Project 1
Updating of HSE Cases, Major Accident Hazard Risk Assessments (MAHRA), Performance Standards and Verification Schemes for the following Jack-Up Units working in the Americas and Mexico for Seadrill.
The following Jack-Ups where included in the work programme:
• West Castor
• West Intrepid
• West Titania
• West Freedom
• West Courageous
• West Defender
• West Oberon
The project scope of work included:
• Full HSE Case update to include IADC Guidance and the Seadrill HSE Case Directives
• MAHRA updates, MAH BowTie ratification and QRA and Sensitivity studies ratification
• Performance Standards and Verification Scheme of SECE’s
This work has been undertaken with direct consultation and input from both the Crews, Management and Regional HSEQ inputs.
Project completion is due in October 2017 in line with the established and agreed work programme with Seadrill Americas and Mexico Region.

Project 2
Development of a suite of SECE Performance Standards and Verification Scheme for Edrill ED1 and ED2 Tender Assist Barges in China.
Review of existing risk assessment and CMMS for listed Safety Critical Equipment.
Listing of SECE’s from the process linked to MAH (Major Accident Hazard), MEI (Major Environmental Incident) and MPI’s (Major Pollution Incident) and their respective BowTie barriers.
Development of the SECE Performance Standards for verification.
Development of Verification Scheme for Performance Standard assurance linked with CMMS and the HSE Case. Reports on all deliverables.
This work has been undertaken with direct consultation and input from both the ED1 and ED2 Crew, Management and Regional HSEQ inputs.
Project completed in April 2017 in line with the established and agreed work programme with Edrill Singapore and ED1 and 2 Rig Management Team.

Project 3
Management of Change Independent Audit for the TaqTaq facilities in Kurdistan
Development of Terms of Reference to cover all aspects of the current MOC process use in TTOPCO. Setting up target of acceptance criteria Previous audit reviews to establish current status of carry over actions
Site visit, interviews and assessment of the MOC process with the objective of:
• Determine the adequacy of the Taq Taq documentation in supporting the overall MOC process with findings and recommendations
• Assess the correct use of the documentation to ensure that what is being undertaken does comply with all aspects of the current MOC procedure
• Further assess progression from previous internal MOC audits
Reporting on findings, conclusions and recommendations for TTOPCO consideration against the Terms of Reference and target criteria set.
The audit took place at TaqTaq between 13th and 16th March 2017 with prework and reporting at the HRM offices in the UK with project completion in March 2017.

• Performance Standards and Verification Scheme of SECE’s
• Compliance with Chevron Safeguard Verification Programme
This work has been undertaken with direct consultation and input from both the Crews, Management and Regional HSEQ inputs.
Project completion is due in September 2017 in line with the established and agreed work programme with Seadrill Singapore and Thailand.

Project 5
OPHIR Energy engaged HRM to provide Learship and scribe for a Hazard Identification and Risk Assessment for a Deepwater Drill Campaign of the coast of Cote d’Ivoire. This included the use a Seadrill Deepwater 6th Generation Drillship West Saturn to drill the well and have three supply vessels out of Abidjan and a helicopter out of San Pedro to support the project, along with a security vessel on location during the drilling campaign.
• Workshop to conduct a Hazard Identification (HazId) of Major Accident Hazards (MAH) using BowTie analysis to determine barrier management and consequence outcomes.
• From the HazId develop a set of MAH Bowties with supporting hazard register
• Report on the process.
The key deliverable was to:
• Consolidated Hazards using the Ophir Risk matrix and record the results on the hazard identification sheets
• Major Accident Hazard Bowties and associated hazard management barriers (Safety Critical Equipment/Safety Critical Activities)
• Control measures and interactions of Safety Critical Equipment/Safety Critical Activities
• Ranking of barriers (with selected verification checks)
• Bowtie profiles and Major Accident Hazard list
This work has been undertaken with direct consultation and input from West Saturn representatives and OPHIR with HRM providing Facilitation, Scribe and reporting functions. Project completion was April 2017.

