Account Information: a. Create user name b. Create password 6. Member Information: a. Enter First Name b. Enter Last Name c. Enter Date of Birth (mm/dd/yyyy) d. Enter Member ID: This is the employee's social security number e. Enter Group ID: riseup (not case sensitive) 7. Challenge Questions: a. Please choose a different question for each of the 3 areas b. Create an easy to remember answer 8. Select “Create Account.” 9. Once your account has successfully been created, click “Continue.” 10. Login with your newly created User Name and Password. FOR RETURNING USERS: 1. Go to: www.umsriseup.info. 2. Select "200 reasons to RiseUP" from the left navigation bar. 3. Click 200reasons to RISE UP. 4. Login with your User Name and Password.
