SUMMARY

The Maritime and Coastguard Agency (MCA) exercises Central Government's statutory responsibilities for taking action when oil or hazardous and noxious substances emanating from any at-sea activity threatens the United Kingdom or its surrounding waters. As part of its commitment to provide cleaner seas around the United Kingdom, the MCA publishes, via ACOPS, an annual survey of reported discharges and releases attributed to vessels and offshore oil and gas installations operating in the United Kingdom’s Exclusive Economic Zone (UKEEZ). The annual survey results complement historical data and records and enable MCA's Counter Pollution and Response (CPR) team to monitor trends in the number of incidents, the amount of oil and chemicals spilt, the geographical distribution of spills, the sources of pollution and the nature of the pollution.

Following analysis of 1,749 incident reports and supporting information, 671 accidental discharges from vessels and releases from offshore oil and gas installations were identified in the survey area during 2015. An overall reduction of 10.3% was evident in the total number of incidents reported from all sources over the previous year’s total.

A total of 139 vessel-sourced discharges were reported during 2015 which was 7 fewer than the previous year’s total and significantly fewer than the mean annual total of 186 reported discharges between 2000 and 2014.

A total of 531 accidental releases were reported by offshore oil and gas installations operating in the open sea during 2015 representing an 11.6% decrease on the previous year’s total.

Further analysis of these statistics showed that the total of 337 oil releases from offshore oil and gas installations operating in the open sea during 2015 was 51 more than the mean annual total of 286 releases reported between 2000 and 2014. Estimated volumes of releases showed that 72% were less than 5 litres.

The total of 194 non-mineral oil releases by offshore installations during 2015, primarily those involving chemical substances, was 27 fewer than the total recorded during the previous year.

The reported incidents from all sources during 2015 comprised 70% mineral oils, 29% chemicals (including oil-based mud) and 1% other substances (including vegetable and animal oils).

Approximately 85 % of all accidental discharges and releases were reported in the open sea, 13% were in ports and harbours and 2% were across the remaining marine environmental zones. Survey respondents submitted details of a total of 87 incidents involving accidental discharges from vessels and oil rigs operating in ports and harbours

during 2015 which was 14 fewer compared with the mean annual total of 101 discharges between 2000 and 2014.

Three beach pollution incidents were reported during 2015 which was 4 fewer than the mean annual total of 7 incidents reported between 2000 and 2014. The most extensive incident occurred during October after varying quantities of solidified rancid palm oil residues were washed ashore along a 5-km stretch of shoreline at Hastings. Clearance operations were completed over a 14-day interval. No reports were received of any beach pollution incidents involving mineral oils during 2015.

Estimated volumes of accidental discharges and releases were recorded in 686 incident reports (including ongoing releases), or 94% of the total. The modal class for oil discharges and releases was less than 455 litres but larger estimated volumes of between 456 litres and approximately 25 tonnes were recorded in 5 of the 11 survey enumeration areas. In total, 4% of all oil discharges and releases (including ongoing releases) were greater than 455 litres, when this information was recorded, compared with 6% during the previous year.

The largest oil incident, with an estimated maximum volume of 25 tonnes, occurred after the general cargo vessel Lysblink Seaways (7,409 gt, built 2000) ran aground near Kilchoan in the western entrance to the Sound of Mull on 18 February.

The modal class for chemical incidents was also less than 455 litres but 35 releases, or 17% of the total (including 13 ongoing releases), were of 1 tonne or more. The largest chemical incident involving a release of 45,254.15 kg of Oceanic HW540 v2 was caused by a hydraulics open system SCM failure on the Ninian Central Platform on 18 March.

At least 28 confirmed discharges were attributed to vessel casualties comprising 9 fishing vessels, 7 pleasure craft and workboats, 3 barges, 2 general cargo vessels, 2 tugs, a bunker tanker, a car carrier, a dredger, a tall-ship and a research vessel.

Reporting organisations submitted details of 2 successful prosecutions brought by the Crown Office and Procurator Fiscal Service in Scotland for marine pollution offences. On 1 July 2015 Shell UK Limited pleaded guilty at the Aberdeen Sheriff Court to an unlicensed release of 13.886 tonnes of diesel oil at the Brent Bravo platform on 11 May 2013 and was fined £6,500. On 24 November 2015 Shell UK Limited was fined £22,500 following an unplanned release of 218 tonnes of crude oil and 403.502 kg Biocide 220 from the Gannet F pipeline bundle between 11 and 19 August 2011.

SECTION 1: INTRODUCTION AND SURVEY DESIGN

1.1 INTRODUCTION

1.1.1 Since 1965 the Advisory Committee on Protection of the Sea (ACOPS) has compiled statistics and other information on different types of marine pollution in the waters around the British Isles. The competent authorities and other stakeholder groups have used this information for a wide range of purposes including policy and planning decisions, evaluations of particular programmes, assessments of regulatory compliance levels, development of risk assessment models and to support educational campaigns. Examples of organisations that have consulted the survey data include the Department for Transport, European Commission, International Maritime Organisation, Local Authority Associations, Marine Management Organisation, the Scottish Government and the Welsh Assembly Government.

1.1.2 The Maritime and Coastguard Agency (MCA) exercises Central Government's statutory responsibilities for taking action when oil or other hazardous and noxious substances emanating from any at-sea activity threatens the United Kingdom or its surrounding waters. 1.2 SURVEY DESIGN

1.2.1 In accordance with the MCA's survey specification a two-page questionnaire was designed to capture information relating to discharges of oil and other hazardous and noxious substances from vessels and unpermitted oil and chemical releases from offshore oil and gas installations. The survey area included the UKEEZ and adjacent areas of jurisdiction including ports, harbours and tidal stretches of rivers and estuaries. The boundaries of the UKEEZ are shown in Figure 2. The survey questionnaire and accompanying guidelines are provided in Appendix 2.

1.2.2 The survey questionnaire was distributed to the appropriate representatives of coastal local authorities, port and harbour authorities, national environment agencies and animal welfare groups. In addition, questionnaire responses were supplemented with Petroleum Operation Notices (PON 1s) provided by the Department of Energy and Climate Change’s (DECC) Offshore Environment Unit (Offshore Petroleum Regulator for Environment and Decommissioning, Department for Business, Energy and Industrial Strategy from 14 July 2016) and Pollution Reports (POLREPS) provided by the MCA. PON 1s are submitted to DECC by offshore oil and gas

installations operators for all unpermitted oil or chemical releases to sea regardless of the volume lost. The resulting information was entered into a database identifying separate pollution incidents by date, location and other survey attributes.

1.2.3 Following analysis of data the survey results were presented in a range of different formats. In Section 2 descriptive statistics are provided for reported discharges and releases including some numerical trends with 2000 as the base year and also including categories and types of polluting substances, geographical locations and distributions of incidents, amount and extent of reported pollution, sources, details of prosecutions brought by the competent authorities and reporting effort.

1.2.4 In Section 3 summary reports are provided for all larger discharges and releases of 2 tonnes or more. Further information is provided in Section 4 for each of 11 geographical areas around the United Kingdom including the causes of particular incidents and counter-pollution responses where appropriate.

1.2.5 Elements of the survey quality assurance programme are outlined in Section 5 including the number of incident reports compiled from information submitted by two or more reporting organizations. Moreover, the current database is provided in Appendix 1 as part of the quality assurance programme in order to ensure the reliability and accuracy of the survey results. This is further enhanced by posting the survey report on the MCA’s website as a publicly available document for scrutiny by all interested stakeholder groups.

SECTION 2: DESCRIPTIVE STATISTICS AND TREND ASSESSMENTS

2.1 NUMERICAL TRENDS IN REPORTED DISCHARGES

2.1.1 Following analysis of 1,749 incident reports and supporting information, 671 separate discharges from vessels and releases from offshore oil and gas installations were identified within the survey area during 2015. An overall reduction of 10.3% was evident in the total number of incidents reported over the previous year’s total.

2.1.2 Trends in annual totals for reported discharges between 2000 and 2015 are displayed in Figure 1, including separate line graphs for vessels and offshore oil and gas installations.

2.1.3 A total of 139 vessel-sourced discharges were reported during 2015 which was 7 less than the previous year’s total and significantly fewer than the mean annual total of 186 reported discharges between 2000 and 2014.

2.2 CATEGORIES OF REPORTED DISCHARGES

2.2.1 In Table 1 statistics are provided showing the different categories and numbers of discharges and releases reported during 2014 and 2015 for each of 11 enumeration areas. Geographical boundaries of each enumeration area and the complete survey area are shown in Figure 2. The statistics for discharges and releases in the United Kingdom Continental Shelf (UKCS) Oil and Gas enumeration area have been presented in two source categories: offshore oil and gas installations, and vessels.

2.2.2 The discharges and releases from all sources reported during 2015 comprised 70% mineral oils, 29% chemicals (including oil-based mud) and 1% other substances (including vegetable and animal oils).

2.3 LOCATIONS OF REPORTED DISCHARGES

2.3.1 The geographical location of each reported discharge or release has been identified in Appendix 1 together with other information for each incident. To facilitate comparisons with previous years’ data, reported locations of all vessel-source oil discharges in the open sea, nearshore waters and estuaries during 2015 are plotted in Figure 3. Data for ports have been grouped separately for England, Wales, Scotland and Northern Ireland.

2.3.2 The overall geographical pattern for vessel-source mineral oil discharges in the survey area during 2015 showed more frequent reports of incidents across 6 enumeration areas, with the highest density of reported incidents in the open sea occurring in the vicinity of North Sea oil and gas installations.

