intrOdUCtiOn TheResidencyProgramdrawsuponthebestresourcesoffouraffiliatedinstitutionsintheLosAngelesmetropolitanareatoprovideabroad,well-balancededucationalexperience.Thefacultyconsistsof30full-timeclinicalandresearchspecialistsinallareasoforthopaedics,trauma,reconstructivesurgery,pediatricorthopae-dics,handsurgery,footsurgery,sportsmedicine,jointreplacement,spinesurgery,orthopaediconcology,orthopaedicinfection,andthebasicsciences.Inaddition,150voluntaryfacultyalsocontributetoresidenteducation.Eachfacultymemberbringsexpertiseinhisorherspecializedareathroughparticipationinteachingrounds,instructioninclinicsandsurgery,formalteachingconferences,andresearch.

Master Schedule
Resident rotations are determined a year in advance, based on the grid provided in the ResidentScheduleMonthlyRotations.Amorespecificindividualscheduleisprovidedayearinadvance.
Call Schedules
WithintheMasterScheduleofrotations,eachhospitaldevelopsitsowninternalcallschedules,whichdetermineweeklyworkinghours.AswithallresidencytrainingprogramsatLAC+USCMedicalCenter,theDepartmentofOrthopaedicscomplieswiththebasicguidelinesestablishedbytheOfficeforGradu-ateMedicalEducationattheMedicalCenter.Theseguidelinesarebasedona1991Californialegislativebill,whichhaditpassed,wouldhavebecomelaw.
• Regularworkinghoursgenerallydonotexceed80hoursperweekonanyservice• On-Callassignmentsarelimitedtoamaximumof1in3nights• Residentsreceiveatleastone24-hourperiodperweek,freefromallpatientcareresponsibilities• EmergencyRoomshiftsarelimitedto12hours

Inadditiontoatleastone24-hourperiodeachweek,everyresidentreceivesonemonthpaidvaca-tion,3daysofeducationalleave,and12daysofsickleaveannually.
Curriculum is based upon requirements set by the American board of orthopaedic Surgery.
rESidEnCy prOgrAM (pgy-1 & pgy-2)internship: pgy-1
TheBoardrequires12monthsofgeneraleducation,whichissatisfiedintheinternshipyear(PGY1).Thefirstyearoftrainingisorganizedintothirteen28-dayrotationsinorthopaedicsandotherservicesrelevanttothecareofpatients.Asorthopaedicinternsareconsideredpartofthesurgicalinternshippool,theserotationsareassignedbytheOfficeforGraduateMedicalEducation,withconsiderationofthespe-cializededucationalneedsoforthopaedicresidents.

TheformalresidencybeginsinPGY-2.TheBoardrequiresallresidentstocomplete12monthsoftrauma,12monthsofadultorthopaedics,6monthsofbasicscienceand/orclinicalspecialities,and6monthsofpediatrics.Theresidentmustspend4yearsinaprogramwhosecurriculumisdeterminedbyanorthopaedicresidencydirector.Rotationsareorganizedtoassureincreasingresponsibilityandexposuretoallaspectsoforthopaedicsasresidentsgrowinknowledge,skills,andmaturity.
Orthopaedic trauma (4 months)
Intraumarotations,PGY-2residentslearntomanageacuteorthopaedicproblemsemergentlyandelectively.Residentslearnpre-andpost-operativecareofpatients.Theybegintooperateunderthesuper-visionofattendingphysiciansandseniorresidents.Inthetraumaout-patientclinic,PGY2residentsseepatientsandpresentthemtotheseniorresidentandattendingphysicianswhostafftheclinic.Thisencour-agestheresidentstomakerecommendationsregardingpatientmanagement.Theresidentsalsolearntechniquesintraction,casting,andminorsurgeryduringthistime.

Residentsspendabout2monthsatChildrensHospitalLosAngelesgainingexposuretoneurologi-cal,congenital,developmental,andtraumaticconditionsinchildrenthroughsurgeryandout-patientclin-ics.
rehabilitation/Amputee Service (1 month)
Residentsspend1monthatRanchoLosAmigosMedicalCenterwheretheylearntomanagechron-icanddebilitatingconditionsofneurological,congenital,metabolic,infectiousandtraumaticorigins.Theyaretaughtbasicprostheticsandorthoticsduringthistime.

