Welcome!

We are pleased to provide you with this Employee Benefits Overview for purposes of assisting you in your benefit se-lections during our open enrollment process. This booklet contains highlights for benefit options offered by your Lexington-Fayette Urban County Government beginning January 1, 2015 through December 31, 2015.

LFUCG takes great pride in offering a comprehensive benefits package that can meet the needs of our diverse workforce and their families. NEW THIS YEAR, our medical benefits will be of-fered through Anthem BlueCross BlueShield. We will continue to offer employees the opportunity to choose among four (4) health care plans, two (2) High Deductible Health Plans (HDHP) with Health Savings Accounts (HSA) and two (2) Preferred Provider Organization (PPO) plans. All medical plans will use the same great Anthem network and have access to the same medical services. Also NEW THIS YEAR we are adding a universal life voluntary insurance policy with a long term care rider. The long term care benefit rider is totally new and worth checking out. We are also pleased to offer benefits to Qualified Adults as well as spouses and dependents. Please be sure to check the eligibility section for more details.

As you know, each year health care costs continue to rise. Because LFUCG makes a substantial contribution toward your

benefits, we want to continue to encourage you to be a wise consumer of health care. During the past year, employees have increased their use of the Samuel Brown Center and the City Pharmacy. Not only do both of these facilities offer excellent services, but they are also cost-effective and help to control our overall health care costs. Using these facilities, participating in our LexThrive incentives and making healthy choices, are some of the best ways to help control health care costs. We are also happy to offer again this year the non-tobacco use credit toward health care premiums.

Star Robbins and Company will be assisting employees along with our Human Resources Benefit team and members of Benefit Insurance Marketing (BIM) to assure a smooth and seamless enrollment.

Open EnrollmentAs with every year at open enrollment, it is your respon-sibility to make certain you are enrolled in the benefits of your choice and update any information that may have changed throughout the year.

If you wish to continue to waive medical coverage, you will not need to resubmit proof of other coverage to the Division of Human Resources.

Remember, if you are choosing a flexible spending account, you must Enroll every year to continue your participation.

Eligibility Full time employees are eligible for all benefits outlined in this summary on the first of the month following your date of hire. Eligible employees may elect to cover a Spouse, Dependent or Qualified Adult.

A Qualified Adult is defined as a person of the same or opposite sex who meets the following criteria (See HR for Affidavit and required documentation):

• Age 18 or older. • Not related to the employee.• Must reside with employee for at least 12

months prior to enrollment.• Agree to inform LFUCG within 30 days of

any change in the circumstances attested to in the affidavit.

• Must be unmarried

• Financially interdependent with the covered employee for at least twelve (12) months & will certify, via an affidavit signed by both the Qualified Adult and the employee, that at least two (2) of the following conditions have existed for twelve (12) months. (choose one (1) from the left column & one (1) from the right column). See chart below.

Part-time employees who have been employed a minimum of two consecutive years may choose to purchase health insurance at the full premium cost.

Joint ownership of real estate property or joint tenancy on a residential lease

Will designating the eligible Qualified Adult as primary beneficiary

Joint ownership of an automobile Retirement plan or life insurance policy beneficiary designation form designating the eligible Qualified Adult as primary beneficiary

Joint bank or credit account Durable power of attorney signed to the effect that the employee and eligible Qualified Adult have granted powers to one another

Who staffs the Center?The Center is staffed by a full-time clinical staff of seven, including a physician, two physician assistants, three medical assistants and a receptionist.

Who can use the Center?The Center is open to all LFUCG employees, retirees and family mem-bers age 6 and older who are on the 2015 PPO or HSA health plans.

How do I schedule my appointments?To schedule appointments you can call the Center at 859-425-2555 or schedule your appointment online at www.marathon-health.com/myphr .

Laboratory tests: You may have a patient responsibility or payment for laboratory services. This will depend on your health plan and the terms of coverage that are applied.

Dr. Samuel Brown Health CenterThe Dr. Samuel Brown Health Center is a ZERO-COST alternative to office visit co-pays (certain lab costs will be extra). The Health Center provides a variety of health & wellness services similar to any doctor’s office, but with no co-pays! In addition to primary and acute care, the Center offers Health Coaching for those individuals looking to succeed with long term goals such as weight loss, health improvement or smoking cessation.

Preventive Care Benefits Preventive Care benefits are covered at 100% on both PPO and HSA plans. Preventive Care benefits cover basic annual exams and services medically suggested for age and gender.

