You’re a partner, not a patientWe work in concert with each guest and their family members to develop a holistic, person-centered care plan that fulfills a guest’s specific discharge goals. We provide ongoing caregiver education during the rehabilitation stay to provide our guests with a supported return home to their independent living apartment.

CONTINUING CARE OPTIONSAssisted Living Convenient, on-site medical careAs a resident of Ann’s Choice, your loved one may benefit from Erickson Health, one of the nation’s largest and most integrated health and wellness systems for seniors. Our on-site medical center is staffed by full-time health care providers who make regular rounds in the continuing care neighborhood. Specialists also have regular hours in the medical center. Thanks to our on-site providers, you don’t have to worry about taking time off from work to drive your loved one to and from medical appointments. The care you need is conveniently located on campus.

Special programs to enhance strength and independenceOur preventive approach to care means your loved one will have access to unique programs that focus on reducing common health care risks. For example, we offer a falls prevention program designed to help residents reduce their risk of falls by assessing medications, balance, strength, vision, flexibility, and dietary habits.

That’s why we employ a memory care manager who provides all types of caregiver support, from educational seminars and workshops to support groups and one-on-one consultation.

Quality health care right on campusResidents in the memory care neighborhood are supported 24 hours a day, 7 days a week, by their care associates and the charge nurse. Full-time

physicians and nurse practitioners who specialize in the health needs of older adults practice at our campus medical center and are available to provide prompt, personalized care.

Chronic disease managementWe strive to reduce the number of medications administered to residents by focusing on creating strong relationships between residents and their care associates. Using complementary therapies like music, massage, and engagement in meaningful activities can often reduce or mitigate the need for pharmaceuticals.