2.3.3 The geographical locations of all discharges and releases are further analysed by their distribution across five marine environmental zones in each enumeration area: open sea; tidal river or estuary; bay or nearshore waters; beach or shore; and port or harbour. The results are presented in Table 2, which shows that 85% of all discharges and releases were reported in the open sea, 13% in ports or harbours and 2% across the remaining zones.

2.3.4 Reporting organisations submitted details of 87 incidents involving discharges of polluting substances from vessels and oil rigs located in ports or harbours during 2014, which was 14 fewer than the mean annual total of 101 discharges between 2000 and 2014. The largest port incident occurred at the Inchgreen Dry Dock in Greenock on 15 March following a 14-tonne marine diesel spill from the damaged general cargo vessel Lysblink Seaways (7,409 gt, built 2000) (Refer 4.8.2).

2.3.5 Three beach pollution incidents were reported during 2015 which was 4

fewer than the mean annual total of 7 incidents reported between 2000 and 2014. No reports were received of any beach pollution incidents involving mineral oils during 2015. Further details are provided in Section 4.

2.4 AMOUNT AND EXTENT OF REPORTED POLLUTION

2.4.1 Information describing observed dimensions and volumes of pollution for each enumeration area is presented in Table 3. It should be noted that the extent of pollution for incidents occurring within the UKCS enumeration area 11 was determined by the observed approximate release area of water surface pollution in 2 categories: up to 1.6 km2; or above 1.6 km2. Estimates for volumes of discharges and releases were not confirmed in written reports for 39 incidents, or 6% of the total reported (including ongoing releases).

2.4.2 Excluding releases from offshore oil and gas installations and vessels operating in the UKCS area 11, less than 1% of oil slicks or sheens were greater than 1.6 km in length when this information was reported. Broadly comparable statistics from the 2000, 2005 and 2010 surveys were 12%, 7% and 10% respectively. Within the UKCS enumeration area 11 a release area of water surface pollution by oil greater than 1.6 km2 was reported on a

single occasion and this was attributed to an unidentified vessel (Refer 4.11.11).

2.4.3 The modal class for all oil incidents was less than 455 litres but larger estimated maximum volumes of between 456 litres and 25 tonnes were recorded in 5 of the 11 survey enumeration areas. In total, 4% of all oil discharges and releases (including 3 ongoing releases) were greater than 455 litres where this information was recorded, compared with 6% during the previous year (Refer to Table 3 and Appendix 1 Table 11).

2.4.4 The largest oil spill, with an estimated maximum volume of 25 tonnes, occurred after the general cargo vessel Lysblink Seaways (7,409 gt, built 2000) ran aground near Kilchoan in the western entrance to the Sound of Mull on 18 February (Refer 3.3 WS/366).

2.4.5 The modal class for chemical incidents was also less than 455 litres but 35 releases, or 17% of the total (including 13 ongoing releases), were of 1 tonne or more. The largest chemical incident involving a release of 45, 254.15 kg of Oceanic HW540 v2 was caused by a hydraulics open system SCM failure on the Ninian Central Platform on 18 March (Refer 3.1 UKCS/1279).

2.4.6 In Section 3 details are provided for all reports involving releases or discharges of 2 tonnes or more during 2015 including 5 ongoing incidents that commenced during previous years.

2.5 CATEGORIES AND TYPES OF POLLUTION

2.5.1 Statistics showing the different categories and types of polluting substances are presented in Table 4. Mineral oils were again the most frequently recorded category of polluting substances and were identified in 70% of incident reports. Crude oils accounted for 15% of the identified types of mineral oil discharges. The largest crude oil incident, with an estimated maximum volume of 2 tonnes, was reported by the MacCulloch FPSO on 14 April. The incident was caused by human error following the incorrect routing of fluids. ConocoPhillips carried out a full review of operational activities and procedures to prevent reoccurrence (Refer 3.20 UKCS/1316).

2.5.2 Bunker, diesel, fuel and gas oil incidents were attributed to vessels and oil and gas installations on 137 occasions, 16 fewer than the previous year’s total, across all enumeration areas. The discharges and releases were reported most frequently in the UKCS, Irish Sea and south-west England enumeration areas.

2.5.3 Lubrication and hydraulic mineral oil incidents were recorded in 10 of the 11 survey enumeration areas with 169 releases attributed to offshore oil and gas installations. Of these, 53 hydraulic releases in quantities of up to 15 litres were identified by DECC as Known Third Party reports and attributed to remotely-operated vehicles (ROVS) and vessels.

2.5.4 Respondents described the remaining types of mineral oil discharges or releases as bilge oils, oily scum, oily water, sludge and various waste oils.

2.5.5 A total of 201 reports referred to incidents involving substances other than mineral oils during 2015 compared with 235 reports during 2014. Offshore operators reported 193 unpermitted chemical releases and a single vegetable oil release during 2015 compared with 217 chemical releases and 4 vegetable oil releases during the previous year. The identified substances were associated with exploration and production operations and included ammonium bisulphite, barite, calcium bromide brine, calcium carbonate, calcium chloride brine, caustic soda, glycol solutions, hydraulic fluids, methanol, potassium chloride brine, soda ash and sodium solutions. The estimated volumes of unpermitted chemical releases ranged from 0.0002 kg to 45,254.15 kg and 35 releases (including ongoing releases) were 1 tonne or more. Further details are provided in Section 3 and Appendix 1.

2.5.6 Various chemicals need to be used as part of offshore exploration and production operations. The vast majority of chemicals spilled are regulated under the provisions of the Offshore Chemicals Regulations 2002 (OCR) for normal operational use and discharge to sea. Under the provisions of OCR each operational chemical is assessed to determine its potential environmental impact. Of the total chemicals spilled in 2015 approximately 90% were considered under the Commission for the Protection of the Marine Environment of the North East Atlantic (OSPAR) list of substances used and discharged offshore as Posing Little or No Risk to the Environment (PLONOR). None of the chemicals spilled was included as a substance listed in the OSPAR list of chemicals for priority action. None of the spill incidents gave rise to a significant environmental impact.

2.5.7 A total of 7 non-mineral oil incidents were attributed to identified or suspected vessel discharges in ports or in the open sea. Palm oil residues were washed ashore on 3 occasions in southern England and south-west England during 2015. The most extensive incident occurred during October after varying quantities of solidified rancid palm oil residues were washed ashore along a 5-km stretch of shoreline at Hastings. Clearance operations were completed over a 14-day interval. Lewes District Council submitted details of another palm oil incident after clumps of the waxy material were washed ashore along a stretch of Seaford beach during November and December (Refer 4.4.6).

2.5.8 Respondents also submitted details of 4 incidents involving discharges of fish oil, an undentified chemical, soot and paint. An inert gas boiler failure onboard the crude oil tanker Botofogo (58,418 gt, built 2010) berthed at Esso Fawley resulted in a carbon soot deposit along a 400-metre stretch of shoreline in the vicinity of the Calshot Activity Centre. The Centre suspended all waterborne activities until a clean-up operation had been completed by an Esso pollution response team. A gust of wind blew a near empty pot of yellow paint off the bulwark of the supply vessel FS Taurus 2,610 gt, built 1991) and into Aberdeen Harbour on 2 March. The oil based paint later dispersed naturally. In the open sea CleanSeaNet radar satellite images identified a discharge from the chemical tanker Hanne Theresa (2,682 gt, built 2002) operating off the Humber Estuary on 11 August. Subsequent communications with the vessel determined a permissible discharge of tank washings.

2.6 REPORTED SOURCES OF POLLUTION

2.6.1 The figures in Table 4 show for each enumeration area the numbers of incidents in which reporting organisations either identified a specific source of pollution or suspected a vessel or offshore pollution source. The statistics for the identified sources of pollution are further analysed in Table 5 to show the different types of vessels. Additional details including the names of vessels or installations are provided in Appendix 1.

2.6.2 This method of analysis showed that releases from oil and gas installations operating on the UKCS accounted for 83.9% of all identified reports of pollution compared with 87.1% during the previous year. Corresponding statistics for fishing vessels, pleasure craft, general cargo vessels and the remaining types of vessels were 3.8%, 3.2%, 0.8% and 8.2% respectively.

2.6.3 Reporting organisations identified discharges from tankers on 8 occasions during 2015 which was the same number of incidents reported during the previous year. Port and harbour authorities reported tanker incidents at Falmouth, Finnart, Peterhead, Sullom Voe and the Tetney Monobuoy in the Humber Estuary. The largest port incident occurred at Falmouth Docks on 2 June after a split discharge hose caused a 4-tonne heavy fuel oil spill from the oil products tanker Wisby Verity (4,295 gt, built 2004). The oil was contained by booms and recovered by skimmers and absorbents to a reception barge alongside (Refer 4.5.3). The shuttle tanker Loch Rannoch (75,526 gt, built 1998) reported a one-tonne crude oil spill after moorings parted during a discharging operation at the Tetney Monobuoy on 24 January. The resulting sheen dispersed naturally and no further actions were required (Refer 4.1.2). The oil tanker Ashna A (156,417gt, built 1999) reported a 40-litre crude oil discharge at the Finnart Oil Terminal in Loch Long on 25 June.

