The following items must be included and submitted to the assistant principal’s office in the main building at BAHS. Students that live outside of the Bel Air High School feeder pattern need to submit a transfer request form. Transfer forms can be picked up in the assistance principal’s office in the main building. This form is submitted each year for approval as a campus requirement for enrollment at BAHS. NOTE: The Bachelor of Science in Nursing (BSN) requires a separate application.

Students in entering 11th grade may be considered based on their academic background or previous enrollment history in a health profession program in the region or state of Texas.

NEW YISD H.S. STUDENTS 9TH/10TH

Students who are enrolled in another district high school or transferring from another YISD high school, must fill out this packet to be considered for the Center for Health Professions Program. Students applying for the program must submit all of the above requirements except for 7th and 8th grade STAAR Scores. Students need to submit 9th and or 10th grade E.O.C. results (i.e. Algebra I, Biology, English I, English II and a high school transcript that shows the student’s GPA. Finally, new students need to enroll in either/both Principles of Health Science and or Health Science for 9th/10th and Medical Terminology 11th grade year to establish an academic foundation with our programs.

CURRICULUM CHOICESBel Air High School Center for Health Profession students will be enrolled in advanced version of academic courses for 9th / 10th grade i.e. PAP English I, PAP Algebra I, PAP Biology, AP Human Geography. BSN students follow an alternate curriculum that meets high school and EPCC requirements for their respective diploma/degree.

IMPORTANT NOTE:Schedules are subject to change up until the third week of school due to the balancing of class loads and

agreements with outside medical and affiliations, which supports our different programs.

Students entering 11th/12th grade – HP courses take priority over sports/elective periods during the school day due to specialized clinical/practicums. The Clinical/Practicum period is a 4 hour block that include the mandatory course, Anatomy and Physiology. To ensure all core coursework for high school graduation is met, a student’s sport/elective participation will need to be before or after school.

We, the student and parent, understand that students who plan to attend Bel Air High School’s Center for Health Professions must

meet the minimum standards for acceptance. We recognize that attendance at Bel Air High School Center for Health Professions is

a privilege not a right. Admission to the Center for Health Professions is competitive and admission is based on the student’s ability

to meet the set academic, attendance, behavior and citizenship criteria. Students are held to a higher standard than required by

non-Health Profession schools. With this acknowledgment, we understand that the following standards and requirements must be

complied with in order to be considered for enrollment and continued enrollment at the Center for Health Professions. Failure to

adhere to the following guidelines will lead to the student being placed on probation and /or separated from the program.

As a future student & parent we understand and agree to:1. Students must prioritize their schedule by not overextending themselves and making commitments with other programs that can affect

their status in the Center for Health Professions. Especially during the11th and 12th-grade years.2. Attend all mandatory H.P. meetings, attend tutoring in the No Excuse Zone tutoring lab if a student's grade falls below 70%, and attends

Saturday/Summer T.S.I. tutoring. The T.S.I. is an exam for enrollment in dual credit courses.3. Maintain positive behavior without any discipline referrals. (NOTE: Students placed at Cesar Chavez Academy, those who are

involved with controlled substances or alcohol, fight, bullying, break any rule(s) at clinical sites, inappropriate behavior on social media, and or other behavior violating YISD Code of Conduct will be considered a severe violation and will be immediately separated from the Center for Health Professions Program).

4. Maintain exemplary attendance; in-class attendance for all classes must meet the 90% rule.5. Maintain an 85 cumulative G.P.A. yearly – B.S.N. students must maintain a 90 cumulative G.P.A. (or 3.0 or higher on a 4.0 scale).6. Enroll and complete 2-3 Advanced Classes in core content area (based on school year) every year (ie. Dual Credit, Pre-AP, AP, or

OnRamps).7. Based on incoming credits in Algebra, students might need to double up on math courses in their sophomore year.8. Students must have all cell phones off while at their clinical/medical/affiliation sites. Using a cell phone, social media, and or taking

pictures or being in a picture while at a clinical site/medical center is viewed as a severe violation of a patient's privacyrights. (Please see #2) and can lead to removal from the program.

9. A student must take all required exams for certification to include courses/exams after the high school graduation date.10. Students in the 12th grade must provide their transportation as needed to fulfill all course requirements. (NOTE: failure to have

steady transportation at the start of the clinical rotation might lead to the removal from the program during any part of the semester. No carpooling.).

11. The student must follow the district and the Center for Health Profession dress code, including but not limited to beards, hair length/color/style, grooming, fingernail length, facial piercings, gauges, dress, and footwear.

12. As an H.P. student, I will abide by all YISD discipline policies to include no involvement with gangs or gang activities, including exhibiting gang graffiti, gang clothing, or gang insignias. (Please see #2)

13. The student will demonstrate the appropriate social manners while interacting with different health care professionals and adhere to all rules and regulations established at school, medical center, or H.P. event.

14. The student will return all required paperwork the following day. Failure to do so is an act of refusal to comply with H.P.requirements.

15. The student will abide by ALL HIPAA regulations and not jeopardize BAHS clinical teacher's certification and or credentials by violations of HIPAA, YISD regulations, and or the medical center/clinical site rules.

16. The student will pass all E.O.C. exams on the first proctoring.17. The student will have all Titers, updated immunizations record, and other required paperwork completed as of the first day ofthe school

year. Not complying can lead to removal from designated H.P. programs (i.e. Dental, LVN, E.M.T., Clinicals)18. The student must be enrolled in a H.P. course to be a member of the Center for Health Professions. Once in a program, I cannot move to

another H.P. pathway during the school year19. If a clinic/affiliation requests that an H.P. student not return to the site, the student cannot enroll in a different clinic. If the student is

NOT in good standing, the student will exit from the Health Professions program. If a student is in good standing, the student will be placed in a non-clinical/affiliation H.P. course to remain in the program.

We understand that because of the health/medical focus of the Center for Health Professions, students may work directly with blood and body fluids, and they will study sexually transmitted diseases. We understand that students who attend the Center for Health Professions are involved in activities at school and while at health care facilities such as hospitals that may require handling of highly confidential patient information, assisting with patient care and transporting patients in beds or wheel chairs. Students involved in such activities will be required to abide by the standards set at such facilities. Health care facilities may have rigid requirements concerning the aforementioned.
