On Saturday, July 2, all participants can gather at the River Crescent Centre, eMalahleni. Entries open at 06:30 and the race will start at 08:00. The 4.9km fee will be R40, 10km, R60 and for the brave, the 21.1km will be R80. The 10km and 21.1km runners must have a valid licence as per ASA on the day, but temporary licences will be avail-able at R25.Everyone who crosses the finish line will receive a medal and there are also great prizes up for grabs. Bursary fund a beacon of hopeThe annual Paul Mthimunye Bursary Fund Gala Dinner was hosted at the Middelburg Banquet Hall, and thousands of rand were pledged towards a noble man, and his efforts to uplift the marginalized. The bursary fund was started by Mr Mthimunye during August 2001, and was officially launched by the former Mpumalanga MEC for Education in the same month of 2003.

Tails were literally wagging at the Wags for Cansa fundraising event, where Amore Vittone stole the limelight alongside the dogs who took part in a glitzy fashion show, to raise much needed funds for cancer research and awareness. The glamorous event was hosted by local Cansa ambassador, Mr Andries du Preez, who ensured once again that the Wags for Cansa evening is set to become the highlight of Middelburg’s social calendar. The evening was characterized by fashion, enter-tainment, good company and of course the Italian flair only Amor can bring to the stage. Attendees were on their feet the whole night, dancing to the music of the Fashionista dressed especially for the evening by YDE. Local pooches, all blinged out, strutted their stuff on the catwalk like professional models, and though everyone held their breath, luckily there were no surprises on stage...

Dié oud-hoofdogter van Hoërskool Middelburg gee werklik vir haar pasiënte om, kom jy sommer gou agter.

“Jy moet bereid wees om ’n pad met mense te stap en hul héle prentjie in te kleur. Dit is iets wat deesdae baie maklik verlore gaan.”

Na haar jare by MHS het sy keuring by MEDUNSA gekry om medies te studeer en sy het haar gemeenskapjare by Chris Hani Bragwanath Hospitaal gedoen – die derde grootste hospitaal ter wêreld.

En dit is létterlik ’n mediese druk-koker: daar is ’n gemiddeld 160 pasiënte per maand wat koeëlwonde het en daar is meer as 350 noodgevalle. Elke dag.

“Dit was aanvanklik intimiderend, maar tog ’n baie goeie leerskool en ’n tydperk wat ek vir niks sal verruil nie. Bara raak amper soos jou tweede huis as jy so lank daar is.”

“Dr. Nadine”, soos sy deur inwoners gedoop is, het by die staat aangebly en in psigiatrie gespesialiseer. Sy is tans besig met haar diploma in dié mediese rigting by die Universiteit van die Witwa-tersrand.

“Ek kry met pasiënte te doen wat met angs en depressie sukkel, of selfs aan skisofrenie of verslawing ly. Dit beteken vir my só baie as ek mense kan help.”

Maar jy moet ook nie die waarde van ’n algemene praktisyn onderskat nie, maan sy.

“Spesialiste fokus byvoorbeeld net op kinders, óf ’n spesifieke mediese kwes-sie. Daarteenoor moet jou huisdokter kennis oor ’n breë spektrum mediese aangeleenthede hê. En dit is nie altyd so eenvoudig soos wat dit lyk nie.”

Wat is ’n dokterspreekkamer nou sonder lekkergoed vir die jongspan? En op haar lessenaar is daar omtrent ’n em-mer vol suigstokkies.

“Hoe dan anders? Ek het ’n baie sagte plekkie vir kinders in my hart en deur bloot net na die dokter te kom, verdien hulle al ’n suigstokkie!”

Natuurlik het sy ook net lof vir haar oud-skool. Buiten haar akademiese prestasies, was sy ook op die atletiekveld in aksie, het provinsiale tennis gespeel én vir die eerste dogtershokkiespan uitgedraf.

“Die wete dat ek ’n verskil in iemand se lewe kan maak, laat my in die oggende met ’n glimlag opstaan. Ek lééf my passie hier in Middelburg. Dit is wat ek nog altyd wou doen,” sluit sy die onderhoud af met ’n glimlag nét so warm as waarmee sy jou gegroet het.

Hospice is afgelei van die Latynse woord, “Hospes”, wat beteken om uit te reik na dié wat lei, “die laaste tyd van jou lewe.”

Die Hospicevereniging het in 1999 hul deure in Middelburg geopen, met sr. Judy Toüa aan die stuur van sake.

Sy is vandag nog die bestuurder en saam met haar regterhand, sr. Corné Adendorff, hanteer hulle hierdie organ-isasie wat deur die gemeenskap vir die gemeenskap begin is.

Hul dienste is beskikbaar vir die hele groter Middelburg, asook alle kragstasies en plase in die omgewing.

Volgens sr. Toüa is hulle ’n outonome, onafhanklike organisasie wat die pasiënt bystaan vanaf die oomblik wat hulle met kanker gediagnoseer is, tot by die herstelpad.

