“Manifestaciones clínicas y de laboratorio que cumplan con los criterios establecidos por la ACR”
Proteinuria de mas de .5 grs/ dia o mas de 3 +++ en tira reactiva
y/o cilindros celulares que incluyan granulares, tubulares o mixtos
Sedimento urinario activo (5 hematíes / 5 células blancas)
Biopsia renal que demuestre que demuestre GNF mediada por complejos inmunes compatible con NL.
Opinión de un reumatólogo u nefrólogo.

“CRITERIOS DE WALLACE Y DUBOIS”
AL MENOS 1:1.- Biopsia renal que demuestre GN mesangial clase IIb, proliferativa focal, proliferativa difusa o membranosa.
2.- Disminución del 30% de la DepCr en un periodo de 1 año en LES activo.
3.- Proteinuria mayor de 1 gr en orina de 24 hrs.
AL MENOS 3 DE LOS SIG EN UN PERIODO DE 12 MESES:1.- Albumina sérica menor de 3 grs/dl.
2.- Proteinuria sostenida de 2+ a 4+ .
3.- Cpos ovales grasos o cilindros granulosos, hialinos o eritrocitario en orina.
4.- Hematuria persistente (> de 5 en HPF)

CONCLUSIONES TERAPIA DE MANTENIMIENTO
MMF y AZA son seguros y con excelentes resultados a 3-4 años
El tratamiento de mantenimiento es bien tolerado por periodos prolongados
en pacientes embarazadas o que planean un embarazo AZA es el tratamiento de elección

CONCLUCIONES
• El tratamiento de la NL al día de hoy continua siendo un reto terapéutico
• La inducción con micofenolato de mofetilo es segura y tiene menos efectos adversos que la ciclofosfamida
• El tratamiento a largo plazo con micofenolato de mofetio y con azatioprina es equiparable
• El uso de rituximab no a resultado sr efectivo en los ensayos clínicos
• Se requieren ensayos clínicos controlados para conocer la utilidad de los agentes nuevos propuestos
• Las estrategias de renoproteccion deben implementarse en todos los pacientes con nefritis Lúpica.
