Florida Third-Party Voter Registration Organization (3PVRO) Kit This kit contains: 1. Form DS-DE 119. This is the two-page form you can use to register yourself as a 3PVRO. In order to collect Voter Registration Applications from people wishing to register to vote, you must be registered as a 3PVRO and use your 3PVRO ID#. (Anyone may hand out V oter Registration Applications without being registered as a 3PVRO.) To fill out the form, you can use your own name as the name of the 3PVRO. You may wish to add “FTB” for Feel the Bern, so for example the name of your 3PVRO would be “Jane Doe – FTB.” Use your own name and home address for the registered agent, and choose All counties. You may list yourself as the officer. You can leave item 6 blank. People who volunteer with your 3PVRO do not need to be listed as employee registration agents. Sign the form and scan and email it to [email protected] or fax to 850-245-6291.
