The UIHA Accountable Care Organization (ACO) is now formed and funded, and work is underway to consolidate various member ACOs under the alliance banner. The first consolidated contract will be with the Centers for Medicare and Medicaid Services. Two physicians will lead change for the alliance by working with member organizations to design and implement clinical integration and population health management strategies. “By its very nature, a value-based care system is physician- led and focuses on the patient experience. We’re pleased to welcome two new alliance staff members to the ACO leadership team,” says UIHA President and CEO Dan Kueter. Both physicians will also participate in the alliance Physician Leadership Council. “These two physicians have already made key contributions to the alliance through their work as part of the clinical integration team. Now, we look forward to them building upon those experiences to lead and support our clinical efforts,” he says. Brad Archer, MD – UIHA Chief Clinical Officer As the UIHA Chief Clinical Officer, Dr. Archer is responsible for the design, implementation and management of value-based care delivery. His focus is on care innovation and redesign. He will work with alliance members at the chapter- level for practice transformation, quality improvement, and patient- engagement strategies. He begins his new role July 27. Dr. Archer comes to the alliance from MercyCare Community Physicians, Cedar Rapids where he served as Executive Medical Director. “Dr. Archer will take the outstanding work he has done at Mercy with population management, medical home and practice transformation and apply that through the alliance. His unique experience and success at Mercy make him the right fit for this critical position, and this will allow Dr. Archer to impact all alliance members,” says Mercy – Cedar Rapids President and CEO Tim Charles. His innovative focus on quality metrics and cost of care make him uniquely well qualified for this new assignment. IT TO HELP ENHANCE PATIENT EXPERIENCEUIHA has selected Optum One of Eden Prairie, Minn., as its vendor to provide an analytics platform for managing population health. The more than $7 million investment over four years in information technology infrastructure will give the alliance better access to health intelligence and workflow applications for enhancing the patient experience, increasing quality and reducing health care costs.

These population management tools can help care providers manage the increased clinical responsibility and financial risk assumed as part of a value-based care delivery model. The alliance will gain the ability to collect and share data from multiple sources, stratify the risk of patient populations, identify care interventions and support multiple care management programs, as well as support patient engagement tools.

As a participating ACO in Iowa Medicaid Enterprise Iowa Wellness Plan, the University of Iowa Health Alliance is measured by a Value Index Score (VIS). The score measures system change and the components necessary for better patient health outcomes and lower costs. Quarterly bonuses are made to the ACO for improving patient outcome performance against a baseline. The latest UIHA bonus was $44,730 for 3Q 2014 performance.

As one of the first ACOs for UIHA, this is an opportunity to learn and refine how to work with patient

populations. In this case, the population is adults 19 to 64 years old at or below the federal poverty line. Measurement is based on the successful implementation of the Healthy Behaviors Program, a means of encouraging IME members to take personal responsibility for their health and wellness. Completing online Health Risk Assessments (HRA) and Wellness Exams (EXAM) are key activities for the program.