2.6.4 Respondents also submitted details of 3 discharges from warships operating in the open sea and in ports and harbours. The Duke-class Type 23 Frigate HMS St Albans (built 2000) reported a 2-litre aviation fuel spill whilst alongside the Fountain Lake Jetty at Portsmouth on 2 April. A clean-up operation was completed at HMNB Devonport following a fuel oil discharge from the Duke-class Type 23 Frigate HMS Sutherland (built 1996) on 4 April. In the open sea the Royal Norwegian Navy Frigate Thor Heyerdahl C (built 2009) reported a 100-litre gas oil spill off Tiree on 13 April (Refer 4.8.6).

2.6.5 At least 28 confirmed discharges were attributed to vessel casualties including 9 fishing vessels, 7 pleasure craft and workboats, 3 barges, 2 general cargo vessels, 2 tugs, a bunker tanker, a car carrier, a dredger, a tall ship and a research vessel. Five vessel-casualty spills occurred in nearshore waters, 18 in ports and the remainder in the open sea. Moreover, vessel casualties accounted for 37% of all discharges attributed to fishing vessels and likewise 29% of all discharges attributed to pleasure craft and workboats. The corresponding statistics from the 2014 survey were 50% and 72% respectively. Further details identifying the likely causes of vessel casualties, where known, are provided in Section 4 and Appendix 1.

2.6.6 Twelve vessel-casualty spills were reported in England during 2015. On 1 November the converted fishing vessel Jolly Roger (built 1971) sank in the Floating Dock at Penzance causing a spillage of 150 litres of diesel oil and 50 litres of lubrication oil. Clean-up and salvage operations were completed over a 14-day period and the costs were later charged to the owner of the vessel (Refer 4.5.6). The hopper barge Jan Leeghwater (3,315 gt, built 2010) accidentally discharged 200 litres of light fuel into the River Mersey following a collision with a tug on 17 June. A workboat ran aground and broke up at New Brighton causing a 180-litre diesel oil spillage. The vehicle carrier G Poseidon (72,408 gt, built 2011) was identified as the source of a 50-litre hydraulic oil spill which was caused by damaged machinery and hull plating (Refer 4.4.4).

2.6.7 Two vessel-casualty spills were reported in Wales during 2015. The 14-metre crabbing vessel CSK ran aground in West Angle Bay on 28 October causing a 10-litre diesel spill. As a consequence of extensive hull damage the vessel was later winched onto a nearby beach and dismantled for disposal ashore (Refer 4.6.5). A Natural Resources Wales report referred to a 50-litre diesel spill after the derelict coastal sailing barge Eben Haezer sank in Cardiff’s Bute East Dock on 23 November.

2.6.8 Reporting organisations in Northern Ireland identified 3 vessel-casualty spills during 2015. A diesel sheen was observed after the 17-metre wooden twin-rigged stern trawler Silver Dee (63 gt, built 1971) sank following a collision with another fishing vessel off County Down on 29 July. An absorbent boom was deployed after the 9-metre fishing vessel John Boy grounded and tipped over in Strangford Lough on 16 October. The 17-metre research vessel Ostrea sank alongside a pontoon in Loch Foyle causing a small fuel oil spillage.

2.6.9 Scottish reporting organisations identified 11 oil discharges caused by vessel casualties during 2015. The cement carrier Cemfjord (1,850 gt, built 1984) capsized and sank in heavy seas off the Pentland Firth on 3 January. A slight sheen was later reported which was dispersed by wave action. The general cargo vessel Lysblink Seaways (7,409 gt, built 2000) ran aground near Kilchoan on 18 February causing a 25-tonne marine gas oil spill, while in a second incident on 15 March approximately 14 tonnes of marine diesel were discharged into a dry dock from the vessel’s ruptured and damaged fuel tanks (Refer 4.8.2).

2.6.10 A Tier 2 contractor was engaged to assist in a counter pollution operation in Peterhead Harbour following a gas oil spill from the damaged single-hulled bunker tanker Erin Wood (70gt). The tanker had earlier collided with a general cargo vessel off Peterhead on 29 August sustaining damage to the port side main cargo tank and wing tank. A boom and absorbent mats were deployed around the vessel whilst the remaining oils were pumped ashore to road tankers (Refer 4.10.3). The 18-metre scallop dredger St Apollo (51gt, built 1979) ran aground on rocks and capsized near Inninmore Bay to the eastern end of the Sound of Mull on 24 August. A slight surface sheen was identified which was later broken up and dispersed

by wave action. The vessel was declared a constructive total loss and hence all potential pollutants and contaminants, including 10.5 tonnes of marine diesel oil, were removed from the wreck. A small patch of diesel oil was reported after the 16-metre wooden-hulled trawler Karinya (built 1982) caught fire and sank north of Rattray Head on 4 October.

2.7 PROSECUTIONS

2.7.1 Details of 2 successful prosecutions brought by the Crown Office and Procurator Fiscal Service in Scotland for marine pollution offences are summarised in Table 6.

2.7.2 On 10 August 2011 a quantity of oil extending for 4.5 kms was spotted on the sea surface in the vicinity of Shell’s Gannet F field, 180 kms from Aberdeen. The oil release was subsequently observed to be emanating from a subsea relief valve attached to the Gannet F pipeline bundle in the vicinity of the Gannet F towhead protection structure. The oil release was secured on 19 August 2011 when the subsea relief valves were manually isolated and closed by divers. Shell subsequently estimated that a total of 219 tonnes of crude oil and 403.502 kg of Biocide 220 had been released to sea during the incident.

2.7.3 On 24 November 2015 Shell UK Limited admitted breaching the Regulation 3A of the Offshore Petroleum Activities (Oil Pollution Prevention and Control) Regulations 2005 and Regulation 13 of the Pipelines Safety Regulations 1996 and was fined a total of £22,500 at Aberdeen Sheriff Court. The Health and Safety Executive (HSE) and DECC carried out a joint investigation into the incident which concluded that the 8-inch Gannet F oil production pipeline within the pipeline bundle had failed. As a result, crude oil entered the insulated area between the production pipeline and its sleeve and was ultimately released to sea through the relief valves. It concluded there was no significant environmental impact or any risk that anyone would be harmed.

2.7.4 In another case heard at the Aberdeen Sheriff Court on 1 July 2015 Shell UK Limited pleaded guilty to an unlicensed release of 13.886 tonnes of diesel oil at the Brent Bravo platform on 11 May 2013 and was fined £6,500. The incident occurred during a bunkering operation between a supply vessel and the platform. During a subsequent investigation DECC inspectors found 2 damaged sections of hose which had not been fully inspected before starting operations due to the way the bunkering station was set up.

2.7.5 In addition Total E&P Limited was fined £1.25 million after pleading guilty to breaching Regulation 13(1)(a) and (b) of the Offshore Installations and Wells (Design and Construction, etc) Regulations 1996 and Section 33 (1)(c) of the Health and Safety at Work Act 1974 which caused uncontrolled releases of condensate, gas and mud from the Elgin platform between March and May 2012 (Refer Table 11).

2.8 REPORTING EFFORT

2.8.1 The number of incident reports received from each of 11 reporting organisations is shown in Table 7. The total number of reports received and processed was 1,749 from which 671 separate incidents were subsequently identified and entered into the 2015 database.

2.8.2 Observations from air surveillance patrols and a satellite surveillance programme were again employed to help identify discharges from vessels and releases from offshore oil and gas installations operating in the UKEEZ (Refer Section 4).

SECTION 3: SUMMARY OF LARGER INCIDENTS (TWO TONNES OR MORE) DURING 2015

(Refer to Section 4 and Appendix 2 for further details of each incident)

The figures given below are indicative of the reported volumes of releases confirmed in written reports. However, incidents in which significant amounts of beach material were mixed with pollutants, or where the volumes released were not indicated or measured, are not included. The incidents are ordered according to the volume of the discharge or release.

In all cases where the source of the release was from subsea control equipment the fluids lost were associated with oil and gas exploration and production operational processes. The operator continued the release to maintain the integrity and functionality of the wells/equipment and dive support vessels could be resourced to complete the necessary repairs.

Each release required the operator to assess the potential environmental impacts associated with the release. The operator in each case was required to provide regular updates on their management plan as part of investigations being conducted by DECC (BEIS from 14 July 2016). Where necessary, meetings with the operator took place to discuss aspects associated with the incident, environmental impact and the operator’s short- and long-term plans to stop the release and, where relevant, effect a final repair.

3.1 UKCS 18 March: CNR International U.K. Limited Ninian Central Platform (11-UKCS/1279)CNR International U.K. Limited reported an incident resulting in the release of 45,254.15 kg of Oceanic HW540 v2 due to hydraulics open system SCM failure. DSV IRM work at Strathspey was completed. Suspect hydraulic accumulators at the Strathspey manifold were isolated and the HP hydraulic rails A and B pressure tested successfully. The usage was monitored for 24 hours and found stable, and the incident closed.

3.2 UKCS 31 August: BP Exploration Operating Company Limited Schiehallion Subsea Clusters (11-UKCS/1518)BP Exploration Operating Company Limited reported an incident resulting in the combined release of 30,340.78 kg of monoethylene glycol, RX-5248 and RX-9022 due to a release during a leak testing program, designed to identify any leakage within the system. The chemical was permitted to be discharged to sea should the leak test have been successful.

3.3 Western Scotland 18 February: MV Lysblink Seaways (8-WS/366)The general cargo vessel Lysblink Seaways (7,409 gt, built 2000) ran aground on rocks near Kilchoan in the western entrance to the Sound of Mull on 18 February. Some of the vessel’s fuel tanks were breached resulting in 25 tonnes of marine gas oil entering the water. Booms were deployed around the vessel and a salvage operation was later completed.

