committee for several years, worked many shifts at fall fest, a chair of the hospitality committee, supported

athletics by working the gate, concession stand and organizing the hospitality suite for referees, and she's also

A MESSAGE FROM NURSE GERI

5

The End is Near!

Can you believe this school year is ending? It is so nice to talk about NORMAL end of the school year procedures.

Is your child turning 11 years old or going to the 6th

grade?

If so, it is time to have some important milestone immunizations. An official copy with an expiration date and office

signature is required to be sent to school. My chart and other self-serve portal copies are not considered official

copies and cannot be accepted. Please send the updated record to [email protected] by July 31st. If

your child is going to 6th

grade and will not have their 11th

birthday yet, do not worry. Updated records sent within a

month of their birthday are still considered on time. Most pediatricians will now administer this set of vaccines early.

If you have medications in the nurse’s office:

Medication pick up for the end of the year will begin on Monday, May 17th

at 9am and end on Friday, May 21st

at 12 noon.

Any medications left after noon on May 21st

will be destroyed.

Planning for Medication Administration in the 2021-2022 School Year:

1. Medication administration forms, food allergy certifications, and emergency plans for your child’s specific illness will

be handed out to parents at medication pick up and will be available on our website. These forms must be received

by July 31, 2021.

2. Parents must schedule an appointment to review medications, medication forms and allergies with the nurse the

week before school begins. Early and late appointment options will be available. An email will be sent to parents

with more details.

3. Any medication appointment requests after the first day of school will be scheduled for the second week of school

during regular office hours.

4. Your child cannot have medications left at school until a completed medication packet has been received and at

least one parent/guardian has met with the nurse.

5. No medications will be received on the first day of school by the office, or myself.

6. Duty teachers cannot accept medications.

School Nurse Hours for June and July

I will not be in town most of June and July. Appointments are reserved for new students with health concerns and newly

diagnosed health concerns of existing students. Please email me if you have a need to meet and I will try to

accommodate you the best I can.

Lastly, it has been an absolute pleasure to take care of your children this year. I will miss them and look forward to

seeing how tall they have grown in the fall!

Yours in Christ,

Nurse Geri
