ADMISSION REQUIREMENTS AhighschooldiplomaorGEDisarequirementfortheVocationalNursingprogramandMedicalAssistantprogram.TheprogramsofferedbyICHareopentobothmenandwomenwhopossessahighschooldiploma/GEDoritsequivalent(evidencedbyanevaluationoftranscriptsbyareputableevaluator), and pass a Wonderlic entrance exam. We are unable to accommodate “ability-to-benefit”studentsandwearenotabletosponsoranystudentsfromoutsidetheUnitedStatesofAmerica.Wedonotprovidevisaservicesorvouchforstudentstatus.Thecollegedoesnotofferhighschooldiplomaprograms.Toqualifyforenrollment:

1. Theapplicantmustbeatleast18yearsold(forVNstudents;requirementbyBVNPT).2. TheapplicantmustbeahighschoolgraduateorGEDequivalent.3. Theapplicantmusthaveforeigntranscriptsformallyevaluatedandresultsmustbeonfile.4. Theapplicantmustbeabletomeetfinancialrequirements.5. Theapplicantmustcompleteanoralinterview.6. Theapplicantmusthavehadaphysicalexamwithin the lastyear,withnoevidenceofa

PHYSICAL REQUIREMENTS Collegedoesnotdiscriminatebasedonphysicalhandicaps.However,studentsmustbementallyandphysicallycapableofsafelyparticipatinginclassroom,clinical/externinstructiondailyto

successfullycompletetheprogram.IntegrityCollegeofHealthencouragesallstudentswhomayhaveaconcernregardingaphysicalissuetovisittheschoolandallowtheschooltoevaluateindividualcircumstances.Theschoolmayallowastudenttoattendaportionofthetrainingonatrialbasis(uptofivedays)withoutbeingobligatedfortuitioncharges.Anydisabilitiesmusthavemedicaldocumentationandmadeavailableatthetimeofadmission.(Foradditionalinformation,referto“StudentswithDisabilities”).

ADMISSIONS PROCEDURES Applicantsmustapplyforadmissionsatanytimepriortoenrollment.ApplicantsmustsubmitacompletedAdmissionsApplicationtothecollegetoapplyforenrollment.Proofofidentificationintheformofadriver’slicense,socialsecuritycard,birthcertificateorotherformofidentificationmustbesubmittedpriortoadmission.TheWonderlictestmustbepassedatqualifyingpassratefor thedesiredprogram: VN20 correct,MA13 correctwithin12minutes. FinancialAid orpaymentarrangementsmustbearranged,andstudentmustbeabletomeetfinancialobligations.ProgramDirector or designeemust complete admission interviewandprovide approval beforeenrollment.Studenttopayapplicableregistrationfee.BACKGROUND CHECKS & DRUG SCREEN Acriminalbackgroundmaypreventsomeonefromobtaininglicensure,clinicaltraining,externshipplacement or employment in their chosen field. When a criminal background must be fullyinvestigated,andbasedontheoffense,itmaytakeuptooneyearinordertoobtainadecisionfromalicensingboard.Applicantsorstudentswithacriminalbackgroundshouldcheckwiththerelevantlicensingbodyfortheirstancetowardsvariouscriminalbackgroundsituations.Studentswill not be allowed to continue their enrollment in the following programswithout acurrentcriminalbackgroundcheck.Aoffensewillnotnecessarilypreventastudentfromobtaininga license, but again, this is a case by case basis. A student’s background checkwill be initiatedimmediatelyuponenrollment&nolaterthanthefirstdayofclass.Thecostofacriminalbackgroundcheckisincludedinthetotalcostoftheseprograms,butstudentsintheVocationalNursingprogrammustpayforthecriminalbackgroundcheckrequiredbytheCaliforniaBoardofVocationNursing&PsychiatricTechnicianswhenapplyingfortheVocationalLicenseatcompletionoftheprogram.Studentsmaybesubjecttoadditionalbackgroundchecks,possiblydrugscreening,atanytimeifrequestedbyaclinicalfacility,employer,orexternshipsite.AnInstructororDirectorfor ICH may request student to submit to drug screening and/or medical attention whenimpairmentissuspected,studentappearsdisorientedorunsafe.Failuretopassanybackgroundcheckordrug screen requestedby an ICH,Externship,EmployerorClinical Sitemayprevent astudentfromgoingtothatExternshiporClinicalSite&mayputtheirprogramcompletionatrisk.Astudentmaybedeniedaccesstoclinical facilitiesbasedonoffensesappearingonthecriminalrecordeventhoughsuchoffensesmayhaveoccurredmorethansevenyearsago.

ALTERNATE STUDENT POLICY WhenanewVNCourseisscheduledtostart,andhasthemaximumnumberofstudentsenrolled,astudentmaybecomeanalternatestudentincaseofacancellationordropinthecourse.Eachnewcohortmaytemporarilyaccept2additionalstudentstemporarilyadmittedtoIntegrityCollegeofHealthfornomorethansix(6)weeks,whilewaitingforapositiontoopen.Thestudentwillattendeachscheduledclass,andparticipateinlectures,includingtesting.AsanAlternate,thestudentwillnotberequiredtopayanymonthlyfeesordeposit. Ifnopositionbecomesavailablebeforethescheduledclinicalexperienceatseven(7)weeks,thealternatestudentwillbedroppedfromtheprogram,andplacedatthetopofthenextclasswaitinglist, ifthestudentchoosestodoso. Ifa

positionbecomesavailable,thestudentwillbeexpectedtocompletetheapplicationprocesswiththeregularfeesanddepositandcancontinueinthecourse.(AppendixE,AlternateStudentPolicyForm).

