q & A with the PresidentMOX BOX

Q. I recently wItnessed a patIent who was on precautIons walkIng the hallways wIthout any personal protectIve eQuIpment. Is thIs allowed? I know that when these patIents are transporterd throughout the hospItal that they have a yellow gown on top of them but are there other precautIons to follow?

A. In general, patients on precautions (other than standard precautions) should remain in their room for the length of their hospital stay. The exception applies to patients who are on contact precautions for colonization with MRSA, ESBL, or VRE only, or those with infected wounds that are covered and contained. In these cases, the patient should change gowns, perform hand hygiene, and have oversight from staff who can monitor hand hygiene and observe limited contact with surface environment. Patients assigned to droplet or airborne precautions should remain in their room, as well as any patient infected or colonized with CRE. For any questions on this topic, call Infection Control at

Q. durIng mornIng rounds, I have notIced an Increase In crItIcally low glucose levels wIth our dIabetIc InpatIents. In the past, the kItchen sent daIly afternoon brown-bag snacks for dIabetIc patIents to help avoId lowered glucose levels overnIght. sInce thIs servIce ended, the nursIng staff Is responsIble to ensure that proteIn-based snacks are gIven but thIs Is eIther mIssed or thIngs get too busy to prepare snacks IndIvIdually. can we consIder reInstatIng the afternoon brown bag snacks to better care for our dIabetIc patIents?

A. After formally tracking the snack service, we found that the majority of brown bags and snacks were being thrown out each day. To prevent significant food waste, we stopped packing the brown bags and instead stock snacks for patients in the unit refrigerator such as crackers and cheese, toast and peanut butter, and cereal and milk. Patients can also - and often do - request a specific snack that staff can manually enter into the room service computer and also can also order an extra snack with dinner.

In an effort to improve this process, we have posted a sign on the refrigerator listing what snacks are appropriate for diabetic patients as well as designated a drawer in the fridge that will be labeled and stocked on every unit for diabetic patients. Par levels will be increased on each unit for protein and carb snacks that will be tracked by our Dietitians.

Q. I notIced on the aJh websIte that the blood donor program eIther Isn’t lIsted or Is too hard to fInd. It’s a great program and more people should be made aware of It.

A. The Blood Donor Program can be found under “Service & Departments” under the Laboratory. In an effort to make the program easier to find on the website, we have also listed it as its own department. We have also promoted the program on social media, and will include it in the next issue of In Case You Missed It (ICYMI).

Lisa Higgins, RN, shared her concern via Mox which resulted in these improvements.

She is holding a new sign designating proper diabetic snacks.

Anna Jaques was recognized in the “Silver” category for Best Hospitals in the 2017 Wicked Local Readers’ Choice awards published this summer. For more than 20 years, Wicked Local readers have been voting on their favorite places, services, and things to do in and around their communities.

The “Gold” award went to Beverly Hospital and the “Bronze” award to Addison Gilbert Hospital in Gloucester.

Congratulations to all employees, medical staff and volunteers who make Anna Jaques a place people recognize as among the best in the area.

PATIENT KUdOS“I’d like to submit a Shining Star to the entire Telemetry Department who recently took care of a senior citizen I care for. Every time I visited, my friend was clean, cared for, and the staff repeatedly checked in to see if she needed anything. Everyone from the nurses to housekeeping and dietary were so good to her. They treated her with respect and with dignity. One of the biggest issues I’ve seen when caring for an elderly person is that they are spoken to like children. Not once was this evident in any of the current staff. Instructions for her care and diet were followed to the letter. We could not have been more impressed nor could we have been more appreciative. All of the aides were very helpful in her personal care and this is the first time she has had a stay in any hospital where she had no breakdowns in her skin. She was washed and she was freshly powdered every day I came to see her. I care for her in her home and am exceptionally protective of her. I did not have a single moment’s concern on this stay.”

MARK’S MESSAGEOne of the hallmarks of Anna Jaques is our commitment to provide a superior patient experience and in order to do that, we need our hospital to be a safe environment. Hospital-wide, we have instated programs to improve safety measures and learn from experiences to continuously improve our performance in everything we do. One of these initiatives include the sharing of “safety stories.”

Safety stories can originate at Anna Jaques or may have happened at other hospitals, and may be near misses or involve actual events. Often, the “Swiss cheese” analogy best describes a scenario where even after multiple layers of protections are put into place, an error can occasionally still get through. “Orange juice in the coffee” is another phrase used to describe the tendency we all have in getting too comfortable with our routine and not always paying attention to details or asking questions. By sharing these stories at senior leadership and department meetings, we provide continuous opportunities to learn and

help to identify patterns or themes such as education needs or barriers to overcome.

As an example, on a rare occasion two patients with the same name are admitted to the hospital. It would be easy to inadvertently select the wrong patient with the same name. We have double-checks built into our system to ensure that there is no confusion between two patients with the same name. This is one of the reasons we ask patients repeatedly to provide their name and birth date during their stay - to help keep patients safe and allow our staff to deliver the best, most appropriate care. It is often said to keep safety and an awareness of your surroundings “in the back of your mind.” I challenge you all to keep safety at the forefront always. If you notice something that seems off, a situation in the parking lot or Cafeteria, or a patient that seems out of place – take action and call Security knowing that it could help avoid a potential dangerous situation for your hospital.