Project 6
HRM conducted on behalf of TTOPCO a Quantitative Risk Assessment (QRA) for their CPF1, TPF1, TPF2 and TLS
facilities at the Taq Taq field. Based on the outputs of this QRA study, a Risk Reduction Workshop was held to provide TTOPCO with a listing of potential risk reduction measures for consideration, in order to improve the ALARP status of the
facilities and to provide a structured way forward for continuous improvement. HRM where requested by TTOPCO to
conduct a review of the risk reduction measures and re – run of QRA sensitivities associated with these supported by CBA.
Review of recommended risk reduction measures against production criteria that encompassed part shutdown of facilities.
Using PHAST and Jeopardy software conduct a re-run of sensitivities based upon the review criteria to produce risk contours and PLL, IRPA and FAR rates associated with risk reduction enhancement or otherwise.
Cost Benefit Analysis to support judgment and approvals and ALARP demonstration of selected or discounted measures against the review criteria.
Reporting and overall Management presentation of findings, conclusions and recommendations at TaqTaq facilities in Kurdistan.
The work was completed in June 2017.

Project 7
SECE Performance Standard development and integration with Seadrill CMMS (IFS) and new MAXIMO project (RAMP) for 37 assets including UDW, Drillships, Semi Submersibles and Jack-Up Assets worldwide. Project executed over two workphases.
Phase 1 Development of SECE lists for a Drillship (West Neptune), Jack-Up (West Castor) and Semi-Submersible (West Capricorn) from BowTie barrier analysis into a suite of Performance Standards to be placed in MAXIMO for all assurance and verification requirements. This further included updating:
• MAH Bowties with SFI links and MEI/MPI considerations
• Development of MAHRA connecting the barriers, SECE and Performance Standards
• Suite of Performance Standards by asset
• Verification Scheme by Assets
• Performance Standard Assurance Sheets to be placed in MAXIMO for assurance activities relating to each Performance Standard requirement (Function Testing, etc).
• Use of the deliverables as a template to roll out over the remaining 37 Seadrill assests in Phase 2.
Phase 2 Using the templates from Phase 1 development by asset type of deliverables to support the intergration (RAMP Project).
• MAHRA update or development as required.
• SECE and Performance Standards development based on the templates taking account of design or operation differences.
• Verification Scheme by Assets.
• Performance Standard Assurance Sheets for MAXIMO.
• Actions listing as required from verification findings from overall process.
• Updates as required. Phase 1 was completed in July 2017
Phase 2 commenced August 2017 and will run until June 2018.

CE Marking and Technical file support for can conveying systems in Bulgaria for ECi Engineering.
Review of design basis and integration with existing canning conveying systems.
Development of the Technical File and conformity statements/attachments to meet all relevant and applicable EU Directives.

The work programme has commenced in January 2016 with a delivery date of February 2016. Project complete to ECi and their Client satisfaction.

Development of a CDM Compliance Document and Guidance with Vessel HSE Case for non-Oil and Gas production working (non regulatory).
Development of CDM 2015 Guidance document relating to Offshore Wind Farm Installation for Jan De Nul’s Vol Au Vent Vessel which included a breakdown of requirements, compliance matrices and description of roles and duties under Principal Contractor or Contractor activities.
Development of Major Accident Hazard Risk Assessment report using BowTie analysis for all operations and vessel associated risk. Listing of SECE’s and their performance standards and assurance requirements.
Development of a HSE Case document (non regulatory) to demonstrate ALARP across the vessel and its operations for prospective clients and for worker education.
The work programme completed in April 2017.

Workshop for SECE and Integrity system integration workshop to be held at Seadrill in London.
Work Task 1 Develop review sheets and overall outline of SECE and Maintenance systems acceptance/criteria matrix for work shop discussion.
Work Task 2 Attendance at a workshop to develop:
▪ Strategies for incorporating SECE and Performance Standards into the Seadrill maintenance and integrity system.
▪ Application of barrier management with the context of BowTies and integrity systems tagging, etc.
▪ Directive and guidance for application with template examples.
▪ Use of “mind mapping” to determine the points above and assist in the development of logic for the computerised systems.
