FINAL REPORT - UNIT 1 INTIEIM REPORT - UNIT 2

Description of Deficiency

During an environmental qualification review for equipment at Watts Bar Nuclear Plant (W1O), a deficiency was identified in TVA General Construction Specification G-38, "Installing Insulated Cables Rated V .o 15,000 Volts." The deficiency was that G-38 allowed the use of electrical cable splicing methods and materials (e.g., tapes, preinsulated butt splices, and nonnuclear Raychem heat shrinkable tubing) which are not qualified for class 1I applications in harsh environments. The cause of this deficiency is that while G-38 did specify that nonqualified materials could be used in nonc.ass 1B applications, it did not specifically require nor clearly state that only qualified materials could be used for class 1I applications in harsh environments.

Safety Imlications

Unqualified insulation on class 1I terminations in harsh environmertz could deteriorate due to the effects of heat, moisture, or radiation. This could allow affected esbles to short circuit to other cables or to conduit. This could result in a failure or spurious actuatior of affected class 1i equipment. This could adversely affect the safe operation of the plant.

Corrective Action - Unit I

TVA is revising G-38 to more clearly define the requirements for the use of qualifie4 termination and splicing materials for class 1I applications. All class 1B terainations and splices located in harsh

navironments will be identi:'ed. Those affected splices or terminations which are comprksed of unqualified materials will be reworked using qualified materials as specified in TVA Standard Drawings SD-E12.5.3, SD-412.5.4, SD-B12.5.5-1, SD-312.5.5-2, SD-E12.5.5-6, SD-E12.5.7-1, SD-112.5.7-2, and SD-B12.5.b, and C-38.

The revision to C-38 will be completed by November 30, 1985. All corrective actions for WBN unit 1 for this item will be completed by fuel load for unit 1.

All future terminations and splices for class 1i applications in harsh environments will be instailed using only qualified materials as specified above. This will prevent recurrence of this deficiency.

Interim Progress - Unit 2

TVA is still evaluating this deficiency for WBN unit 2. Corrective actions for unit 2 will be similar to those for unit 1. The next report on this item will be submitted on or about December 6, 1985.