Thetraumarotationconsistsoftraumasurgery,emergencyroom,andtraumaconsultations.IntheEmergencyRoomattheLAC+USCMedicalCenter,residentsparticipateinthemanagementoforthopaedicpatients.DuringthethirdrotationofPGY-3trauma,residentsareresponsibleforallroutineortho-paedicconsultationsinLAC+USCMedicalCenter.Dailyroundswithfacultyandseniorresidentsarecarriedout.
pediatric Orthopaedics (2 months)
ThePGY-3residentisgivenincreasedclinicandsurgicalresponsibilitiesonthepediatricservicesateitherChildrensHospitalortheLAC+USCWomen’sandChildren’sHospital
Adult reconstructive Service (2 months)
AtUSCUniversityHospital,PGY-3residentsparticipateintotaljointsurgeryandinthemanagementofadultorthopaedicproblemswiththefulltimefacultywhosepracticesarelimitedtospinesurgery,handsurgery,jointreplacements,arthroscopy,orthopaedicinfections,oncology,footandanklesurgery,andtraumacomplications.
Orthopaedic infection (1 month)
AttheLAC+USCMedicalCenter,residentsparticipatewiththefacultyinthetreatmentofboneandjointinfections.Theorthopaedicinfectionservicehasadedicatedwardthataverages25in-patientsperday.Electivesurgeryisperformedoneandahalfdaysaweekandpatientsareseenintheclinictwodaysaweek.
Sports Medicine (1 month)
Residentsspend1monthatLAC+USCMedicalCenter,gainingexposuretosportsmedicine.
Vacation/Elective (1 month)
Clinicalandresearchexposuretoanorthopaedicsubspecialtyselectedbytheresident.

trauma (4 months)
PGY-4residentsreturntoLAC+USCMedicalCenterwheretheyresumeparticipationintheiroriginalteamtoprovideamoreadvancedleveloftreatment.Theywillbeabletofurtherevaluatetheresultsofthetreatmentprovidedbytheirteamthepreviousyear.Inadditiontotheemergencytraumaadmittingday,eachteamisassignedtoelectivesurgeryandclinics.
Arthritis/Orthopaedic reconstruction (1 month)
ResidentsspendonemonthontheArthritisserviceatRanchoLosAmigosMedicalCenter.Residentsareexposedtothreedaysofsurgeryperweekateachinstitutionandofficevisitsforpre-andpost-operativemanagement.
Adult reconstructive Surgery (1 month)
ResidentsspendtimeatKaiserBaldwinParkconcentratingonthemanagementofadultorthopaedicproblemswithexposuretoarthroscopic,sportsmedicine,andgeneralorthopaediccases.
Orthopaedic Oncology Service (1 month) AtLAC+USCMedicalCenter,NorrisCancerHospital,andUSCUniversityHospital,residentsworkincloseassociationwiththeOncologyfacultyinthemanagementofadultandpediatricbenignandmalignantmusculoskeletaltumors.Electivesurgeryisperformedthreedaysperweek.Clinicsandofficevisitsprovideresidentspre-andpost-operativemanagement.
RESIDENCY PROGRAM (PGY-3 & PGY-4)
Hand Surgery (1 month)
ResidentsontheHandServiceparticipateinsurgeryatLAC+USCMedicalCenterandtheH.ClaudeHudsonComprehensiveHealthCenter,andUSCUniversityHospital.Thisrotationprovidesresidentswithcomprehensivetraininginallaspectsofhandsurgery,includingacutetrauma,congenital,reconstructive,andmicrovascularsurgeryofthehand.HandclinicisheldatLAC+USCMedicalCenter.
Spinal disorders (2 months)
ThisserviceatUSCUniversityHospitalissplitbetweenin-patientandout-patienttreatmentofthespine.Particularemphasisisonspinaldeformitiesandspineinjuries.
Sports Medicine (2 months)
ThisserviceatLAC+USCMedicalCenterandH.ClaudeHudsonComprehensiveHealthCenteraredi-videdbetweenin-patientandout-patienttreatmentofsportsproblems.Theparticularemphasisisonacuteandchronicsportsinjuries.
Vacation/Elective (1 month)
Clinicalandresearchexposuretoanorthopaedicsubspecialtyselectedbyresident.

PGY-5iscomposedofrotationsintraumaorthopaedics(LAC+USCMedicalCenter),adultrecon-structiveorthopaedics(LAC+USCMedicalCenterandKaiserBaldwinPark),pediatricorthopaedics(LAC+USCMedicalCenterorChildrensHospital),sportsmedicineandhandsurgery(LAC+USCMedicalCenter).TheserotationsforPGY-5residentsaresimilarinformattothoseoutlinedforPGY-2,3,and4residents.How-ever,thelevelofresponsibilityandindependenceismuchgreaterforseniorresidents.Residentsparticipateinconsultationandpre-operativedecisionsregardingsurgery,serveasprimarysurgeonorfirstassistant,andorganizepost-operativemanagementwithinthehospitalandout-patientclinicswithinthedepart-ment.PGY-5residentsalsoserveasadministrativemanagersoftheirrespectiveservices.
TwoofourPGY-5residentsareselectedyearlybythefacultytofunctionasChiefResidents.Theyaregivenadvancedresponsibilitiesinteaching,organizationandadministration.ChiefResidentsattendfac-ultymeetingsandretreatsaswellasprovideinputfromtheresidents.Nonetheless,theChiefResidentsareexpectedtocontinuewiththeirregularinvolvementintheabovecurriculum.

