Message from CEOGreetings! You spoke, we listened. Last month, we conducted an employee satisfaction survey in IL and the results were clear. You work here because you believe in the work that we do. My values and passions are your values and passions. That is what sets us apart. We also heard your concerns about work/life balance and our need to improve company wide communication. This monthly newsletter will be just one way for us to share updates, news, and happenings across the companies. While this first edition is focused primarily on hospice, each month we will be adding new information and updates about all of our companies so that everyone can feel more connected as we continue to grow and expand.

I hope you all took the time to watch my podcast last week. I am so proud and grateful to have all of you- 800 people working strong every day to live the mission and values of this company! At the end of the day, I could not have imagined a greater outcome. Enjoy the rest of the summer.

Volunteer News!Things are beginning to pop in

volunteer services and the volunteer roster continues to grow! In June we

on-boarded six new volunteers: Swapnil (CW), Eesha (SE-summer),

Bethany (NE- summer intern), Rrezarta (SE), Alex (NE-summer intern) and

Stephanie (SE).  We have three additional prospective volunteers for the SE area and one in the CW area. Please extend a warm welcome to

these new volunteers when you see them in the office or in the field!

Social Services with Heather Barrera

Social Workers are an important part of the hospice team.  A hospice social

worker advocates for the patient and focuses on their psychosocial needs.

Some common examples of what hospice social workers do include:

Provide support/counseling to patients and families, offer education/resources,

assist in completing Advance Directives, help facilitate difficult

conversations regarding end of life care, along with much more!  If you

ever need help in any of these areas, please call your team social worker!

Hello all! I am Miranda Harris, the new Bereavement Coordinator for Transitions Hospice (but not new to the company). I had been a field social worker for the Chicagoland areas for just over a year before making my transition. For this first letter, I figured I would tell my story of why hospice and my passion for bereavement.

On March 12, 2016, I suddenly and very unexpectedly lost my mother from medical complications. It was a very tough grieving process not only for myself, but my immediate family as well. About 8 months later after her death, I realized there was more that I could do as a social worker other than be a discharge planner. Then, a close friend steered me towards Transitions. On the day of my interview, with Sara Dado and Heather Barrera (previous bereavement coordinator), I was graced by the presence of a beautiful cardinal, to add onto this, it was just a year after losing my mom. I knew it was my mom telling me this is where I should go and grow as an individual. Well, here I am.

There are plans to grow the bereavement program, and I am super excited to assist in making those changes. Sara and I are in the works of setting up memorial services in late July/early August for all regions. Just some food for thought, I did find some statistics in an article written in 2000 by Lattanzi-Licht showing those that bereaved, within 1 month of death, 5% are at a high risk (needing referrals to mental health professionals), 22% are moderate risk (in need of additional support outside of friend/family) and 70% are low risk (individuals deal with grief with support of family and friend support).

June 2018, we transitioned our PBM to Optum and Deliver Care Rx is our new hospice pharmacy. During this

time we provided 9 training sessions for the nurses to ensure they would be comfortable with using the new pharmacy. We believe this change

will greatly benefit our patients, families and company.