In die geval wat die siekte te ver gevorder is, stap hulle ’n pad met die terminale pasiënt en sy familie.

Hul 24 uur dienste vir terminale pasiënte sluit in:

• Professionele pyn- en simptomebe-heer.

• ’n Hospi-tuisversorgingsdiens deur vrywillige werkers en susters Toüa en Adendorff.

• Adviserende verpleging.• Sterwensbegeleiding en berading

aan die families.Hul Hospi-tuisversorgingsdiens

behels dat hulle letterlik “die hospitaal na die pasiënt toe vat.”

’n Hospitaalbed, asook rippelmatras word voorsien, om die pasiënt gemak-liker te laat lê. Hulle verskaf ook bed-panne, krukke, rolstoele en looprame aan Hospice pasiënte.

Ander pasiënte, wat nie versorg word deur Hospice nie, kan hierdie toerusting teen ’n minimale bedrag by hulle huur.

Hul versorging by die pasiënt se huis, sluit in: bedbaddens en voorkoming en

behandeling van bedsere en kankerwonde.

“Ons bied hierdie diens aan kankerpasiënte, ongeag of hulle mediese fondse het of nie.”

Sr. Toüa sê dat daar drie tipes behan-deling van kanker is, wat insluit: chirur-gie, chemoterapie en bestraling.

By die Middelburg Hospicever-eniging kan pasiënte Dinsdae gehelp word met chemoterapie (mediese fonds pasiënte).

Op hierdie dag kom die praktyk van Drs Alberts, Bouwer en Jordaan na Middelburg waartydens die pasiënte gesien en chemoterapie vanaf 08:00 gedoen word. Volgens sr. Toüa hanteer hulle soms tot 15 pasiënte vir chemoterapie per dag.

Aangesien van die pasiënte vir etlike ure moet sit met die chemo-behan-deling, maak die vrywilligers seker dat hulle iets te ete en drinke kry en gesels opbeurend met almal.

In die Wintermaande word ’n snoesige kombersie oor hul bene gegooi en kan die pasiënte selfs kies uit ’n mus en ’n serp. Hierdie komberse en mussies word geskenk deur organisasies soos die Vrouelandbou Unie en persone wat die Hospice ondersteun.

Omdat die organisasie geen subsidies ontvang nie, is hulle totaal afhanklik van die gemeenskap se helpende hand en hul eie fondsinsamelingsprojekte vir oorlewing.

Van hierdie projekte sluit onder andere in:

• Die jaarlikse Hospicetee deur vrywilligers. Vanjaar was Jakkie Louw die gaskunstenaar by die tee wat by Rustique (Riverside) gehou is.

• Hul maandelikse pannekoekdag by die organisasies se kantore in Kogel-straat. Die Hospice pannekoektent word ook gereeld gesien by funksies en

sportby-eenkomste.

• Donasies uit die gemeen-skap.

• Pink Trees for Pauline projek wat hanteer word deur Lee Brandmuller en Jacqueline de Kok.

Die span word verder by-gestaan deur die Hospice Raad wat hulle help met hul fondsin-samelingsprojekte en die administra-tiewe sy van die vereniging.

It started off very small, with no casualty unit, only a few patient beds and a small number of theatres. It is all about service quality, but the thing is this, we as emergency medi-cine practitioners see this whole busi-ness from a total different perspective.For one we can be certain, Prof Bof-

fard will call this whole phenomenon a snake!

So I went to get a second opinion. The Marketing professor called this the “service quality expectation gap”.

It made sense: as provider I have certain expectations and so does my patient. These two sets of expectations are different - the “gap”.

Although I do not disregard the expectations of our patients, the very nature of our business dictate us to do things in a certain way - even if this will mean “poor service” from a marketing perspective.

Without referring to someone specific the following situation arises on a fre-quent base:

Mr Mike Migraine presents at the A&E, he explains to the triage officer that he has been experiencing another relentless migraine for the past hour. The triage officer request Mike to open a file while waiting for the doctor (who happen to be busy resuscitating another family’s loved one in ICU).

Within 10 minutes Mrs Dora Disloca-tion enters the unit - she was chasing after a thug who grabbed her purse, she fell and dislocated her shoulder.

The triage officer explained that the doctor would see her in 15 minutes.

The doctor, who is now trying to explain that the sixteen year old girl died from severe complications as a result of cerebral malaria, is called by a registered nurse (RN) saying that “the unit is starting to get busy and we are expecting an am-bulance within an hour - traumatic am-putation of the arm, apparently ripped of by a tractors’ PTO shaft”.

Cutting short the “breaking of bad news” the doctor rash down to A&E. As he enters the unit the triage officer ac-companies Mr Harry Heart-attack to the resuscitation bed.

