At the heart of our care plan we offer person-centred care, which includes a varied and stimulating programme of therapies and daily activities designed to meet individual needs and wishes to promote our residents reablement and enhance their experiences of care and quality of life. We provide a comprehensive package of person centred care for all people who use our services that encourages and supports their individuality and encourages participation in the planning process with the individual and family if appropriate.

We are a fully intergrated service which works in partnership with people who use our services, carers and all relevant agencies and professionals in order to achieve the best possible outcome for individuals.

Our Facilities We have 16 bedrooms avaliable for older people with a primary diagnosis of a complex mental health illness which is possibly complicated by organic mental health illnesses or physical health problems, leading to a risk to self and others.

Our 16 private rooms all have equipt en-suit pathrooms. Our bedrooms have telephone

points for personal, direct lines. Although all our bedrooms are fully furnished we understand that this is your home, which is why we encourage you to personalise your rooms.

All our facilities are on one floor for easy access to all our residents including wheelchair access. We offer homely, comfortable communal areas including a lounge with TV, video, DVD, Game consoles, the internet and WIFI. We have quiet areas which are available for reading and relaxation and personal laundry facilities are available however people who use our services are supported to take care of their own laundry. Our spacious and attractive gardens and courtyard have sitting areas which encourage relaxation and daily activities such as gardening.

We provide a varied choice of menus. Our meals are always freshly prepared to meet the individual dietary, religious and cultural needs, wishes and preferences of our residents. You are able to enjoy your meal in the company of others in any one of our two dining areas, or in the privacy of your own room.

At Beyer Lodge we offer short to medium term health and social care support from a multi-disciplinary team of staff. We provide appropriate care that will stabilise presenting mental health problems with the intention of moving our residents onto a more appropriate setting once their symptoms have improved.

Our treatment and care plansAs part of their personalised care plan all our residents recieve:

• An OT assessment to inform the activities of daily living/ living skills programme, an integral domain of the clinical recovery star based needs assessment and care plan

• The opportunity to access and engage with community leisure activities and vocational rehabilitation activities. Individual and group activities in the clinical setting and in the community.

• Techniques such as motivational interviewing and behavioural programmes, supervised by the clinical psychologist, psychological therapies (such as cognative behaviour therapy for psychosis)

As a crucial part of our rehabilitation programme, we work with patients individually to problem solve and set goals. In addition our clinical psychologist team offers consultation to the staff team to develop psychological formulations of resident’s difficulties, supporting positive relationships between staff and clients, therapeutic optimism and creative interventions.

Wherever possible our staff will work with residents to support them to develop strategies that will allow those to self manage their lives. Our clincial psychologist provides training and supervision to other staff to provide “low intensity” psychological interventions such as behavioural activities, anxiety management, relaxation techniques, relapse prevention and motivational interviewing

Each of the people who use our services recieve a personalised care plan which includes a programme of theraputic activities that are avaliable. Wherever possible our staff will work with residents to support them to develop strategies that will allow those to self manage their lives.

Referrals Referrals must be made by secondary mental health services and, wherever possible discussed and agreed with the service user and/or family/carer. Discharge planning will be discussed on admission and action taken to ensure that the person can be discharged seamlessly to a suitable environment as identified throughout the CPA process.
