Audit Concerns

Since 1997-1998, the integrity of leaver records has been evaluated annually by TEA through various indicators and data analyses. The system is designed to identify districts that are a high risk of having inaccurate dropout records and that, as a result, require on-site monitoring of dropout records. The data validation indicators include the following:

1. Dropout Trend Analysis – This indicator identifies districts that reported a decrease in dropout rates that was significantly greater than the reported decreases of other similar districts over either two or three years.

2. Underreported Students – This indicator identifies districts not meeting the state standard for the count and/or percent of underreported students.

3. Zero Dropouts and High Use of Other Exit Leaver Codes – This indicator identifies districts with zero dropouts and an unusually high use of leavers with intent codes. Intent codes include 16, 24, 60, 81, and 82.

4. High Use of One or More Leaver Codes – This indicator identifies districts with an unusually high use of one or more leaver codes. Leaver codes include 03, 16, 24, 60, 66, 78, 81, 82, 83, 85, 86, 87, 88, 89 and 90.

5. Continuing Students’ Dropout Rate – This indicator reports the dropout rate of students from the previous year graduating Class who returned to school after their expectant cohort graduation year. This is a Report Only indicator for and is reported for district information and planning purposes.

SECTION 5 – STUDENT LEAVER DATA

Because of the monitoring from TEA it is very important that these guidelines are strictly followed.

PROCEDURE OVERVIEW

Leaver reports include data for two different groups of students: Report 1: Current Year

Includes students who withdraw during the current year (2013-2014).

Report 2: Prior YearIncludes students who completed the prior year (2012-2013) but did not return for the current year (2013-2014) and students who withdrew during the previous school year (2012-2013).

Documentation that must be kept in the student’s PRC includes, but is not limited to, the student’s withdrawal form and enrollment form (or a copy of the enrollment form). Both must be filled out completely and accurately and signed by the parent/guardian, if possible. It should be understood that the enrollment form is the district’s proof that the student is/was enrolled. In addition, the withdrawal form must be signed and dated by an authorized administrator. Signatures on the withdrawal form from the clerical staff are not permitted unless prior approval has been received by the Information Management Services (IMS) Department.

Included in this information are detailed operational procedures for accomplishing the data entry and other necessary tasks. Also included are relevant excerpts from the PEIMS Data Standards, listing the acceptable withdrawal codes, definitions, and documentation requirements for each code. Please keep in mind that proper documentation, as defined in the PEIMS Data Standards, will be a key issue when we are audited.

CURRENT YEAR

1. As withdrawals occur throughout a school year, authorized personnel should assign the proper withdrawal code, based on the PEIMS Data Standards and available documentation. Please note - you must have documentation as noted in the PEIMS Data Standards to support the code assigned.

2. In addition to procedures normally followed by the campus, the withdrawal form, enrollment form, and any other relevant documentation should be placed in the student’s PRC. You must also upload the signed withdrawal form along with approved documentation supporting the code into ITCCS.

3. Periodically during the school year, the campus will need to review current year withdrawal reports. Updates will need to be maintained and acceptable documentation filed. If documentation is obtained which causes a change in the leaver code, the change should be authorized in writing by the appropriate staff member, reflected on the Leaver record (WST1345). The new documentation should be attached to the original documentation and then placed in the student’s PRC and uploaded into ITCCS.

Please Note: NEVER UPDATE THE WITHDRAWAL CODE IN THE LEAVER RECORD (WST1345). ONLY UPDATE THE LEAVER CODE.

4. Periodically during the school year, District leaver audits will be conducted at each campus to verify that the documentation on file meets the documentation requirements outlined in the PEIMS Data Standards.

5. Campuses will be provided a blank copy of the MISD Leaver Form. This form may be utilized to verify Leavers and updates discovered from the annual Recovery Walk.

6. Campuses will be provided a blank copy of the MISD Student Verification Form. This form must be utilized to verify Movers.

7. Campuses will be provided a blank copy of the MISD Home School Verification Form. This form may be filled out by the parent/guardian and notarized to verify that the parent/guardian plans to home school the child.

PRIOR YEAR

1. Campuses will complete a no-show withdrawal on the student database for each student who does not attend the first day of school. (as explained in section 4) For these students, no withdrawal form is required. Students who are no-shows on the first day of school but start later will have their withdrawal deleted and their entry date corrected to the first day of attendance. Students who remain on the no-show list will be moved to the no-show campus approximately two weeks after the first day of school. This process will create leaver records for those students who are true no-shows.