ENGLISH PROFICIENCY CollegedoesnotprovideEnglish-as-a-SecondLanguage(ESL)instruction.Alltesting,lecturesandclinicalcommunicationareinEnglish,andmusthavedemonstratedproficiencyofatleast10thgradelevel. This will be evaluated by theWonderlic entrance exam. Students are required to passWonderlicentranceexamatleast10thgradelevel.StudentsarerequiredtospeakEnglishwhenaninstructionalsettingnecessitatestheuseofEnglishforeducationalorcommunicationpurposes.AllcoursesaretaughtinEnglish.

TRANSFER OR EXPERIENCE CREDIT Theawardingofcreditsforcourseworkcompletedatanyotherinstitutionisatthesolediscretionof College. Students who have previously attended another school or college accredited by anagencyrecognizedbyUSDE(UnitedStatesDepartmentofEducation)orCHEA(CounselforHigherEducationAccreditation)mayreceivecredits(maximumof54hours) forsuchattendancewhenproofofsaidattendanceispresentedatthetimeofenrollmentanduponreviewofthetranscriptbythe College administration. AP (Advanced Placement), USAFT (United States Armed ForcesInstitute)andothernontraditional,butwidelyaccepted,meansofearningcollegecreditwillberecognizedwhenrelevant.Alimitednumberofcreditsformilitaryservicemayalsobeawarded.Studentsreceivingveteran’sbenefitsmusthaveallprioreducationandevaluateduponenrollment.Creditwillbeawardedwhereapplicablewiththeprogrambeingshortenedaccordinglyortuitionreductionappliedtoapplicablecontent.ThestudentandtheDepartmentofVeteranAffairswillbenotifiedinwriting.ProcedureforrequestingTransferorExperienceCredits:

Ø Official Transcriptsmust be sent directly from the other schools or the armed forces toIntegrityCollegeofHealthforreviewandevaluation.

Ø Ifrequested,prospectivestudentsmayberequiredtosupplycoursedescriptions,syllabior

othersupportingmaterialsasprooffortheequivalencyforanycourseinquestion.

Ø TheProgramDirector or the SchoolDirectorwill do transfer evaluations.An evaluationsheet is completed during the review of the documentation. This sheet is kept in thestudent’spermanentfile.

Ø Ifastudentwishestoappealthedeterminationofcreditsgranted,theymaydosoinwritingandbysupplyinganyothersupportingdocumentationfortheirclaim.TheSchoolDirectorwillreviewtheadditionalinformationandhasfinalauthoritytograntordenyanytransfercredits.

Ø Studentwillnotberequiredtopayanychargesfortheapplicationorawardofcreditforpriorexperientiallearning.

ADVANCED PLACEMENT (Vocational Nursing) 1. Students transferring from other collegesmust have achieved a “C” or better in any course(nursing or other) to be considered for advanced placement andmay be eligible for advancedplacementduringdesignatedtimes.

Ø Applicationmustbecompletedsixweeksbeforethestartoftheprogram.Maximumhoursacceptedforeachcourseis54hoursforthosecoursessuccessfullycompletedintheareasofNutrition,AnatomyandPhysiology,Psychology,GrowthandDevelopment.

Ø StudentswhohavepreviouslysuccessfullycompletedaspecificcourseintheVNProgram(ex.1st54hrs.Med.Surg.),maybegivencreditforthecoursewithwrittenpermissionfromtheVocationalNurseProgramDirector.Theapplicationtoreceivecreditforthecoursemustbecompletedbeforethestartofthecourse.

2.ApplicationsforAdvancedPlacement/reinstatementareavailablefromtheVNProgramDirector.AdvancedPlacementstudentsmustmeet theprogramcriteria foradmissionandanyadditionalcriteriaasdelineatedbytheV.N.DirectorandSection2535oftheVocationalNursingRulesandRegulations,whichinclude:Transfercreditshallbegivenforrelatedpreviouseducationcompletedwithinthelastfiveyears.

A competency-based credit shall be granted for knowledge and/or skills acquired throughexperience. Credit shall bedeterminedbywritten and/orpractical examinations. Creditwill beawarded where applicable with the program being shortened accordingly or tuition reductionappliedtoapplicablecontent.3. B. StudentsmustbecoveredbyStudentLiabilityandMedicalinsurance.

OffCampusIllnessorAccidentswhenstudentisatafacility.Whenastudentbecomesillorisinjuredatanaffiliatinghealthcareagency,thefollowingprocedureistobefollowed:

A. Iftheinjuryisofanabsoluteemergencynatureorisareportablehospitalincident,thestudentreceivesinitialemergencycareattheinstitutioninwhichtheywereinjured.

B. If the student belongs to an HMO, a pre-paid medical pan, or has a private healthinsuranceplantheyobtainallexceptessentialemergencycarefromtheirownagency,ordesignatedphysician.

C. If the student has no health insurance, they contact Integrity College of HealthAdvisementCenterOfficeandreceivedirectionfromthatoffice.

D. Allinjuriesincurredatanaffiliatingclinical/externagencymustbereportedtotheICHAdvisementCenterOfficeregardlessofwheretheyareobtainingcarewithin24hoursoftheoccurrenceoftheincidenttocompletenecessaryforms.

E. All injuries incurred at an affiliating clinical/extern agency must be reported to theinstructorinchargeoftheclinicalcourseandProgramDirectorwithin24hoursoftheoccurrenceoftheincident.