3.4 UKCS 18 January: Chevron North Sea Limited Captain WPP’A’ (11-UKCS/1213)Chevron North Sea Limited reported an incident resulting in the combined release of 24,512 kg of sodium chloride, caustic soda, soda ash, BARAZAN, DEXTRID E, barite, CFS-476, STARCIDE and OXYGON due to a small subsea release from the well. An investigation was opened into the release of water based mud to the environment during planned pressure testing of a well. The release was a result of casing failure due to corrosion. The investigation was closed following Chevron North Sea Limited’s investigation of the casing corrosion potential of Captain wells and their development of a comprehensive strategy to manage corrosion.

3.5 UKCS 20 August: CNR International (U.K.) Limited Strathspey Manifold (11-UKCS/1499)CNR International (U.K.) Limited reported an incident resulting in the combined release of 20,290.89 kg of Oceanic HW540 v2 and Oceanic HW540 E due to hydraulics open system fittings/connections. All external leaks associated with M2z and M9 have been rectified and the situation is no longer ongoing.

3.6 UKCS 11 June: Total E&P UK PLC Dunbar Platform (11-UKCS/1386)The Dunbar Platform reported an incident resulting in the combined release of 18,974.95 kg of TETRACIDE D, OXBAN HB, STARCIDE, OXYGON, monoethylene glycol (all dilutions), calcium chloride brine and CORSAF CR-I due to water based chemicals discharge via platform drains whilst flushing of the trip tank was taking place. Total E&P undertook a review and revision or relevant risk assessments and procedures and delivered additional crew training. An investigation was carried out into relation to this incident, which resulted in an Enforcement Letter being issued.

3.7 UKCS 7 March: Shell U.K. Limited Pierce FPSO (Haewene Brim) (11-UKCS/1261)Shell U.K. Limited reported an incident resulting in the release of 16,240 kg of Transaqua HT2 due to hydraulics open system fittings/connections. Leak from the SSSV associated with the Pierce FPSO (Haewene Brim) installation. The SSSV Control Line was repaired, successfully stopping the release in August 2015. An investigation was carried out into relation to this incident, which resulted in an Enforcement Letter being issued.

3.8 Western Scotland 15 March: MV Lysblink Seaways (8-WS/375) Approximately 14 tonnes of marine diesel oil were accidentally discharged into the Inchgreen Dry Dock at Greenock from the damaged general cargo vessel Lysblink Seaways (7,409 gt, built 2000). The oil spill had been anticipated and equipment and resources had been deployed in advance by contractors to capture and remove the pollution.

Chevron North Sea Limited reported an incident resulting in the release of 13,770 kg of potassium chloride brine due to a pinhole leak in pipework. Cause of release was lack of ownership to track and manage integrity anomalies raised during inspection of drilling rig pipework. Investigation closed following repair and with Chevron North Sea Ltd confirming that drilling rig pipework is now included in the asset integrity management system.

3.10 UKCS 7 August: Apache Beryl I Limited Beryl Alpha (11-UKCS/1475)Apache Beryl I Limited reported an incident resulting in the release of 9,369 kg of Oceanic HW 443 due to hydraulics open system SCM failure. The Subsea Control Module (SCM) had been in place for a number of years and by the nature of its location cannot be readily inspected. Oceanic HW 443 has a low environmental hazard potential. Upon identification of the release Apache expedited the replacement of the SCM. No further action was required.

3.11 UKCS 27 April: Maersk Oil UK Donan FPSO Global Producer III (11-UKCS/1338)Maersk Oil UK reported an incident resulting in the release of 8,780 kg of Transaqua HT2 due to hydraulics open system fittings/connections. Maersk Oil had a release of Transaqua HT2 from a subsea infrastructure. The operator has reviewed and improved hydraulic use in order to facilitate early identification of abnormal fluid use. The release was reported to DECC and was repaired following a DSV intervention.

3.12 UKCS 10 June: Perenco UK Limited Leman AP Platform (11-UKCS/1384)Perenco UK Limited reported an incident resulting in the release of 6,791 kg of MEG/water mixes and CRW85689 due to a release from a pipework joint. The release occurred due to injected chemical pooling in a deadleg being released on the removal of a spade during a deisolation. Perenco were required to produce and implement an improvement plan to prevent future such occurrences. Confirmation of full implementation of the plan was confirmed during a BEIS inspection in August 2016.

3.13 UKCS 3 June: BG plc Armada Platform (11-UKCS/1380)BG plc reported an incident resulting in the release of 5,509 kg of Transaqua HT2N due to hydraulics open systems fittings/connections. This was an ongoing subsea release of PLONOR fluid which only occurs when the well is open flowing. Cessation of production for the well is imminent at which stage well will be closed in, stopping the release.

3.14 UKCS 29 March: Nexen Petroleum UK Limited Golden Eagle W Platform (11-UKCS/1296)Nexen Petroleum UK Limited reported an incident resulting in the release of 5,301.71 kg of methanol (all grades) due to a storage tank open drain line. Loss of full methanol inventory to sea due to inadvertent opening of safety valve. An investigation was carried out into relation to this incident, which resulted in an Enforcement Letter being issued.

3.15 South-West England 2 June: MT Wisby Verity (5-SW 203)A split hose from the oil products tanker Wisby Verity (4,295 gt, built 2004) was identified as the cause of a 4-tonne heavy fuel oil spill at Falmouth Docks eastern wharf on 2 June. The oil was contained by boom and recovered by skimmers and absorbents to a reception barge alongside.

3.16 UKCS 14 March: Fairfield Betula Limited Dunlin A Platform (11-UKCS/1271)Fairfield Betula Limited reported an incident resulting in the release of 2,798 kg of calcium chloride brine due to a sheared bunkering hose. An investigation was opened by the Department which concluded the root cause of the failure appeared to be off-centre tensile loading of the hose, through the actions of wind and waves. The company carried out a number of actions and changes in procedures to prevent reoccurrence. The environmental impact from the release was considered to be negligible.

3.17 UKCS 16 January: Prospector Offshore Drilling Prospector 5 (11-UKCS/1212)Prospector Offshore Drilling reported an incident resulting in the release of 2.52 tonnes of hydraulic fluid due to a missing capping plug on a flange on derrick pipework. It is assumed this was due to vibration. All capped flanges were swapped out to prevent future occurrence and the alarms on the hydraulic fluid supply tank were changed to ensure earlier identification of a release.

3.18 UKCS 17 June: Oranje-Nassau Energie UK Limited Sean RD Platform (11-UKCS/1401)Oranje-Nassau Energie UK Limited reported an incident resulting in the release of 2.2 tonnes of rapeseed oil due to annular communication failure. The release point was subsequently engineered out. Environmental damage was negligible.

3.19 UKCS 18 September: Chevron North Sea Limited Captain WPP’A’ (11-UKCS/1550)Chevron North Sea Limited reported an incident resulting in the release of 2,041 kg of Oceanic HW540 E from a Captain subsea control module due to hydraulics open system SCM failure. Release monitored and investigated by DECC. The release ceased following work during the 2016 annual shutdown.

3.20 UKCS 14 April: ConocoPhillips (U.K.) Limited MacCulloch FPSO (11-UKCS/1316)ConocoPhillips (U.K.) Limited reported an incident resulting in the release of 2 tonnes of crude oil due to the incorrect routing of fluids. An investigation was opened by the Department and a team was deployed offshore and reviewed the incident, subsequently an enforcement letter was issued. A number of operational and procedural factors were identified, the company subsequently carried out a full review of the operational activities and procedures to prevent reoccurrence. The installation ceased production shortly thereafter and is off location for decommissioning.

UKCS INCIDENTS OCCURRING IN A PREVIOUS CALENDAR YEAR BUT WITH QUANTITIES RELEASED DURING 2015

The following reported UKCS incidents occurred during a previous calendar year, but still continued over a period of time during 2015.

3.21 UKCS 1 January: Premier Oil UK Limited Balmoral FPV (ongoing incident from 11 November 2014 UKCS/1687)Premier Oil UK Limited reported an ongoing loss of 28,898 kg between 1 January and 31 December 2015, of Pelagic 100 Production Control Fluid due to all subsea hydraulic system failures. The source of the hydraulic fluid release was identified and corrective action taken to prevent further loss. The operator undertook a full review of hydraulic fluid tracking and management processes and introduced additional controls to allow for the early identification of system anomalies and minimise the amount of hydraulic fluids released to sea.

3.22 UKCS 1 January: CNR International Ninian Central (ongoing incident from 3 May 2014 UKCS/1350)CNR International reported an ongoing loss of 15,022.16 kg between 1 January and 31 December 2015, of Oceanic HW540 v2 due to a leak of subsea hydraulic control fluid from the system controlling pipeline SSIVs from the Ninian Central installation. CNRI were continuing to carry out investigations via subsea intervention vessels divers and ROVs to identify source during 2015.

3.23 UKCS 3 May: Marathon Oil UK LLC Brae Central Template (ongoing incident from 23 December 2014 UKCS/1737)Marathon Oil UK LLC reported an ongoing loss of 5,424 kg of HW443 due to hydraulics open system SCM failure which resulted in the loss of hydraulic fluid to sea due to incorrect alignment of a valve. Marathon increased monitoring of hydraulic fluid usage to allow early indication of releases from the system. An investigation was carried out into relation to this incident, which resulted in an Enforcement Letter being issued.

