Sakra Institute of Digestive and HPB Sciences

The Sakra Institute of Digestive and HPB (Hepato-pancreato-biliary) Sciences has two divisions, i.e. Medical and Surgical. At Sakra World Hospital, we strive to offer the patients a comprehensive multidisciplinary approach for the full spectrum of gastrointestinal, liver, pancreas and biliary diseases. The surgical division has a state-of-art facility for the early detection, diagnosis, and surgical treatment of all complex diseases. There are few key areas of focus, namely Minimally Invasive Organ Preserving Surgery, Pancreas Care Program, Bariatric Surgery and Gastrointestinal Oncology Program.

- 2 -

Introduction

The surgical division of the Sakra Institute of Digestive & HPB (Hepato-pancreato-biliary) Sciences has the following

key areas of focus. They are:

1. Minimally Invasive Organ Preserving Surgery

At Sakra Hospital, majority of gastrointestinal surgical procedures can now be performed by a minimal invasive approach (laparoscopic surgery) with small incisions, quick, painless recovery and early return to work. All intestinal surgeries (esophagus or food pipe, large intestine), a majority of biliary surgery (gallbladder surgery, surgery of the bile duct) and selected pancreatic surgery are performed by the laparoscopic approach. The next natural step in advanced laparoscopic surgery is to combine the minimally invasive approach with organ preservation to enhance outcomes. At Sakra, with scrupulous planning and expertise we strive to safeguard the complete organ and accomplish the objective of treating the disease. Organ preserving approaches are an advantage in the surgery of the liver, pancreas where removal of the critical part of the organ leads to a long term functional disability.

2. Pancreas Care Program

The Pancreas Care Program at the Institute of Digestive & HPB (Hepato-pancreato-biliary) Sciences, at Sakra World Hospital is a comprehensive program for management of pancreatic diseases with a MULTI DISCIPLINARY team. Pancreatic disorders present unique challenge to the physicians in terms of diagnosis, selecting

the most appropriate treatment option and execute the same with the skill and precisionusing skilled personel and technology.

3. Bariatric Surgery

There are many types of weight-loss surgery, known collectively as Bariatric Surgery. Gastric bypass and other weight-loss surgeries make changes to your digestive system which would help you to lose weight by limiting you from the intake of food.

4. Gastrointestinal Oncology Program

The Gastrointestinal Oncology Programundertakes the care of all GI malignancies, including pancreas, liver, biliary tract, and the complete gastrointestinal tract including that of esophagus, stomach, colon and rectum .A multidisciplinary tumor board reviews the cases and management strategy for the patient is charted. The teams comprises of physicians and surgeons with international experience and renowned nationally for their expertise in dealing with these cancers. All diagnostic facilities with expertise in surgical treatment and /or endoscopic palliation is available in the program. The team has considerable expertise in procedures such as hepatectomy, Whipple's Procedure, total mesorectal excis ion and laparoscopiconcology procedures.

5. Biliary Surgery

Injuries of biliary tract after cholecystectomy (gallbladder surgery) are serious problems which require expertise and understanding in successful management. The surgical team has national and international recognition in the treatment and management of bile duct injury following cholecystectomy. The center is a referral center for management of this complex problems.

Our expertise is also in the treatment of choledochal cyst (congenital dilatation of the bile passage) in all age groups, apart from other diseases like gallbladder cancer and bile duct obstruction.

6. Liver Surgery

The team has a large experience of performing liver surgery for liver cancers, cancers of the bile duct and for other liver tumors. The center has a skilled team of surgeons, complimented by intraoperative tools (intraoperative ultrasound, USA and Waterjet dissectors, Bipolar radiofrequency dissecting device etc) to perform a safe and blood less liver surgery. A well trained anesthesia team and a well-equipped ICU for postoperative care thus ensures that all major and complex liver surgery is performed safely and successfully.

Every year, millions of women undergo gynaecological procedures to diagnose, evaluate, or treat gynaecological problems. Gynaecological surgery or treatment procedures are performed on the female reproductive system. These procedures can range from minimally invasive to open surgery. A variety of gynaecological illnesses and conditions can be diagnosed and treated this way including uterine fibroids, cysts, uterine prolapse, etc.

The field of gynaecological surgery is ever-expanding and encompasses the surgical interventions related to women’s health, which includes minimal access surgery, oncology, urogynecology, and fetal surgery. The gynaecological field is rapidly evolving with respect to new developments and innovations in robotics, endoscopy, and interventional procedures.

Laparoscopy/keyhole surgery/minimal access surgery

In order to perform open abdominal surgeries, the surgeon makes a cut which is large enough to have adequate visibility, access to the abdominal organs and allow the use of hand-held surgical instruments. Depending on the size of the patient and the type of operation, the cut may be 6 to 12 inches in length. There is a significant amount of discomfort associated with these cuts. They have more blood loss, prolonged hospital stay, and longer recovery time post-surgery. These traditional techniques have long been used and taught to generations of surgeons, and are widely available.

Advantages of laparoscopy over open surgeryŸ Small incision- less pain and short recovery time, wounds heal quickly. There is

less scarring with better cosmetics.

Ÿ The pain is less. So less administration of pain medication.

Ÿ Reduced bleeding and need for blood transfusion is negligible.

Ÿ 2-3 days stay in the hospital after surgery. Few are discharged on the same day. This enables the patient to return to normal routine life quickly.

Ÿ There is a reduced chance of infections & contamination

Understanding Interventional RadiologySakra institute of Interventional Radiology

The Department of Interventional Radiology (IR) at Sakra is a subspecialty of radiology that uses image-guided, minimally invasive techniques for both diagnosis and therapy. In other words, it is a medical procedure performed by Interventional radiologist using x-ray fluoroscopy, computed tomography, magnetic resonance imaging, or ultrasound.

Interventional radiology, however, is both a diagnostic and therapeutic discipline, where operations are performed using the normal highway system of the body, the network of veins and arteries, through a puncture the size of a pinhole.

The purpose of using Interventional radiology is to diagnose and treat patients using the least invasive techniques to reduce the risk and improve the overall health of the patients.

The range of Interventional radiology techniques is divided into two main types of procedures such as diagnostic and therapeutic.

Diagnostic Procedure:

Diagnostic procedure employs anatomical images either to precisely direct an abnormality biopsy, such as a cancerous tumor, or to insert a radiopaque agent to imagine a hollow structure, such as a blood vessel or canal.

Therapeutic Procedure:

Therapeutic procedure specifically direct treatments to an abnormality, such as stents, laser treatment or clotting agents.

Types of interventional radiology:

1. Diagnostic interventional radiology: The procedures of diagnostic IR include Angiography, Cholangiography and Biopsy.

2. Therapeutic interventional radiology: The procedures of Therapeutic IR include Balloon angioplasty, Endovascular aneurysm repair, Embolization, Thrombolysis, IVC filters, Dialysis, TIPS, Endovenous laser treatment, Cholecystostomy, Catheter placement, Embolization and Percutaneous nephrostomy.

The range of conditions which can be treated by Interventional radiology is vast and constantly expanding. While technology advances and the availability of high-

Overview

quality imaging equipment grows, interventional radiology will bring a host of new treatment options to patients and referral specialists.

The benefits of Interventional radiology are:

1. Reduced risks and shorter hospital stay

2. Lower cost than surgery

3. Greater comfort and quicker

4. Lower costs, greater comfort

5. Quicker recovery time

The Department of Interventional Radiology at Sakra world hospital is offering a full range of advanced interventional radiology services.