As Harry removed his, now sweat soaked, shirt an ECG is done - bad news. Harry will eventually be the proud win-ner of a helicopter ride to one of the few cardiac units in the country.

While working on Harry, the doctor is informed that Mandy Malingering is (again) waiting on the other resuscitation bed - another fake epileptic attack.

The drama continued, but at the end

Mike went home without pain, Dora’s shoulder was back in place and Harry’s coronary artery was pumping blood again.

The doctor is heading back home - although a busy day at the “office”, all patients were managed within target times (triage) and all, except one, had a favourable out come.

When the lights go out at home he still thinks about the young girl that died - if she only took the prescribed prophy-laxis.

Three weeks later and the complaints is piling up: “I had to wait more than 90 minutes with a terrible headache, the doctor was on the telephone - some medical aid had an issue to fly a person around.

“Why should I suffer because of another patient’s poor medical aid? To make matters worse they helped at least three patients before me!

“I would say one of them was serious - she had a kind of epileptic attack! When I tried to complain to the sister-in-charge she just looked at me and say I must write this complaint because she needs to go, a farm worker injured his arm. Re-ally, does anyone care?”

Well, this is a long story, but at least explain the point: Emergency Medicine and Service Quality rarely co-exist in the

endorse the concept of “triage”, derived from the French word “trier”, meaning “to sort”.

This concept was adopted from the military to sort between the large num-bers of casualties on the battlefield. The country of freedom, dreams and consum-erism started with the idea that a patient is a client. It came back to bite them (as usual) and A&E now became better known as “All and Everything” instead of “Accident and Emergency”.

Simply adding more doctors was not the solution. To create a balance between scarce skills and funding issues a process was adopted (triage) to assign an objec-tive priority to each patient coupled with a target time to see the doctor.

The degree of perceived urgency from the patient’s side plays little role in a changing the priority level. In practical terms this might include a nasty head-ache or crying child that take second place to another life on the edge.

It is well accepted in America, Aus-tralia, Europe and South Africa that a patient with a low triage score can wait for 240 minutes!

At Middok24/7 the triage protocol is easy to understand and highly efficient: the triage officer meet you at the door to take a brief history and a set of physi-ological measurements.

This will give an objective screen-ing of all patients in order to prioritize everyone. With “life or death” conditions excluded, a request to start the registra-tion process will be made.

While opening the file, the triage score will be discussed with a senior experienced healthcare worker. The score will then either be confirmed or changed

according to senior advice.Seems like a mouth full, but the

system is designed “for the greater good” - saving lives not earning satisfaction scores.

If still not certain about this unit in the “Platteland”, have a look at the following facts:• Nearly all medical practitioners holds a post-graduate qualification. These include post-graduate diploma in EM, masters degree in clinical pharmacology or family medicine.• All practitioners are Advanced Cardiac Life Support and Paediatric Life Sup-port accredited, some as experienced providers. Ongoing studies include E-FAST certification (a new concept of sonar being used in high priority cases to reduce time to diagnosis) and Member-ship of the Royal College of Emergency Medicine (UK).• Our unit is certified as a stroke unit with the Boerhinger initiative known as “mysstroke.co.za”.Other hospitals include Life Cosmos and Life Groenkloof (the latter being the only registered unit in Pretoria).• The heart attack protocol is fast and very successful. The process that includes initial evaluation, rescue thrombolysis (to buy critical time), stabilization, transfer and up until exiting the cardiac theatre with a stent can be done within four hours.Driving straight to the cardiac unit will add no benefit. While you drive without thrombolysis, the heart basically dies due to a lack of oxygen.Once you get to the city a cardiac work-up still needs to be done. At the end of the day you will be unstable and the time from your heart attack and a stent place-ment will be longer. The damage to the

heart ends up significantly worse.• With one of the highest number “Prior-ity 1” cases per doctor in the Life Hospital group, the doctors can be regarded as highly experienced in managing critical injured patients.We have a very good team of dedicated doctors, but recently had to say good-bye to one of them. The grass is quite green in Europe, or that was the impres-sion. On a recent visit I was handed a London newspaper. Some of the reports were: • “..the number of people waiting longer than four hours in hospital accident and emergency units...rose by a third...”• “...thousands of patients are waiting more than 18 weeks for operations... and at a particular hospital there were 5 524 patients waiting (last week) on a trolley for a ward bed between four and 12 hours...”• “Sick patients have been forced to wait up to eight hours in ambulances queuing outside A&E units...”

Not quite my cup of tea. I can proudly say that I am part of an emergency medi-cine team that can be matched-up with the worlds’ best units.

As jy die toonbeeld van ’n ge-sonde lewe wil sien, hoef jy nie verder as Magda Nieuwoudt te soek nie.

Nie net het hierdie jong vrou ’n ge-sonde liggaam nie, maar huisves sy ook ’n gesonde gees, hart, siel en buitengewone sterk kop.