2. In preparation for the fall PEIMS submission, the IMS Department will provide campuses with the PEIMS Leaver Report in mid-October. This report includes students who will be reported as leavers on the fall PEIMS submission as well as movers. It lists the student demographic data, attendance data, and leaver data all taken from the prior school year for graduate students, dropout students, and all other students in grades 7 through 12 who either withdrew from school during the prior school year or completed the prior school year and have not enrolled in MISD this year.

3. Assigned campus personnel must review the PEIMS Leaver report to verify the data is accurate. Authorized changes or updates are to be completed following the procedures found in section 4.

4. The IMS Department will provide a deadline for which review and updates must be complete. At completion, the campus will receive an updated leaver list for final review and sign off by the campus Principal. At this point all leaver changes need to be made by the Dropout Prevention office. It is critical that all student data on the list be verified since these are the codes that will be submitted to TEA on the Fall PEIMS submission.

5. Because of the PEIMS regulations for leaver reporting, the current year and prior year procedures overlap at certain times. For example, during the fall of the current school year, campuses will be following the current year procedures for students who withdraw during the current year. At the same time, they will be following prior year procedures to prepare for the fall PEIMS submission. Because TEA makes changes to the Leaver codes and documentation periodically, withdrawal codes can vary from one year to the next, meaning that two different sets of codes could be in use simultaneously: one set for prior year, and another set for current year. Details of these issues will be described in the Leaver Data Monitoring section if this occurs.

Leaver Data Monitoring

LEAVER DEFINITION

There are three ways to define students who were enrolled in the prior school year and who do not reenroll in the following school year. These include:

1) Leavers A leaver is a student who was enrolled at least one day in a Texas

public school the prior year and does not return the following fall. These are categories of students that TEA cannot determine the

status unless a leaver record is submitted. A PEIMS 203 record is submitted.

Withdrawal codes include: 01 - graduated; 03 - died; 16 - return to home country; 24 - college, pursue degree; 60 - home schooling, online diploma program; 66 - removed/child protective services; 78 - expelled, cannot return; 81 - enroll in a Texas private school; 82 - enroll in a school outside of Texas; 83 – not entitled to enroll; 85 - graduated outside of Texas/returned, left again; 86 - GED outside of Texas; 87 – enroll in university high school diploma program; 88 – Court order GED has not earned GED; 89 – Incarcerated State or Federal as an adult; 90 – Graduate out of state under ICEOM (Military).

2) Movers A mover is a student who will be reported to PEIMS by another

district or entity. A mover is a student who, through the TEA PEIMS/PET submission,

can be located at another Texas public school as enrolling during the school-start window or has obtained a GED certificate at a Texas examination site by August 31.

Students who have been coded with a mover code but cannot be tracked through the PEIMS/PET submission as enrolling in another Texas public school during the school-start window or obtaining a GED by August 31will be considered a dropout.

A PEIMS 203 record is not submitted unless the student is determined to be a leaver or a dropout.

Withdrawal codes include: 21 - transfer to other campus in district; 30 - enter health care facility in Texas and receiving educational services; 31 - completed GED in Texas by August 31; 61 - incarcerated outside of district in TX and receiving educational services; 63 - graduated/returned, left again; 64 - GED/returned left again; 80 - enroll in other Texas public school.

3) Dropouts A dropout is a student who is not a leaver or a mover. Students who have been coded with a mover code but cannot be

tracked through the PEIMS/PET submission as enrolling in another Texas public school during the school-start window or obtaining a GED by August 31will be considered a dropout.

A PEIMS 203 record is submitted. Withdrawal codes include: 02 - pursue job/job training; 04 - join the

military; 08 - pregnancy; 09 - marriage; 10 - alcohol/other drug abuse problems; 14 - age; 15 - homeless or non-permanent resident; 19 - failed exit TAAS-TAKS/met graduation requirements; 22 - enroll in alternative/GED program; 44 - no-show and whereabouts are unknown; 47 – incarcerated inside or outside of Texas and not

receiving services; 59 - summer withdrawal and whereabouts are unknown; 79 - expelled, can return, has not; 84 - academic performance; 98 - other/unknown.

SUMMARY OF IMPORTANT POINTS TO REMEMBER

School districts report data on all students they served in grades 7-12 during the prior school year.

The school-start window extends from the first day of school through the last Friday in September.

Leaver records are submitted for students who left the district before the end of the school year, did not return in the same year, and did not return in the fall in the school-start window. The students leaver record will be reflected on the last campus of attendance. Leaver records are not submitted for students who enroll in other Texas public school districts, students who obtain GED certificates at Texas examination sites by August 31st, and students accounted for through other state reconciliation processes.

