VI. ADMINISTRATIVE RESPONSIBILITIES

Clinician lead – CUPS Health Centre General Internal Medicine/Primary Care Shared Care clinic: Dec 2016 – Present

Re-developed a traditional GIM consultancy clinic into a novel shared-care delivery clinic for socially vulnerable and homeless complex chronic disease patients in Calgary in partnership with primary care

Embedded 4 general internal medicine specialists in a primary care setting to co-manage complex chronic disease patients and improve access to specialists services for patients that traditionally have many social barriers affecting access to specialty care

Directing medical education for IM/GIM residents within CUPS as well as initiating both quality improvement and research programs for this clinic through coordination with existing CUPS C2C program.

Clinician lead - East Calgary Health Family Care Centre General Internal Medicine/Primary Care Shared Care clinic: Dec 2014 – Present

Designed and created a novel shared-care delivery clinic for socially vulnerable complex chronic disease patients in Calgary in partnership with primary care

Embedded general internal medicine specialists in a primary care setting to co-manage complex chronic disease patients and improve access to specialists services for patients that traditionally have many social barriers affecting access to specialty care

Directing both quality improvement and research programs for this new clinic through initiation of ongoing standardized data collection including granular clinical, utilization, self-reported sociodemographic and material deprivation data

Leading quarterly focus groups with the center’s physicians, nurse practitioners, nurses, multidisciplinary team members and management for iterative improvement of the current model and assessment of the quality of care delivered.

Early data suggests a significant improvement in bi-directional communication with family physicians, satisfaction with the model among all care providers, and access to GIM and specialty care for patients within the first 2 years of the clinic’s initiation.

Health outcomes for shared-care patients are being tracked longitudinally

Planning for external grants to support ongoing research program of this novel intervention and care model

Mosaic PCN Calgary Refugee Health Clinic: 2014-Present

Provide clinical service as an embedded Internal Medicine consultant for recently arrived refugee patients with complex disease within the clinic.

Work in collaboration with Mosaic Primary Care Network within a specialized refugee health clinic to direct, design and implement a research program around the care and health outcomes of recently arrived refugees to Canada

Initiated and developed a comprehensive quality improvement and research database with multiple database linkages across different domains.

Coordinate among multiple research assistants, IT support and privacy experts to collect, link, clean, process and analyze large volumes clinical data for QI and research

Leading coordination of a provincial approach to refugee care provision and provincial research network

Junior Faculty Research Committee Chair: October 2014 - July 2015

Created new research group for junior academic faculty members in the University of Calgary Faculty of Medicine to foster peer-mentoring, collaboration for research and increase research productivity among junior research faculty.

Membership includes health services researchers, epidemiologists, biostatisticians, basic and clinician scientists. Currently there are twenty-two active members.

Participation and support from senior faculty is scheduled to foster ongoing senior mentorship for junior faculty.

Chief Medical Resident University of Calgary Internal Medicine Program: 2010-2011

Elected as one of four Chief Medical Residents for the 2010-2011 academic year

Multiple administrative responsibilities including: Academic rotation review, R1/R4 CaRMS residency and fellowship matches, organization of the IM academic half-day, IM resident scheduling, organization of IM resident’s retreat and facilitating communication amongst staff and residents as needed

Created and organized a restructured overnight on-call system on the Medical Teaching Units of the IM program to a night float system of call for the 2010-2011 academic year, now permanently adopted

Coordinated the creation and implementation of a simulation curriculum for the IM program. This program has been subsequently enhanced, broadened and formalized into the IM curriculum

Coordinated, implemented and studied new electronic handover tool on the Internal Medicine clinical Medical Teaching Units, now permanently adopted in IM and expanded to other residency programs

Participated in the design and execution of a study of the senior resident night float system, simulation curriculum, and electronic handover document for publication through the Ward of the 21st Century research group

Evaluated and participated in selection of International Medical Graduate trainees into IM program for the 2011-2012 academic year

Created recruitment program for visiting medical students and organization of PGY1 Internal Medicine CaRMs selection process

Participated in the planning and reorganization of Medical Teaching Units (MTUs) in 2 major teaching hospitals including the expansion of clinical services and initiation of new MTUs

Reorganized IM Medical Teaching Units academic rounds

Created a clinical rotation for the care of Vulnerable Populations for the IM program to provide clinical experiences focusing on the care of immigrants, refugees, First Nations peoples and homeless and low-income populations

Invited to sit on AHS zone leadership committee focused on operations and management of high needs and high costs populations within the Calgary Zone

Membership representation includes senior provincial and regional AHS operational leadership and management

Representation also includes, primary care, public health, addictions and mental health, emergency department, aboriginal health, data integration and clinical leadership

Given research and clinical focus, was asked to contribute to zone planning and operations for high needs patients with an aim improve care and cost efficiency

Mosaic Refugee Health Clinic Research Committee: Nov 2016-Present

Co-created and organized a research committee of clinicians, staff and researchers to coordinate and direct a comprehensive research program focused on recently arrived refugees in Calgary.

This committee meets quarterly and duties include: reviewing all research applications to the Mosaic Refugee Clinic from external parties, trainees and researchers

Coordinating and prioritizing multiple research projects to maximize efficiency and ensure maximal benefit to refugee patients and improve care by refugee care providers, both locally and nationally