Magda is ’n sportwetenskaplike by Hoërskool Middelburg en ’n aspirant driekampatleet wat op pad is om ’n pro-fessionele driekamper te word.

Van haar prestasies sluit in, die 2015 Ironman 70.3 wêreldkampioenskap in Oostenryk waar sy 33ste in haar ouder-domsgroep gekom het, haar onlangse oorwinning tydens die MiWay SunCity Ultra en nog vele meer.

Magda het ’n BSc graad in Mens-like Bewegingskunde en Voeding, en honneursgraad in BSc Sportwetenskap wat sy aan die Noordwes Universiteit se Potchefstroomkampus verwerf het.

Get It het met Magda gaan gesels oor ’n paar van haar geheime oor ’n gesonde lewe.

“Van kleins af het die menslike lig-gaam my geïnteresseer en wou ek ge-sond lewe. Toe ek nog baie klein was, het ek altyd die hoender se velletjie afgehaal en sou dit nie eet nie - dit was nou voor ek die lae koolhidraat en hoë vet leefstyl begin volg het,” lag Magda.

Nog nooit kon jy haar bederf met ’n wegneemete of uiteet nie.

Van graad vier af het Magda elke maand die Runner’s World ontvang en het dit gelees soos haar tweede Bybel.

“Al die fisiologiese inligting het my fassineer. Wat om te doen, wat om te eet

en wat nie. Ek dink ek het onbewuste-lik reeds besluit dat ek eendag fisiolo-gie en voeding sal gaan studeer.”

“Hoe jy grootword het beslis ’n invloed op jou lewe en ek glo dit het bygedra tot my keuse van ’n gesonde lewe. My twee ouer broers het rugby gespeel en ook tweekamp gedoen.”

Magda het ook nog altyd self sport gedoen en het reeds in graad 6 besluit dat sy ernstig gaan begin oefen en eendag wil kompeteer in sport.

“Ek het elke dag voor skool gaan hardloop en elke middag na skool ook geoefen. Ek sou selfs my laaste periode by die skool mis om te gaan oefen, want netnou oefen iemand meer as ek,” vertel Magda laggend.

“Ons het elke Saterdag rugby gekyk en sodra dit halftyd is, sou ek dan gou in daardie 10 min my drie kilometer roete gaan doen het. Sondae het ek ‘gerus’ wat behels het dat ek ’n easy run gaan doen het.”

Magda sê jy moet die liggaam ken en verstaan as jy werklik goed wil oefen.

“Ek het op universiteit die teorie vinnig deurgegaan en dan dadelik daarna gaan oefen. Soos ek geoefen het, het ek vir myself gesê wat nou presies met my liggaam gebeur.”

Magda oefen deesdae by Trifactri, ’n driekampoefengroep wat deur die professionele driekamper, Lucie Zelen-kova, afgerig word. Haar droom is om eendag ’n professionele driekamper te wees en sy oefen elke week tussen 20-27 ure – dit is nou terwyl sy ’n voltydse werk het en naweke op die sportveld

moet wees vir die leerlinge.“Jy moet nooit sê jy’t nie tyd om te

oefen nie. Alles in die lewe is op tyd en juis daarom sal jy alles kan inpas. Dit is vreemd hoe baie jy kan doen as jy eers bewus word van tyd.”

’n Gesonde lewe is vir Magda so een-voudig soos ’n besluit wat jy neem.

“Moenie te veel daaroor redekawel nie. Besluit dit en doen dit. Op die ou einde gaan dit eintlik oor jou gees wat gesond is. Daar is niks lekkerder as om gemaklik te voel in jou eie self nie. As jy gemaklik is in jou eie self, kan jy enigiets in die lewe doen.”

Maar ’n mens bly maar ’n mens sê Nieuwoudt en vertel dat sy wel ’n swak-punt vir kondensmelk en koekies het.

Magda ontspan wel ook graag wan-neer sy ’n tydjie kry en, glo dit of nie, sy is ’n groot aanhanger van die kunste.

“Gee vir my ’n smaakvolle toneelstuk of musiekvertoning, en ek ontspan heerlik. Maar my gunsteling is om blok-kiesraaisels in te vul met ’n glas goeie rooiwyn in die hand - vir die antioksi-dante, jy weet.”

Magda glo dat sy op 80-jarige ouder-dom steeds aan die Ironman sal deel-neem en som die besluit om gesond te lewe soos volg op:

“Vra jouself af of jy volheid wil lewe tot op jou oudag. Dit wat jy eet, kan of sleg of goed wees vir jou liggaam. Jy word ouer en sal sterf omdat jou liggaam degenereer, so jy wil alles in jou vermoë doen om daardie proses te vertraag. Genoeg slaap, reg eet, oefen en jou Godsdiens moet alles in balans wees om gesond te kan lewe.”