Rewards for participating in a wellness program are available to all employees. If you think you might be unable to meet a standard for a reward under this wellness program, you might qualify for an opportunity to earn the same reward by different means. Dental Benefits Staying healthy includes obtaining quality dental care for you and your family. LFUCG’s dental plan is provided through Delta Dental, which includes an extensive network of providers. Once enrolled, you may visit Delta Dental’s website at www.deltadentalky.com or call (800) 955-2030 to locate a provider. See chart below for dental summary.

You can purchase the Dental Plan even if you are not on the health plans.NOTE: Out-of-network providers can balance bill above what Delta Dental pays.

City Employee Pharmacy The City Employee Pharmacy is available to all employees and their dependents that are enrolled on the LFUCG PPO or HSA health plans. Medications through the On-Site Pharmacy are offered at wholesale cost.

Vision Benefits LFUCG is pleased to offer a Vision Program through Anthem Vision using the Anthem Blue View Vision network. This vision plan is included with your medical plan. The amount you pay for vision services depends on whether you visit a network or non-network provider. Network providers can be found at www.Anthem.com. See chart below for vision summary.

USE IT OR LOSE IT RULE IRS guidelines stipulate that if you contribute dollars to a Flexible

Spending Accout and do not use all the money you deposit, you will lose any remaining balance in the account at the end of the eligible claims period.

Any remaining balance will not be carried over to the next year. You must submit all 2014 qualifying expenses for reimburse-ment by March 31, 2015. If the expenses are not submitted before March 31, 2015, no reimbursement will be made to you.

Plan election does not roll over. You must sign up each year if interested.

Flexible Spending Accounts (FSA) LFUCG offers two Flexible Spending Accounts (FSAs) which can save you money! You can elect a medical FSA to complement your PPO medical plan or if you are waiving medical coverage.

A Medical FSA allows you to put aside tax free money to pay for qualified medical, dental and vision expenses not covered by your insurance plans (expenses such as co-pays, or deductibles). You can payroll deduct up to $2,500/each year to be used on expenses incurred during that year. Note, you must use all funds set aside during the plan year or lose the funds per IRS guidelines. Account is funded in its entirety at the beginning of the plan year.

Dependent Care FSA allows you to set aside pre-tax payroll deductions to reimburse the expenses associated with daycare for your qualified dependents. Eligible expenses must be for the care of dependents under the age of 13 or for a qualified adult dependent (subject to IRS regulations).

Maximum annual limit is $5,000 (or $2,500 if married, filing separately). Money can only be used as it accumulates.

Health Savings Account Plan (HSA) HSA 1 or HSA 2 medical plan options may make you eligible for a Health Savings Account (HSA). This is a savings account that can be funded with pre-tax dollars to be used for qualified medical, dental and vision expenses. It is similar to the flexible spending account with the exception that the money is rolled over year to year and cannot be lost. You can use the funds in your HSA Account for qualifying medical expenses in future years. This Account goes with you even if you leave employment with LFUCG.

Eligibility requirements for the Health Savings Account: You must be covered by a High Deductible Health Plan (HSA 1 or HSA 2).

You cannot be covered under another medical plan that is not an HDHP.

You are not eligible for an HSA Account if either you or your spouse contributes to a general purpose Medical Flexible Spending Account.

You are not eligible for an HSA Account if you are entitled to a government sponsored medical plan (i.e. Medicare, Medicaid).

LFUCG contributes a sum of money to to your HSA Account as indicated below. Half is deposited in January and half in July.

New for 2015! Health Equity will manage all of the 2015 contributions into the Health Savings Accounts. Any contri-bution you make can be payroll deducted and deposited by LFUCG. For more information on Health Savings Accounts, call 866-346-5800, or visit www.myhealthequity.com.

Contribution limitations apply since this is an IRS regulated account. The 2015 limits include both Employer & Employee contributions (see chart at below):

Anyone between the ages of 55-65 may elect to contribute an ad-ditional $1,000 to the account each year. There is no limit on how much money can accumulate year after year. The above limits apply to annual contributions from both you and your employer.

You will receive a new card if you sign up for an HSA with Health Equity.

IN THE EVENT OF AN IRS AUDIT you will need to provide all receipts for withdrawals made from the HSA Account to verify that they were Qualified Medical Expenses.

Be aware that it is ultimately your responsibility to provide the IRS with evidence of the expense when audited. The item(s) you purchase with the debit card may process to pay at the point of purchase, but the item(s) may not be an IRS qualifying expenditure.