1. COMMUNICABLE DISEASE Studentsknowntobeinfectedorsuspectedofbeinginfectedwithacommunicabledisease(asdeterminedby theRegulationsof theCalifornia StateDepartmentofHealth Services for theControlofCommunicableDiseases)willbelimitedorrestrictedfrompatientcontactuntiltheyarefreeofsuchdiseaseorincapableoftransmittinginfection.Theprocedure,shouldthisoccur,is:

A. Students will report infections immediately to their Clinical Instructor, and theVocationalNursingorMedicalAssistingProgramDirector.

B. TheClinicalInstructorandtheProgramDirectormutuallyagreeuponareassignment,whichwill limitorrestrictstudent frompatientcontact.Consultationwiththe facilityinfectioncontrolnurseshouldbeobtainedifindicated.

C. If reassignment cannot bemade, studentswill be sent home. (SeeAttendancePolicy,whichremainsinforce.)

D. Ifastudenthasbeensenthome,medicalclearancefromthestudentphysicianmustbeobtainedbeforereturningtotheclinicalfacilityorclassroomsetting.

2. BLOODBORNE PATHOGENS AND INFECTIOUS DISEASES ICHacknowledgesthatglovingiscriticaltoallhealthcareprofessionalsworkingandhandlingwithsharpinstruments(needles,lancets,etc.)andbodyfluid.Wearingglovesisthemostimportantmeasuretopreventthespreadingofinfectiousdiseases,especiallyHepatitisBandAIDS.TheICHcampusutilizestheengineeringcontrolsandworkpracticecontrolstominimizeoreliminatestudentexposuretothebloodbornepathogens.Studentsareexpectedtoutilizethefollowingpersonalprotectiveequipment:

3. HAZARDOUS MATERIALS

IntegrityCollegeofHealthunderstandsthathazardousmaterialisasubstance,beitsolid,liquidorgas,iscapableofharminghumans,propertyandtheenvironmentifmishandled,stored/disposedincorrectly.ICHmaintainssafehandlinganduseofthesematerialsthroughtrainingwithproperinstructions,preciselabelling,storage,disposal,and,provisionofmaterialsafetydatasheets(MSDS).Contaminatedmaterialssuchasneedles,syringes,etc.,mustbedisposedofinthesharpcollectors’containers.Theuseofthesecontainersismandatory,and,studentsarenotallowedtodisposethesharpcollectorunderanycircumstances.

4. EXPOSURE TO RADIATION TheICHcampusdoesnothaveanyradiationequipmentoncampusbutdependingontheclinicalfacilitiesandontheassignedarea,havethepotentialriskforstudentswhichmayincludetheexposuretoradiation.ThepolicyofICHforallstudentswhoareexposedwhileparticipatingintheeducationalprogram,istoobtainimmediatemedicalattentionand/orlaboratoryfollowupsasnecessary.

5. FIREARMS AND WEAPONS IntegrityCollegeofHealthhaszerotolerancetoanytypeofviolencecommittedbyoragainstanyofICHfaculty,staff,and/orstudentsincludingphysicaland/orverbalthreats.Threateningstudents/employeesinengaginginviolentactsareprohibited.Firearmand/orweaponpossessionisabsolutelybannedincampusandwithinthebuilding’sproperty.Violationofthepolicyconstitutegravemisconductthatmayleadtostudentterminationandnotificationtothelocalpolice/lawenforcement.IntegrityCollegeofHealthhavesurveillancecamerastomonitorthecampus.

6. HEALTH SERVICES INJURY PROTOCOL:

FollowingisareviewoftheprotocoltobefollowedwhenastudentreceivesaneedlestickorbreakageoftheskinbyanobjectorinstrumentwhichhasbeencontaminatedwithbloodorotherbodyfluidsORifthestudentisexposedtobodyfluidsviamucousmembranesornon-intactskin.

1.ReportinjurytoAdvisementCenterOfficeimmediately.2.Referstudentforhepatitis-BSurfaceAntibodyandHIVtesting.a.IfthestudenthasKaiserorotherHMOinsurance,refertoHMOproviderfortestingandappropriatefollowupcare.

b.Ifthestudenthasotherinsuranceornoinsurance,refertoPrivateMedicalDoctor.If the student does not have insurance, charges will be submitted to StudentInsurance.

3.Shouldthestudenttestnegativeforimmunitytohepatitis-Bitisrecommendedthatshe/he receive a second series of two injections. If after receiving a total of fiveinjectionstheimmunitylevelremainsnegative,nofurthertestingorvaccinationisrecommendedas the student is presumedunresponsive/insensitive tohepatitis-Bvaccine.

4.RepeattheHIVtestsixmonthsaftertheincident.

5.Advisethepatientthattheincidenthasoccurredandrequestthepatienttoconsenttohavingahepatitis-BpanelandHIVtest.Thepatientisnotrequiredtodothisbutisrequestedtoconsenttothetesting.Ifthetestsareperformed,theHIVtestisrepeatedinsixmonths.Resultsofthetestsaremadeavailabletothestudent.

FINANCIAL AID EffectiveOct13,2013,IntegrityCollegeofHealthoffersFinancialAidforqualifyingstudents.WedoparticipateinFederaland/orStateFinancialAidprograms.Aftersuccessfulcompletionofentranceexam,theFinancialAidOfficerisavailablebyappointmenttodiscussfinances.

HOUSING IntegrityCollegeofHealthdoesnotofferonoroffcampusdormitoryaccommodationsandhasnodormitoryfacilitiesunderitscontrol.StudentsmayfindapartmentsnearCollege.Rentchargesvaryper location, sizeofapartment,and facilities in thecomplex.Cost rangesofapartments inclosevicinityrangefrom$1,095(studio)to$3,000(3bedroom).Thecollegehasnoresponsibilitytofindorassistastudentinfindinghousing.

