Biography:

Dr. John R. Langefeld is vice president of clinical integration and quality innovation for Vizient Southern States. He is a board certified family physician and physician executive with health care experience in clinical care that ranges from rural primary care to large delivery systems. Dr. Langefeld also has expertise in the areas of provider management, managed care organization leadership, consulting for large employer-sponsored benefit programs, worker’s compensation and disability management, and information

management.

Prior to joining Vizient Southern States, Dr. Langefeld was chief medical officer for the Kentucky Cabinet for Health and Family Services - Department for Medicaid Services. Other past positions include chief medical officer for Artemetrx, a company specializing in how data analysis can impact clinical quality outcomes; medical director and vice president of health care management for Bluegrass Family Health, a regional health plan in Lexington, Kentucky; chief medical officer for Health Associates of Kentucky, a management service organization (MSO) supporting a large primary care network affiliated with the University of Kentucky Medical Center; as well as several other medical director and clinical care positions. Dr. Langefeld has also served on the faculty for two academic medical centers and in 2015, was selected as Citizen Doctor of the Year by the Kentucky Academy of Family Physicians.

Biography:

Colleen Meiman joined NACHC as the Senior Policy Advisor in January 2015. Her department is responsible for working with the Administration – particularly CMS and HRSA -- on policy issues that affect Health Centers and their patients. Previously, Colleen spent over two decades in the Federal government, including in the Office of the HHS Secretary, in the US Senate, and in numerous positions within HRSA, including in the Bureau of Primary Health Care, which oversees the Community Health Center program. Colleen’s expertise is in the regulatory, administrative, and financing aspects of Medicaid, Medicare, 340B, and the

Affordable Care Act, and how they relate to health centers.

Nathan Baugh is the Director of Government Affairs for the National Association of Rural Health Clinics (NARHC) where he has worked since April of 2015. Nathan works on both regulatory and legislative policy at the federal level. He has been involved in issues

such as the CPT reporting policy, the Chronic Care Management benefit, and the Emergency Preparedness rules.

John Inman is the Director of Advocacy and Government Affairs for the Kentucky Primary Care Association (KPCA). His primary responsibilities include staying in contact with the Kentucky state government agencies, advocating on the behalf of the KPCA membership, managing the grassroot advocacy program for KPCA clinics and acting as a liaison with National Association of Community Health Centers (NACHC) and National Association of Rural Health Centers (NARHC) on federal issues.

John is a native Kentuckian and spent his childhood helping his father and grandfather raise tobacco and cattle in rural Taylor County. He then went on to become a deputy sheriff in Grant County and later a police officer with the Kenton County Police. While at Kenton County John served in many capacities and was a detective assigned white collar crime and public corruption cases. In 2016 he was appointed to Deputy Inspector General at the Cabinet for Health and Family Services, where he oversaw all special investigations, healthcare facility licensing and surveys, and childcare facility licensing and inspection. In 2017 John transferred to Medicaid where he supervised Program Integrity, served as legal advisor to the Commissioner, and was Chief of Staff. John holds a bachelor’s degree from Eastern Kentucky University, a Juris Doctorate and Master of Accountancy from Northern Kentucky University, and is a licensed attorney.

Biography:

Mr. Bledsoe (Andrew) has a number of years of managing health Information exchanges in the State of Kentucky. He began his career at St. Claire Regional Medical Center, obtaining insight into rural medicine for both hospitals and ambulatory providers. Leveraging his rural experience, Andrew moved to the Northeast Kentucky Regional Health Information Organization (NeKY RHIO), serving as the Network Director initially, later moving into the role of Executive Director. Andrew is very well versed in national quality standards (MU, PQRS, VBM, MIPS, MACRA, HEDIS, PCMH, HIPAA) and holds multiple credentials focusing on health information technology. Andrew’s current position is the Deputy Executive Director for the Cabinet for Health and Family Services,

Office of Health Data and Analytics where he oversees the Kentucky Health Information Exchange (KHIE). With a primary focus on the state-wide initiative for health information exchange, Andrew coordinates and facilitates the KHIE Strategic & Operational Plan, which is focused on connecting all providers across the state of Kentucky to the health information exchange. Andrew is a graduate of Morehead State University with a MBA in Management and BA in Business Administration.