IRS has increased the HSA contribution limits for 2015:

Qualifying Events IRS Guidelines are adhered to for the medical, dental and vision plans at LFUCG which are administered in accordance with the IRS Section 125 Tax Code. This allows premiums for benefits to be deducted on a pre-tax basis from your paycheck.

Plan changes cannot be made after open enrollment without a Change in Family Status or Qualifying Event that is con-sistent with the IRS guidelines. A change in Family Status allows you to add or remove dependents from your plan but does not allow you to change from one plan to another. Some examples of Qualifying Events are:

• Legal Marital Status Changes: Marriage, divorce, death of a spouse (copy of marriage certificate, copy of final court divorce decree or death certificate required).

• Cancellation of dependent coverage due to dependent ineligibil-ity: Dependent gains or loses coverage under another plan.

• Birth or adoption of a child (copy of birth certificate or adoption papers required).

• Change in employment status from part-time or temporary to full-time.

• Change in employment status (full-time to part-time or temporary) or termination of employment.

• Loss of coverage under spouse’s employer plan due to a spouse’s death, termination of employment, divorce or loss of eligibility for other coverage

• Qualified medical child support court order (documents required).

CHANGES IN FAMILY STATUS MUST BE REPORTED TO HR WITHIN 30 DAYS OF THE EVENT IN ORDER TO MAKE A PLAN CHANGE.

HEALTH EQUITY / HSA Help Center If you need help with your claims status, card swipes, account balance, or eligible expenses contact the Health Equity Help Center. They are available every hour of every day at the phone number listed on your card or: 866-346-5800.

You can also view your personal Health Savings Account Information on line at www.myHealthEquity.com... click ‘begin now’. The site will walk you through the initial set up of your account.

After your first site visit, you can log in to www.myHealthEquity.com with the User Name and Password that you set up previously.

•If you will be transferring money from your current HSA plan through UMB Bank, LFUCG will add an additional $15 to your January contribution to cover the rollover fee. You must turn in a transfer form to a Star Robbins enroller, or the Division of Human Resources, if transferring money from a UMB account.

Let a nurse help you decide what a true emergency is. The number is on the back of your member ID card. In the time it takes to make a quick call, the nurse can help you decide what kind of care makes the most sense and can find a provider near you.

LiveHealth OnlineSM

LiveHealth Online is a convenient way for employees to interact with a doctor via live, two-way video on their computer or mobile device. LiveHealth Online provides access to U.S. board-certified doctors at the same price as a regular doctor visit co-pay ($49 charge for HSA medical plan) and Doctors can ePrescribe to local pharmacies. The LiveHealth Online service is available anywhere you have an internet connection (at home, in the office or on the go) 24 hours a day 7 days a week / 365 days / year.

Little Clinics & Urgent Care CentersLittle Clinics and Urgent Care Centers are now available to the LFUCG employees through Anthem for walk in appoint-ments to treat symptoms that should be looked at right away but that don’t require an emergency room visit. All facili-ties below offer extended hours and/or weekend hours.

Your primary care doctor or the Samuel Brown Clinic may be your first choice when you need care, but now you have other options for after hour care.

Additional Anthem Resources• Care Comparison: Pricing comparison for 130+ procedures, at a variety of facilities in your

geographical area. This service allows Anthem members to be wise healthcare consumers.

• Condition Care: Assistance in managing conditions such as asthma, diabetes, chronic obstructive pulmonary disease or cornonary artery disease.

• Complex Care: Anthem will identify and assist members in managing complex and/or multiple disease states.

• Future Moms Maternity Program: Access to an OB nurse who can advise/answer pregnancy related questions as needed. Access to maternity web site with educational resources.

Group Life & AD&D Benefits

Short Term Disability through Short Term Disability protects your paycheck if you cannot work due to off the job illness or injury. You will have the ability to select the amount of Short Term Disability that coordinates best with your current sick pay. Accident Assistance through Accident Assistance through Allstate Accident coverage offers peace of mind when an accident or injury occurs. The coverage can provide cash to cover unexpected expenses. Individual Term Life through Term life insurance is designed to provide affordable life coverage during a specific ‘term’ of your life, typically, your younger years when your expenses are the highest. Whole Life Insurance through Whole Life insurance is permanent coverage and does not go away like ‘term’ life does. Benefit is also available for spouses, children and grandchildren. At age 65, policy owners will have the option of taking a cash payout or having the life benefit paid up with no additional premiums due. Dependent coverage is available. If you elect life insurance for dependents, you must elect optional life insurance for yourself. Life insur-ance for dependents may not exceed 50% of the employee benefit.