Appendix D provides an expanded definition and specific guidelines on acceptable documentation for each of the leaver reason code.

Grade level of a school leaver refers to the last grade of enrollment . Students who complete a grade, but do not return to school within the school-start window the following year, will be reported with the grade level during the last year of enrollment.

CAMPUS-ID refers to the identification number of the campus in which the student was last enrolled. If a student completes the eighth grade at a middle school (Grades 6-8), and then fails to return to school at a high school (Grades 9-12) the following fall, he is shown as a school leaver in Grade 8 from the middle school campus.

Students who complete a GED program are not reported as graduates. Students who have previously completed all graduation requirements in one

school year, but do not pass the exit-level TAKS until the next year, are reported as graduates in the year in which the TAKS test was passed.

LEAVER DOCUMENTATION

All leaver codes, which include dropout codes and non-dropout codes, used to withdraw a student must be supported by documentation. Documentation supporting the use of a leaver code must exist in the district at the time the leaver data are submitted. This documentation must be retained for at least five years.

TEA is very specific about what is considered acceptable documentation. These documentation standards are to be strictly followed.

In addition to the specific requirements for each leaver reason code, use the additional “tips” below:

Do not use post it notes. They are easily lost or overlooked. Use formal documentation. Outside (someone other than MISD staff) verification is better than inside

(MISD staff) verification. Be precise in your documentation. Remember that an auditor does not

know the student. He/she cannot go by your “belief” that the student enrolled elsewhere.

Date and sign every item of documentation. If an auditor finds that you consistently omit the date, he/she may not consider your documentation as valid proof.

PROCEDURES FOR MAINTAINING LEAVER DOCUMENTATION

Students with non-dropout leaver codes as well as those with dropout leaver codes must have documentation to support the leaver code.

In regards to Completion Rate and Cohorts Reports – only current year leavers or those on the Leaver update list provided by the IMS department from the prior year can be updated. Students who withdrew 2-3 years ago or more are not allowed to have changes made to their old leaver data. Once they re-enroll and withdraw again then that current withdrawal data can be updated during the window of time allowed.

VERIFICATION

All leaver codes with the exception of Dropouts must have supporting documentation. The designated campus personnel must verify that the assigned leaver code is supported by the acceptable documentation as explained in the PEIMS Data Standards leaver documentation table. Please note: Leaver codes that cannot be verified will be assigned a dropout code.

Graduates Leaver code 01 – No withdrawal form is necessary. Transcripts must reflect that the student has the necessary credits per their graduation plan and have passed the necessary Texas state exams. Students reported with a LEAVER-REASON-CODE of 01 must also have valid entries for DATE-OF-GRADUATION and GRADUATION-TYPE-CODE.

Leavers with Codes 03, 66, 78, 85, 86, 87, 88, 89, 90 – Will need a withdrawal form and have official documentation as explained in the PEIMS Data Standards leaver documentation table.

Leavers with Codes 24, 81, 82 – A Withdrawal form signed by the legal guardian and campus administrator indicating intent to enroll will be acceptable if attained within 10 days of withdrawal. If signatures are not attained within 10 days then the designated campus personnel will need to contact the intended institutions to verify enrollment.

Leavers with Codes 83 and 16 - Will need a withdrawal form and the MISD leaver form documenting the reason for the code.

Leavers with Code 60 – Will need a withdrawal form and the MISD Homeschool/Online diploma program form. This is a form that must be notarized. If parent/guardian or student refuses to sign the notarized form please note the refusal on the form, sign, date and file.

Movers Leaver Code 00 – Will need a withdrawal form and the MISD Verification form. Receiving institution must check that the student is attending not just enrolled to qualify as a mover. If the student is not attending the student is a dropout.

Dropouts Leaver code 98 – Will need a withdrawal form. Designated campus personnel should make attempts to contact the dropouts to find out their whereabouts.

The actions to be taken should include, but are not limited to, the following: Verify with records request personnel that no record requests have been

made. Check PID. Call parent, guardian, emergency contact, friends of student, etc. to obtain

information of student’s whereabouts. Check the enrollment form as well as the student’s PRC for additional

information that may help you determine the student’s whereabouts.

Take action to document information that indicates the Leaver code may now be changed to a non-dropout code, including the following:

Write the new code on the enrollment form, withdrawal form, and other paper documentation indicates a withdrawal code.

