Wellborn Cabinet, Inc is pleased to introduce our Cash Rewards Program. This program is designed for Designers* to earn cash rewards on their specific brand purchases from Wellborn Cabinet participating dealers.• Promotions will run throughout various time periods of the year - Promotional flyers will provide detail• Earn cash rewards by cabinet brands sold• Cash Rewards Program launch date was February 9, 2015• Dedicated website for Cash Rewards reports - Designer Views• Rewards will be deposited on a monthly basis to a Visa Debit Card• Use the card wherever Visa is accepted (ATM option)• Designers of participating dealers are eligible for the program*• Participants are required by law to submit their W-9 tax information• Information for tax purposes is provided through a secure fulfillment company• Debit Card is provided by a fulfillment company• Based on cabinet invoices not orders• Wellborn reserves the right to amend the terms and conditions of any of its promotions or terminate promotions without notice• Cash Awards can not be combined with other rewards• Dealer must select Designer on appropriate order for Designer to receive Cash Rewards*Based on a Wellborn participating dealer and verification of Designer

• Cash Rewards will not be applied after order is placed with Wellborn if Dealer does not select appropriate Designer. NO backdating rewards program• Wellborn brands included in the Cash Rewards program: • Select Series • Premier Series • Estate Collection • Elegant Bath Collection • Wellborn Closets (remember only cabinet skus)• NOTICE: Home Concepts Is NOT Included• Cash earned will be based on a specified dollar amount per promotion period and per brand as stated in the promotion on each cabinet* purchased

Note: Promotion Periods and Rewards per Cabinet are Subject To CHANGE*A cabinet is defined as any SKU that is 3 dimensional with volume that has space requirements and is not included inside another box. This definition would not include parts, trim items, moulding, accessories, and marketing sample doors and items.

The Cabinets that Apply to Cash Rewards are listed below: • All Cabinets with doors and accessories that are larger cabinets like bookcases & hoods • Kits, mouldings, fillers, small accessories, etc. do not apply • You Draw It! • UDI cabinets will apply • UDI items that are filler and moulding type products will not apply • Rush Items—yes, but have to be on the above Cabinet List

• Accessories, kits, mouldings, fillers, panels, etc• Display Orders—these orders receive 50% to 75% display credit• Parade of Homes—these orders receive 50% Coop Credit• Model Homes—these orders receive 50% Coop Credit• Marketing Samples—including all Wood Samples, Design Centers and Doors, SFKs, Display Doors etc. • On Line order—dealer will be required to check if one of the above. Participating Wellborn Dealers• Current Wellborn dealers who wish to participate in the Cash Rewards program must sign up for the program. All Dealers participating in Cash Rewards must register and accept the Terms and Conditions of this Program on the WAY site in addition to their Sales Addendum• Once Dealer has registered for the program, dealer must list all eligible Designers for the program on WAY• Designers Receive the Cash Rewards

Registration for Dealers through WAY:• Dealers must e-accept participation in Cash Rewards on the WAY site• Link will be provided to the Dealer from the Sales Department• This registration is in addition to the Sales Agreement• Wellborn will track registrations for the program• Dealer must accept all Terms and Conditions of Cash Rewards Program

• Note: This registration is just the first step in the total enrollment process. Designers MUST enroll on the Cash Rewards website and with the debit card company

• In the details of this program, you will see the word, Designer, in reference to the Cash Rewards Program. To cover all bases and give rewards where due, please see the definition below concerning the use of these terms

• The definition of a designer for this program is anyone deemed eligible by a participating Wellborn dealer who orders Wellborn products through that dealer. Whether you are an employee, freelancer, independent contractor, salesperson or other, you must be approved by the participating Wellborn dealer to be eligible for the program. Any reference to “Designer” in the following presentation and program details, refers to this definition

Designer status in the program will have three designations• Pending Eligibility—Designer will be pending until all information is filled out and submitted and W-9

information is submitted and approved• Active—will show as a status after approval. If a designer does not submit orders within a one year

time-frame, they must re-apply for the program• InActive—No orders have been placed for 12 months. Designer will have to re-enroll in the program

