Benefit of up to 66 2/3% of your weekly pay to a maximum of $3,462 a week for up to 26 weeks if you become disabled.
Employees may elect up to $130 per month for mass transit and up to $250 per month for eligible parking with pre-tax dollars.
Paid Time Off (PTO) for vacations, illness or personal time. Employees accrue 3, 4 or 5 weeks of PTO per year, depending on years of service.
Contribute pre-tax or ROTH dollars up to the IRS maximum. Equinix will match 50 cents on every dollar of your pre-tax or ROTH contributions up to 6% of your eligible earnings.

All employees who are regularly scheduled to work 20 or more hours per week are eligible for benefits.
When Coverage Begins
When you join the team at Equinix, your employee benefits begin the day you start. You have 31 days from your start date to make your selection. If you take no action, you will be defaulted into the Cigna Open Access Choice Fund (HRA) Plan at the employee only coverage level.
Who Is Eligible?
You have the option to enroll eligible dependents for health care benefits, including:• A spouse or domestic partner• Children or children of your domestic partner; up to age 26• Dependent children without self-support due to a disability
Making Changes to Benefits – Qualifying Events
You cannot add or drop coverage until the next open enrollment period, unless you have a change in family status or experience a “qualifying event.” Your benefit changes must be consistent with the specific life event.
Qualifying Events Include:
• Marriage or divorce• Birth or adoption of a child• Change in spouse’s employment status• Death of a spouse or dependent

HOW TO ENROLL IN BENEFITS
On the landing page, click on Benefits Enrollment when you are ready to enroll in your benefits.
After adding and reviewing your personal information, click next to continue and begin your benefits enrollment process.
To begin your enrollment process, click Benefits Enrollment then choose the plan you wish to enroll in and select the dependents you wish to enroll. Click on the next plan to continue your election process.
Confirm your elections by clicking Finalize Enrollment Elections at the bottom of the page. A confirmation e-mail will be sent to you once you finalize your enrollment.
When you log in for the first time, you will be taken to the New Account Setup page, where you will complete your personal information.

Your contributions for medical, vision, dental and flexible spending accounts, as well as your contributions to the 401(k), are taken on a pre-tax basis.
Employee, Dependent Voluntary Life and your ROTH contributions are taken on an after-tax basis.
Keep in mind that if you cover a domestic partner, the cost per pay period for medical, dental and vision coverage for your domestic partner is taxable for both federal and state taxes (unless you reside in a state that recognizes domestic partnership, and you and your domestic partner are registered according to the state law).
If you receive coverage elsewhere and opt out of the Equinix medical, dental, and vision plans, you will receive a $200 credit per month.

UNDERSTANDING THE CIGNA OPEN ACCESS CHOICE FUND (HRA)
What is an HRA?
A Health Reimbursement Account (HRA) is a healthcare plan combined with a reimbursement account that is funded by Equinix. The Cigna Open Access Choice Fund (HRA) plan provides a way for employees to be better consumers of health care services and in turn provides an affordable healthcare option with low payroll contributions. How does an HRA work?
Equinix provides funds in an HRA account for you to use towards your healthcare expenses. Equinix funds 50% of the annual deductible into this account. If you terminate employment with Equinix, you can’t take the money with you.
Once you’ve used your funds and met the deductible, your coinsurance kicks in. If you don’t use all of the money in the account, it will roll over into the next plan year, and Equinix will still fund 50% of the deductible into this account.
Why you might want to choose the Cigna Open Access Choice Fund (HRA):
The HRA plan has the lowest payroll contributions (No deductions for employee only level) of all of Equinix’s health plans. The reimbursement account is funded by Equinix to help you pay some of the costs of eligible health care expenses. If you don’t use all of the funds during the year, the remainder gets rolled over for future years, allowing you to continue saving for future health care expenses.

The Cigna prescription drug program uses a three-tier design. The amount you pay varies by the type of drug and whether you fill your prescription at a retail pharmacy or through mail order.
KAISER PRESCRIPTION DRUG PLAN The Kaiser Prescription drug program uses a two-tier design. The amount you pay varies by the type of drug. Kaiser offers you the convenience of refilling your prescription online or through the mobile app. You have the option to pick up your prescription at a Kaiser pharmacy or have it mailed directly to your home.
Using a Cigna participating pharmacy is a simple way to save a little money. Cigna has a large network that includes more than 61,000 retail pharmacies nationwide.
Using Cigna Home Delivery Pharmacy is an easy, reliable way to get your medications. The medications often cost less than at a retail pharmacy, and you receive an extra month of medication for FREE. The Cigna Home Delivery Pharmacy is a great choice for anyone who takes long-term maintenance medications regularly, such as those prescribed for allergies, anxiety, arthritis, asthma, birth control, diabetes, high blood pressure, infertility and many other medical issues. When you fill your prescription through Cigna Home Delivery Pharmacy, you get up to a 90-day supply delivered to your home. To transfer your prescriptions to Cigna Home Delivery, call 1.800.285.4812.
Cigna Specialty Pharmacy Services provides resources and support when managing a complex condition. With Cigna Specialty Pharmacy Services, you’ll have a clinical team of specialty pharmacists and technicians available to help you understand your medication and possible side effects, day or night. Call 1.855.326.7463 to transfer your specialty medications. With your okay, Cigna will call your doctor to transfer your prescription on your behalf.
If a brand-name prescription drug becomes available as a generic drug, the prescription drug tier and the amount of the coinsurance may change. You will pay the coinsurance based on the prescription drug tier to which the prescription drug is assigned.
When you receive a prescription from your physician, the pharmacy will fill the prescription with the equivalent generic drug, if one is available. If your physician specifies “Dispense as Written,” the prescription will be filled with the specified brand-name drug. If you choose to have the prescription filled with a brand-name prescription, and “Dispense as Written” was not included on the prescription, you will pay the difference between the cost of the brand-name and generic drugs.

