o Two databases are created each semester.
o Three campuses are assigned to each database.

Collection Tool
• Populating the Section Selection e-Form Course section
reports are pulled from the People’s Soft database after Official
Day and uploaded to the e-form database.
• Selecting Sections: Assessment leaders then select sections for
assessment (Follow 25/4 Rule: 25% sections/4 sections
minimum)/Include online, dual, centers as well)

SharePoint acts as the “storefront” for the databases
o The Access databases are linked to SharePoint sites
for users to access the site through LSC’s intranet.
o This allows us to protect the faculty’s confidentiality.
because faculty need to enter their employee ID to
access the data entry tool.
What Faculty See from
the SLO Data
Collection Tool
• Once sections are selected, we identify any new users
based on their selections and update permissions on the
Share-point site.
o We email the identified instructors/sections to let them
know they have been selected, and provide them the
link to the data collection tool created for their campus.
o When assessment is complete, instructors access the
site to enter their section level data.

• Curriculum Team Form (CT Form): Situated in the SLO Form
section;
• SLO Form: For assessment plans and closing the loop
reporting—Mainly Fall semesters assessment/Spring
semesters are for catch-ups
• PLO Form: For workforce programs and other 60 credit hour
programs to develop assessment plans and closing the loop
reports – Annual process

attend class.”