Place the original withdrawal form and documentation in the student’s PRC and Upload into ITCCS.

Do not destroy any existing documentation . Access the student’s withdrawal information on the Leaver screen

(WST1345). Leave the withdrawal code alone, change the Leaver reason only. Click SAVE to update. This will update the Leaver record.

REMINDERIn regards to Completion Rate and Cohorts Reports – only current year leavers or those on the Leaver update list provided by the IMS department from the prior year can be updated. 6. Identify the leaver row that pertains to the year in question.

7. Enter any changes necessary keeping the following in mind:

If a records request has been received, enter a “Y” in the Records Requested Code field for the appropriate leaver row. In the explanation field, enter the district, city or state indicated where the student will be attending.

If the leaver reason code, Leaver Reason, is incorrect, enter the appropriate code.

Leaver Camp identifies the last campus the student was enrolled. This is the campus that the dropout, graduate, or leaver will be counted at.

Grade Level is the last grade of enrollment. Students who complete a grade, but do not return to school the following year, will be reported with the grade level during the last year of enrollment.

Changes to the following fields cannot be updated on this screen: Rec Camp, Sex, Eth, Econ Disadv, Spec Ed, or Birth Date.

Withdrawal Date/Code identifies the last withdrawal date and code during the last year of enrollment. The withdrawal date is reported to PEIMS only when the student graduates. In this case, the withdrawal date is the graduation date.Please note: The withdrawal date cannot be changed. You must delete the leaver row, and then add a new leaver row. See the section TO DELETE A LEAVER RECORD for instructions on deletions.

Helpful hint: Add the new leaver row with the correct withdrawal date before you delete the incorrect row. This way, you will have all of the pertinent data on the screen which you will need for the new leaver row.

8. When you have completed the changes on screen WST1345, click Save.

DELETING LEAVER INFORMATION

1. Ensure that the proper student data is displayed on screen WST1345, Leaver Record.

2. Select the row that needs to be deleted by clicking in the left hand box under the Del column. A check mark will appear.

3. Click Save. 4. That particular leaver row should now be deleted.

PROCEDURES FOR UPDATING GRADUATE INFORMATION

GRADUATION TYPE CODES 2013-2014 PEIMS Data Standards

Graduation Type Codes Decision Charts

The following three GRADUATION-TYPE-CODE charts differentiate the requirements for coding general education and special education students.

GRADUATION-TYPE-CODE Chart 1 applies to all students who completed the curriculum and credit requirements for graduation, including satisfactory performance on the exit-level assessment.

GRADUATION-TYPE-CODE Chart 2 applies only to students who received special education services, completed the curriculum and credit requirements for graduation, and participated in the statewide assessment instrument identified in the IEP.

GRADUATION-TYPE-CODE Chart 3 applies only to students who received special education services who completed credit requirements for graduation, but whose curriculum content was modified or substituted.

PLEASE NOTE: The content of the curriculum determines the graduation type and drives the applicable coding.

Chart 1 applies to all students in general education and to students receiving special education services for whom the required curriculum content was not modified or substituted and satisfactory performance on the exit-level assessments in place at the time of the student’s enrollment in grade 9 was required.

The codes in this chart apply to students graduating under the minimum, recommended, or distinguished programs, including students receiving special education who graduate according to TAC §89.1070 in effect at the time of the student’s enrollment in grade 9. See

Charts 2 and 3 for students receiving special education services graduating under TAC §89.1070 for whom the student’s ARD committee determined that satisfactory performance on a required state assessment was not required. Students receiving special education services for whom the curriculum content was not modified, and who meet the minimal requirements of the recommended or distinguished program must have that achievement recognized on the Academic Achievement Record.

Instructional accommodations for students with disabilities that do not result in curricular content modifications do not impact graduation requirements. Examples of accommodations that do not impact the graduation plan include one-on-one instruction, small group instruction, multi-sensory approaches, extended time on projects, study guides, highlighted texts, programmed materials, preferential seating, immediate feedback, Braille, books on tape, screen readers, the use of an interpreter or word processor, oral testing, un-timed testing, extended time to complete assignments, shortened tests, alternative evaluation strategies (draw a diagram, develop a model, perform the answer), etc. The use of these and other accommodations is allowed under all three graduation programs.

Amendments to TAC §89.1070 adopted to be effective Nov. 11, 2007, require satisfactory performance on the statewide assessment for the recommended and distinguished achievement high school programs for all students entering the ninth grade in 2007-2008 and thereafter; therefore, graduation codes 19 and 20 may be used for students entering the grade 9 prior to the 2007-2008 school year only.