Hierarchical Condition Categories (HCCs) - What Are They & How Do They Impact Value-Base Payment

Session Description:

The Centers for Medicare & Medicaid Services (CMS) is moving aggressively to shift health care payments from the traditional volume-based fee-for-service model towards value-based payment models, also called value-based reimbursement (VBR). This shift is focused on reducing the cost of health care in the U.S. while also giving healthcare providers incentives to improve their quality of care. These models require a risk-adjustment methodology, so payment can be adjusted based on the burden of illness of each individual.

Following the presentation, the participants will be able to:

1. Understand the new value-based reimbursement models CMS is implementing

2. Discover the role HCCs and diagnosis code assignment have in affecting value-based payments

3. Learn best practices for accurately capturing HCCs across the patient population.

Marla has more than 20 years of experience in the health care industry. She provides reimbursement and compliance audit services as well as education to physician clinics, provider based clinics, rural health clinics, federally ‐qualified health centers and outpatient behavioral health providers.

Her experience is in professional coding, documentation and billing compliance audits, analysis of medical practice charge master description or

fee schedule descriptions, CMS 1500 and UB 04 claims detail and charge capture review, hierarchical ‐ ‐condition category (HCC) and clinical documentation analysis, analysis of risk adjustment reimbursement impact, quality measure analysis, physician and staff education on coding, billing and regulatory compliance issues.

Marla frequently gives lectures about evaluation and management coding and documentation guidelines, CPT/HCPCS code updates, rural health clinic/federally qualified health centers coding, billing and compliance issues and ICD 10 CM and HCC coding accuracy and clinical documentation ‐ ‐improvement at events hosted by state and national primary care and rural health associations and chapters of The American Health Information Management Association (AHIMA), AAPC, Healthcare Financial Management Association (HFMA), Medical Group Management Association and for BKD internal and external webinars. Biography:

Barry is the Chief Executive Officer of Primary Care Centers of Eastern Kentucky (PCCEK). PCCEK operates four rural health clinics in the heart of Appalachia in southeastern Kentucky. He started the organization in 2003 with the goal of providing quality healthcare to the residents of the region and to contribute to the communities where the patients live.

He began his career in healthcare straight out of college as a Kentucky Medicaid Auditor. He worked as a Controller in a long-term care facility after he left State

Government. His next move was to the position as the CFO of Shriners Hospitals for Children in Lexington, KY. During his experience at Shriners he remained involved in working with RHCs as a consultant and this helped him decide to return to his home region to make a difference.

Barry’s work ethic, along with his 30 years of diverse health care experience has given him the business understanding and industry knowledge to build a clinically integrated, multi-tiered, patient centered primary care system using the RHC model. The practice includes Family Medicine, Pediatrics, OB/GYN, Behavioral Health, Dentistry, Diabetes Center, Pharmacy and other ancillary services all in one location. Additionally, he has developed unique relationships with various partners to meet the needs of the patients PCCEK serves. Working with the local hospital, the University of Kentucky, and a regional Community Mental Health Center he has developed innovative approaches to issues faced by the patient population served by PCCEK. Barry has the willingness to adapt to an ever-changing healthcare environment and he makes needed investments to serve the patient population and meet their needs.

He graduated from the University of Kentucky in 1986 with a Bachelor of Science in Accounting and obtained a Master of Health Administration degree, also from UK, in 1995.

Barry is the Past Chair of Kentucky Primary Care Association (KPCA) and has been a board member and/or committee member of the organization for over 20 years. In addition to his duties as Chair he currently sits on the organizations Executive Committee, IPA Committee and Kentucky Medicaid Primary Care Technical Advisory Committee. He is also a member of the KPCA ACO Board of Directors and was involved in the creation of this RHC/FQHC driven organization. Some of his past community service includes Chair of Kentucky Mountain Health Alliance, President of the Hazard Rotary Club and a Board of Regents for the Kentucky Community and Technical College System.

In today's fast-paced and ever-changing health care environment, Federally Qualified Health Centers (FQHCs) are subject to an array of federal legal requirements, including the Health Center Program Requirements issued by the Health Resources and Services Administration as well as various laws and rules that apply to federal grantees in general. While some of these requirements are ongoing, others appear to change often. This session will provide brief overviews of the latest developments in HRSA and other federal requirements applicable to FQHCs. Participants will explore how these developments coulq impact health center operations, both in theory and in practice.

