Minor Injury: reported in Aug (1) and in Oct (1), Non-Work Related
Fatality: reported in Nov- two(2) deaths from a vehicular accident
from an outsource IGT service vehicle used in the project was
traversing in a hi-way going to a site about 50 kms away, met a
motorbike moving from an opposite direction who tried to overtook a
truck then encroached the other lane, and accidentally hit the
approaching outsource IGT service vehicle that have resulted to 2
fatalities (the motorbike driver and its passenger)

Near Miss: (1) Re-bar stirrup stored at edge of the excavated pit that
may cause injury if it fall to workers, (1) sub-standard construction
ladder that can cause injury if used by heavier working personnel. (1)
earthing system are not in-place in the tower leg that may endanger
the equipment. Non-Work Related Fatality: reported in Nov- two(2)
deaths from a vehicular accident from an outsource IGT service

Near Miss: One(1) unsafe Earthing system that can damage
Equipment, Two(2) scattered formworks with nail on it that may cause
injury to workers, One(1) soil erosion might extend to the equipment
pads and damage it, One(1) substandard ladder used on site that may
collapse and can cause injury to worker, One(1) broken mechanical
winch that send the tower material being lifted to descend back to fall
to ground that resulted to damage tower member but no injury, One
(1) sub-standard tower lifting equipment that may cause to
breakdown if use on-site and may cause injuty to workers. One (1)
Minor Injury due slip while going up and using staircase in the office.
One(1) vehicular incident carrying tower materials and experience
mechanical trouble (brake system) while ascending to a mountainous
road that resulted to fall to a ravine but cause no Injury.

DEATHS
If there is an accident connected with work and your employee, or self-employed person working on the
premises or a member of the public is killed.
MAJOR INJURIES
If there is an accident connected with work and your employee, or self-employed person working on the
premises sustains a major injury, or a member of the public suffers an injury and is taken to hospital from the
site of the accident
Reportable major injuries are:
fracture
amputation ( is the removal of a body extremity by trauma or surgery) ;
dislocation of the shoulder, hip, knee or spine;
loss of sight (temporary or permanent);
chemical or hot metal burn to the eye or any penetrating injury to the eye; injury resulting from an electric shock or electrical burn leading to unconsciousness, or requiring
resuscitation ( is an
emergency procedure which is performed in an effort to return life to a person in cardiac arrest ) or
admittance any other injury: leading to hypothermia ( is a condition in which core temperature drops below the
required
temperature for normal metabolism) , heat-induced illness or unconsciousness; or requiring resuscitation;
unconsciousness caused by asphyxia ( is a condition of severely deficient supply of oxygen to the body
that acute illness requiring medical treatment, or loss of consciousness arising from absorption of any
substance by
Acute illness requiring medical treatment where there is reason to believe that this resulted from exposure
to a

If there is an accident connected with work (including an act of physical violence) and your employee, or a self-
employed person working on your premises, suffers an over-three-day injury you must report it to the enforcing
authority within ten days.
An over-3-day injury is one which is not "major" but results in the injured person being away from work OR
unable to do their full range of their normal duties for more than three days.
MINOR (ABSENCE FROM WORK 1 – 3 DAYS) INJURIES
A minor injury is one which is not "major or moderate" but results in the injured person being away from work
OR unable to do their full range of their normal duties for 1 – 3 days.
REPORTABLE DISEASES
If a doctor notifies you that the employee suffers from a reportable work-related disease
Reportable diseases include:
certain poisonings;
some skin diseases such as occupational dermatitis, skin cancer, chrome ulcer, oil folliculate/acne;
lung diseases including: occupational asthma, farmer's lung, pneumoconiosis, asbestosis, mesothelioma;
infections such as: leptospirosis; hepatitis; tuberculosis; anthrax; legionellosis and tetanus;
Other conditions such as: occupational cancer; certain musculoskeletal disorders; decompression illness
and hand-
REPORTABLE DANGEROUS OCCURRENCES (NEAR MISSES)
If something happens which does not result in a reportable injury, but which clearly could have done, then it
may be a dangerous occurrence which must be reported immediately
Reportable dangerous occurrences are:
Falling of person, material or equipment into the excavation (more than 1 meter deep)
Falling of material while lifting with crane
Falling of material while manual lifting
Failing of lifting equipment (breaking of rope or breaking of pulley)
Falling of tools from more than 2 meter height (tower, scaffold, ladder, building, etc.)
Falling of a person from more than 2 meter height but there is no injury
Electrical shock (resulting in no injury)
Breaking of disk of a cutter or bit of a drill machine,
Collapse of scaffold over 2 meter height
Any fire incident at site

plant or equipment coming into contact with overhead power lines;
electrical short circuit or overload causing fire or explosion;
collapse or partial collapse of a scaffold over five meters high, or erected near water where there could be
a risk of
explosion or fire causing suspension of normal work for over 24 hours;
· Accidental release of any substance which may damage health.