Students receiving special education services for whom the curriculum content was not modified and who meet all of the requirements of the recommended or distinguished program, including satisfactory performance on the standard exit-level assessments, must have that achievement recognized on the Academic Achievement Record, even if the student originally participated in a modified assessment (or was previously exempted from satisfactory performance on the standard exit-level assessment requirement in place at the time of the student’s enrollment in grade 9). In the event that a student completes all of the requirements of the recommended or distinguished program, including satisfactory performance on the standard exit-level assessments, that student should be coded with one of the codes in Chart 1.

SECTION 5 – STUDENT LEAVER DATA

Leaver Reason Codes and Documentation RequirementsThe attached table provides expanded definitions and specific documentation guidelines for each of the leaver reason codes listed in Code Table C162 of the Texas Education Agency (TEA) PEIMS Data Standards. The table is organized into the following broad categories of leavers:

completed high school or General Educational Development (GED) program moved to other educational setting withdrawn by school district other

Leaver records are not submitted for students who enroll in other Texas public school districts or charters and students who obtain GED certificates at Texas examination sites by August 31. Students who move to other Texas public school districts are considered movers, not leavers. Documentation as described in Appendix D is not required for movers; districts may wish to develop local policy on documentation for movers. The Person Identification Database (PID) Enrollment Tracking (PET) may be used to establish tentative local documentation that students are movers. The final determination of whether students are movers will be made by TEA.

Local policy in this appendix refers to a district's or charter's leaver policies and procedures.

General Documentation RequirementsDistricts must document the withdrawal of students and maintain on file the appropriate paperwork associated with student withdrawals. Documentation is required for all leaver reason codes. Documentation requirements for leaver codes are provided in the Documentation Requirements by LEAVER-REASON-CODE section. Documentation supporting use of a leaver reason code must exist in the district at the time the leaver data are submitted, i.e., no later than the PEIMS Submission 1 January resubmission date.

Timelines for establishing leaver reasons and obtaining documentation.

Students who leave during the school year.

For students who leave during the school year, leaver reasons apply at the time of withdrawal, and documentation should be obtained at that time. For example, for students who are withdrawn by Child Protective Services (CPS), LEAVER-REASON-CODE 66, documentation would be obtained when the student is removed.

Students who fail to return in the fall.

For students who fail to return the following fall, leaver reasons apply on the first day of school or its approximation, the school-start window. The school-start window is the period of time between the first day of school and the last Friday in September. Districts should use the

LEAVER-REASON-CODE that most appropriately describes the student's whereabouts during the school-start window. For example, to use LEAVER-REASON-CODE 60 student withdrew from/left school for home schooling, a district would establish that a student was being home schooled at some point during the school-start window. The district could obtain the documentation to support the leaver code at any time up until the PEIMS Submission 1 resubmission date.

Students who were attending and were withdrawn under LEAVER-REASON-CODE 83 when the district discovered that the student was not entitled to public school enrollment in the district.

LEAVER-REASON-CODE 83, not entitled to public school enrollment in the district, applies to students who are attending and are withdrawn by the district because the district discovers, when verifying enrollment information, that the student was not entitled to enrollment in the district because the student was not a resident of the district or was not entitled under other provisions of TEC §25.001 or as a transfer student. It is not for a student who was a resident and stops attending because he/she has moved out of the district.

This code is also used for those rare situations in which the student has not met the requirements under TEC §38.001 or a corresponding rule of the Texas Department of State Health Services for immunization, provisional enrollment, or exemption. Leaver reasons for LEAVER-REASON-CODE 83 apply at the time of withdrawal and documentation showing due process supporting the withdrawal should be obtained at that time.

Signatures on documentation. Documentation must be signed and dated by an authorized representative of the district. The district should have a written policy stating who can act as an authorized representative for purposes of signing withdrawal forms and other leaver reason documentation.

Withdrawal documentation should also be signed for the student by a:

parent, or guardian, or responsible adult as recorded in school records, such as a foster parent or a probation

officer, or qualified student. A qualified student is one who, at the time he/she stops attending

school: is married, or is 18 years or older, or has established a residence separate and apart from the student’s parent, guardian, or

other person having lawful control of the student

An original signature is not required on withdrawal forms received in the district by fax. Withdrawal forms received by e-mail do not need to be signed by the parent/guardian or qualified student. Written documentation of oral statements made in person or by telephone by the parent/guardian or qualified student is acceptable documentation in some situations if it is signed and dated by the district representative.