REVISION OF PROGRAMS AND TUITION ICHreservestherighttochangeormodify,withoutnotification,theprogramcontent,equipment,staff,instructionalmaterials,equipment,clinicalsitesandorganizationasnecessarywithapprovalfromtheappropriateStateagency,ifrequired.Suchchangesmaybeneededtostayabreastwithever-evolving technological advances, legislation in the allied healthcare field and to improveteachingmethods.Nochangeswillbemadetodiminishthecompetencyofanyprogramorresultintuitionchangesforstudentscurrentlyenrolledinaprogram.ForcurrenttuitionandfeespleasecontactAdmissionsOffice.

CANCELLATION & REFUND POLICY

POLICY OVERVIEW Astudenthas theright tocancel theenrollmentagreementandobtainarefundofchargespaidthroughattendanceattheinitialclasssession(excludingregistrationfee),ortheseventhdayafterenrollment,whicheverislater.CancellationshalloccurwhenyougivewrittennoticeofcancellationattheaddressoftheSchoolshownonthe topof the firstpageof theEnrollmentAgreement. Youcando thisbymail,handdeliveryore-mail.Thewrittennoticeofcancellationsentbymail,iseffectivewhendepositedinthemailproperlyaddressedwithpostageprepaid.Thewrittennoticeofcancellationneednottakeanyformandiseffectiveifthewrittennoticeshowsthat you no longer wish to be bound by this Agreement. You will be given two [2] Notice ofCancellationformstouseatthefirstdayofclass,butyoucanuseanywrittennoticethatyouwish.Iftheschoolhasgivenyouanyequipment,includingbooksorothermaterials,youshallreturnthemtotheschoolwithinthirtydaysfollowingthedateofyourNoticeofCancellation.Ifyoufailtoreturnthisequipment,includingbooksandothermaterials,ingoodconditionwithinthethirty[30]dayperiod,theSchoolmaydeductitsdocumentedcostfortheequipmentfromanyrefundthatmaybeduetoyou.Onceyoupayfortheequipment,itisyourstokeepwithoutfurtherobligation.

IfyoucancelthisAgreement,theSchoolwillprorateanymoneythatyoupaid,lessanydeductionfor equipment not timely returned in good condition, within thirty days after your Notice ofCancellationisreceived.Note:

Intheeventtheeligiblepersonfailstoenterthecourse,withdraws,ordiscontinuesattendanceatanytimepriortothecompletion,theamountchargedtotheeligiblepersonfortuition,fees,andotherchargesshouldnotexceedtheapproximatepro-rataportionoftotalchargesforthatprogram.

Ifastudentobtainsaloantopayforaneducationprogram,thestudentmustrepaythefullamountoftheloanplusinterest,lesstheamountofanyrefund.Ifthestudenthasreceivedfederalstudentfinancialaidfunds,thestudentisentitledtoarefundofmoneys not paid from federal student financial aid programs. CEC94919 or94920.Pleaserefertorefundpolicyspecifictofinancialaidandfinancialaiddisclosures.Withdrawing from a Course after the Seventh (7th) Day after enrollment Youhavetherighttowithdrawfromacourseofinstructionatanytime.YouhavetherighttocancelthisAgreementandobtainarefundofchargespaidthroughattendanceatthefirstclasssession,orthe seventh day after enrollment, whichever is later. The School will remit a refund less aregistrationfee,ifapplicable,nottoexceed$75.00withinthirtydaysfollowingyourwithdrawal.Youareobligatedtopayonlyforeducationalservicesrenderedandforunreturnedequipment.Therefundshallbetheamountyoupaidforinstructionmultipliedbyfraction,thenumeratorofwhichisthenumberofhoursofinstructionwhichyouhavenotreceivedbutforwhichyouhavepaid,andthedenominatorofwhichisthetotalnumberofhoursofinstructionforwhichyouhavepaid.Ifyouobtain equipment, as specified in the Agreement as a separate charge, and return it in goodconditionwithin thirty days following the date of yourwithdrawal, the School shall refund thechargefortheequipmentpaidbyyou.Ifyoufailtoreturntheequipmentingoodcondition,allowingforreasonablewearandtear,withinthisthirty-dayperiod,theSchoolmayoffsetagainsttherefundthedocumentedcosttotheschoolforthatequipment.Youshallbeliablefortheamount,ifany,bywhichthedocumentedcostforequipmentexceedstheproratedrefundamount.Thedocumentedcostoftheequipmentmaybelessthantheamountcharged,andtheamounttheSchoolhaschargedinthecontract.Inanyevent,youwillneverbechargedformorethantheequipmentchargesstatedinthecontract.Foralistofthesecharges,seethelistonthefirstpageofthisagreement.IFTHEAMOUNTYOUHAVEPAIDISMORETHANTHEAMOUNTYOUOWEFORTHETIMEYOUATTENDED,THENAREFUNDWILLBEGIVENWITHINTHIRTYDAYSOFWITHDRAWAL.IFTHEAMOUNTYOUOWEISMORETHANTHEAMOUNTYOUHAVEALREADYPAID,THENYOUWILLHAVETOMAKEARRANGEMENTSTOPAYTHEBALANCE.Studentswhoseentiretuitionandfeesarepaidbyathird-partyorganizationarenoteligibleforarefund. HYPOTHETICAL REFUND EXAMPLE: Assume that a student, upon enrollment in a 400-hour course, pays $2,000 for tuition, $75 forregistrationand$150,documentedcost to school, forequipmentas specified in theEnrollment