3.24 UKCS 1 January: EnQuest Heather Limited Don SW and West Don Northern Producer (ongoing incident from 14 March 2013 UKCS/1285)EnQuest Heather Limited reported an ongoing loss of 4,130.17 kg between 1 January and 31 December 2015, of Castrol Transaqua from a leaking fitting on the West Don PO3 wing valve. The Department investigated the circumstances of this release and several attempts were required to resolve the issues. The predominant release was an issue with an actuator valve which required a bespoke engineering fix. This was further exasperated by availability of an appropriate dive support vessel (DSV) during a period of benign environmental conditions to allow a safe operation. An environmental assessment of the release demonstrated a negligible environmental impact. The company reported a full repair was successful to the Department during May 2015.

3.25 UKCS 1 January: EnQuest Heather Limited Don SW and West Don Northern Producer (ongoing incident from 8 December 2011 UKCS/1669)EnQuest Heather Limited reported an ongoing loss of 3,565.88 kg between 1 January and 31 December 2015, of Gyptron SA3460 from a leaking subsea distribution unit. An investigation was carried out by the Department, the company were required to locate and rectify the sources of the release. Operationally this was a complex task due to the requirement for diver intervention and challenging environmental conditions, which included several attempts with a dive support vessel (DSV) to intervene. The company continued to liaise with the Department until all releases had ceased and provided an environmental assessment which demonstrated a negligible environmental impact.

4.1 North-east England (Northumberland, Tyne & Wear, Durham, Yorkshire and The Humber)

4.1.1 Ten vessel-source pollution incidents were identified by reporting

organisations during 2015 compared with 11 incidents during 2014. Seven discharges occurred in local ports and harbours and three discharges were recorded in the open sea. A source of pollution was identified on 5 occasions, the sources comprising a chemical tanker, an oil tanker, a fishing vessel, a tug and a dredger. Releases from offshore oil and gas installations operating off north-east England were included in the statistics for the UKCS (Refer to 4.11).

4.1.2 Three incidents were reported in the River Humber area during 2015. A POLREP referred to a one-tonne crude oil spill from the shuttle tanker Loch Rannoch (75,526 gt, built 1998) at the Tetney Monobuoy on 24 January. The incident occurred after moorings parted during a cargo discharging operation. The resulting sheen dispersed naturally and no further actions were required by tugs located in close proximity to the tanker. A leaking windlass was identified as the cause and source of a 30-litre hydraulic oil spill from the tug Svitzer Trimley (371 gt, built 1991) off Spurn Point on 6 February. The oil dispersed naturally and no further actions were required. A POLREP and an Environment Agency (EA) report referred to a diesel spill from an unidentified vessel at Immingham Dock on 7 September. Booms were deployed in a clean-up operation.

4.1.3 Two pollution incidents were described in reports submitted by PD Teesport. An unidentified vessel was believed to have been the source of a 4-litre diesel oil spill at Hartlepool on 26 January. A clean-up operation was completed following a 6-litre hydraulic oil discharge from the trailing suction hopper dredger Cleveland County (1,265 gt, built 1973) on 30 September.

4.1.4 The 10-metre fishing boat Tom Boffin sank in the mouth of the River Tweed at Berwick on 5 May and a light diesel sheen was later observed extending 150 metres downstream from the vessel. The sheen dispersed naturally and the vessel was successfully re-floated on the following day. An unidentified vessel was believed to have been the source of a diesel oil discharge in Warkworth Harbour on 3 November.

4.1.5 Three incidents were reported in the open sea during 2015. European Maritime Safety Agency (EMSA) CleanSeaNet radar satellite images identified a discharge from the chemical tanker Hanne Theresa (2,682 gt, built 2002) operating off the Humber Estuary on 11 August. Subsequent communications with the vessel determined a permissible discharge of tank washings. Two small surface sheens attributed to unidentified vessels were reported by offshore installations on 15 July and 3 November.

4.1.6 In addition to the pollution incidents above POLREPS also referred to a lost cargo incident. During rough weather 4 iso-tank containers and a box container were washed overboard from the containership Vohburg (7,852 gt, built 2005) off Teesport on 31 January. One iso-tank container was filled

with 26,800 kg of dicyclopentadiene (UN 2048) and was later recovered off Spurn Head on 6 February. A second lost iso-tank container was retrieved in the nets of a fishing vessel operating off Whitby on 2 October.

4.2 Eastern England (Lincolnshire, Norfolk & Suffolk)

4.2.1 Six incidents were recorded during 2015, two more than the total recorded during the previous year. Accidental releases from offshore oil and gas installations operating off eastern England were included in the statistics for the UKCS (Refer to 4.11).

4.2.2 Three incidents were reported by the Broads Authority during 2015. A clean-up operation was completed following a 2-litre diesel oil spill from a motor cruiser in Oulton Broad on 16 January. A residential boat sank on its moorings alongside the ABC wharf at Norwich on 9 July causing a diesel sheen which was broken up and dispersed by propeller wash. A boom was deployed in a clean-up operation following a diesel oil discharge from an unidentified pleasure craft at the same location on 15 August.

4.2.3 An unidentified vessel was believed to have been the source of 5-litre diesel oil discharge at the port of Great Yarmouth on 11 August. The pollution dispersed naturally on an ebb tide. An EA report referred to an incident at the port of King’s Lynn following a 5-litre diesel oil discharge from an unidentified fishing vessel.

4.2.4 In the open sea a POLREP described an incident at the Sheringham Wind Farm following a 10-litre hydraulic oil spill from a remotely operated underwater vehicle (ROV).

4.3 Essex and Kent

4.3.1 Five vessel-source pollution incidents were identified by reporting organisations during 2015 compared with 9 incidents during 2014. Four discharges occurred in local ports and harbours and a single discharge in the open sea. The source of pollution was identified on 4 occasions comprising 2 pleasure craft, a tug and a cruise ship.

4.3.2 Port and harbour authorities reported vessel discharges at Dover, Medway and Ramsgate. The dawn-class cruise ship Norwegian Star (91,740 gt, built 2001) was identified as the source of a 2-litre oily water sludge spill at the Port of Dover on 6 October. The incident occurred after a hose between the ship and a road tanker was disconnected following a discharge operation. A clean-up operation was completed by the vessel’s crew.

4.3.3 Two incidents were reported by Peel Ports Medway during 2015. The motor cruiser Khamsin sank whilst berthed at Rochester Pier on 10 March causing a 5-litre diesel spill. The privately owned 20-metre tug Pelikaan (built 1957) sank on her moorings at Cuxton on 24 July. A counter-pollution operation was completed which included the removal of all oil from the vessel which was later salvaged and removed to a local boat yard for repairs. An

accidental bilge discharge from the pleasure craft Lady Kelsey was identified as the cause of an incident at Ramsgate on 15 December.

4.3.4 In the open sea a small surface slick composed of fish oil residues was identified in the Dover Strait by a surveillance aircraft on 8 July. The source of the pollution was believed to have been a tank washout from an unidentified passing vessel.

4.4 Southern England (East and West Sussex, Hampshire, Dorset and the Isle of Wight)

4.4.1 Reporting organisations submitted details of 16 pollution incidents attributed to vessels during 2015 compared with 19 incidents during 2014. Thirteen recorded incidents were in ports and harbours compared with 16 during the previous year. A source of pollution was identified on 13 occasions comprising 5 pleasure craft, a fishing vessel, a vehicle carrier, a high speed catamaran passenger vessel, a coastal survey vessel, a tug, a warship, a crude oil tanker and an all-weather lifeboat. Fuel oils were identified as pollutants on 10 occasions and there were 2 references to beach pollution incidents.

4.4.2 Oil discharges resulting mainly in surface sheens were reported in local ports and harbours at Bridport, Cowes, Hamble, Littlehampton, Lymington, Poole, Portsmouth, Shoreham, Southampton and Weymouth. An unidentified pleasure craft was believed to have been the source of a diesel discharge in Bridport’s West Bay Marina on 28 October. Cowes Harbour Commission submitted details of an incident after the 19-metre converted trawler Mabuhay sank on her moorings near to the Medina Wharf causing a 4-litre diesel oil spill. Containment and absorbent booms were deployed during a counter-pollution operation. Three reports referred to fuel oil sheens attributed to pleasure craft operating in ports and marinas at Hamble, Littlehampton and Lymington.

4.4.3 A Poole Harbour Commissioners report described a 3-litre diesel spill from the 16-metre RNLI Tamar class all-weather lifeboat Baltic Exchange III at Holes Bay on 25 August. The incident occurred during a refuelling operation and a clean-up operation was completed. The Duke-class Type 23 frigate HMS St. Albans (built 2000) reported a 2-litre aviation fuel spill (AVCAT) whilst alongside the Fountain Lake Jetty at Portsmouth on 2 April. A clean-up operation was completed at Shoreham on 19 March following a one-litre diesel oil spill from the coastal survey ship Flatholm (128 gt, built 1977).

4.4.4 Three incidents were recorded at ABP Southampton during 2015. The vehicle carrier G Poseidon (72,408 gt, built 2011) was identified as the source of a 50-litre hydraulic oil spill which was caused by damaged machinery and hull plating. Tier 1 preventative action was taken by the port authority to contain and remove the pollution from the water surface. On 30 March the tug Asterix 75 gt, built 2014) capsized in strong winds and sank in Southampton water at the Fawley Marine Terminal. Absorbent and fence booms were deployed to contain a light diesel sheen. During a salvage operation dives were undertaken to close fuel tank vents and complete other

actions to prevent any further leakage of fuel oil and hydraulic fluid. The vessel was successfully raised to the surface on 10 April and pumped out with the dirty water transferred to holding tanks on supporting vessels. An inert gas boiler failure onboard the crude oil tanker Botofogo (58,418 gt, built 2010) berthed at Esso Fawley resulted in a carbon soot deposit along a 400-metre stretch of shoreline in the vicinity of the Calshot Activity Centre. The Centre suspended all waterborne activities until a clean-up operation had been completed by an Esso pollution response team.