Following the presentation, the participants will be able to:

1. Understand the latest developments in compliance with the Health Center Program Requirements of the Health Resources and Services Administration

Biography:

Marcie H. Zakheim is a health care Partner at the firm, specializing in Federal grants and grant-related requirements for health care services (including Section 330 of the Public Health Service Act); non-profit corporation law, including governance; health care provider networks and affiliations; managed care contracting; corporate compliance programs for health care providers; and federal health care fraud and abuse law. Ms. Zakheim’s work includes providing advice to community health centers, other providers, and provider consortia on governance, administration and operations (including evaluating, developing and negotiating agreements, organizational documents, and operational policies). In

addition to reviewing, developing and providing advice on federal grant applications and federal legal requirements. Her work also includes reviewing, analyzing, and drafting formal comments and advice on statutes, regulations, and policies impacting community health centers and the health care industry in general.

Ms. Zakheim is counsel to the National Association of Community Health Centers and she provides technical assistance services and advice to health care providers and provider-based consortia, national and state associations and public agencies nationwide. Ms. Zakheim also conducts training programs, presents seminars and webinars, and has written various issues briefs, information bulletins, advisories, manuals and articles on a wide array of health care-related issues.

Prior to joining the firm, Ms. Zakheim served as a consultant to the George Washington University (GWU) School of Public Health and Health Services, where she co-authored "Negotiating the New Health System: A Nationwide Study of Medicaid Managed Care Contracts” (editions one through four). Ms. Zakheim continues to serve as a Professorial Lecturer of Health Policy at the George Washington University School of Public Health and Health Services.

Stacy Stanifer, MSN, APRN, AOCNS is an Advanced Practice Registered Nurse, a Certified Oncology Clinical Nurse Specialist, and PhD candidate at the University of Kentucky College of Nursing. Ms. Stanifer works as an Oncology Clinical Nurse Specialist at Baptist Health Lexington and her research interests lie in cancer prevention. Ms. Stanifer's research focuses on radon testing and mitigation, as well as HPV vaccination.

Ms. Kilpatrick serves on the Board of Directors for Radio Eye, Inc., the University of Kentucky Law Alumni Association, and the Fayette County Bar Association. She serves on the Board of Trustees and on the Scholarship Committee of the Energy & Mineral Law Foundation. She has been recognized in the listing of Kentucky Super Lawyers from 2012 to the present and the listing of U.S. News – Best Lawyers for 2020.

Ms. Kilpatrick was Articles Editor (1995-96) and a Staff Member (1994-95) for the Kentucky Law Journal. She served as law clerk to the Honorable Eugene E. Siler, Jr. on the United States Sixth Circuit Court of Appeals from 1996-97. She was an associate and then partner at Wyatt, Tarrant & Combs, LLP, where she worked from 1997-2005. Ms. Kilpatrick received her B.A. degree with High Honors in 1992 from the University of Virginia and her J.D. degree (valedictorian) from the University of Kentucky in 1996.

Tricia Shackelford joined Williams Kilpatrick in September 2019. Her practice focuses on all aspects of health law, including corporate issues, employment issues, bankruptcy issues, Medicare Fraud and Abuse, billing and coding, EMTALA, HIPAA, Accountable Care Organizations, Managed Care Organizations, Independent Physician Organizations, mergers and acquisitions, Certificate of Need, licensure defense, and peer review and credentialing. Her clients include hospitals, hospital systems, hospice providers, rural health clinics, practice management companies, primary care centers, ambulatory surgery centers, community health centers, public health departments, home health agencies, nursing homes, durable medical

equipment companies, pharmaceutical marketing companies, and individual providers (physicians, advanced practice nurse practitioners (APRNs), physician assistants, audiologists, certified registered nurse anesthetists (CRNAs), physical therapists, and dentists).

Tricia graduated, cum laude, from the University of Miami School of Law in 1999 and started her legal career at Steel Hector & Davis, one of South Florida’s leading business and international law firms. Biography:

David Bolt joined KPCA with a vast amount of experience in health care and in the FQHC, RHC and Primary Health Care world, specifically. Prior to joining the KPCA, David was Director of Network Management for WellCare of Kentucky adding to his involvement and knowledge in managed care which began in 1976. David has experience in clinic operations, recruitment, pharmacy, dental clinic operations, school health, EMS, grant development and other areas. Previously, he served as CEO of a new start health center in western Kentucky achieving a no deficiency OSV

within the first 24 months of operation. David served as the COO of a large FQHC system for 10 years

adding 4 new sites, pharmacy, dental services, integrated BH and primary care and managed the adoption of the first integrated EMR in a KY FQHC. David spent 19 years running inpatient and outpatient services at a rural regional hospital. There he set up the first telehealth health program in the state in 1994, established one of the first ER Fast Tracks in the state, developed and operated 4 LPCC/RHCs and established the first regional AHEC in Kentucky. While with the AHEC he was the lead in establishment of the UK PA and NP programs in Morehead, developed the program that lead to a rural track FP residency program, assisted in the development the first rural based Pharmacy Residency Program, and developed the first regional Paramedic Program outside of major urban areas in Kentucky, among other accomplishments.

