Tariffs range of Internet Plus are provided within a Special offer of a cheaper tariff against the signature of a 12-month

contract and the correct payment of all relevant bills by the subscriber. If the Subscriber is not a consumer (ie a natural

person who has set up a service for a birth number, not for the purpose of his / her self-employment). After the expiry of the

12-month period, the contract will be automatically prolonged. Subscribers have the right to refuse the prolongation at any

time. The refusal will result in the subscriber’s, which is not a consumer, loss of entitlement to a benefit provided within this

Special offer and the tariff will be automatically converted into a relevant tariff from the commitment-free part of the service

portfolio. Subscribers will be entitled to automatically prolong the Special offer only when all their payments related to O2 are

O2 is entitled to request a financial deposit when a request for Service activation is submitted and when the Service is

provided especially in case of a Special Offer for a subsidized price. This deposit is automatically returned if the Subscriber

settles their billing for provided services in the last three subsequent billing periods, if they do not have any commitments

after due date and if the providing of their services has not been limited or cancelled in the last 12 months for not settling

their bills.

In the case of O2‘s reasonable suspicion of the Subscriber’s not meeting the obligations set out in the Agreement and the

General Terms and Conditions (namely when black-listed in the SOLUS register or any similar database or committing fraud)

O2 has the right to receive a deposit during the service application period of maximum of CZK 10,000. The deposit will be

returned based on a written request by the Subscriber at the earliest 12 months from the signature of Service Specifications

provided that the Subscriber’s recent payment history is clear (i.e. three consecutive bills are paid in due amount and within

the due date, no outstanding debt on the account, no service reduction / suspension over the recent 12 months due to

payment delinquency).

As of February 8, 2014, if a service is cancelled by the user (a natural person registered by their birth certificate number to get

the service) or based on an agreement by the parties but prior to the Special Offer running out O2 is entitled to charge the

user a fee for a premature service termination (a lump sum compensation) amounting to one fifth of the total of monthly

fees (based on their current tariff including VAT) remaining until the end of the agreed-on commitment period. If a service is

cancelled prior to the Special Offer running out by a Subscriber who is not a user (if the notice under the General Conditions

might) or or an agreement by the parties, O2 is entitled to charge the Subscriber a fee for a premature service termination

amounting to the total of monthly fees (based on their current tariff including VAT) remaining until the end of the agreed-on

commitment period.

All deposits will be returned to the Subscriber, unless used for the settlement of the Suibscriber’s debt with O2, not later than

on the expiry / termination of the contract.

If a subscriber contract for the O2 Internet service is terminated earlier than the Subscriber has made all payments for the

hardware all outstanding payments will become due and the Subscriber will be obliged to pay the service charges to O2

immediately. If the Subscriber fails to pay any of the payments for hardware all outstanding payments will become due and

the Subscriber will be obliged to pay the service charges to O2 immediately. When requested by the Subscriber O2 will make it

possible for the Subscriber, during the effective period of the contract of O2 Internet, to pay the outstanding amount of fees

for the hardware.

Prices and tariff conditions specified in the The Price list of electronic communication services: O2 Internet Connection will be

applied to all current subscribers having the tariff activated before July 31, 2015.

c) sharing the capacity items of the fixed line by more subscribers up to the number of a maximum ratio set in the pricelist (so called

aggregation);

e) simultaneous running of a different service of electronic communications on the given connecting line, e.g. Digital O2 TV (so called

flexi regime);

Especially in case of subscribers with a commitment running out starting on September 1, 2017, and a negotiated renewal of

a commitment (automatic prolonging) it is possible to renew the commitment once the current commitment runs out only

with a corresponding HD line tariff and only for 24 months under the terms and conditions under which a commitment can be

arranged as of March 1, 2017, if they are informed of this change in accordance with the law and using the method that is

used to send their billing. For end-of-date consumer subscribers starting on 1 June 2018, the renewal of the commitment

(auto-tranche), if agreed, will be canceled on the expiry date of the current commitment; this change is notified in advance to

the participants in accordance with the law. Subscribers who are informed of this change in accordance with the law and

using the method that is used to send their billing will be gradually migrated to the HD tariffs after September 1, 2017.

The PROfi tariffs are not intented for consumers.

The Internet Start tariff, the Internet Start Plus tariff, the Internet Start PROfi tariff and the Internet Start Plus PROfi tariff

could only be set up on July 31, 2017 established only on PBX line, where due to its insufficient capacity, a speed profil max.

4096/256 kb/s and less can be configured.

If the subscriber when specifying the O2 Internet service agrees with O2 on paying the installation charges by instalments (first

payment plus 24 monthly payments) then the subscriber, in the case of terminating the service before paying the full price of

the agreed installation, will have to settle the remainder of the charges for the installation of the service.

The service is configured to the highest possible speed profile of the DSLAM device available to the subscriber with regards to

the connecting line quality and the negotiated service tariff. The subscriber can check the configured speed profile in their

modem settings.