4.4.5 Two oil spills were reported in Weymouth Harbour during 2015. A hose failure was identified as the cause of a 40-litre hydraulic oil discharge from the twin-hulled fast catamaran ferry Condor Express 5,005 gt, built 1996) on 10 March. A boom and absorbent pads were deployed in a clean-up operation. Approximately 500 litres of diesel oil leaked from a defective fuel tank onboard the 12-metre motor boat Larissa on a return voyage from Weymouth Harbour to Portland Bill on 15 May. Booms were deployed around the vessel when it berthed at Weymouth Marina and the pollution within the harbour was broken up and dispersed by propeller agitation from workboats and a RIB. Most of the oil was lost in the open sea where it was broken up and dispersed by wave action.

4.4.6 Two beach pollution incidents were reported by local authorities. During October palm oil residues were washed ashore in varying quantities along a 5-km stretch of shoreline at Hastings. Clearance operations were completed over a 14-day interval. Lewes District Council submitted details of another palm oil incident after clumps of the waxy material were washed ashore along a stretch of Seaford beach. Clearance operations were carried out on five days between 11 November and 12 December and warning notices were erected advising dog owners of the potential hazards to their pets.

4.4.7 In the open sea a rainbow sheen with a gasoline smell was sighted in the south west shipping lane of the English Channel on 10 August.

4.4.8 In addition to the pollution incidents outlined above a POLREP and an EA report referred to an incident in which a number of green 25-litre plastic and steel drums were washed up on a beach at Ryde on the Isle of Wight. Markings suggested these contained benzene alkaline for tank cleaning. Subsequent investigations revealed that the containers had fallen from a broken pallet being loaded aboard the tanker Stavanger Breeze (30,093 gt, built 2004) at Fawley Marine terminal on the previous day.

4.5 South West England (Devon, Cornwall, Isles of Scilly, Channel Islands and the Western Approaches)

4.5.1 Eighteen vessel-source pollution incidents were identified by reporting organisations during 2015 compared with 16 incidents during 2014. Fifteen discharges occurred in ports and harbours, 1 in the open sea, 1 in nearshore waters and there was also a reference to a beach pollution incident. A source of pollution was identified on 16 occasions, the sources including 4 pleasure

craft, 4 fishing vessels, 2 passenger vessels, a diving charter vessel, an oil products tanker, a research vessel, a dredger and a warship.

4.5.2 Reporting organisations provided details of 2 incidents in the open sea and nearshore waters during 2015. The Royal Research Ship Ernest Shackleton (4,028 gt, built 1995) reported a small gas oil spill from an anti-roll tank in Falmouth Bay on 22 June. A POLREP referred to a slick of light fuel oil curving into Watergate Bay off Newquay on 19 October.

4.5.3 Fifteen oil spills were reported in ports and harbours at Falmouth, Fowey, Padstow, Penzance, Plymouth, St. Helier and Torbay. Falmouth Harbour Commissioners provided reports of 2 pollution incidents during 2015. A split discharge hose from the oil products tanker Wisby Verity (4,295 gt, built 2004) was identified as the cause of a 4-tonne heavy fuel oil spill at Falmouth Docks eastern wharf on 2 June. The oil was contained by boom and recovered by skimmers and absorbents to a reception barge alongside. A small oil sheen was reported after a pleasure craft sank alongside Falmouth’s Fish Strand quay on 16 August.

4.5.4 Fowey Harbour Commissioners submitted details of 2 pollution incidents during 2015. An overflow during a bunkering operation caused a 20-litre diesel spill from the ex-fleet tender Loyal Watcher (12 gt, built 1978). Absorbents were employed in a clean-up operation following a 20-litre hydraulic oil discharge from the grab hopper dredger Lantic Bay (147 gt, built 1958) at Mixtow Reach on 1 July.

4.5.5 Three oil spills were reported in Padstow Harbour. A pleasure craft was believed to have been the source of a light lubrication oil sheen observed in the Inner Harbour on 19 February. A clean-up operation was completed by harbour staff following a diesel discharge from the pleasure craft Nsanga. The clean–up costs were later recovered from the owner of the vessel. Another pleasure craft was identified as the source of a 20-litre lubrication oil spill on 7 June.

4.5.6 On 1 November the 26-metre wooden-hulled converted fishing vessel Jolly Roger (built 1971) sank in the Floating Dock at Penzance causing a spillage of 150 litres of diesel oil and 50 litres of lubrication oil. Clean-up and salvage operations were completed over a two-week interval and the costs were later invoiced to the owner of the vessel.

4.5.7 Four oil pollution incidents were reported at Plymouth during 2015. A clean-up operation was completed at HMNB Devonport following a 2-litre fuel oil discharge from the Duke-class Type 23 frigate HMS Sutherland (built 1996) on 4 February. The Sutton Harbour Company provided details of 3 incidents that occurred during October following diesel oil discharges from fishing vessels. A clean-up operation using mats and booms was completed on each occasion.

4.5.8 The twin-hulled fast catamaran ferry Condor Rapide (5,007 gt, built 1997) was identified as the source of 2 hydraulic oil discharges in Jersey’s St Helier

Harbour on 15 and 18 November. The pollution dispersed naturally on both occasions and no further actions were required. A Torbay Council report described a one-litre lubrication oil discharge from the passenger vessel Dart Venturer (94 gt, built 1982) on 9 August. A Tier 1 kit was employed by harbour staff in a clean-up operation.

4.5.9 Palm oil residues were washed ashore on beaches in Widemouth Bay during the last week of September. Contractors were engaged to complete clearance operations on Cornwall Council-owned beaches and advice was offered to the owners of private beaches on how to dispose of the substance. In addition warning notices were erected advising dog owners of the potential hazards to their pets.

4.5.10 In addition to the pollution incidents above POLREPS also referred to a lost cargo incident. During rough weather 3 containers were washed overboard from the containership DS Blue Ocean (7,545 gt, built 2007) off Lands End on 19 May. An RVL aircraft was tasked to search for the lost containers but none were sighted.

4.6 Bristol Channel and South Wales (Somerset, Bristol, Gloucestershire,

Monmouthshire, Newport, Cardiff, Vale of Glamorgan, Bridgend, Neath Port Talbot, Swansea, Carmarthenshire and Pembrokeshire)

4.6.1 Eight oil pollution incidents were reported during 2015 compared with 3 incidents during 2014. Five discharges occurred in ports and harbours, 2 in the open sea and 1 in nearshore waters. Sources of pollution were identified on 8 occasions comprising 2 pleasure craft, an oil products tanker, a fishing vessel, a suction dredger, a tug, a passenger vessel and a coastal sailing barge.

4.6.2 A bilge pump-out from a pleasure craft was identified as the source of a light diesel sheen identified in Cardiff Harbour on 26 March. A Natural Resources Wales report to a 50-litre diesel spill after the derelict coastal sailing barge Eben Haezer sank in Cardiff’s Bute East Dock on 23 November. A clean-up operation was completed and the barge was later raised and removed for scrap. The Port of Bridgwater reported a 10-litre diesel oil discharge from the section dredger Arco Dart (1,309 gt, built 1990).

4.6.3 The Milford Haven Port Authority submitted details of 2 incidents at the Port of Pembroke during 2015 and clean-up operations were completed on both occasions. The tug Svitzer Tacu was identified as the source of a 5-litre lubrication oil discharge on 28 March. A defective pressure gauge caused a 5-litre hydraulic oil spill from the Ro-Ro ferry Isle of Inishmore (34,031 gt, built 1997) on 30 March.

4.6.4 In the open sea the yacht Angel reported an 8-litre diesel oil discharge

following a mechanical failure. A leaking stern gland was identified as the cause of a lubrication oil discharge from the oil products tanker Lizrix (1,343 gt, built 2008) on 12 July. The pollution was quickly dispersed by wave action.

4.6.5 In nearshore waters the 14 metre crabbing vessel CSK ran aground in West Angle Bay at the mouth to Milford Haven on 28 October causing a 10-litre diesel oil discharge. A dive survey revealed damage to the vessel’s hull such that it was not considered safe to refloat and tow to a local berth. Hence the vessel was winched onto a nearby beach and dismantled for disposal ashore.

4.7 Irish Sea (Ceredigion, Gwynedd, Isle of Anglesey, Conwy, Denbighshire, Flintshire, Merseyside, Lancashire, Cumbria, Isle of Man and all Districts of Northern Ireland)

4.7.1 Respondents identified 14 discharges from vessels during 2015 compared with 19 discharges during the previous year. Ten incidents occurred in ports and harbours and 4 in the open sea, estuaries and nearshore waters. A source of pollution was identified on 14 occasions and 6 spills were caused by vessel casualties. Details of accidental releases from oil and gas installations operating in Morecambe Bay are included in the statistics for the UKCS (Refer to 4.11).

4.7.2 Seven incidents were reported in the ports and harbours of north-west

England area during 2015 including 4 oil spills at the port of Liverpool. Booms were deployed following a 20-litre heavy fuel oil discharge from the cruise liner Boudicca (28,551 gt, built 1973) on 5 January. The hopper barge Jan Leeghwater (3,315 gt, built 2010) accidentally discharged 200 litres of light fuel oil into the River Mersey following a collision with a tug. A workboat ran aground and broke up at New Brighton causing a 180-litre diesel oil spillage; a clean-up operation was completed and contractors were engaged to remove the wreck from the beach. The square-rigged sail training brig Zebu (built 1938) sank on her moorings in the Albert Dock on 4 September. An unknown quantity of diesel oil escaped from the vessel until it was raised and refloated 6 weeks later.