Evaluation of documentation

Merits of leaver documentation are assessed at the time the documentation is requested during a data inquiry investigation. Determination of the acceptability of documentation is made by the professional staff conducting the investigation. These guidelines describe the most common types of documentation the investigator would expect to find supporting use of each leaver reason code. Other documentation that represents good business practice and shows a good faith effort on the part of the district to properly report leaver status will be evaluated on a case-by-case basis.

Completeness of documentation

Withdrawal documentation shall be considered incomplete without a date, signature(s), and destination. Documentation will not be deemed insufficient when information is missing because the parent or parents refuse to provide information requested by the district. A district should document at the time of the conversation that the information was requested, and the parent refused to provide it. Appropriate documentation of a parent refusal to provide information includes the date, content of conversation, name of person with whom the conversation was conducted, and the signature of the school official verifying the conversation.

Changing LEAVER-REASON-CODEs

Once a district meets the documentation standard that supports the leaver reason code used, the district is not required to obtain additional information on the student. Local policy will determine if an existing leaver reason code is updated for a student when additional information is received. The policy should be clearly stated in the district's published guidelines on leaver procedures. For example, if a district assigned LEAVER-REASON-CODE 60 student withdrew from/left school for home schooling for a student and later received a request for transcript information from a private school for the same student, the district may change the LEAVER-REASON-CODE to 81 enroll in a Texas private school but is not obligated to.

Documentation Requirements by LEAVER-REASON-CODEIn addition to general documentation requirements, requirements specific to leaver reason codes also apply. Following are the specific documentation requirements by leaver reason code. They are grouped into four major categories: graduated or received an out-of-state GED, moved to other educational setting, withdrawn by school district, and other reasons. School leavers with this LEAVER-REASON-CODE are counted as dropouts for state accountability purposes: code 98. School leavers with this LEAVER-REASON-CODE are counted as dropouts for federal accountability purposes: codes 88, 89, 98. These designations are provided for information purposes only. They are not the final or comprehensive description of the definitions used for dropout and completion processing. Definition and use: Use for students who meet all high school graduation requirements (which includes passing the exit-level TAAS or TAKS) at any time during the prior school year, including the summer (through August 31) following the close of the prior year.

To graduate, a student must satisfy the requirements under 19 TAC Chapter 74, Subchapter B. Special education students must satisfy requirements under 19 TAC §89.1070. Students, who complete all graduation requirements in one school year but do not pass the exit-level assessments until a later year, are reported as graduates in the school year in which the exit-level assessments are passed and the diploma is issued.

Documentation requirement: Transcript showing sufficient credits, successful completion of the exit-level assessments (including testing dates), graduation seal, school official signature, and date of completion.

85 Student graduated outside Texas before entering a Texas public school, entered a Texas public school, and left again

Definition and use: This code may be used for students who graduated in another state or country before entering Texas public schools. This code may also be used for students who graduated from Texhoma High School, Texhoma, Oklahoma.

Documentation requirement: Transcript showing sufficient credits, date, and school official signature, and a diploma with a graduation seal.

86 Student completed the GED outside Texas

Definition and use: This code may be used for students who earned GED certificates outside Texas, including students living in Texas and earning GED certificates online from a testing company in another state, before enrolling or after leaving Texas public schools.

Documentation requirement: Acceptable documentation is a copy of the GED certificate or some other written document provided by the testing company showing completion of the GED. Written documentation from the testing company must include the date of GED completion, location, address, and contact information of the company.

90 Student graduated from another state under provisions of the Interstate Compact on Educational Opportunity for Military Children

Definition and use: Per TEC §162.002, student lives in the household of an active-duty military serviceperson, transferred into Texas public schools at the beginning of or during his or her senior year, did not meet requirements to graduate from Texas public schools, did meet requirements to graduate from a school in the sending state, and, under provisions of the Interstate Compact on Educational Opportunity for Military Children, graduated from a school or district in the sending state.

Texas Academy of Mathematics and Science at the University of North Texas, the Texas Academy of Leadership in the Humanities at Lamar University, the Texas Academy of Mathematics and Science at the University of Texas at Brownsville, and the Texas Academy of International Studies at Texas A&M International University.

