Outcomes:

(To be filled in by end of Semester)

1. To provide the appropriate education to all populations seeking health care at the clinic. I will be very attentive to clients, listen to my

preceptor (mentor) and ask any questions that I may have.

I will make sure the clients successfully understand the education that they received.

-Clients felt comfortable discussing their care with me and asking questions

-Provide education to patients including handouts/brochures.

-Assisted my mentor with the EDCD waiver assessment and gained insight of the client’s life and education. She did not have the best living condition, but she stated she was as happy as can be. This is a human being that truly deserves approval for the waiver. Therefore, an aid can come to her house and assist her with her healthcare. She would also be approved for transportation that would take her to and from doctor appointments.

-Clients felt more comfortable discussing information with me

-I provided education and the clients were able to explain what was taught to them (teach-back method) Before the end of the appointments, clients stated an understanding due to the education I provided.

-I was able to gain knowledge and experience another area of nursing.

-I provided a caring attitude and compassion to all clients regardless of age, sex, and ethnicity.

2. To be sensitive to cultural backgrounds Be understanding of one’s culture as well as their values and beliefs

Treat everyone with equal respect-No one is better than anyone else

.-I provided care and education specific to the client. For

example, I cared for a Muslim client (female) and two Hispanic clients (female). To make sure these clients received the appropriate care, a female nurse was assured for the Muslim client and an interpreter was provided to the two Hispanic clients.

-I was able to accomplish one on one care. I did not have to worry about running back and forth to rooms and answering call bells.

-I left Hanover Health Department feeling very confident that I provided

compassion and was respectful to all different backgrounds.
