Prompts for the Performing Authority PA confirm (initial)
The Issuing Authority or site delegated representative has demonstrated all the isolation points for this task including PBU points AND I HAVE EXPLAINED THESE TO THE WORK PARTY.
I have demonstrated to my Work Party all prerequisite controls required for this permit.
Explain the hazards identified in the Risk Assessment for this Permit to Work (refer to Risk Assessment sheet). Ask your work party What could go wrong with this job? If new hazards are found STOP THE JOB.
Ask all the Work Party to confirm what their role is in this task and what controls measures in this task they are responsible for (e.g. who is the standby man?)
How you will be communicating to others during this task? (CHECK! Do your radios have full batteries? Does the Work Party ALL understand the hand signals?).
Show the work party any other worksites or activities that could affect your safety (refer to your Permit)
