LAKESHORE LEGAL AID, Southfield, MI Aug. 2013- Sep. 2014Long-term Care Ombudsman Investigated facility violation complaints (internal reviews) of residents living in long-term care facilities;

provided residents with information about their rights, community services available to them, laws (Medicare & Medicaid), regulations and standards that govern long-term care facilities, and good health and safety practice.

Interviewed witness (facility employees, volunteers, families, etc.) Planned and conducted initial investigation to make recommendations and reports to the state of Michigan. Documented daily findings; completed monthly report Planned and coordinated monthly visits to facilities to review changes and assisting state with plan of correction. When the complaint was not resolved, referred the complaint, together with all documentation, to the Regional

Long-Term Care Ombudsman under whose direction worked, with the resident’s consent, referred the complaint to the appropriate regulatory agency if necessary.