The upgrade saw the best combination of equipment to offer the latest generation of technology. Neurosurgeon, Dr Gerrit Engelbrecht stated “The equipment includes the Brain Lab, Leica (operating microscope) and Hybrid Neuro Table. The Brain Lab navigation system enables accurate pre-operative planning of procedures and the Leica Operative Microscope allows for better visualisation. As for patient safety and comfort, we have a specialised hybrid operating table with carbon fi bre frame to position our patient for optimal intra-operative fl uoroscopy.

There is also excellent news for individuals with vertebral compression fractures. To provide relief from the pain of a vertebral fracture and prevent further collapse, two types of minimally invasive procedures are available. These procedures, Vertebroplasty and Kyphoplasty, are used in cases of functionally disabling pain caused by a vertebral fracture that does not improve over a number of weeks with pain medication and immobilisation.

“Ek is ’n gekwalifiseerde kraamsuster, of eerder bekend as ’n vroedvrou. Hierdie is my passie,” het sy na haar kantoor gewys. Mariska is oorspronklik van Vereeniging, waar sy in 2000 gekwalifiseer het en vir sewe jaar in die kraamsaal by Medi-Clinic gewerk het. Daarna het haar pad met eMalahleni gekruis en hier het sy vir ongeveer twee jaar in Klipfontein by die Suid-Afrikaanse Nasionale Bloeddiens (SANBD) gewerk. “Dit is hier waar die saadjie geplant is om my eie kliniek te open. Soveel mammas het ingekom en gevra ek moet hulle kinders inent. Maar omdat ek nie by ’n kliniek werksaam was nie, was dit teen die reëls. Ek het begin navorsing en bemarking doen, en kon nie glo dat hier nie so plek is, behalwe die staatkliniek nie. Ek hou daarvan om meer van die kind te weet en om ook eers bietjie met mamma te gesels,” het sy verder vertel. Haar twee kinders is die riglyne agter haar advies, aangesien die persoonlike ervaring eerstehands betrokke is by elke mamma wie sy onder haar vlerk neem. “Boeke leer jou net soveel. Elke kind verskil. My dogtertjie is op 32 weke met ’n noodkeiser gebore en het maar ’n

skrale 1.7kg geweeg. Sy is al 11 jaar oud en dan het ek ’n seuntjie van 21 maande. Hy is op sy tyd gebore en albei is perdfris en gesond. Met al die mammas met wie ek praat, sê ek vir hulle wat ek gedoen en gebruik het. Ek verduidelik wat die beste vir my gewerk het, en hoekom. Daar is niks wat my meer grief as mammas wat vir ‘Google’ gaan raad vra nie. Mammas wat sommer net iets probeer wat hulle op Facebook gesien het. Dit is ’n verskrik-like gevaarlike roete om te volg. Ek het ’n Facebook bladsy waarop ek raad aanbied en ek maak ’n punt daarvan om twee keer ’n dag mammas se vragies te beant-woord. Ek is altyd beskikbaar, maar ek trek die streep wanneer mammas bel vir ’n uitslag of ’n hoes of enige soort siekte, as ek nie die kind gesien het nie. Ek is verantwoordelik vir elke stukkie raad wat ek uitdeel en as ek nie die kind gesien het nie, kan ek nie net links en regs sê wat ek dink makeer nie,” het sy ook vertel.

Om ’n dokter te word was nooit vir haar ’n opsie nie. Sy het vertel dat dit die persoonlike effek uit haar werk uit haal en haar grootste vrees, was dat sy haar passie sou verloor en ook die persoon-like kontak met haar mammas. “Ek het ook besef dat die aanvraag hoog is vir ’n vroedvrou in eMalahleni, maar ek moes kies tussen dit en my kliniek. Die kliniek lê my nader aan die hart. Ek het nie kans gesien om onbepaalde tye nie by die huis te wees, wanneer ’n vrou moet kraam nie. Ek kan nie my kliniek opgee nie. Ek is te lief hiervoor,” het Mariska ingelas. Voorgeboorte klasse, borsvoedings-kon-sultant, immunisering van babas, advies oor koliek, slaap probleme en ‘potty-train-ing’ is maar net van die goed wat op haar advertensieborde pronk. “Soveel kinders verdrink en ek voel daar is ’n verskriklike groot behoefte aan die soort opleiding vir mammas. Hoeveel kinders val in swembaddens en sit met breinskade na die tyd? Mammas moet kardiopulmonêre resussitasie (KPR)

kan doen om daardie kinders uit die gevaarsone te probeer kry. Ek is ’n KPR en eerstehulp instrukteur, en hierdie oplei-ding gaan ook binnekort begin. Daar is nog baie dinge in die pyplyn waarmee ek besig is. ‘Nanny training’ en ’n mobiele kliniek is net ’n paar dinge wat ek in ge-dagte het,” was haar verdere woorde.Nie net vestig sr. Mariska haar by haar kliniek nie, maar sy gaan ook na skole toe om kinders se immunisasies daar te doen. Sy het verduidelik dat kleuterskole haar mark is en dit is waar die mobiele kliniek sal floreer. Sy doen dit reeds, maar op kleinskaal. “Ek leer my mammas wat ek wou weet met my kinders. Dinge wat nie in die handboek geskryf staan nie en dinge waarop jy as mamma nie voorbe-reid is nie. Ek fokus op die onverwagte en wat jy in sulke situasies moet doen,” het sy ook vertel. Haar warm gevoel wanneer mammas terugkom met ’n tweede of derde kind, is wat haar kliniek se hart laat klop.