• Email Notifications and Updates: Participants of the Wellborn Cash Rewards program are authorizing Wellborn to communicate with them via email, mail or any other means regarding, but not limited to program updates, promotional opportunities, and any Wellborn marketing information

• Go to www.wellborncashrewards.com and click on the New User Tab• Enter all information for your Designer Profile, read and agree to the Terms & Conditions of the program and click on Submit• Your pre-qualification will be verified for the following: • That you are applying for the program under a Cash Rewards participating dealer • Verification of your eligibility status by Wellborn• Once this is completed you will receive an email

with your Designer ID and your ID number will show up under your Profile on the Cash Rewards website

• Only eligible designers of participating Wellborn dealers are allowed in the program• Designers must enroll on the Cash Rewards website and complete their enrollment and tax

information on the debit card fulfillment site once a Dealer has registered them for the program

How does the Designer sign up for the Cash Rewards program?• Dealers list Designers on the WAY site who are eligible for the program and are sent a unique URL for

their Designers• Designer will use that link and go to the Cash Rewards site and complete the Designer participation

consent process, including personal address and contact information. Personal tax information will be entered on a secured website administered by an independent company, NOT WELLBORN. Personal tax information is not shared with Wellborn

How does the Designer become a user of the Cash Rewards website?• Go to Cash Rewards site and create a Cash Rewards username and password. You will receive a

Designer ID account number• A one time activation card fee of $25 is required by the participant and will be deducted from your

rewards card

How does the Designer start earning?• Complete consent process via Cash Rewards site• Submit personal tax information on Debit Card Administration site• Upon eligibility verification and notification, start putting your Designer ID on all your orders in 2020 or

choose the designer name in a dropdown at Order Submittal in Direct Connect• Rewards will accrue when your orders invoice. Rewards will not accrue until vendor verification,

personal info verification and invoicing occur. System will not allow accrual until all is completed

Can Designer ID be added after an order has been placed?• No, You must include your Designer ID on each order at the time of order placement. Wellborn will not

add Designer ID’s after the order has been placed, unless it was missed due to a Wellborn system error

Can more than one Designer ID be on an order?• No, only one Designer ID can be put on each order. This must be the responsibility of the dealer at

order placement

Can the Designer transfer his/her accrued cash to another Designer ID?• No, cash accrued is non-transferable

What is the definition of a cabinet that accrues?• A cabinet is defined as any SKU that is 3 dimensional with volume that has space requirements

and is not included inside another box. This definition would not include parts, trim items, moulding, accessories, and sample fronts. Do not include your Designer ID on orders consisting of Non qualifying products or order reasons. See details under “Orders That Do Not Qualify for Cash Rewards”

Who does the Designer contact if he/she has a discrepancy in the rewards accrued on an order?• Technical disputes on any order must be initiated within 90 days of invoice by contacting either

Christy Saxon, Wellborn Customer Service Representative or Emily Johnson, Wellborn Credit Department

What happens if a Designer has rewards accrued in the program and leaves the dealer?• Designer must be registered by a participating, qualified dealer to earn Cash Rewards• If a designer leaves a dealer for any reason, the accumulated rewards will be available for redemption

for 12 months from date of last order activity• If a designer signs up under one dealer and leaves to transfer to another participating, qualified dealer,

existing Designer ID and any accumulated rewards can be transferred by contacting Emily Johnson at ejohnson@wellborn.com

• Upon verification of eligibility, you will receive an email containing your Designer ID for the Cash Rewards program from Wellborn. This ID will also be visible on your Designer Profile screen on the Wellborn Cash Rewards website

• This Designer ID will be the number you will enter on all eligible Wellborn orders and in your 20-20 CAD orders. This number will also be used to complete your enrollment in the Debit Card Administration site

• An email will be sent to you from the Debit Card Administration site that will contain a link to their site and your username and password for their site. This is where you will complete your tax information and monitor when funds will be added to your Debit card once issued