Documentation requirement: Documentation of enrollment in a college or university must indicate that the student is enrolled full-time in an academic program. Per federal requirement, it is not permissible for a district to document that, at the time of withdrawal, the student intended to enter a post-secondary educational setting. Beginning with students leaving in the 2011-12 school year, a district must document that the student has actually entered a post-secondary educational setting. One of the following types of documentation is required to verify enrollment:

Transcript request. Acceptable documentation of enrollment in college is a records request from the college in which the student is enrolled. Telephone requests must be documented in writing, including the date of the call, the name of the college requesting the records, the name of the person making the request, and the name of the person who received the call. Telephone requests should appear on a standardized, district-approved form. The original of the form should be included in the student's permanent file. Documentation of the method of records dissemination also must be included in the student’s permanent file (e.g., copy of fax activity log, certified mail receipt, encrypted email receipt confirmation, or postage/mail log with complete address information for requesting school).

Verification by an authorized representative of the college. A signed letter from the college verifying enrollment is also acceptable documentation. The letter must state the name and location of the college in which the student is enrolled and the date of enrollment. Other acceptable documentation is written documentation of an oral statement by a representative of the college providing the name and location of the college and verifying that the student is enrolled, signed and dated by an authorized representative of the district.

Verification by the parent/guardian or qualified student. Acceptable documentation includes a letter, signed and dated from the parent, guardian, or qualified student stating that the student has enrolled in college in a program leading to an associate’s or bachelor’s degree.

60 Student is home schooled

Definition and use: Student is being home schooled. This code may be used only for a student whose parent/guardian confirms that the student is pursuing, under direct supervision of the parent/guardian, a curriculum designed to meet basic education goals. The district is not required to obtain evidence that the program being provided meets educational standards.

Documentation requirement: A district must document that the parent/guardian is home schooling the student. Per federal requirement, it is not permissible for a district to document that, at the time of withdrawal, the student intended to be home schooled. Beginning with students leaving in the 2011-12 school year, the following documentation is required to verify enrollment:

Verification by the parent/guardian. A letter, signed and dated, from the parent/guardian stating that the student is being home schooled is acceptable documentation. Letters from parents/guardians must indicate the actual date home schooling began.

Student enrolled in a private school in Texas

Student enrolled in a public or private school outside of Texas

Definition and use: Student is enrolled in a private school in Texas (code 81), the Texas Job Corps Diploma Program (code 81), or a public or private school outside Texas (code 82). Documentation of actual enrollment is required. This code is also used when a student moves from the district without withdrawing but the district receives a records request.

If the student enrolls in another school in the district or another public school district in Texas, a leaver record is not submitted.

If the district did not assign code 81 or code 82 when the student stopped attending, the district can change the original code assigned to the student when the records request or communication from the parent/guardian or qualified student is received. If the original withdrawal date for the student is later than the date the student enrolled in the other school, the withdrawal date must be changed and all attendance accounting records affected by this change must be updated.

Diploma Program or the University of Texas at Austin High School Diploma Program

Definition and use: Student was withdrawn from school and parent/guardian or qualified student indicated at time of withdrawal that the student has enrolled in the State Board of Education-authorized Texas Tech University ISD High School Diploma Program or the University of Texas at Austin High School Diploma Program.

Documentation requirement: The district must receive either a) a records or transcript request from the high school diploma program or b) a letter from the high school diploma program stating that the student is enrolled.

Withdrawn by school district

78 Student was expelled under the provisions of TEC §37.007 and cannot return to school

Definition and use: This code may only be used when:

the student was expelled under the provisions of TEC §37.007, and the term of expulsion has not expired or the student’s failure to attend school is due to court

action.

This code may only be used for a student who was expelled for an offense included in TEC §37.007. This code is not intended for use by districts which assign students to a Juvenile Justice Alternative Education Program (JJAEP).

Documentation requirement: Due process documentation supporting the expulsion.83 Student was attending and was withdrawn from school by the district when the district

discovered that the student was not entitled to enrollment in the district because a) the student was not a resident of the district, b) was not entitled under other provisions of TEC §25.001 or as a transfer student, or c) was not entitled to public school enrollment under TEC §38.001 or a corresponding rule of the Texas Department of State Health Services because the student was not immunized.

Definition and use: This code is for situations in which the district discovers when verifying enrollment information that the student is not entitled to enrollment in the district because the student is not a resident of the district or is not entitled under other provisions of TEC §25.001 or as a transfer student. It is not for a student who was a resident of the district and who stops attending because he/she has moved. This code is also for rare situations in which the student has not met the requirements under TEC §38.001 or a corresponding rule of the Texas Department of State Health Services for immunization, provisional enrollment, or exemption.