David is a long-standing member of KPCA and served as chair the Board several terms. He has served on the Board of the KRHA, is a past Chair of Foundation for a Healthy Kentucky, served a six-year term on the Foundation’s CAC, provided volunteer support to the Foundation’s efforts before becoming formally involved with the CAC, and was a member of the Kentucky Health Information Exchange Coordinating Council, among others. He has published articles on rural health care issues and holds Voluntary Community Based faculty appointments in the Colleges of Medicine, Health Services and Public Health at the University of Kentucky.

David has been recognized for his leadership by several organizations over the years. He was awarded the Dan Martin Award by the Kentucky Rural Health Association; the UK College of Health Sciences and the Kentucky Academy of Physician Assistants have recognized his efforts He has been honored on the floor of the Kentucky House and Senate for his contributions and efforts in behavioral health and for other services.

David is currently on the Board of Directors and a long-time member of the Prichard Committee for Academic Excellence among other state committees.

David has served as a consultant to the National Health Service Corps, various Primary Care Associations throughout the Midwest and Southern US and has served as a HRSA grant reviewer. In his, almost 5 years in State Government he ran the KY ARC Health and Human Services programs helping to integrate those programs into overall economic develop efforts for the Appalachian area of the state. In his last two years with State Government he oversaw the Office of Local Government Assistance overseeing city and county budget and technical assistance programs, in addition to his other ARC program responsibilities.

He lives in and served as the elected Commissioner and Mayor of the City of Lakeview Heights, KY for 35 years. David holds a bachelor and master’s degrees from Morehead State University where he is a member of the Alumni Hall of Fame and was recognized as the Outstanding Fundraiser for the University’s Foundation.

This session aims to help increase awareness and completion rates of vaccine rates in adolescents. The

focus includes the HEDIS Immunizations for Adolescents (IMA) measure of one dose of the

meningococcal vaccine, one Tdap vaccine, and the complete human papillomavirus (HPV) vaccines

series. According to the American Academy of Pediatrics (AAP), Kentucky lags the nation in cumulative

flu season coverage and HPV vaccination rates for both boys and girls. Kentucky adolescents also lag

the Healthy People 2020 goal for the Combined 7-vaccine series in children 19-35 months old. The

session will provide strategies to enhance immunization rates at the provider and healthcare system

level including a strong provider recommendation, motivational interviewing strategies/techniques,

and clinical/operational enhancements including EHR workflows, data utilization, provider

prompts/reminders, and standing orders. Also provided will be information on best practice resources

to enhance clinical staff influence, answering parents’ questions, minimizing missed opportunities and

creating a pro-immunization environment through staff, leadership, and cultural engagement.

Following the presentation, the participants will be able to:

experience with EHR implementation and optimization in the clinical setting. She is

a graduate of the University of South Carolina.

Jeff Vella is a Health Systems Manager at the American Cancer Society. In his role,

he works with FQHCs to increase screening measures through provider and staff

education/engagement, provides technical project assistance, and helps to direct

organization’s to screening resources. His passion is quality and performance

improvement at the health system level. He is a graduate of the University of

Ms. Crawford is a current member of the National Association of Rural Health Clinics Board of Directors. She also serves on the Board of Directors of the Kentucky Primary Care Association and the Board of Directors for the Tennessee Rural Health Association.

She is the Vice President of Operations and Director of Nursing with Wilkens Medical Group in Jellico, Tennessee and manages clinics in Kentucky and Tennessee.

Elsie attended Cumberland College and Carson Newman College for Business Administration and Roane State Community College and The University of Tennessee for Nursing.

She has been the Vice President of Operations and Director of Nursing for Wilkens Medical Group in Jellico, Tennessee, since 1974. She is a certified Medicare Rural Health Clinic surveyor and works with AAAASF doing clinic surveys.

Elsie has served on the board of The Tennessee College of Applied Science (Nursing Division) since 2004, Board Member of NARHC since 2006, Jellico SDA Church School Board Member (Treasurer) for 9 years & Jellico City Council Member for 12 years.