There are always limiting technical factors that cause a difference between the configured speed profile and the current

efficient transmission speed of the subscriber. Technical factors that cause a difference between the configured speed profile

and the current efficient speed of the service include mainly:

a) the connection quality of the subscriber’s computer or a different displaying device to the modem (e.g. WiFi connection); b) the quality and configuration of the subscriber’s computer or a different displaying device;

f) the contents of the subscriber’s targeted request in the internet network and other factors of the internet network outside O2’s

influence.

Frequently available speed is at least available in 95 % of measurements carried out during one calendar day on the

subscriber’s connecting line. An individual measurement of the regular speed corresponds to the overall real volume of data

transferred to the subscriber (or from the subscriber) during the measured time to a maximum possible volume of data

transferred during the measured time.

d) simultaneous connection of more computers or different displaying devices to the end point of the network;

Changes in the pricelist version effective as of September 15, 3017, caused the by the amendment no. 252/2017 of the

Collective (especially the exclusion of automatic prolonging for consumers) apply to subscribers right from the beginning of

the effect of the amendment, i.e. from September 2, 2017. Subscribers with contracts signed prior to September 15, 2017,

will be informed about these changes in accordance with law.

22. Special provision about mobile data services faults and responsibility for them

To find out the service performance and its faults the measuring between the end telecommunications device and the access

point in the internet networks plays a decisive role, it is done on the transport layer of a TCP/IP model.

If the subscriber finds out current changes of the service performance, which could be responsible for the fault, they can

submit a complaint about the service quality at the latest within two months from the day the fault appeared. If this is not

and outage or a temporary interruption of operation that O2 is aware of from their own operating data and about which O2

informs at a toll-free line 800 184 084, to secure customer’s rights from the responsibility for faults it is necessary that they

report the fault at the time when the service performance is change at a line 800 184 084 so that O2 can carry out a timely

measurement of the current service performance.

If O2 finds out that the complaint was justified, they remove the fault, if it can be removed, and within one month from

setting the complaint they return to the subscriber the amounts paid for services that they complained about. If the fault

cannot be removed the subscriber and O2 can withdraw from the contract if they provably deliver the notice of withdrawal to

the other party within 30 days from delivering the notification of setting the complaint to the subscriber at the latest. The

Specialized services and their impactO2 provides two specialized services using Internet access and optimized for specific content: the O2 digital TV service and

the IP telephony service (phone calls using voice transfer via an IP protocol). The current usage of specialized services has the

following impact on the internet access service: a) if a data limit is set up with the specific tariff of the internet access service,

the specialized service does not draw this limit; b) the real internet access speed may decrease by the data flow to each

device, where the special service using the same access is currently being used. For example, if the overall current connection

speed is 20,000 kb/s and IPTV is being watched in HD quality using a set-top box (with data flow of about 7,100 kb/s for HD

quality and 3,500 kb/s for SD quality) and two mobile devices (with data flow of about 3,500 kb/s per device) and at the same

time voice transmission on one VoIP device (with data flow of about 200kb/s per device), the internet access speed might

decrease to 5,700 kb/s, which equals 20,000-(1*7.100+2*3.500+1*200).

A fault of a service is a change of service performance meaning that there is a decrease in the downloading or uploading

speed under 25% of the advertised speed for a continuous period longer than 30 minutes (long lasting divergence) or during

one hour at least ten times for a period longer than 1 minute (large repeated divergence).

Large divergences from the advertised downloading or uploading speed can result in the slowing down or in an extreme case

in a complete suspension of internet access. This can result in a decreased quality of streamed video in high definition, longer

response time, longer updating or longer downloading or uploading of data in applications and services that use the internet;

and in the worst case in non functioning of these applications and services.

The Subscriber with services or commitments agreed before September 1, 2017, who receive a notification in

accordance with the law of the billing process, will be gradually migrated to HD tariffs.

Unless otherwise agreed between the participant and O2, a new commitment will be negotiated from the same date by

negotiation of the new tariff with commitment from 1 September 2017; this does not apply when changing a tariff

within the HD tariff series, if the change is negotiated before June 1, 2018.

F. JOINT REGULATIONS FOR SERVICES AND COMMITMENTS WITH

NETWORK AND ACTIVATED AFTER FEBRUARY 1, 2017

Tariffs with a commitment are provided in the Special offer regime. The subscriber commits to properly use the tariff for

24 months in exchange for a subsidized price of the monthly fee. If the participant is not a consumer (ie the natural

person who set up the service for a birth number, not for the purposes of his / her self-employment),the 24-month

period runs out the commitment arising from the subsidized price of a monthly fee is automatically prolonged by the

same period; nevertheless the subscriber has the right to refuse this future automatic prolonging at any time. If they

refuse the automatic renewal, the subscriber loses their right to the benefit provided as part of this Special offer once

the commitment period runs and terms and conditions of a tariff without a commitment apply to them.