Agreementandwithdrawsaftercompleting100hourswithout returning theequipmenthe/sheobtained.Theproratarefundtothestudentwouldbe$1,462baseduponthecalculationsstatedbelow.Ifthestudentreturnstheequipmentingoodconditionwithinthirty[30]daysfollowinghis/herwithdrawal,theSchoolshallrefundthechargefortheequipmentpaidbythestudent.RefundCalculation

Forthepurposeofdeterminingtheamountyouoweforthetimeyouattended,youshallbedeemedtohavewithdrawnfromthecoursewhenanyofthefollowingoccurs:

Ø YounotifytheSchoolofyourwithdrawalortheactualdateofwithdrawal.Ø TheSchoolterminatesyourenrollment.Ø Youfailtoattendclassesforathree[3]weekperiod.Inthiscase,thedateofwithdrawalshall

bedeemedtobethelastdateofrecordedattendance.Ifanyportionofyourtuitionwaspaidfromtheproceedsofaloan,thentherefundwillbesenttothelenderortotheagencythatguaranteedtheloan,ifany.Anyremainingamountofrefundwillfirst be used to repay any student financial aid programs fromwhich you received benefits, inproportiontotheamountof thebenefitsreceived.Anyremainingamountwillbepaidtoyou. Ifthereisabalancedue,youwillberesponsibletopaythatamount.

STUDENT TUITION RECOVERY FUND (STRF) EffectiveJanuary1,2015STRFwillbereported,butnofeesarerequiredtobecollected.TheStudentRecoveryFund (STRF)wasestablishedby theLegislature toprotectanyCaliforniaresidentwhoattendsaprivatepostsecondaryinstitutionfromlosingmoneyiftheyprepaidtuitionand suffereda financial lossasa resultof the school closing, failing to liveup to its enrollmentagreementorrefusingtopayacourtjudgment.TobeeligibleforSTRF,youmustbeaCaliforniaresidentandresideinCaliforniaatthetimetheenrollmentagreementissignedorwhenyoureceivelessonsataCaliforniamailingaddressfroman approved institution offering correspondence instruction. HOMEWORK/COURSE PREPARATION Studentsshouldconsiderhomeworkpreparationforeachclassan integralpartof their trainingprogram.Studentsareexpectedtobepreparedforupcomingcoursematerialandreviewpreviouslylearnedinformationonaregularbasis.Thismayincludecompletingchapterreadingassignmentsbeforelectures,expectationsof“pre-lecturequizzes,”andtheabilitytoparticipateindiscussionsregarding assigned topics and homework. Homework projects may be assigned by individualinstructorsonanas-neededbasis.Allstudentsshouldbepreparedtodemonstratepreparationinassignedchaptercontent.Studentswillhavethescheduleofallcontentforeachcourse.Itiscritical students allow time each day outside of class for reading, studying and preparation forassignedtopicsinclass.Studentsmustreadassignedchaptersbeforeclasstomaximizethelearningexperiencesincoreprograms.

CLASSROOM AND CLINICAL BEHAVIOR Studentsareexpectedtomaintainrespectfulandprofessionalbehavior.Ifastudentbecomesdisruptiveorargumentativewiththeinstructororotherstudents,therewillbeadisciplinarywarning.Ifbehaviorcontinuesstudentwillbedismissed.Ifthestudentdisagreeswithaninstructors’assignmentorbehavior,thestudentistospeakwiththeInstructorinaprivatesettingduringbreakorafterclass.Ifthestudentdoesnotfeelissuewasresolved,studentistomakeawrittenstatementandreporttotheProgramDirector.UndernocircumstanceisastudenttohaveaverbalorphysicalaltercationwithanInstructor,staffmember,patient,orpeer.

SATISFACTORY ACADEMIC PROGRESS (SAP) SatisfactoryAcademicProgressappliestoALLstudentsregardlessofthemethodofpaymentfortheireducationalprogram.Ifthestudenthasfederalstudentloanfinancing,meetingtimelySAPrequirementsaffectsfinancialaideligibility.Tocontinueintheprogram,allstudentsmust:QualitativeRequirement-CumulativeGradePointAverageMaintainagradeof“C”(2.0or75%)orbetterintheorycourses.Ifthecumulativecurrentsemestergradeisfallsbelow75%thestudentwillbereferredtoremediation.SPECIALNOTE: If thestudent isreceivingFinancialAidand isplacedonremediation forcumulativegradesbelow75%,thestudentwillbealsoplacedonSAP-Warning.Thismeansthe student must meet satisfactory academic progress before next payment period. ThebehavioralobjectivesforeachcourseandevaluationratingscaledefinethestandardsforaCR(credit) grade for the clinical portion of the Vocational Nursing Courses. Medical AssistantpreceptorsandMAInstructorwillcompletetheexternshipevaluation.Ratingscaleandevaluationcriteriaaredistributedtostudentsinwritingatthebeginningofthecourse.2.StudentsmustearnafinalgradeofCR(credit)inclinicalperformanceportionsofnursingcoursesand externship of the Medical Assisting program. Thismeansthestudent must meet satisfactory academic progress before next SAP Evaluationpoint.Ifstudentdoesnotmeetsatisfactoryacademicprogress,financialaidwillceaseforthosewithfinancialaideligibility.Studentwillberesponsibleforearnedtuition.Studentcanappealthedecision,andifthestudentprevailsuponappeal,can be placed on SAP-Probation. Eligibility for aid may be reinstated for onepaymentperiod.

B. Students,whodonotearnafinalgradeof“CR”(credit)inlaboratoryorclinicalportionsof program, after remediation, will be dropped from the program. To continue inprogram,studentmustrepeatthesemester,includingtheoryandclinical(appropriatetuitionchargesapply).