RESIDENCY PROGRAM (PGY-4 & PGY-5)

LAC+USCMedicalCenterisoneofthelargestteachingcentersintheworld,withover1,400bedsinitstwohospitals(GeneralHospital,Women’sandChildren’sHealthHospital).ThisistheparentinstitutionoftheResidencyProgram.TheDepartmentofOrthopaedicsisorganizedbyspecialtyservices:trauma,recon-structivesurgery,sportsmedicine,handsurgery,orthopaedicinfections,footsurgery,spinesurgery,pediat-ricorthopaedics,musculoskeletaloncology,andpelvicfracturesandacetabularsurgery.
Thereareover3,733orthopaedicadmissionsandmorethan52,035adultorthopaedicout-patientseachyear.Thislargepatientpopulationaffordstheresidentsawiderangeofclinicalproblemstoevaluateandtreat,providessignificanthandsonexperienceinsurgeryandclinics,andoffersawealthofmaterialforclinicalresearch.OurbiomechanicslaboratoryislocatedontheUSCHealthScienceCampus,adjacenttothemedicalcenter.Residentsspendapproximatelyone-halfoftheirtrainingatthemedicalcenter,workingwiththefull-timeandvoluntaryclinicalfaculty.
USC University Hospital
TheUSCUniversityHospitalservesasheadquartersfortheprivatepracticein-patientactivitiesoftheUSCmedicalfaculty,andoffersasophisticatedlevelofdiagnosticandtreatmentservicesthatisbeyondthescopeofcaretypicallyfoundincommunityhospitals.The238-bedhospitalisbestdescribedasaspecialtyteachinghospital,whichhousestheInstituteforBoneandJointInfections,theCenterforArthritisandJointReplacement,theCenterforMusculoskeletalOncology,OrthopaedicSpineCenterandHandCenter.Ortho-paedicspecialtyareasincludetrauma,arthroscopicsurgery,foot/anklesurgery,arthritisandjointrecon-struction.PediatricpatientsareseenatChildrensHospital,LosAngeles.
rancho Los Amigos Medical Center
RanchoLosAmigosMedicalCenterisa245-bedfacility,with556adultorthopaedicadmissionsand7,600adultoutpatientvisitsannually.ResidentsonrotationsatRanchoLosAmigosMedicalCenterreceivetraininginadultreconstructionandrehabilitation.Residentrotationsincludearthritis,jointreplacement,spinalcordinjuryandthediabeticfootservice.Rehabilitationmanagementisincorporatedinallservices.RanchoLosAmigosHospitalhousesaworld-renownPathokinesiologyLaboratory,whichconductssophisti-catedstudiesdealingwithnumerousacquiredandtraumaticneuromusculardisorders.
Childrens Hospital Los Angeles
ChildrensHospitalLosAngeles(CHLA),oneoftheleadingpediatrichospitalsinthecountry,pro-videspediatricorthopaedicrotationsfortheresidents.AwidevarietyofpediatricorthopaedicconditionsaretreatedatCHLA,fromthesimpletothemostcomplex.Theonlylevel1PediatricTraumaCenterinLosAngelesislocatedhere.ChildrensHospitalprovidestheopportunityfortheresidentstogainexposuretoneuromuscular,congenital,developmental,andtraumaticconditionsinchildren.Anactivegaitlaboratory,forbothclinicalandresearchpurposes,islocatedinthehospital.
AffiLiAtEd HOSpitALS
Kaiser permanente Hospital, Baldwin park
TheKaiserPermanenteHospitalatBaldwinPark,CA,isacommunity-basedhospitalwith175in-patientbeds.TwoorthopaedicresidentsfromUniversityofSouthernCaliforniarotatethroughthisfacilityandareexposedtoadultreconstructiveprocedures.Theyworkwithsixattendingswhohavefacultyclini-calappointmentswiththeUSCDepartmentofOrthopaedicSurgery.Thereareapproximately1,500adultreconstructiveproceduresperformedannually.Approximately500casesarejointarthroplastycaseswithover95%beingprimarycases.Over1,000arthroscopicandsportsmedicinecasesareperformedannually.Thebestaspectofthisrotationisthatresidentsareexposedtogeneralorthopaediccasesseeninthecom-munity,andthattheyreceivea“hands-on”experience.
