Singapore Patient Conference Organised by: Members of the The Ng Teng Fong Healthcare Innovation Programme - Transforming workforce and care delivery, today and tomorrow Supported by: Recovery, Rehabilitation, and Resilience after Trauma MR AUYONG HAWYEE suffered a stroke in May 2011 and is the Founder of the Rehabilitation Assistance to Cure Heal and Enable Lives (RACHEL) Fund administered by Tan Tock Seng Hospital Community Charity Fund, which provides assistance to low- income families for stroke rehabilitation therapy. He currently works as a Research Associate at the Case Study Unit of the Lee Kuan Yew School of Public Policy. Neighbours for Active Living: Better Health, Better Care, Together with the Community DR EUGENE SHUM (Chief Corporate Development Officer), together with MS ZAHARA MAHMOOD (Assistant Director, Community Care) of Eastern Health Alliance, are reponsible for the ‘Neighbours for Active Living’ programme, which provides holistic support for clients with high healthcare and social care needs after they have been discharged from the hospital. The programme comprises a team of healthcare professionals and dedicated volunteers living in the neighbourhood. My Life Before and After my Two Encounters with Cancer MR CAYDEN CHANG is a two-time cancer survivor and Founder of Mind Kinesis Group of companies. Despite his condition, he wrote two books and donated their sales proceeds to ‘The Straits Times School Pocket Money Fund’ and helped raise funds for cancer research for National Cancer Centre and Singapore Cord Blood Bank. In March 2016, he established an endowment fund with Singapore Management University called ‘Cayden Chang Inspire Award’ to help financially-challenged students. Healing Through Community Involvement MS LEE YING YING is a Peer Support Specialist at the Early Psychosis Intervention Programme department of the Institute of Mental Health. Together with a multidisciplinary team, she supports persons in recovery by sharing her journey of recovery from psychosis. She believes in breaking the silence surrounding mental health issues and has taken baby steps towards advocacy for persons with mental health concerns. Activating the Seniors in Leading an Active and Fulfilling Lifestyle DR NOOR HAFIZAH BTE ISMAIL is a Senior Consultant in the department of Continuing and Community Care in Tan Tock Seng Hospital, where she continually builds a more robust and healthier community through various department initiatives. She plays an active role in the ‘Step it Out’ programme, a 24-week programme designed to reduce falls by targeting fall risk factors for seniors in the community. Building the Bridge of Empathy between People with Dementia and Caregivers MS WONG YOCK LENG is a Senior Care Manager (Social Worker). She works closely with people with Dementia who have given her deeper understanding into their innerself. Ms Wong graduated from the Arts and Social Sciences Faculty at the National University of Singapore and engaged in work on larger social issues such as migration, globalisation and urban poverty with various regional Non- Government Organisations from the period of 1997 – 2012.
