Care Plan• A care plan sets out in detail the way daily

care and support must be provided to an individual. Care plans may also be known as ‘plans of support’ or ‘individual plans’.

• Care plans are legal documents which might be needed as evidence if an individual makes a complaint.

• Care plans are written in the first person to make it clear that it is the individual who owns it.

What do we mean by psychological barriers?

Psychology is about how we think. Some people have fears, for example going to the dentist. Some older people may fear moving into a home as they feel they will lose their independence.

Can you think of anymore psychological barriers that people may face that could stop them from using health care services?