When it comes to having a baby, there is so much to prepare for – choosing a hospital, buying a car seat, preparing the nursery…The Cigna Health Pregnancies, Healthy Babies program provides education and support throughout the entire pregnancy and after. If you enroll in the program during your first trimester and complete the program, you will receive $150. If you enroll in the second trimester and complete the program, you will receive $75. This program is designed to help those at risk or living with cancer manage and treat their complex condition. It provides access to specialized nurses, health educators and behavioral health experts equipped to assist members and their families with preventive care, understanding treatment options, coordination of care, and lifestyle issues. The program also offers members support to help diagnose potential cancerous conditions early and prevent the development of the disease.

Pre-Tax Spending Accounts provide a tax-advantaged way to pay for eligible health care, dependent care and commuter expenses. Pre-Tax Spending Accounts let you pay for eligible health care, dependent care and commuter care expenses with dollars that are automatically withheld from each paycheck before taxes. They reduce your taxable earnings so you pay less in income taxes. You decide the total amount you want withheld for the plan year. That amount is then deducted in identical increments from your paycheck in each pay period during the plan year.
The Health Care FSA gives you a tax break on many healthcare expenses that are not covered by other plans. By anticipating your expenses and arranging for deductions to be made from your paycheck each pay period, you lower your tax bill. You must enroll each year, as contribution amounts are not carried forward from one year to the next. You can set aside up to $2,550 each plan year on a pre-tax basis to cover eligible health care expenses not reimbursed by any other health plan.
The Dependent Care FSA allows you to pay for eligible child care or elder care expenses while you are working. You must enroll each year, as contribution amounts are not carried forward from one year to the next. You can set aside up to $5,000 each plan year on a pre-tax basis to cover the cost of dependent day care expenses.
The amount you contribute to a Dependent Care FSA cannot be greater than your income or your spouse’s income, whichever is less. If your spouse contributes to a Dependent Care FSA through his or her employer, your combined contributions may not exceed $5,000. If you are married and file separate tax returns, you can contribute up to $2,500 per year.
For expenses to be eligible, they must be incurred because you and your spouse both work or attend school full-time.
• Children must be under age 13• Care providers can be anyone except your spouse, a dependent claimed on your tax return, or a child under age 19
• Disabled dependents (a child, a spouse or parent) also qualify for reimbursement if they spend at least eight hours a day in your home
Eligible dependent care expenses include nursery schools, day camps, licensed day care centers and day care in your home (except if the provider is the child’s parent or your child under age 19). Continued on page 25.

Careful Planning Required
You should plan your FSA contributions carefully. Here’s why:
• Use it or lose it. IRS rules require that any money left in your flexible spending account at the end of the plan year must be forfeited
• Money set aside for health care expenses cannot be used to reimburse dependent day care expenses, and vice-versa
• Any health care or dependent care expenses that are paid from FSAs cannot be claimed as a deduction or credit when filing your income tax return
• You cannot stop or change contributions during the year, unless you have a qualified status change
• Once you terminate employment, only expenses incurred before you terminated are eligible for reimbursement from your FSA, unless you elect to continue your Health Care FSA through COBRA
You will be reimbursed for dependent care expenses only up to the amount of your dependent care spending balance and only after the care has been provided.
Your Health Care and Dependent Care FSA accounts include a grace period. The grace period extends 2 1/2 months after the end of the plan year, until March 15, when you can continue to incur expenses and use the funds remaining in your account to cover expenses. The grace period ensures that you have the opportunity to maximize your FSA funds and avoid forfeiting money through the IRS use-it-or-lose-it rule. It is not an extension of the plan year.
Your FSA plan also includes a run-out period which allows you to submit claims incurred during the plan year and the grace period. The run-out period ends 195 days after the end of the plan year. After the close of the run-out period, you forfeit any of the funds remaining in your account.
The Commuter Benefit plan allows employees to pay mass transit and eligible parking expenses with pre-tax dollars. Employees may elect up to $130 per month for eligible transit expenses and $250 per month for eligible parking expenses. The plan allows you to pre-order your transit passes or commuter vouchers, pay directly to parking vendors or pre-order commuter check cards.
Navia, our flex plan services administrator, has a platform called Flexi-Pass which is a single card solution allowing you to load your monthly transit/parking amounts onto an existing Benny card. The card can be used with the transit or parking merchant to make monthly purchases.