QuantitativeRequirement-PaceofCompletionVNstudentsmustsuccessfullycomplete100%ofallattemptedcumulativeclockhourstograduate fromaprogram. MAstudentsmustsuccessfullycomplete80%ofallclockhours.TheMaximumTimeframe to completeallhoursarewithin150%oroneandahalf times thescheduled program length. For example, if a program is 12months in length, then thestudentwillhaveamaximumof18monthsintotaltosuccessfullycompleteallrequirementsfortheprogram.SAPEvaluationPeriods:Therearespecificandroutineevaluationperiodsduringtheprogramofstudytodetermineifthestudentismeetingsatisfactoryacademicprogress.TheICHSAPEvaluationPeriodisperformedforallstudentsatthesametimesduringtheprogram.TheSAPEvaluationPeriodisthesameastheFinancialAidpaymentperiodforthosestudentsthatareeligibleforFinancialAid.Presently,thecoursesofferedatICHareclockhourprograms.TheSAPEvaluationPeriodisdoneateach450th,900thand1230thhourperiodforVNstudents.Forexample,theVNProgramisatotalof1560hours(60weeks).Whenstudentsstarttheirprogram,theSAPEvaluationPeriodwilloccurwhentheprogramhasreached450hours(17thweek)andagainat900hours(35thweek).MAProgram

isatotalof800hours(33weeks),theSAPEvaluationPeriodisonlydoneatthe400-hourperiod(17weeks).SAPEVALUATION1.Whenstudentsbegintheirprogram,theyareconsideredmakingSatisfactoryAcademicProgress(SAP).AteachSAPEvaluationPeriodstudentsmustmaintainatleasta75%intheory(or2.0)andaCredit(CR)gradeforClinical/Externship.IfstudentsarenotmeetingSAPatanyevaluationpoint,thenthefollowingprocedurewillbe:SAPWarning,SAPProbation,orDismissal.

LeaveofAbsenceAnystudentwishingtotakeaLeaveofAbsencefromtheVNorMAprogram,mustpresentarequestinwritingatleasttwo(2)weekspriortothebeginningofLeaveofAbsence.SuchrequestshouldsetforththereasonsforrequestingtheLeaveofAbsenceandthestudent’sprojectedreturndate.AllsuchrequestsshallbeconsideredbytheDirectoroftheProgram.

TheDirectorwillapproveordenytherequestandinformthestudentinwriting.Studentswhointerrupttheirownprogressintheprogram,duetopersonalreasons,andwhohavesatisfactoryacademicandattendancerecords,andpassingclinicalmaybereinstatedintheprogramatthenextavailableentrypointprovidingspaceisavailable.DISMISSAL/TERMINATIONPOLICYAstudentissubjecttodismissal/terminationforviolatinganyofthefollowing:1.FailuretomaintainSatisfactoryAcademicProgress(SAP).2.FailuretocomplywiththeICHAttendancePolicy.3.FailuretocomplywiththeICHStudentConductPolicy.4.FailuretosatisfyallfinancialobligationstotheCollege.

5.ConsideredunsafebyClinical/ExternshipInstructor.

REINSTATEMENT AstudentwhosesuspensionhasresultedindisqualificationfromaProgrammaybereinstatedintheProgramatthenextavailableentrypoint,ifspaceisavailable;approvaloftheProgramDirector,theproblemcausingthedismissalhasbeenappropriatelyremediated.A student who is disqualified from the Program for unsatisfactory attendance or forunsatisfactorycompletionofalectureportionofacourse,orfailuretomeettheprerequisiterequirementsforthecourse,willbereinstatedintheprogramatthenextscheduledentrypoint,upon application by the student and upon satisfactory completion of the written terms forreinstatement, if any.Reinstatement is contingentuponavailable spaceandapprovalby theProgramDirector.

A student who is disqualified from the Program for not passing may be reinstated in theprogram at a time determined by the Program Director. Students thus disqualified mustcompletethetermsforreinstatementasdelineatedinwrittencommunicationgiventhestudentat the time of the disqualification. Students are permitted only one reinstatement fordisqualificationfromtheprogramforunsatisfactoryacademicand/orclinicalperformance,andexcessiveabsence.Astudentwhohastwicebeendisqualifiedshallnotbere-admittedtotheprogram.

Studentswhointerrupttheirownprogressintheprogram,andwhohavesatisfactoryacademicand attendance records, and passing clinical may be reinstated in the program at the nextavailableentrypointprovidingspaceisavailable.

Studentswhofailtomeettheprerequisiterequirementforacoursewillbereinstatedatthenextavailable entry point providing space is available, after they complete the prerequisiterequirementandsubmittherequireddocumentation.

Studentswhowishtore-entertheProgrammustbeevaluatedbytheProgramDirectorpriortore-entry.Conditionsforre-entrywillbeoutlinedinacontractgiventothestudentandinsertedinhis/herstudentfile.TheentireVocationalNursingProgrammustbecompletedwithinafive-yearperiodorpreviouslypassedcoursesmustberepeated.

