The goal of Park Ridge Wellness is to improve the health and well-being of all Park Ridge Health

associates and their families. By encouraging healthy behaviors through a nourishing

environment and providing tools, resources and education, we can support a healthful, vibrant

workplace. Your Wellness Team is a progressive and talented group of trained professionals,

and we are here to support and guide you along any path of your wellness journey.

The Point System Program is intended to improve overall health in both the work and home life of our

associates and their families, as well as improve camaraderie between team members. We promise that

working toward your personal goals using the Point System will be fun, effective and empowering.

Park Ridge Wellness is also excited to bring you the Creation Health Series. This program reflects

our belief in teaching and learning the best lifestyle practices for the whole person, both mental

and physical. This wellness program is based on Biblical principles found in the Creation story

and supported by evidence-based science. This eight-week series offers many tools such as

daily health tips, mobile apps, recipes and healthy news to make change a little easier.

Please take the time to enjoy all of your wellness options in 2013. We are looking forward

to a culture of wellness radiating throughout our workplace and homes.

Interpersonal relationships strengthen our well- being. Social connection fortifies our resolve and improves health.

Nutrition is the fuel that drives the whole system. Small improvements and strategic substitutions produce profound results.

Associate Wellness Points System

All Park Ridge Health associates are encouraged to participate in the Employee Wellness Program. Participation in the Employee Wellness Program is without cost to all full-time and part-time employees.

AssociAte FinAnciAl incentives: Participants must be covered under the Park Ridge Health health insurance plan to be eligible for the financial incentives. The Employee Wellness Program financial incentives are available as a method to offset health plan premiums.

Wellness Points: 1 point = $1 Maximum Eligibility: Employees 100 points per quarter Spouses 50 points per quarter

Procedure For submitting wellness Points:

1 - ARC Submission: Visit the Wellness Arc page and submit points under the “My Park Ridge Points” link 2 - Accountability Form: Some activities may require this form. Each activity specifies need. The form can be found on the Wellness Arc Page.

To obtain a copy of the Wellness Program policy or to obtain more information on wellness activities, please contact Park Ridge Wellness at 828.687.5290. If it is unreasonably difficult for a Wellness Program participant to follow the standards in this program, or if it is medically inadvisable for a Wellness Program participant to follow the standards in this program, please contact Julie Palmer, Wellness Coordinator, at 828.687.5290. The Wellness Department will work with participants to develop an alternative method to qualify for incentives. The Wellness Points System program may be modified or cancelled at any time at the sole discretion of Park Ridge Health.

HrA - AnnuAlly 80 points employee/40 points spouse(points to be rewarded on a quarterly basis = 20 points per quarter/10 points per quarter)Participating in a Health Risk Assessment will provide a snapshot of your current health status and enable you to track your health over time. An HRA can give you the concrete information you need to prepare for lifestyle modifications.

It can judge whether a treatment plan is working or show you how healthy choices can make a difference in diabetes control.

body comPosition AnAlysis - QuArterly 20 points employee/10 points spouse Using the Tanita body composition analysis to determine body fat vs. lean muscle mass is the best way to ensure you are staying on track with your wellness goals. ACSM guidelines are the gold standard in body composition.

It is designed to meet you where you are in your journey to wellness. Coaching provides the information and support needed to reduce unhealthy behavior and obtain your wellness goals.

Smoking is dangerous to your health. The American Lung Association’s gold standard in smoking cessation assistance is “Freedom from Smoking.” This highly structured program focuses on behavior change, and has been evaluated and validated by people just like you.

stress mAnAgement - AnnuAlly 10 points employee/5 points spouse In partnership with Pfizer, Getting Healthier Together Stress Management is designed to help you make positive changes to decrease risk factors for cardiovascular disease. This program is highly interactive to engage participants and support self-learning.

rivAl Fusion –QuArterly 30 points employeeRival Fusion is an inexpensive comprehensive online fitness and nutrition program for those who prefer a more structured program. Enjoy a daily email that specifies your custom created workout and meals for the day. Rival Fusion can accommodate specific diets (vegetarian, high blood pressure, etc) as well as injuries or disabilities. Earn Wellness Points by taking a quarterly “Fit In” to determine your progress. vitAmin-d testing - AnnuAlly 10 points employee/5 points spouse Research suggests vitamin-D is linked to 17 varieties of cancer, as well as heart disease, stroke, hypertension, autoimmune disease, diabetes, depression, chronic pain, osteoarthritis, osteoporosis, muscle weakness, birth defects, periodontal disease and other concerns. weigHt mAnAgement - QuArterly 100 points employee/50 points spouse Dr. Richard Schaffer of M.D. Wellness and Weight Management offers a three-month package that includes six visits. Dr. Schaffer works one-on-one with his patients, teaching them how to eat soundly for healthy weight loss. With the resources, tools and expertise from Dr. Schaffer, you will be well on your way to a healthy weight. There is an out-of-pocket charge to the employee of $419. YourHSAfundscanbeapplied.

The Spa at Park Ridge Health is now offering monthly memberships! For the low employee rate of $45 monthly, you will receive one hour of services each month.

Fitness facilities will generate an automatic report each quarter to track your number of times to the gym. However, remember you need to submit your points on the ARC each quarter. If exercising at another facility, please fill out the accountability form and submit your points on the ARC.