NewCo News... A monthly status update on the new health care system

➝ On September 14th, the AJH Corporators unanimously voted to move forward with the transaction.

➝ The deal is currently in the regulatory review phase, seeking state and federal approval to move forward with our plans. The entire regulatory review process - including review by the U.S. Federal Trade Commission – is expected to be completed in about a year.

➝ Two events will take place this fall: 1. Filing of a Determination of Need (DON) application with the MA Department of Public Health. The DON will spell out goals for the new system and demonstrate how the system will help improve the health of the communities we serve.

2. A meeting of the MA Health Policy Commission. At this meeting, we fully expect that the Commission will make a formal determination to conduct a “Cost and Market Impact Review” (CMIR) of our proposed new system, in order to assess the potential impact on health care costs, quality and access. The CMIR is a standard part of the review process, and we welcome the opportunity to make our case and highlight the many benefits our new system will create.

O’cONNOR’S VIEW: SUPPORT fOR AJH IN NEW HEAlTH SySTEMBy Delia O’Connor, Published in the Daily News, September 18, 2017

As the former president and CEO of Anna Jaques Hospital, a resident of Newburyport, and along with my husband, a patient of AJH-affiliated physicians and the hospital, I would like to express my full support for Anna Jaques becoming a founding member of the new health system with Beth Israel Deaconess Medical Center, Lahey Health, New England Baptist Hospital and Mount Auburn Hospital.

When I came to Anna Jaques over 10 years ago, one of the things I heard repeatedly from area residents was how grateful they are to have a hospital locally, period.

If we want to have that resource (and more) for our patients and communities into the upcoming decades, we need a larger system blocking for us. Many, many improvements to what the hospital and medical staff can offer have been engineered in recent years by a loyal medical staff, board members, donors, employees and volunteers.

And when I say loyal, among other things, I recall the united fight we and the whole community put up against a pushy and overbearing outside union during the major capital campaign to update the hospital. Remember the yard signs everywhere — “We Support

Anna Jaques Hospital” — and we won.

This opportunity comes during a very tough health care period with near-universal understanding that the growing cost of care must be reduced. That means that some facilities will not survive.

Anna Jaques and its medical staff have worked hard to solidify a reputation for high-quality care and an excellent patient experience at relatively lower cost. AJH is therefore well suited to join this new system in eastern Massachusetts, which will emphasize coordinated access to great, complex care when it is needed, and excellent, service-oriented care in the community when that is the best choice.

The integrated system will carry the very desirable brand of the system’s members, including Harvard teaching hospital Beth Israel Deaconess Medical Center, with whom we and the medical staff have worked successfully for seven years as an affiliate.

And the new system is also designed to be more efficient than the current choices in the market, which addresses the demand for more affordable care. The community hospitals in this system have very good reputations, including our near neighbor, Beverly, and the teaching and specialty hospitals, Beth Israel Deaconess, Lahey Health, New England Baptist and Mount Auburn, are outstanding, nationally known institutions.

I was very proud for Anna Jaques that we were included, and this prospect redoubles my optimism about our future.

BIRTH cENTER REcEIVES BlUE dISTINcTION+ AWARd fROM BlUE cROSS BlUE SHIEld

On September 18th, the Daily News of Newburyport featured an article on Christine Graham, a 36-year-old mother of two with a storied history with the Anna Jaques Birth Center. Christine was born at AJH at just two pounds and was the smallest baby to have been born here at the time. She has since gone on to deliver two sons at the hospital, as well. As published in the paper….

Dr. Steven Mollov, a veteran physician in obstetrics and gynecology at the hospital, said it came down to teamwork and providing excellent care.

“There is close coordination between women’s health care and the maternal fetal medicine department when the need arises,” Mollov added. “We are blessed to have our neonatal care center staffed by neonatologists and a specially trained nursing staff to handle most prematurity as well as other medical conditions facing our newborns, allowing them to stay in the community.”

“It is just another example of keeping medical care in the community while taking advantage of Boston resources when needed,” he said.

A SEcONd cHANcE: WOMAN GRATEfUl TO ANNA JAqUESAs published in the Daily News, September 18, by Dyke Hendrickson

Christine and her seven-week-old son who was recently born at AJH.

The Anna Jaques Birth Center was once again recognized through Blue Cross Blue Shield’s Blue Distinction Specialty Care program as a Blue Distinction Center+ in Maternity Care. This recognition reflects the Birth Center’s demonstrated expertise and commitment to quality care for vaginal and cesarean section deliveries, better overall patient satisfaction, and a lower percentage of early elective deliveries. The Blue Distinction Specialty Care program has two

levels of recognition, Blue Distinction Center and Blue Distinction Center+ in the categories of: bariatric surgery; cardiac care; complex and rare cancers; knee and hip replacement; maternity care; spine surgery; and transplants. The Birth Center received the Blue Distinction Center+ level because of its more affordable care in addition to quality care, treatment expertise and better overall patient results. Congratulations to the entire Birth Center!