4.7.3 Reporting organisations submitted details of 2 diesel oil spills from fishing vessels at Silloth New Dock and a clean-up operation was completed after each. A pleasure craft was identified as the source of a one-litre gearbox oil discharge at Fleetwood’s Wyre Dock on 9 October.

4.7.4 Elsewhere in north-west England the trailing suction hopper dredger Deo Gloria (1,512 gt, built 1978) suffered damage to a stern tube and lost approximately 10 litres of lubrication oil in the open sea off Barrow in Furness. The vessel later made its way to Troon for repairs. A 5-metre clinker boat ran aground in the Ravenglass Estuary causing a 22-litre diesel oil spillage.

4.7.5 Two oil discharges were recorded in the ports and harbours of North Wales

during 2015. A POLREP described a 5-litre diesel oil discharge from the 16- metre wind farm service vessel Offshore Progress (built 2008) in Mostyn Harbour on 27 July. Stena Line Ports and Natural Resources Wales submitted reports of a 25-litre hydraulic oil spill from the high-speed ferry Jonathan Swift (5,992 gt, built 1999) at Holyhead on 7 September. The

incident was caused by a ruptured hydraulic line and although booms were deployed the pollution dispersed naturally.

4.7.6 Reporting organisations in Northern Ireland and POLREPS identified 3 oil pollution incidents during 2015. The 17-metre wooden twin-rigged stern trawler Silver Dee (63 gt, built 1971) sank following a collision with another fishing vessel off County Down on 29 July. A diesel sheen with an estimated volume of 3.8 litres was later reported above the wreck site by an MCA counter-pollution aircraft. The 9-metre fishing vessel John Boy grounded and tipped over in Strangford Lough on 16 October. An absorbent boom was deployed around the vessel and collected a small amount of oil. In a salvage operation the vessel was pulled upright at low water and later refloated on a flood tide. Three reporting organisations submitted reports of an incident on 14 December after the 17-metre research vessel Ostrea sank alongside a pontoon in Loch Foyle. A boom was placed around the vessel to retain a small quantity of fuel oil.

4.8 Western Scotland (Dumfries and Galloway, Ayrshire, Inverclyde, Argyll & Bute and the Highlands & Islands Region to Cape Wrath)

4.8.1 Reporting organisations submitted details of 20 vessel-source pollution incidents during 2015 compared with 9 during the previous year. Thirteen oil discharges were reported in local ports or harbours and 7 oil discharges in nearshore waters and the open sea. Further analysis of the information contained in annual returns showed 8 references to oil discharges caused by vessel casualties including 2 fishing vessels, 2 pleasure craft, 2 barges and a general cargo ship. Although there were no references to any successful prosecutions brought by the competent authorities against alleged offenders, the costs of clean-up operations and related activities were often charged to the owners of vessels.

4.8.2 The general cargo vessel Lysblink Seaways (7,409 gt, built 2000) ran aground near Kilchoan in the western entrance to the Sound of Mull on 18 February. The vessel remained on the rocky foreshore for 2 days during adverse weather conditions resulting in material damage to its hull. Some fuel tanks were breached resulting in 25 tonnes of marine gas oil entering the water. Booms were deployed around the vessel to contain the pollution. Following salvage and inspection works it was towed to a repair facility at Inchgreen in Greenock. In a second incident on 15 March approximately 14 tonnes of marine diesel were discharged into the Inchgreen Dry Dock from the vessel’s ruptured and damaged fuel tanks. The oil spill had been anticipated and equipment and resources had been deployed in advance by contractors to capture and remove the pollution. The vessel was later declared a constructive total loss and scrapped.

4.8.3 On 20 January the twin-rig trawler Iuda Naofa (198 gt, built 1978) sank in the open sea north of the Butt of Lewis. A number of small surface oil sheens were observed during a search and rescue operation. Booms were deployed after a crane barge capsized and sank at Largs Yacht Haven. A Scottish Environment Protection Agency (SEPA) report provided details of a fuel oil

leakage from a damaged pleasure craft at Kippford Marina on 4 February. A crane barge under tow capsized in the open sea off Tiree causing a small hydraulic oil spill. An absorbent boom was deployed in Stornoway Harbour to collect an oil sheen after a 5-metre wooden pleasure craft sank in the inner harbour on 6 July. The 18-metre scallop dredger St Apollo (51 gt, built 1979) ran aground on rocks and capsized near Inninmore Bay to the eastern end of the Sound of Mull on 24 August. A slight surface oil sheen was identified which was later broken up and dispersed by wave action. The vessel was declared a constructive total loss and hence all pollutants and contaminants, including 10.5 tonnes of marine diesel oil, were removed from the wreck.

4.8.4 Reporting organisations identified 5 oil discharges from fishing vessels at Lochinver and Stornoway during 2015. Absorbent booms were deployed by crew members following a 150-litre diesel spill from the trawler Mariette Le Roch II (999 gt, built 2004) on 30 December. A Highland Council report and a POLREP described a 50-litre diesel spill from the trawler Mar Mares (409 gt, built 2003) on 10 September. The incident occurred during a bunkering operation and a clean-up operation was completed. Absorbent booms and matting were deployed in another clean-up operation following a 20-litre diesel spill from the trawler Hermanos Busto (220 gt, built 1996) on 18 February. A Stornoway Port Authority report referred to a 10-litre diesel oil discharge from the trawler Northern Star (39 gt, built 1968) whilst the vessel was beached for a maintenance operation. A rapid response by the owner of the vessel ensured that the pollution was quickly contained and removed for disposal ashore.

4.8.5 Elsewhere, a POLREP provided details of a 40-litre crude oil discharge from the oil tanker Ashna A (156,417 gt, built 1999) at the Finnart Oil Terminal in Loch Long. The spill originated from a loading arm and a clean-up operation was completed. A ROV was deployed to help identify the source of 5-litre lubrication oil leak from the RO-RO ferry Loch Shira (1,024 gt, built 2007) berthed in Largs Harbour. The skipper of the Caledonian MacBrayne ferry Loch Seaforth (7,800gt, built 2014) reported a small hydraulic oil discharge during a maintenance operation in Stornoway Harbour on 6 October.

4.8.6 In the open sea the Royal Norwegian Navy frigate Thor Heyerdahl C (built 2009) reported a 100-litre gas oil spill off Tiree on 13 April. The incident occurred during a refuelling operation from a tanker and the pollution was quickly broken up and dispersed by wave action.

4.9 Orkney and Shetland Islands

4.9.1 Respondents identified 9 oil discharges during 2015 compared with 6 discharges during the previous year. Eight incidents occurred in ports and harbours and the sources of discharges included 2 fishing vessels, 2 workboats, a crude oil tanker, a pleasure craft and a tug. Accidental releases from oil installations operating in the West Shetland Basin were included in the statistics for the UKCS (Refer to 4.11).

4.9.2 Orkney Islands Council Marine Services provided details of 6 incidents in local ports and harbours during 2015. A fishing vessel was identified as the source of a 5-litre fuel oil spill in Kirkwall Harbour on 27 January. A clean-up operation was required following a 10-litre hydraulic oil discharge from the 18-metre wooden hulled trawler Flamborough Light (44 gt, built 1965) at Stromness on 15 June. A pleasure craft was believed to have been the source of a diesel sheen reported in Kirkwall Harbour on 15 July. The salmon tender workboat Robina III reported a 10-litre diesel spill in the same location on 18 November. A clean-up operation was completed following a 10-litre diesel spill from the workboat John Jolly in Kirkwall Harbour.

4.9.3 Shetland Islands Council submitted details of 2 incidents in ports during 2015. Human error during a transfer operation was identified as the cause of a 20-litre diesel oil spill from the tug Dunter (797 gt, built 1996) in Sullom Voe Harbour on 7 July. The pollution was broken up and dispersed by propeller wash. A stern gland leak was identified as the source of a light sheen emanating from the crude oil tanker Navion Oslo (55,756 gt, built 2001) in Sullom Voe on 14 September.

4.9.4 In the open sea a diesel sheen with an estimated volume of 3.8 litres was reported by the Solan installation on 2 August and attributed to an unidentified passing vessel.

4.10 Eastern Scotland (the northern and eastern shores of the Highlands and

Islands Region from Cape Wrath, Moray, Aberdeenshire, Angus, Fife, Lothian and Scottish Borders)

4.10.1 Twelve incidents were recorded by reporting organisations during 2015, six fewer than the previous year’s total. Analysis of incidents by marine environmental zones showed 8 spills occurred in ports and harbours, 2 in the open sea and 2 in nearshore waters. The source of the pollution was identified on 11 occasions, sources comprising 3 fishing vessels, 2 offshore support/supply vessels, a cable laying vessel, a standby safety vessel, a bunker tanker, a cement carrier, a pleasure craft and a semi-submersible rig. At least 4 incidents were known to be caused by vessel casualties.

