Transcript (School Record) Release Form Allentown High School has always honored student requests to forward transcripts. Effective November 15, 1974, Federal and State Law prohibit the release of pupil records without parent or adult student written authorization. The school cannot release records without this written permission. Ref. New Jersey Administrative Code #6:3-6.1 states, “Organizations, agencies and persons from outside the school shall have access to pupil records if they have written consent of parent or adult pupil (age 18)”.

In addition, some colleges (particularly more selective ones) request information on any suspensions or expulsions. We can release this information only if you sign the additional consent below. If you do not authorize below, we will answer any such questions from colleges by saying, “Release of this information required an express consent by the student/parent which we do not currently have on file.”

Parent College “Brag” Sheet

Please use the following suggestions to help you type a recommendation for your child (minimum 1 page, maximum 4 pages). No one knows your son or daughter better than you and your input is most helpful while writing your son or daughter’s recommendation. Please share any anecdotal stories (that occurred in high school) about your son or daughter that may demonstrate his or her personality traits and characteristics. Tell us those qualities that make him/her unique. Here are some suggestions:

Tell us what you consider your son or daughter’s strongest asset (as a person).

Describe how you have seen your son/daughter grow in a variety of ways since entering high school. Has there been an exceptional challenge in the past few years?

Describe an accomplishment, recognition or an incident which made you especially proud of your son or daughter.

Describe your son/daughter’s outstanding personality traits.

What seems to bring your son or daughter the most joy?

Tell us what you see as your son or daughter’s passion and give any information that captures the spirit of your son or daughter.

How to Request a Transcript on Naviance 1. Complete and submit the Transcript Release Form to the guidance department. TRANSCRIPT REQUESTS WILL

BE DISABLED IN NAVIANCE UNTIL THE TRANSCRIPT RELEASE FORM IS SUBMITTED TO GUIDANCE. 2. Complete and submit the application to your college via the Common App or the school web site. 3. Log in to Naviance by going to the High School web site, and navigate to the Guidance Department page (under

Academics). Click on the Naviance link on the left hand navigation. 4. Go to the colleges section.

5. Go to the colleges I’m applying to section.

6. Before you can request a transcript, you must first give us permission to do so on Naviance:


7. Once you hit submit, your page will refresh. Now, click “+add colleges to this list”.

8. Add all of the colleges you have applied to.

9. Your page should now look like this:


10. One last thing! You need to request TEACHER LETTERS OF RECOMMENDATION. Scroll down the page to the

Teacher Recommendation section. PLEASE DO NOT COMPLETE THIS STEP UNTIL YOU HAVE SPOKEN TO YOUR

TEACHERS IN PERSON ABOUT WRITING YOU A LETTER.

11. Select your two teachers, then click “add requests”.
