Wright County Conference Code of Conduct as well as the MSHSL Code of Conduct.

• Academic Progress • Under the age of 20 • Physical Exam • Transfer Rules • Six seasons in grades 7-12 • Dropped out or repeated a grade • $100 Amateur Status Awards • Non-school competition in that sport

NOTE! Both the student and the parent have reviewed the concussion management information contained in the eligibility brochure and on the www.cdc.gov/concussion website. • Have not and will not violate racial/religious or sexual harassment/violence and hazing bylaws • Have not and will not use, consume, possess tobacco, alcohol or other controlled substances • "I agree to fully cooperate in any investigation honestly and truthfully"

WILDCAT ACTIVITIES Eligibility Rules and Policies

Academic Eligibility -- read carefully

./ Student-Athlete must have a GPA of at least 1.67 and have passed every class . .

for the current or previous grading period ./ Ineligibility penalty is two weeks during the season or starting the first week of

the season (Winter & Spring) OR at the start of the Fall season will miss one to two games or two weeks, whichever is greater

./ If a student-athlete is not making academic progress at any point during the current trimester, eligibility in terms of participating in games/events may be suspended until appropriate progress is made by the student-athlete

./ Student-Athlete remains ineligible until his/her GPA meets the 1.67 requirement and/or is passing ALL classes - Student-Athlete's responsibility to get signed off by Teacher & AD

./ If at any point, a student-athlete's progress toward timely graduation with his/her most appropriate grade level is not occurring, than the Principal and AD can declare the student ineligible

WILDCAT ACTIVITIES Concussion Management Info

• State Statute requires that ALL organizations sponsoring sports programs to provide training and/or education on concussion management to participants, parents, coaches and officials

"Any athlete who exhibits signs, symptoms or behaviors consistent with a concussion (such as loss of consciousness, headache, dizziness, confusion, or balance problems) shall be immediately removed from the contest and shall not return to play until cleared by an appropriate health care professional." .

When a player shows any symptoms or signs of a concussion, the following should be applied. • The player should not be allowed to return to play in the current game or practice. • The player should not be left alone, and regular monitoring for deterioration is essential over

the initial few hours after injury. • The player should be medically evaluated after the injury. • Return to play must follow a medically supervised stepwise process.

A player should never return to play while symptomatic. "When in doubt, sit them out!"

• MSHSL has included a document in the eligibility form that you must sign in order for your child to participate in a one of our athletic programs .

. This document includes: - Guidelines that need to be followed if athlete demonstrates symptoms or signs - Return to play protocol - Symptoms or signs of a concussion

• The MSHSL Sports Medicine Advisory Committee highly recommends that every student-athlete and parent successfully complete the Heads Up: Concussion in High School Sports course. The course can be accessed at: www.cdc.gov/concussion

• Mood-Altering Chemicals: Twelve (12) months of the year, a student shall not at any time, regardless of the quantity: use or consume, have in possession (1) a beverage containing alcohol, (2) tobacco or (3) buy, sell or give away any other controlled substance or drug paraphernalia.

• Sexual/Racial/Religious Harassment or Violence - Defined as annoying and inappropriate comments and or actions pertaining to sex, race and/or religion.

• Hazing - Defined as committing an act against any person or coercing a person into committing an act that creates a substantial risk of harm to a person.

WILDCAT ACTIVITIES Consequences 1st Violation - 14 calendar days or two consecutive contests whichever is greater 2nd Violation - 21 calendar days or 6 consecutive contests whichever is greater 3rd and subsequent Violation - 28·calendar days or 12 consecutive contests whichever is greater

* Ineligible to be captain * In-Season Violation -- No letter, No recognition at season ending banquet (student

can attend banquet) * Year around rule * Penalties shall be accumulative beginning with and throughout the student's

participation in their 7-12 educational years *Must be at all practices, scrimmages, games, meetings, etc. during period of

suspension. * Must finish the season with the team where suspension was served. * A student who has been suspended either in school or out of school may NOT

participate in practices/contests and lor scheduled activities during the time of suspension.

* Denial Disqualification - Additional 9 weeks of suspension!

Social Media ... "If you don't have something nice to say ... don't post it, don't text it, don't say it ..... "

• Athletes may not be coached outside of their season. by their coaches (includes volunteer coaches) during the school year. Coaches may coach their athletes during the summer through July 31 (Exception: week of July 4)

Attendance: ../ A student must attend ALL of her/his classes in order to participate in a

scheduled contest on the same day or to practice on that day Exceptions to this is family emergencies or pre-arranged medical appointments. Please contact the high school attendance office or if necessary contact Activities Director. Examples that are not acceptable excuses • Haircuts • Sick in the morning and then come to school • Get out early because you forgot practice/game gear and/or need to get

food before leaving ../ Unexcused Absence or Truancy from school or class during the season will

result in the suspension for one contest ../ Second offense of an unexcused absence or truancy could result in a

possible suspension for 2 contests or for 2 weeks whichever is greater ../ Students will be school excused for school sponsored activities requiring

early departure times, etc.

ALWAYS want the student-athlete present during discussions

Don't approach coaches on game day, before or after, to discuss personal matters, with or without student.

Don't discuss playing time, strategy or other student athletes

There is a difference in making decisions and mistreatment ...

Avoid screaming or yelling inappropriately from the sidelines or stands. If you have to make a noise, make sure you are offering positive reinforcement.

Get involved and enjoy the experience as best you can!

TRANSPORTATION

• The EXPECf~TI0N is tHat student-athletes travel witH the team to events and should also travel back home with tne team

• CSoaches will communicate early out times as well as departure times for away events

• If you have any questions call the middle school or rnigh school office for information

WILDCAT ACTIVITIES Forms, Physicals & Fees To be fully registered for a program, athletes must turn in:

• MSHSL Eligibility Brochure and Sports Health Questionnaire. The MSHSL requires that the Athletic Eligibility Brochure and the Sports Health Questionnaire be filled out and returned every year. The Sports Health Questionnaire is required in addition to the sports physical.

• Sports Qualifying Physical. Athletes must have a current physical on file in the Activities Office before they are able to practice. A reminder that physicals need to be updated every three years. (Incoming Freshman physicals have been transferred from Clearwater. Sophomore athletes are most likely in need of an updated physical before practicing in August.)

• Emergency Information Card. This card must be updated and turned in each year. (Insurance Information) NOTE: Medical Insurance: The Waconia Public Schools does not provide insurance coverage for any medical expenses incurred by a student as a result of injury while participating in school sponsored programs. A medical benefit plan is available through a commercial company for parents and/or students interested in purchasing this coverage

• Fee Payment. Activity Fees - Grades 9-12 $165 Grades 7-8 $110 Please communicate in a timely manner if financial hardship prevents the payment of fees We want EVERYONE who wants to participate on one of our teams to have every opportunity to do so!!

CLEARANCE CARD Upon receipt of this these items, a student athlete will secure a Clearance Card which they must give to the coach in order to participate.

All incoming 9th graders in football, volleyball and boys & girls soccer Retest of all 11th and 12th graders in football, volleyball and soccer that tested in 2012. Retest any football, volleyball or soccer players that had a concussion last year

• Waconia Public Schools #110 2014 Bond Referendum At A Glance

• The Waconia Board of Education is proposing a $75 million bond referendum for the Nov. 4, 2014 election.

• Levy funds will be used to support an expansion project to meet student enrollment projections. • If approved, the District will refinance existing obligations, stretching payments for the new bonds from

a 20-year schedule to 24 years to keep annual property taxes stable •