4.10.2 Aberdeen Harbour Board provided details of 3 discharges during 2015

compared with 4 discharges during 2014. A gust of wind blew a near empty pot of yellow paint off the bulwark of the supply vessel FS Taurus (261 gt, built 1991) and into the harbour at Jamiesons Quay on 2 March. The oil- based paint later dispersed naturally. An overflow from a bunkering point bund was identified as the cause of a 5-litre diesel spillage from the standby safety vessel Vos Grace (1,969 gt, built 2015) at the Regent Quay on 26 July. The supply vessel Skandi Foula (3,252 gt, built 2002) reported an ongoing diesel oil leak whilst berthed alongside the Torry Quay on 10 November. The cause was found to be corrosion in a box cooler contained within a fuel tank. A boom was deployed around the vessel and an internal fuel oil transfer operation was completed to prevent any further seepage. The vessel later entered dry-dock for repairs.

4.10.3 Elsewhere, reporting organisations recorded 5 oil spills in ports and harbours at Buckie, Cromarty Firth, North Berwick and Peterhead. Moray Council reports referred to 2 diesel oil discharges from unidentified fishing vessels berthed in Buckie Harbour. The Cromarty Firth Port Authority provided details of a 5-litre hydraulic oil discharge from the cable laying vessel Seven Discovery (8,248 gt, built 1990) on 15 May. The resulting surface sheen dispersed naturally and no further actions were required. A POLREP referred to a 10-litre diesel spill from an unidentified vessel in North Berwick Harbour. Absorbent pads were used in a clean-up operation completed by harbour staff. A Tier 2 contractor was engaged to assist in a counter pollution operation in Peterhead Harbour following a gas oil spill from the damaged single-hulled bunker tanker Erin Wood (70 gt). The tanker had earlier collided with a general cargo vessel off Peterhead on 29 August sustaining damage to the port side main cargo tank and wing tank. A boom and absorbent mats were deployed around the vessel whilst the remaining oils were pumped ashore to road tankers.

4.10.4 Two vessel-casualty oil spills were reported in the open sea during 2015. The cement carrier Cemfjord (1,850 gt, built 1984) capsized and sank in heavy seas east of the Pentland Firth on 3 January. A slight sheen was later reported which was dispersed by wave action. A small patch of diesel fuel was reported after the 16-metre wooden-hulled trawler Karinya (built 1982) caught fire and sank north of Rattray Head on 4 October.

4.10.5 Two oil spills were reported in the nearshore waters of eastern Scotland during 2015. An overflow from a settling tank was identified as the cause of a one-litre diesel oil discharge from the semi-submersible rig Deepsea Aberdeen (42,000 gt, built 2014) on 1 April. A POLREP referred to an incident on 26 August after a small pleasure angling vessel hit rocks and sank off Cruden Bay causing a small petrol spillage.

4.11 United Kingdom Continental Shelf

4.11.1 The statistics for this enumeration area were compiled from POLREP reports submitted by the MCA and PON1 reports submitted by DECC. Operators are required to notify DECC where an unauthorised release of oil or chemical to sea, regardless of volume, has occurred. Each incident is reported on a PON1 to DECC’s Offshore Environmental Inspectorate, MCA, the Joint Nature Conservation Committee (JNCC) and Marine Scotland.

4.11.2 DECC reviews every PON1 that is received in order to assess the potential for environmental impact and the need for any counter-pollution response actions. In addition, each incident reported is reviewed to ensure actions are being undertaken to prevent recurrence and to assess whether there has been any contravention of legislation. This is undertaken in accordance with DECC's investigation and enforcement policies.

4.11.3 Separate data sets have been presented for releases attributed to offshore oil and gas installations and for discharges attributed to vessels, whether identified or not, operating within the UKCS area (Refer to Figure 2).

Oil and gas installations4.11.4 All unplanned releases of oil or chemicals, regardless of volume, must be

notified to the relevant authorities. Releases of operational chemicals being used subsea as part of the exploration and production process may take place over a significant period of time. In some cases it may be many months before remedial work can be undertaken due to the timescales involved in sourcing vessels and equipment to carry out the work. Environmental risk assessments have shown that these events, which are normally of relatively small volume, usually cause negligible impact on the marine environment. Details of all releases, including a cause where known, are listed in Appendix 1 (UKCS oil and gas installations) and further information is provided in Section 3 for those releases of 2 tonnes or more.

4.11.5 A total of 337 oil releases were attributed to oil and gas installations operating in the open sea during 2015 including the west Shetland Basin, Liverpool Bay and off the coast of eastern England. The 2015 annual total was 51 more than the mean annual total of 286 releases reported between 2000 and 2014.

4.11.6 Analysis of oil types showed that 49% of reported releases were identified as

lubrication and hydraulic oils, followed by 19% fuel oils and 9% crude oils. The corresponding statistics from the 2014 survey were 44%, 24% and 10% respectively. Estimated volumes for 339 oil releases were reported (including 3 ongoing releases) and of these 72% were less than 5 litres.

4.11.7 In addition, the authorities were notified of 193 releases of chemical substances during 2015 compared with 217 releases during the previous year. These chemicals are routinely used in oil and gas exploration and production operations and included ammonium bisulphite, barite, bentonite, calcium chloride, chlorine, caustic soda, glycol solutions, hydraulic fluids, methanol, potassium chloride brine and sodium solutions.

4.11.8 The estimated volumes of unpermitted chemical releases ranged from 0.002 kg to 45,254.15 kg and 35 releases (including ongoing releases) were 1 tonne or more. It should be noted, however, that many of the larger discharges associated with operational chemicals being used sub-sea occurred over an extended interval due to the requirement for operators to source vessels and equipment to locate the source of the release and undertake repairs. Further details are provided in Section 3.

4.11.9 Of the total chemicals spilled in 2015 approximately 90% were considered under the OSPAR list of substances used and discharged offshore as Posing Little or No Risk to the Environment (PLONOR). None of the chemicals spilled are included in the OSPAR list of chemicals for priority action, which

are considered to pose the greatest hazard. None of the spill incidents resulted in a significant environmental impact.

4.11.10 The PON1 reports again indicated that remedial actions were taken by operators following accidental releases. The responses included identification of root causes of spills and improvements in operational control procedures, recommendations concerning preventative actions and carrying out any necessary repairs and modifications to faulty or damaged equipment.

Vessels4.11.11 A further 22 incidents were identified in POLREPS and PON1s. Most

involved surface sheens or thicker oil slicks reported by the nearest offshore installation but attributed to a discharge from a ‘third party’ elsewhere that was generally assumed to have been a passing vessel. The maximum estimated volumes, where known, ranged from 0.00005 tonnes to 1.54 tonnes. In each incident the pollution was believed to have dispersed naturally without posing a threat to wildlife or the coastline. On 1 September the Balmoral installation reported an oil sheen covering an area of 5.12 km2

with an estimated maximum volume of 1.54 tonnes.

SECTION 5: QUALITY ASSURANCE PLAN ANDACKNOWLEDGEMENTS

5.1 Quality Assurance Plan

As part of a comprehensive quality assurance plan data verification procedures were again applied at all stages during the 2015 survey. For example, other sources of information were consulted and any relevant reports were cross-referenced with the appropriate questionnaire returns.

The approaches for data entry and analysis were clearly shown in the relevant tables and all entries to the enumeration area sub-sets were subject to independent checking to detect any errors. On some occasions unexpected values were confirmed with the various data-sources. Multiple reports were again received from DECC (BEIS from July 2016) and MCA describing accidental discharges from oil and gas installations operating in the UKEEZ. For the remaining discharges from vessels 25% of individual incident reports were compiled from questionnaire returns submitted by two or more reporting organisations which was 6% less than the previous year.

As part of the quality assurance plan the complete database is provided for scrutiny in Appendix 1 thereby enabling stakeholder groups to determine the reliability and accuracy of the survey results. Furthermore, the annual survey reports are made available to all interested parties through the MCA's website (www.dft.gov.uk/mca) and the ACOPS website (www.acops.org.uk).

MCA/ACOPS ANNUAL SURVEY OF VESSEL/OFFSHORE SOURCED MARINE POLLUTION

GUIDELINES TO AID COMPLETION OF THE SURVEY FORM

The survey questionnaire requests information on incidents involving a range of pollutants including mineral oil (MARPOL I), noxious liquid substances (MARPOL II), packaged dangerous/harmful goods (MARPOL III) and garbage (MARPOL V).

Reports are requested for pollution incidents attributed to discharges, or suspected discharges, from vessels and offshore installations operating within the United Kingdom’s Exclusive Economic Zone (UK EEZ), harbour waters and national waters. Information is not required for pollution incidents attributed to land-based sources such as industrial premises, sewers and outfalls.

Annual returns can be made either by email or by post. If using email please click on the boxes provided and insert other information in the spaces provided. The email and postal versions of the survey form can be downloaded from the ACOPS website at www.acops.org.uk (under ACOPS Reports on the main menu) together with previous survey reports.

Please complete a separate form for each incident. Thank you for your continued support in providing this important information.

2 Identify the category of vessel/offshore sourced pollution from the options given (tick appropriate box). Note that in the case of garbage, an incident report is requested only in circumstances involving an illegal or accidental discharge from a specific vessel/offshore installation. An incident report is not required following observations of litter at sea or on beaches from an unknown source.

3 Insert dates of the beginning and end of the incident.

4 Please identify one or more zones affected by the pollution (tick appropriate box). The shoreward boundary of the open sea zone is defined as 3 nautical miles (5.6 km) from nearest land.

5 State how long the pollution remained in days or weeks (insert values).

6 Identify the location of the incident using geo-coordinates, national grid reference, place name or other suitable method (insert details).

7 If known, insert the estimated volume of pollution in litres, tonnes or cubic metres. Please state any other units of volume used.