“Daar was al rotse en klippe in my pad gegooi maar ek stof my skoene af en klim oor. Die Here is baie goed vir my en ek gaan net elke aand op my knieë en sê dankie vir waar ek is. Ek voel goed wanneer mammas terugkom en ek kan help wanneer niemand anders kan nie. ’n Mamma verstaan en ek is bly dat ek an-der kan help. ’n Gelukkige mamma sorg vir ’n gelukkige baba en die gelukkige huis pluk ook die vrugte,” was Mariska se laaste woorde. Oor haar toekomsplanne is sy selfverse-kerd. Maar wanneer die onverwagte haar tref, het sy gesê dat sy altyd planne het en nooit gaan lê nie. Sy het gesê dat wan-neer jy uit jou gemaksone geruk word in vandag se ekonomie, sy beslis die behoefte sal raaksien en aanpas, sou die nood dit in die toekoms vereis. Mariska se moeder-hart het haar gedra tot waar sy vandag is en sy sien uit na nog baie jare met die gemeenskap van ons Steenkoolstad.

dis die suksesresep. As jy dit vir jouself gaan moeilik maak deur ’n strawwe dieet te probeer volg, gaan jy vinnig moed verloor en dan kom die wiele af.

Byna elke dag hoor jy van iemand wat net nog ’n dieet volg. In dieselfde asem hoor jy van mislukte diëte en motiverings in maer-maak pilletjies wat die vetjies weg smelt of won-derkure wat die hongerpyne stil om maar net weer van vooraf te begin.

Vandag pas Rochelle gemaklik in ’n broek grootte van 30 of 32 waar sy voorheen in ’n 38 of 40 denim ingepas het. Haar hempies was voorheen hemde van groot tot ekstragroot, nou wissel dit

na gelang van die maak tussen klein en medium, haar borste is ook vier mate kleiner.

Rochelle se verhaal is een waarmee die meerderheid van vroue hulself vereenselwig. So begin haar verhaal, “In 2014 het ek ’n skaal gehaat, ek was toe 82kg. Na die geboorte van my twee woelwaters, het ek gesukkel om die kilogramme af te skud. Ek het met elke swangerskap so 12kg in totaal opgetel,

wat nie baie was nie maar wat moei-lik was om elke keer te verloor. Ek het met altwee ’n keiser gekry, wat dit nog moeiliker gemaak het om my lyf terug te kry. Ek was moeg gesukkel om klere te kry wat pas en mooi aan my sit. Ek het omtrent elke klimtoldieet ná die ander gevolg, maar dit wou net nie werk nie,’ eggo sy miljoene ander vroue se nood-kreet.

’n Verjaarsdaggeskenk van haar man, Wynand, het haar hele lewe verander.

“Na baie ure in die winkelsentrum op soek na ’n geskenk, was ek al moedeloos. Maar ek het ’n advertensie in WITBANK NUUS raakgelees en besluit om nie verder te soek nie, ek het my verjaarsdag-geskenk gekry.”

Sy het by ’n gewigsverlies- en gesond-heidskliniek aangesluit, die eerste stap na ’n leefwyse wat sy sonder moeite haar eie gemaak het.

“In Februarie 2014 het ek by die kliniek begin en met ’n eetplan afgeskop. Ek kan met eerlikheid sê ek was dadelik negatief en het gedink dis té streng en dat ek dit nooit sal kan volhou nie. Ons was gewoond aan baie eet, naweke het leë pizzabokse, ‘chips-pakke’ en leë Coke bottels ons asblik versier. My grootste vrees was dat ek honger sou word met die nuwe eetplan, maar inteendeel. Water drink was nie een van my sterk punte in die lewe nie, ek was verslaaf aan Coke.”

Rochelle moes skielik leer hoe om reg te eet en alles was makliker gemaak met die hulp van die personeel by die kliniek wat haar met elke stap ondersteun het.

“Daardie personeel was kwaai en streng,” het Rochelle tong in die kies gesê.

Elke keer wat sy geweeg en gemeet was en hulle kon sien een gram het van iewers af ingesluip, wou hulle dadelik weet of sy die eetplan noukeurig volg.