Subject to the exceptions in TEC §38.001(c), a student is required to be fully immunized against disease as required by the Texas Department of State Health Services (TEC §38.001(a)). A student may be provisionally admitted if the student has begun the required immunizations and continues to receive the necessary immunizations as rapidly as medically feasible (TEC §38.001(e)). Except as provided by TEC §38.001(c) or by rule of the Department of State Health Services, a student who is not fully immunized and has not begun the required immunizations may not attend school. For further information about immunization requirements, immunization exemptions, and immunization documentation, please contact the Texas Department of State Health Services.

Documentation requirement: Due process documentation supporting the withdrawal. All district actions to withdraw a student must be documented or the documentation for use of this leaver reason code may be considered insufficient. For purposes of leaver reason code 83, due process is defined as completion of

the following steps:

District provides oral or written notice, appropriately documented, to the student’s parent, guardian, or qualified student him- or herself of intent to withdraw the student, reasons for the withdrawal, effective date of withdrawal, and date of hearing or conference at which the parent, guardian, or qualified student will have an opportunity to respond to the allegations that the student is not entitled to be enrolled in the district. Steps 2 and 3 are not required for cases in which the parent, guardian or qualified student agrees that the student is not entitled to enrollment in the district. Step 4 is always required for charter districts.

District provides a hearing or conference at which the district presents the reasons for withdrawal, and the parent, guardian, or qualified student is given the opportunity to respond to the reasons for withdrawal.

District provides a written report to the parent, guardian, or qualified student that contains the findings of fact and district decision following the hearing or conference. The written report shall include notice of the parent’s, guardian’s, or qualified student’s right to appeal the district’s decision.

Charter districts shall notify the school district in which the student resides within three business days of withdrawing a student from a charter school, per Texas Administrative Code §100.1211.

Other reasons

03 Student died while enrolled in school or during the summer break after completing the prior school year

Definition and use: Self-explanatory.

Documentation requirement: Acceptable documentation includes a copy of the death certificate or obituary, a program from the funeral or memorial service, a written statement from the parent or guardian, and written documentation of an oral statement by a parent or guardian stating that the student has died.

16 Student returned to family’s home country

Definition and use: Use for students who are leaving the United States to return to their home country. A student may be leaving with or without family members to live with his or her family, immediate or extended, in the home country. The citizenship of the student is not relevant in assigning this code. This code can also be used for foreign exchange students.

Documentation requirement: Acceptable documentation is a copy of the Transfer Document for Binational Migrant Student completed at the time the student withdraws from school, signed and dated by an authorized campus or district administrator. Acceptable documentation is also a copy of the withdrawal form signed and dated by the parent/guardian or qualified student and a campus or district administrator. The withdrawal form should indicate that the student is leaving school because the student is returning to the home country and should specify the destination. An original signature is not required on withdrawal forms received in the district by fax. Withdrawal forms received by e-mail do not need to be signed by the parent/guardian or qualified student. A signed letter from the parent/guardian or qualified student stating that the student is leaving school because the student is returning to the home country is also acceptable documentation. Acceptable documentation for foreign exchange students includes a written, signed, and dated statement from the student's host family or the foreign student advisor verifying the student's return to his or her home country. Other acceptable documentation is written documentation of an oral statement by an adult neighbor or other adult with knowledge of the family's whereabouts, signed and dated by an authorized campus or district administrator.

Definition and use: This code is for students who are court-ordered to attend a GED program and have not earned a GED certificate at any time during the prior school year, including the summer (through August 31) following the close of the prior year.

Documentation requirement: Acceptable documentation is a copy of the court order stating that the student has been ordered to attend a high school equivalency or GED program. Documentation must include the name of the student, the date of the order, the name of the judge making the order, and the county in which the judge presides. The order should state that, under Article 45,054, Code of Criminal Procedure, the court is ordering the student to attend a high school equivalency or GED program or to take a high school equivalency or GED exam.

89 Student is incarcerated in a state jail or federal penitentiary as an adult or as a person certified to stand trial as an adult

Definition and use: Student is incarcerated in a state jail or federal penitentiary as an adult or as a person certified to stand trial as an adult.

Documentation requirement: Acceptable documentation is one of the following: 1) Oral notification from a law enforcement agency, the office of the prosecuting attorney, or the jail or penitentiary, to an authorized representative of the district, that the student is incarcerated. The written statement of the oral notification shall be signed and dated by the authorized representative. 2) Written notification from a law enforcement agency, the office of the prosecuting attorney, or the jail or penitentiary, that the student is incarcerated.