EXITEXAMS/AssessmentTesting(VocationalNursing)ALLATIASSESSMENTEXAMSMUSTBECOMPLETEDANDPASSEDTOGRADUATEFROMTHEVOCATIONALNURSINGPROGRAM.Integrity College of Health (ICH) provides assessment tools and materials for our students toprepareandassesshowpreparedtheyare to take theNCLEX-PNexamaftergraduation. Theseassessmenttoolsaretothebenefitofthestudenttoidentifyspecificsubjectareasofstrengthsandweaknesses.Therearefocusedreviewmaterialsthatcanhelpthestudentbecomeproficientintheareas of weaknesses. ICH utilizes the preparation and assessment materials from AssessmentTechnologiesInstitute(ATI).ATIisacompanythathasalong-demonstratedhistoryinprovidingtheresourcesandassessmenttoolsfornursingstudentsinpreparationfortheNCLEXexam.Thepurposeofusingtheseassessmentandpreparationmaterials,aretopreparethestudentfortherigorsoftheofficialtest,andwillbetreatedwiththeutmostseriousness,asallexamsarerecognizedthroughouttheprogram.ATI Examinations are taken by second and third semester students. There is a total of sevenspecialtyexamsinpreparationforthefinal“ComprehensivePredictor”exam.Studentsareallowedtwoattemptsperspecialty.Ifuponcompletionofthefirstattemptofanexam,thestudentisnotsuccessfulatachievingatleasta“level2”,thestudentwillhaveremediation.ATIprovidesareviewprogramcalledthe“focusedreview.”Aftertakingtheassessmenttest,itshowsareasofweaknessesandprovidesacomputerizedreviewinthosespecificareas.RemediationwillalsoincludemeetingwiththeProgramDirectorandturninginacopyoftheATIfocusedreviewtoverifythatthereviewwascompleted.AdatewillbescheduledtoretaketheATIexam.

Ifthesecondattemptisnotsuccessfulinmeetingatleasta“level2”,thestudentwillmeetwiththeProgramDirectorordesignee,re-evaluatetheremediationplan,completeafocusedreview,andscheduleathirdtestingdate.AnytimeastudentrequirestakingtheATIexaminationmorethantwice,thestudentmustpurchasetheexamfromATI.Studentwillnotbeabletoproceedtothenextassessmenttestuntiltheyhavepassedthepresenttest.Allmustbecompletedbeforegraduationandstudentcannotgraduateuntilallarecompleted.ScoresfromtheATIAssessmentswillcompriseof10%ofthesemestergrade.TheATIComprehensivePredictorExamisthefinaloftheATIeightexams. TheComprehensivePredictormustbepassedwithascoreof85%orhigherinthe“PredictedProbability”ofPassingNCLEX-PNontheFirstAttempt.Ifthestudentdidnotobtaina“85%PredictedProbabilityofPassing NCLEX on First Attempt,” or above, the student will follow the same procedure inremediation as stated above. The student cannot graduate from the program withoutcompletingallexamsatthelevelsindicated.Dateswillbescheduledforthe“retake”examandmustbeperformedonatimelybasis.StudentswillbeissuedanIncomplete(I)ifallATITestshavenot been completed. Students have 30 days after graduation date to complete ATI exams. Ifassessmentexamsarenotcompletedwithin30daysandpassedallothersemesterrequirements,maintainingatleast75%gradeincoursework,thestudentwillhavetheoption:

1. StudentwillbeeligibletoenrollinIndependentstudy. Studentwillcontinuetocompletetheassessmentexamsuntilmeetinggraduationstandards.Studentwillberesponsibleforthe cost of subsequent remediation ATI assessments exams during Independent Study.Studentmustcompletewithinthe150%programlimit(6-monthsfromprogramenddate)orthestudentwillnotgraduate. Thetranscriptwill indicate,“GraduateNO”,“PassedExitExamNO.”

2. IfstudentisunabletocompleteExitexams,ICHcanassistthestudenttoapplytoBVNPTto“Challenge”theNCLEX-PNexam.StudentwillhavetheoptiontochallengetheNCLEX-PNasa“non-graduate”ofanAccreditedNursingProgram.IfsuccessfulpassingtheNCLEX-PNexam, the studentwill be able to obtain the license as an LVN, but there are importantlimitations.ThestudentcanonlyusethelicenseinCalifornia,anddoesnothavereciprocityto other states; there may be restrictions to applying to Registered Nursing Programs(withoutgraduationfromanaccreditednursingprogram);somehospitalsandfacilitiesmaynothirewithoutgraduationfromanaccreditednursingprogram,evenwiththeVocationalNursingLicense.Thisisnotthecolleges’proceduralrecommendationforastudenttoobtaintheirlicense.Ifthestudentclearlyunderstandsthelicenserestrictions,andwantstopursuethechallengeoption,ICHwillassistwiththeapplicationprocess.Asanote,ifthestudentisunable tosuccessfullypassATIassessment testing in thecollege, thismay indicate thestudentmaynotpasstheNCLEX-PNduringthechallenge.

Remediation/StudentSuccessProgramwillberequiredwhenstudentobtainstestscoreonexamlessthan75%.VNStudentsaregiventwoopportunitiesinthefirstsemestertoretakeregularclasstestsandonechanceforretakesinsecondandthirdsemester.Finalexamscannotberepeated.

E. Failuretomeetclinicalobjectivesstatedinwritingatthebeginningofthecoursetherebyplacingapatientorpatientsinphysicaloremotionaljeopardy.Onewrittenwarningwillbeissued.

ATTENDANCE Studentsareexpectedtoattendallclassesasscheduled.VocationalNursing(VN)andMedicalAssisting(MA)studentsmustdemonstratecontinuouspunctualityanddependabilityduringtheentireprogram.Thisincludesattendancetoallscheduledlecturecourses,VNclinicalshifts,andMAexternships.TheBoardofVocationalNursing&PsychiatricTechnicians(BVNPT)requires100%ofalltheoryandclinicalhourscompletedbynursingstudents.Thismeansalltardiesandabsencesmustbemadeuptocompletetheprogram.Studentswillnotpassanycoursewithoutmeetingthegradeandattendancerequirements.Consistentattendanceisimperativetofacilitateadequatelearning.Punctualitydemonstratesresponsibility,accountabilityandprofessionalism.Studentsmustadheretothefollowingattendancerequirements:TARDINESS

1. Attendancewillbetakenatthebeginningofeachclasssession.(IfforanyreasontheInstructorisnotthere,immediatelycontacttheProgramDirectorand/orICHofficestaff).Ifthestudentistenminuteslatetostartofclass,returningfrombreak,orleavesearly,he/shewillbeconsideredtardy.

