• Purpose: To implement evidence based practices that will enhance advocacy provided to youth 7-17.

• Grant Period: June 1, 2014 – May 31, 2015

• 15 Evidence Based Enhancements new grantees

Importance of Reporting

To show outputs and outcomes in relation to:

• State and local program page

• Reporting tools

• Evidence Based Enhancements, Fostering Futures Training and Youth Advocacy Grants

Data Collection and Reporting

Questions so far?

To help track volunteers and the children/youth they serve

• For volunteers:National CASA Program Number plus unique

three digit identifier (example-12312-001)

• For youth:The ID# of the volunteer assigned plus a one

digit identifier (example–12312-001-1)

Evaluation ID Assignment Instructions

• To track volunteer and youth evaluation ID numbers and other required grant related information

• Tracking sheet has five tabs – Notes, Volunteers, Youth and a tab for each of the reporting periods

• All grantees are required to use the tracking sheet provided by National CASA

Grantee Report Tracking Sheet

Components:• Volunteer name and

evaluation ID#:• CASA Program Number plus a

unique three digit identifier (example – 12312-001)

• Dates related to volunteer participation in an evidence based practice

Tracking Sheet – Volunteer Information

• Volunteer evaluation ID#• Date of Birth• Dates relating to arrest,

sentencing and incarceration • If child promoted to next grade

level (May report only)• Involvement with volunteer

Tracking Sheet – Youth InformationComponents:

• Youth name and evaluation ID#

• The ID# of the volunteer assigned plus a one digit identifier

(example–12312-001-1)

• Monthly Expense Report

• Semiannual Grantee Report

• Grantee Narrative Report

Semiannual Grantee Report

• To report grantee related outputs and outcomes

• Report only on grant-related volunteers and youth served - Volunteers who participate in the evidence based enhancement and youth served by a volunteer who participated in the evidence based enhancement

• All fields must be number/digit

• Semiannual Grantee Report Instructions provide detailed instructions on completing the form including definitions for information requested
