Improving Clinical Outcomes and Chronic Disease Management using Big Data and Predictive Analytics

The Informatics Division at Independence Blue Cross (“Independence”) employs advanced predictive models and leverages big data to improve clinical outcomes and transform chronic disease management for its members. In this presentation, Somesh Nigam, Ph.D., senior vice president and chief informatics officer, will discuss Independence’s likelihood of hospitalization (LoH) model for patients with congestive heart failure (CHF) and Chronic Obstructive Pulmonary Disease (COPD), and the associated care gaps Independence can identify, using a platform from Lumiata, to help it gain a deeper understanding of a patient’s current and future health, risk, and cost in order to deliver higher value care. Dr. Nigam will also highlight Independence’s ongoing research collaboration with New York University and NYU Langone Medical Center, which is helping Independence to identify members with undetected or undiagnosed diabetes to ensure they receive appropriate and timely care.

The Evolving Role of Clinicians in Data-Driven Care Delivery

The capacity to work in a ‘systems-based practice’ – or even understand the basic concepts of data-driven care delivery -- is not necessarily intuitive; successful integration and collaboration of professionals will require more than training programs, but a reconsideration of the fundamental roles and responsibilities of physicians and other clinicians involved in patient care. This talk will reflect on direct experience guiding a group of physicians and advanced practice professionals through this evolution as part of population health management initiative.

While this still may the case today, there are ways to build out valuable Business Intelligence applications at a fraction of the traditional cost and time. I will highlight and demonstrate how United Surgical Partners International built out Business Intelligence capabilities along with a Data Warehouse from scratch, and delivered reporting, dashboards and self-service analytics corporate-wide in under 6 months, at a quarter of the expected cost. By bundling SAP Business Objects with Infoburst, USPI has been able to fast-track Business Intelligence and have attained an instant ROI.

Terhilda Garrido has worked at Kaiser Permanente, an integrated delivery system, for more than 20 years. At present, she is vice president, Health Information Technology Transformation & Analytics within the National Quality and Care Delivery organization. Her team is responsible for realizing the strategic value and maximizing opportunities for Kaiser Permanente’s electronic health record. She also currently co-leads KP’s efforts to qualify for ‘Meaningful Use’. Her areas of focus inc lude : eva luat ion of new EHR-based innovations, strategic impact of PHRs/EHRs, the business case for KP’s investment, leveraging HIT to improve quality, patient safety, efficiency and equity. She has published on these topics and lends her expertise to various organizations within the health care industry. She sat on an IOM committee on HIT and Patient Safety and is currently on an IOM committee on EHR ROI. Prior to joining Kaiser, Terhilda did economic modeling and consulting for the European Economic community among others. Prior to joining Anthem, Tanya was employed by UnitedHealth Group (UHG) for 12 years. Nicholas Marko, MD is Chief Data Officer for Geisinger Health System in Danville, PA. He is a data scientist with expertise in predictive analytics, organizational data strategy, data-driven collaboration, and data science consulting. He heads the Department of Data Science & Engineering in Geisinger’s Division of Applied Research and Clinical Informatics (DARCI) and co-directs the High Performance Computing Center in the organization’s Institute for Advanced Applications (IAA). His specific academic interests include integration of heterogeneous data sources and application of advanced mathematical methods and modeling strategies to generate measurable value for organizations in the healthcare sector and beyond.