If the consumer (consumer = a natural person who activated the service using their birth certificate number) infringes

their commitment, O2 is according to paragraph 13.7 of General Terms and Conditions entitled to the payment of a

contractual fine amounting to a one-fifth of the total sum of all monthly fees (based on the current lump sum fee of the

basic service with VAT) remaining until the end of the commitment period, including the already started month. If a

subscriber who is not a consumer infringes their commitment , O2 is according to paragraph 13.7 of General Terms and

Conditions entitled to the payment of a contractual fine amounting to the total sum of all monthly fees (based on the

current lump sum fee of the basic service with VAT) remaining until the end of the commitment period, including the

already started month. The contractual fine is charged without VAT. If the subscriber’s settlement of the billing is longer

than 65 days, the remaining period of the commitment is calculated from the day the notice period runs out.

If the consumer asks to have the Special offer cancelled before the commitment period runs out, O2 is according to

paragraph 13.5 of General Terms and Conditions entitled to charge them a fee for a premature termination (lump sum

compensation) amounting to a one-fifth of the total sum of all monthly fees (based on the current lump sum fee of the

basic service with VAT) remaining until the end of the commitment period, including the already started month. If a

subscriber who is not a consumer agrees with O2 on cancelling the Special offer before their commitment runs out , O2

is according to paragraph 13.5 of General Terms and Conditions entitled to charge them a fee for a premature

Tariffs of the O2 Internet Connection service offered after March 1, 2017, enable Internet connection on the agreed on

address where the service is provided via a fixed or mobile network, based on the choice by O2.

Roaming cannot be activated with these tariffs.

The service provided via mobile network is available only with a special SIM card (USIM).

The Special tariff offer for beneficial price cannot be combined with the Special devices offer for beneficial price.

[:kůl:], is entitled to the discount of CZK 100 incl.VAT (CZK 82,64 excl. VAT) from the standard price of a service tariff

provided via a mobile network. The Subscriber is only entitled to the discount if they pay for the services, where the

benefit is provided, properly and on time. Otherwise, O2 is entitled to recharge the Subscriber´s provided discounts. The

Subscriber is only entitled to one tariff with benefitial price to one of the combined services.

A subscriber who newly actives the service provided via mobile network and at the same time purchases a Zyxel LTE

4506 or TP-LINK MR200, has the option to cancel the service within 30 days from activating the services at a selling point

where they activated the service; the service is cancelled by an agreement and with immediately effect without a

contractual fine. Only a subscriber who has already paid for all issued billings and who returns an undamaged and

complete modem purchased at the time of the service activation. The subscriber is entitled to a refund of the purchase

price. The subscriber can use this benefit only once in 12 months.

Changes in the pricelist version effective as of September 15, 3017, caused the by the amendment no. 252/2017 of the

Collective (especially the exclusion of automatic prolonging for consumers) apply to subscribers right from the beginning

of the effect of the amendment, i.e. from September 2, 2017. Subscribers with contracts signed prior to September 15,

2017, will be informed about these changes in accordance with law.

Transitional Provision, effective from April 1, 2018 to May 31, 2020: The loyalty fare rule applies to subscribers who have

negotiated the HD tariff or changed the tariff from April 1, 2018. For other subscribers with a tariff HD series are gradually

migrating to such a rule by a notice delivered to them prior to a change in accordance with the law of the billing process.

If O2 finds out that the complaint was justified, they remove the fault, if it can be removed, and within one month from setting

the complaint they return to the subscriber the amounts paid for services that they complained about. If the fault cannot be

removed the subscriber and O2 can withdraw from the contract if they provably deliver the notice of withdrawal to the other

party within 30 days from delivering the notification of setting the complaint to the subscriber at the latest. The contract

terminates when the notice of withdrawal from the contract is delivered to the other contractual party.

Special provision about mobile data services faults and responsibility for them

A fault of a service is a change of service performance meaning that there is a decrease in the downloading or uploading speed

under 25% of the advertised speed for a continuous period longer than 30 minutes (long lasting divergence) or during one hour

at least ten times for a period longer than 1 minute (large repeated divergence).

Large divergences from the advertised downloading or uploading speed can result in the slowing down or in an extreme case in

a complete suspension of internet access. This can result in a decreased quality of streamed video in high definition, longer

response time, longer updating or longer downloading or uploading of data in applications and services that use the internet;

and in the worst case in non functioning of these applications and services.

To find out the service performance and its faults the measuring between the end telecommunications device and the access

point in the internet networks plays a decisive role, it is done on the transport layer of a TCP/IP model.

If the subscriber finds out current changes of the service performance, which could be responsible for the fault, they can

submit a complaint about the service quality at the latest within two months from the day the fault appeared. If this is not and

outage or a temporary interruption of operation that O2 is aware of from their own operating data and about which O2

informs at a toll-free line 800 184 084, to secure customer’s rights from the responsibility for faults it is necessary that they

report the fault at the time when the service performance is change at a line 800 184 084 so that O2 can carry out a timely

measurement of the current service performance.
