    In order to receive the discounted price listedin this brochure, the participant must registerand make full payment two (2) weeks beforethe start of EACH summer camp session. Ifthe participant does not register and make full

    payment during the discount period, the price ofthe camp will be raised by $20 for residents and$30 for non-residents.

    Full payment must be received prior to the startof each session to be allowed to participate.

    Our camp staff is comprised of individualswho have experience working with children ofall ability levels. A 1:4 staff-to-camper ratio ismaintained as much as possible.

    The campers will also take part in gross motorsports and games, arts and crafts, music,swimming, and field trips. Most trips, specialevent days and swim days are included in thecamp fee.

    pants and parents/ guardians of minors register-ing for the above listed programs/activities mustrecognize that there is an inherent risk of injurywhen choosing to participate in recreationalactivities/ programs.You are solely responsible for determining if youor your minor child/ward are physically t and/or skilled for the activities contemplated by thisagreement. CUSR participants explore new areasof interest, make new acquaintances, and learnlifetime leisure skills.     INCLEMENT WEATHER

    POLICYIf inclement weather occurs, CUSR may stilldecide to hold programs. If the weather turnsdangerous, CUSR will call participants priorto the start of the program to cancel the pro-gram. If you are unsure whether a program will

    meet due to inclement weather, call CUSR.Missed programs due to weather are made upthe week after the end of the program season.Credit is given for additional cancellations.

    SUGGESTED PROGRAMSSome of CUSRs programs and special eventslisted in the program guide are suggested forcertain age groups and disabilities classifica-tions so programs can be designed to meet in-

    dividual needs. However, we will make reason-able accommodations for individuals who desireto participate in programs not suggested for hisor her age group or disability. Call the CUSRoffice for details.

    BEHAVIOR CODECUSR promotes the concept of Equal Fun ForEveryone and strives to accomplish this beliefthrough our program goals. However, certainrules are necessary to ensure everyones safetyand enjoyment.

    BEHAVIORCUSR participants are expected to exhibit ap-propriate behavior at all times. The CUSR be-havior code states that participants shall showrespect to all participants and staff, refrain fromusing foul language, refrain from causing bodilyharm, and show respect to equipment, sup-plies, and facilities. Additional policies may bedeveloped for specific programs, camps, andintegrated programs.

    DISCIPLINEA caring, positive approach will be used regard-ing discipline. CUSR reserves the right to dis-miss participants. Each situation will be evalu-ated on its own merit.

    TRANSPORTATION POLICY/

    SAFETY PROCEDURES1. Transportation must be requested at least

    48 hours prior to the program date. Iftransportation is requested with less than48 hours notification from program date,there is no guarantee transportation will be

    available. NO transportation request at thetime of the program will be accepted.

    2. A minimum of three participants mustregister for transportation in order for it torun.

    3. Transportation is provided from YOUR homeback to YOUR home. We will not pick up ordrop off at any other sites.

    4. CUSR staff will wait a maximum of 5 minutespast your pickup time due to a demandingvan schedule. If you miss your pickup time,we will not return to pick you up. It is yourresponsibility to find transportation to theprogram if you miss your pickup time.

    5. CUSR transports in-district participants only.6. Transportation will only be provided for those

    who sign up and pay for the service at thebeginning of the program. We will notprovide transportation for anyone mid-season that is not on the transportationroster at the beginning of the program.

    7. Persons unable to be in a house aloneshould have someone meet the van at thedesignated time of return.

    8. If needed, the CUSR driver will assistparticipants to and from their homes, butdriveways and walkways must be clear ofdebris and obstacle.

    9. All passengers must be sitting in seats orwheelchairs with belts securely fastened inthe correct manner or the driver will notmove the vehicle.

    10. CUSR staff is responsible for determiningwhether or not a participant can be trans-ported safely.

    11. Persons riding in Amigo-type wheelchairsor strollers must transfer into a vehicle seatand use a seat belt.

    CUSR reserves the right to refuse to transportindividuals. These policies have been developed

    dangerous, CUSR will call participants prior tothe start of the program to cancel the program.If you are unsure whether a program will meetdue to inclement weather, call CUSR. Missedprograms due to weather are made up the weekafter the end of the program season. Credit isgiven for additional cancellations.