“Dit was baie hard en moeilik in die begin, maar na ’n paar weke toe ek die

resultate begin sien en voel, was ek net meer gemotiveer. Water is nou omtrent al wat ek drink asook Rooibos, Groentee en Decaf koffie. Ek gaan nêrens sonder my waterbottel nie. Wegneemetes en pizza is nou iets van die verlede en ek drink glad nie meer so vreeslik baie Coke nie. As ons gaan uiteet het sou ons burgers en ‘chips’ bestel, nou bestel ons ’n lekker gesonde ‘wrap’ met slaai en water of koffie,” het sy een van haar geheime gedeel.

Om die vetjies vinniger te laat ver-dwyn, het sy ook begin ‘bootcamp’

Na ses maande het Rochelle aan ’n aanlyn liggaamstransformasie kompeti-sie deelgeneem wat oor 12 weke gestrek het.

“Ek het ’n aanlyn instrukteur gehad wat vir my elke vier weke ’n eet-en oefen-program gegee het. Ek moes elke vierde week vir haar foto’s stuur. Hierdie kom-petisie het vir my daardie ekstra hupstoot gegee en tydens die 12 weke het ek nog 4kg en ’n paar ekstra sentimeters verloor.”

Rochelle het onder die top 30 deelne-mers uit die 250 deelnemers landwyd geëindig. Ongelukkig het sy in die tweede rondte uitgeval.

In Januarie verlede jaar het Rochelle by ’n plaaslike gimnasium aangesluit. Daar is sy gevra of sy sou belangstel om aan liggaamsboukompetisies deel te neem. Vier maande later het sy die lig-gaamsfiksheidkompetisie van die 165cm afdeling gewen en net daar het die kompetisie-gogga gebyt.

Om soos Rochelle te lyk, verg baie deursettingsvermoë. Haar dag begin vroeg in die oggend wanneer sy haar twee woelwatertjies by die skool aflaai. Tussen 07:30 en 08:00 rig sy ‘bootcamp’ af, dan is dit gimnasium toe waar sy op gewigte en kardio konsentreer. As sy klaar geoefen het, gaan sy huis toe waar sy hul besigheid se papierwerk doen, kwotasies en fakture deurstuur en kliënte

bel . Teen daardie tyd knipoog die horlosie al op 13:00 en is dit weer tyd om die woelwaters te gaan haal, dan is dit huis-werk doen. Teen 15:00 sal jy haar weer in die gimnasium kry. Sy moet ook tyd maak vir die kinders se buitemuurse aktiwiteite. Tussen 17:30 en 18:30 rig sy weer ‘boot-camp’ af.

“Ek is 19:00 by die huis. Dit vat my gewoonlik ’n uur om die kinders te bad, kos te gee en teen 20:00 is hulle in die bed. Dan spandeer ek en Wynand kosbare tyd saam terwyl ons, ons etes vir die volgende dag voorberei. Ek eet ses etes ’n dag en elkeen word individueel verpak.”

Wanneer sy voorberei vir ’n kompetisie volg sy ’n spesiale dieet wat afhang van hoe sy wil lyk vir die kompetisie.

“Ek wil nou so bietjie groter word so ek eet heelwat meer proteïene en stysel. My oefen- en eetprogram verander elke vier weke, sodat my lyf nie gewoond raak daaraan nie en heeltyd kan verander. Ons mag geen lekkernye, gaskoeldrank, alkohol, ‘chips’ of sjokolade eet nie. En glo my dis vrek moeilik, want mens raak lus vir daai lekker goedjies. Maar as jy resultate soek moet jy jou eetprogram met 110% volhou. Daar is nie ’n kortpad nie, dis ’n leefwyse.”

Wynand oefen saam met Rochelle en sy beskryf hom as haar grootste inspirasie.

“As dit nie vir Wynand se bystand en ondersteuning was nie, sou ek nie gewees het waar ek vandag is nie. Dis nie maklik om te kom waar ek nou is nie, ek wou al so baie keer opgee en handdoek ingooi. Daar was al baie trane die afgelope twee jaar. Maar hy was daar vir my deur dik en dun en het my aangemoedig en aange-spoor. Hy druk my harder as hy dink ek kan beter doen. Bystand en motivering is die belangrikste om mens te help om aan te gaan. Dit maak die leefstyl net soveel makliker omdat ek en Wynand dieselfde passie deel.”

The initial excitement often gives way to the reality of a chronically sick child. This is only made worse by different advices from every-

body around you mixed with a degree of criticism of your parenting skills if not interference by the grandparents.

It is important for you as parent to understand as much as possible about the reasons behind crèche acquired in-fections, their possible prevention as well as reasonable treatment in order to make the right decisions that will affect your child’s and your own wellbeing.What is Crèche Syndrome

Large numbers of kids in a confined space allow organisms such as viruses to spread easily from child to child. Sharing of dummies, bottles, eating utensils and toys help spread disease even though measures are taken to minimize this sharing.

