We Need Your Enthusiasm!To participate, each team needs a coach, so please consider signing up to volunteer coach. All coaches must be willing to submit to fingerprinting.

Do You Love The Sport?OFFICIALS NEEDED!Individuals, at least 16 years of age, are needed to officiate games in K through fifth grade leagues; Saturday and/or Sunday hours. Salary is determined by previous referee experience and education. Officiating instruction is provided in late November and early December. For more information, call the call the Sports office at 240-777-6870. Help these kids have fun!

Payment InformationFull payment is due with registration. Non-County residents pay an additional $15 per participant, per activity. This includes City of Gaithersburg and City of Rockville participants. If your check is returned unpaid, your account will be debited electronically for the original check amount and electronically or via paper for the State’s maximum allowable service fee. Payment by check constitutes authorization of these transactions. You may revoke your authorization by calling 800.666.5222 ext. 2 to arrange payment due for any outstanding checks and service fees due. We accept checks, Visa, MasterCard, American Express, Discover, or Money Order. Make checks payable to ActiveMONTGOMERY. Checks must have name, current address, phone number, and driver’s license number written or printed on check. Send separate registration form for each child, in each sport. Complete each registrationform fully and legibly. Please print all information.

Withdrawal PolicyRequest for withdrawal must be submitted in writing five business days or more before the start date of the program. Send withdrawal requests via email to rec.refund@montgomerycountymd.gov. If your written withdrawal request is received on or after the start date of the program, your credit will be pro-rated based on the date the request is received. In addition, all refund written requests received after seven days prior to the start date of the program are subject to a withdrawal fee of $25. This request must include the participant’s name, payer’s name, address, phone number, course number, reason for withdrawal, and specify credit or refund. All refunds will be issued to the payer in the same form (check or charge) as the payment was received. Refunds will be processed within 2-3 weeks of the receipt of your written request.

Instructional Winter Basketball (Grades K-2) Increase balance, coordination, and fine motor skills while learning the fundamentals of basketball!! Noncompetitive beginner league emphasizing basic skills, good sportsmanship, and fun! Teams are assigned to the closest available gym.

Instructional teams may be scheduled hourly as follows: Saturdays: 9:00am – 6:00pm • Sundays: 12:00pm – 6:00pm

Registration begins October 5, 2015 and closes November 30, 2015

8 Sessions: Begins the weekend of January 9 or 10, 2016• Four practices at the location and time you are placed• Four low key scrimmages. Scrimmages may be played at alternate sites, at varying times,

but on the assigned day.

Fees: $83 per player (County resident) $98 (Non-County resident, including City of Gaithersburg and City of Rockville). Fee includes eight one-hour sessions, team shirt, and individual participation awards.• All checks are payable to ActiveMONTGOMERY• Parents are encouraged to register online at ActiveMONTGOMERY.org

Register as an individual:• Players who register as individuals will be assigned to teams as space allows• Placement of individuals is contingent on sufficient registrations to form a team and

Youth Winter Basketball (3rd-8th Grades)Dribble….Pass…Shoot….Score!!!! This recreation basketball league is for youth in grades 3 – 8, emphasizing skill development, fundamental strategy, sportsmanship, and fun! Separate leagues for boys and girls for each grade. Every player is guaranteed to participate in each game. Registration begins September 28, 2015 and closes on November 6, 2015.

Practices: Begin November 30, 2015• All teams practice once a week (weekday evenings only)

altered.• Teams may have no more than two players on their roster who participate on any select