    LATE PICK-UP POLICYIn order to be fair to our participants and pro-gram leaders, CUSR has enacted a late pick-uppolicy. The first time a participant is not pickedup within a 5 minute grace period, a writtenwarning will be handed out. For each minute fol-lowing the grace period, a fee of $1/1 minute willbe charged to the parent. After the first warning,each time a participant is not picked up within 5minutes of the program ending, a $1/1 minute

    fee will be charged. Registration for other pro-grams will not be accepted until all outstandinglate fees are paid. Thank you for helping us torespect others through punctual pick-ups.

    INSURANCEIndividual accident liability insurance is not pro-vided by CUSR.

    DISHONORED CHECKSA $25 service charge will be assessed on allchecks returned by the bank for any reason.

    PROGRAM SCHOLARSHIPSBecause some residents are unable to partici-pate in recreation programs due to economichardship, CUSR provides a scholarship policyto reduce certain fees and charges. Scholar-ship applications are not guaranteed and arebased on available funds, program registration,

    or choice of program location. Early applicationsare given priority. Please apply 3-4 weeks beforethe program start date. Applications are avail-able at the CUSR office and can be mailed uponrequest. Need is the primary criteria upon whichscholarship applicants are considered. Scholar-ships are not available for transportation andspecifically identified programs. For more infor-mation, contact the Receptionist at 217-239-1152.

    BROCHURE ACCURACYEvery effort has been made to assure theaccuracy of information contained within thisbrochure. However, errors can occur and cir-cumstances may require adjustments to fees,schedules, locations, or other aspects. CUSRreserves the right to make such adjustmentsand apologizes for any inconvenience their er-rors or adjustments may cause.

    REGISTRATIONYou may register for any program at the CUSRoffice or any Champaign/Urbana Park Districtfacility. These locations in Champaign are HaysCenter, Bresnan Meeting Center, Springer Cul-tural Center, Douglas Community Center, andthe Tennis Center. In Urbana, you can registerat the Phillips Recreation Center. Full pay-

    ment must be received upon registration unlessspecific arrangements have been made withthe CUSR office. Registrations will not be pro-cessed if a fee balance remains from a previousseason. Registrations are accepted in person atthe CUSR office, through mail, and by fax (217-373-7951). When faxing, please include creditcard information so we may process the regis-tration. Program specialists and leaders MAYNOT receive payment or registration forms at

    programs. CUSR does not prorate for late reg-istration. Exceptions may be made for peoplewho are new to the community.

    NON-RESIDENT

    PROGRAM FEEParticipants that do not pay property taxes inChampaign or Urbana will be required to pay50% more than residents for programs. The NRlisted next to the program fee is the Non Resi-dent fee. The lesser fee associated with each

    program is the Resident fee.

    SATISFACTION

    GUARANTEEDCUSR constantly strives to provide participantswith high quality recreation programs, activities,and events. To encourage first-time participation,as well as, repeat participation, CUSR has a re-fund policy highlighted by a satisfaction guaran-tee. This special refund policy provides us withtimely feedback about dissatisfaction regardingour services. If you are not completely satisfied,tell us, and we will either invite you to repeat theclass at no charge, allow you a credit that can beapplied to another program, or issue you a re-fund. A refund will be granted if you were dissat-isfied with the experience due to circumstancesunder CUSRs control. If you are not satisfied, wekindly ask you to give us your input or ideas forimprovement. Requests for the above mentioned

    options must be made before the midpoint of theprogram or within two weeks of a single meetingactivity. All refund requests must be submitted onthe Satisfaction Refund Request form and turnedinto CUSR. A $5 service charge will be deductedfrom all refunds. Satisfaction Guarantee does notapply to trips and special events with prepaid ad-missions and tickets.

    PROGRAM CANCELLATION

    POLICYAll registration cancellations must be made atleast one week prior to the start of the programin order to receive a full refund. For programswith cancellation dates listed, registration mustbe cancelled no later than the specified date toreceive a full refund. Any program cancellationsafter the one week deadline will receive NO re-fund A $5 service charge will be deducted from