In addition little children have an im-mature immune system. They are relative-ly new “earthlings” and as such have not been exposed to all the bugs and germs that an adult has already dealt with and built up immunity against. Hence every virus introduced into the day care centre is most likely unknown to your baby’s immune system and therefore results in a full-blown cold or flu.

Remember also that many children in day care have older siblings in pre-school or school, who are a further outside source for infections brought into the crèche.

Another important point is the ability of viruses to change their appearance (mutate). This allows them to re-infect a child that has already built up immunity to the virus before the mutation. Despite the fascinating abilities of our immune system we have to acknowledge that viruses are in fact two steps ahead of us.

Finally several anatomical features in small children render them more sus-ceptible to infections. This can be simply due to smaller diameters of air pipes in the lungs or length of e.g. the Eustachian tube resulting in bronchiolitis or middle ear infections.

Considering the above conditions, the stage is set for frequently recurring and more often chronic infections in the crèche-going child.

Therefor crèche syndrome is not a disease as such but continuous infec-tions and illnesses that often go over into one another without healthy peri-ods in between.The Maelstrom of Crèche Syndrome

The chronically runny or blocked nose, coughs, raised temperature, lack of appetite, frequent waking at night, fidgeting with the ears, skin rashes, bouts of diarrhoea, sore throats and vomiting become part of your daily life. Feedback from the caretaker that your child is feverish or refuses to eat worsens the already gnawing feeling of guilt. Medical or specialist help often fails to make a difference. Hospitalisation is fre-quently necessary. This in turn puts more

pressure on the parent trying to balance between work, household, family and partner. The resulting stress is further aggravated by confusing and contrarian advice from media, family, friends, col-leagues and medical personnel.

Crèche syndrome does not last forever. Your child’s immune system strengthens and becomes better and better at identifying and dealing with bugs surrounding us. After the first two to four years you will notice that infections become fewer and often less severe. Eventually your child has basi-cally the same number of colds or flu as you have as an adult.

The number of kids in a crèche is directly linked to the frequency of infections. The smaller the number the smaller the pool of organisms. Although children in a small play-group will still share viruses, it is often a distinct episode followed by weeks or even months of no health issues rather than permanent infections that become indistinguishable from one another. Ide-ally groups of not more than five to six children should be sought out.

If at all possible delay, the entry into the crèche until the child is two-and-a-half to three years old. Firstly many of the anatomical shortcomings of very young kids will have been overcome and hence the child will almost have “outgrown” specific “size-related” health problems. Secondly the older child can communicate much better and is able

to e.g. localize pain or cooperate better towards obtaining a diagnosis or during administering treatment.Crèche Syndrome Cannot be Cured. More important than anything else: make peace with the fact that your child’s health is not going to be the same as before they went to crèche. Often the children are less perturbed by their runny nose, cough or temperature than their parents. It is therefor paramount to approach the ensuing medical conse-quences of crèche-going with reason, empathy (for the child and yourself as parent) and above all knowledge. This is equally important for the medical staff that is being consulted.

The desperate parent frequently seeks medical or other help in order to escape the repeated or chronic nature of the child’s symptoms. However parent’s ex-pectations of a completely healthy child can simply not be met. Remember crèche syndrome is “incurable” and you have to accept some symptoms showing that your child’s immune system is working.

Instead parents put pressure on themselves and health professionals to ”cure the incurable”. Both parties are at fault when this results in unnecessary, sometimes harmful and often expensive

tests, investigations and therapies.Due to the ongoing nature of the

child’s symptoms chronic medication is often initiated, which fails to improve the condition. All medication has side effects, which might even cause worsening of your kid’s condition.

Antihistamines or drying-up medica-tion will reduce the runny nose but at the same time thicken mucus in nose, si-nuses and lungs. Thick secretions cannot be cleared so easily by the membranes and can clog passages in nose or lungs leading to worse infections.

Antibiotics are often given liberally for infections caused by viruses, for which they do not work. They will however kill good bacteria in our body. This in turn negatively affects our immune system. Wrong use of antibiotics will also lead to the development of resistance and when it really becomes mandatory to use them they may not be able to eradicate the offending bacteria.

Investigations must have a definite goal and should, depending on the findings, influence further treatment. If regardless of the test result, the treat-ment will be the same, the test might not be warranted. Blood tests are always associated with a painful needle prick.

X-ray and especially CT investigations subject your child to radiation. Especially when such investigations are performed repeatedly it can lead to harmful doses of radiation on the growing child.

Again sound knowledge on your part as parent and mutual trust between you and the doctor will prevent these unfa-vourable scenario.

Braai dit liggies in die kokosneutolie en bêre in ’n bakkie.Braai nou die hamburgerfrikkadel in die-selfde pan en sit die sny kaas bo-op. Haal die hamburgerfrikkadel uit en bak nou die eier ook in dieselfde pan.Pak nou die soetrissie-mengsel, ’n paar snye komkommer en die eier bo-op die hamburgerfrikkadel.