To provide you and your family with financial security, Equinix offers Basic Life and AD&D Insurance through Minnesota Life to all eligible employees at no cost. Basic Life and AD&D insurance provides a cash benefit to you or your loved ones in the event of death or a serious accidental injury.
Basic Life Insurance pays a benefit equal to two times your annual earnings, to a maximum of $750,000. Life insurance pays benefits upon your death. AD&D insurance pays a benefit of two times your annual earning if your death is the result of a covered accident. If you die from an accidental injury, your beneficiary will receive both the life and AD&D benefit. AD&D also pays benefits if you are severely injured and suffer a loss such as the loss of a limb or eyesight. Employer Paid Term Life and AD&D will decrease to 65% of the original coverage amount at age 70.

LIFE & ACCIDENTAL DEATH AND DISMEMBERMENT (AD&D) INSURANCE
You can supplement your basic life coverage with Voluntary Life and AD&D plans that provide additional protection. Voluntary Life and AD&D Insurance allows you to buy additional coverage of one to five times your annual earnings, to a maximum of $500,000. Voluntary Life Insurance can only be purchased for your spouse, domestic partner and dependents if you are insured under the voluntary life plan.
Employees are able to enroll or make changes even if not currently enrolled for coverage or previously declined coverage.

Coverage requested during open enrollment may require evidence of insurability and approval by the insurance carrier. When you enroll on EquiLife CONNECT, you will be able to download an “evidence of insurability” form to complete and submit to Minnesota Life.

Equinix offers eligible employees disability insurance through Reliance Standard that helps protect you and your family from the financial uncertainties and hardships that can accompany long-term illness or disability. Equinix pays the full cost for short- and long-term disability coverage.
If you become disabled, Equinix provides a Short-Term Disability (STD) benefit of up to 66 2/3% of your weekly pay to a maximum of $3,462 a week for up to 26 weeks. Benefits begin on the eighth day for both accident and illness. Equinix provides full pay to employees during the 7 day wait period prior to benefits starting. Short-Term Disability benefits from Equinix are reduced by payments you receive from state plans for employees located in states covered under a state disability plan.
Equinix provides a Long-Term Disability (LTD) benefit of up to 66 2/3% of your monthly pay to a maximum of $15,000 a month if your disability extends beyond 180 consecutive days.

Employees who are traveling on the business of or at the expense of Equinix, outside their country of residence or permanent assignment, are covered by our Cigna Global BTA plan. The plan covers emergency medical and dental services, as well as replacement medicine for lost prescriptions, and medical evacuation.
Chubb provides travel assistance such as emergency travel arrangements, assistance with the replacement of lost or stolen travel documents or luggage and baggage delay. Identity theft and carjacking benefits are available as well as many others.

When you need help with the daily challenges of life, the Employee Assistance Program (EAP) is here to help you. The EAP is administered through ComPsych GuidanceResources and is paid for by Equinix. The EAP provides confidential counseling sessions and referrals, as well as a host of other resources to help you navigate some of life’s changes.
These sessions can help you with emotional problems, relationship issues, substance abuse, legal or financial concerns and more. Other services include health and wellness articles and referrals. The Equinix 401(k) Plan helps build financial security for your future. It also provides important tax advantages. Fidelity administers the plan.
You will be automatically enrolled in the plan and begin contributing 5% approximately 30 days following your date of hire. Fidelity will provide you additional information, which will be mailed to your home. You can choose to opt out of auto enrollment by going to netbenefits.com and setting your contribution to 0%. You may choose to participate again at any time. Your contributions:You may contribute a portion of your pay (up to the IRS annual limit) to your 401(k) account. If you are 50 years of age or older in 2016, you are eligible to make an additional “catch-up” contribution (up to the IRS annual limit).
Your contributions to the 401(k) plan are taken pre-tax. This lowers your taxable income. However, you have the option to pay taxes now called the ROTH 401(k) and you will receive your money tax-free upon retirement.
Any distributions from the plan will be subject to income tax. You may change, stop or start your contributions at any time by going to netbenefits.com. You are always 100% vested in your contributions. If your funds are in a prior qualified retirement plan, Fidelity allows you to roll over your account balance into the Equinix 401(k) plan.

All Equinix regular employees working an average of 20 or more hours per week are eligible for PTO and shall accrue PTO each pay period, beginning on their first day of work. PTO will be prorated for those employees who are scheduled to work less than 40 hours per week (but who regularly work a minimum of 20 or more hours per week). Employees working an average of less than 20 hours per week are not eligible for PTO under company policy. PTO may be used for reasons such as vacation, personal time off, sick days or time off to care for family members. PTO is accrued based on years of service with the company, as described below.
Receive up to one work week paid for the care or adoption of a child.
Up to two days paid annually (one every six months) for volunteer work.
Up to two weeks paid per year for service on a jury or as a witness. Up to two weeks paid per year for military service.
Up to three work days off with pay or up to five work days off with pay if long-distance travel is required.
An employee will cease accruing PTO once she/he has accrued 250 hours. PTO accrual will resume when the employee takes sufficient time off to bring her/his accrual total below 250 hours.