ABSENCES

1. Studentsmustattendalltheoryandclinical/externship.Thestudentistogiveatleast30-minutenoticetoschoolorinstructorofthereasonheorshewillbeabsent.Attendanceisavitalcomponentofthecurriculum.Studentsareallowedonlytwoexcusedclinical/externshipabsences(mustbemadeupinclinical/externship),andtwotheoryabsences(whichmustbemadeupwithassignmentsoncampus)persemester.Allabsencesmustbemadeupinordertosuccessfullypassthesemester.Overtwo(2)absencesareconsideredexcessive.Onthethird(3)tardyorabsence,studentwillbeplacedonprobation.Inabilitytodemonstratecompliancewithattendancepolicywillplacestudentatriskforbeingdroppedfromprogram.Studentcannotobtainanyadditionalabsenceswhileonprobationorwillbeatriskforexpulsion.

2. Ifastudentisabsentfromeithertheoryorclinicalsessions,thestudentisrequiredtomake-upanyobjectivesmissed.

o Theoryassignmentsmustbemadeupwhileoncampus(studentswillnotbegivencreditforassignmentscompletedathome).StudentmustscheduletimewiththeInstructororProgramDirectorfordateandtimetocompletemissedtheoryassignments.

o Clinicalhoursmustbemadeupinclinical.StudentmustplanwiththeProgramDirectorforscheduledmake-up.Iftherearenoavailableclinicalclasses,studentwillhavetwo(2)daysduringmake-upweek(atendofsemester)tomakeuphours.Ifmoreclinicalhoursareneeded,studentmaypossiblyhaveopportunitytoarrangetopayanadditionalfeeforaclinicalinstructortoteachintheclinicalsettingifclinicalsiteandinstructorareavailable.

3. Attendancerequirementsatanassignedclinicalfacilitymaybechangedduetothesuddenandunexpectedinabilityofaclinicalfacilitytofulfillitscommitmenttotheprogram.

4. Astudentmaypetitionforre-admissionbecauseofexcessabsencesonlyonceduringtheentireprogram.

LEAVEOFABSENCE

o IntegrityCollegeofHealthmayreviewandrequireclearanceforhealthconditionsandpregnancy.Ifapregnantstudenthasfullmedicalclearance,withnorestriction,shemaycontinueintheprogram.ALeaveofAbsencemaybegrantedforthebirthofachild,significantillnesswithanotefromthedoctororextenuatingcircumstances.

5. SPECIALNOTE:IfthestudentisreceivingFinancialAidandisplacedonprobationforattendance,thestudentwillbealsoplacedonaFinancialAidWarning.Thismeansthe

student must meet satisfactory attendance and be removed from attendanceprobation before next payment period. If student does not meet satisfactoryattendance,financialaidwillcease,andstudentwillberesponsibleforearnedtuition.Studentcanappealthedecision,andifthestudentprevailsuponappeal,canbeplacedon Financial Aid Probation. Eligibility for aid may be reinstated for one paymentperiod(seeFinancialAiddefinitions)

6. Students are expected to attend clinical or externship assignments in compliance with

Integrity College of Health’s Vocational Nursing Program andMedical Assisting UniformPolicy.StudentswhodonotconformtotheUniformPolicywillbeaskedtoleavetheclinicalareauntilsuchtimethathe/shecancomplywiththepolicy.Timemissedforsuchpurposeswillapplytoabsencetime.InstructorsinPediatricsNursingcoursewilladvise students of appropriate attire for alternate clinical assignments. Attendancerequirements at an assigned clinical facility may be changed due to the sudden andunexpectedinabilityofaclinicalfacilitytoaccommodatestudents.Studentswillbenotifiedinadvanceofanychanges.

FINANCIALAIDFAFSAINFORMATIONReturntoTitleIVPolicy(R2T4)IntegrityCollegeofHealth studentswhoreceive federal financial aidanddonot complete theirprogramcouldberesponsibletorepayaportionoftheaidreceivedaccordingtofederalregulations.TheFinancialAidOfficeissupplyingthefollowinginformationtohelpyoubetterunderstandwhatcouldhappenifyouwithdrawfromorstopattendingourVocationalNursingProgram.Federal lawrequiresschoolstocalculatehowmuchfederal financialaidastudenthasearned ifhe/shewithdrawsorstopsattendingbeforecompletingtheprogram.Federalfinancialaidcoveredunder this regulation includes Federal Pell Grants, Federal Direct Loans (subsidized andunsubsidized),andFederalDirectPLUSloans.This regulationapplies to studentswhoreceive federal financial aidandwhowithdrawor stopattendingallclassespriortothe60%pointoftheprogram.Studentswhoreceivefederalfinancialaidmust “earn” theaid they receiveby stayingenrolled in theprogram.Theamountof federalfinancialaidearned isproportionate to thenumberof clockhours thestudentcompletedwhileenrolledintheprogram.Unearnedaidmustbereturnedtothefederalgovernment.Forexample, ifyoucomplete30%of theprogram,youearn30%of theaidyouoriginallywerescheduledtoreceive.Thismeansthat70%ofyourscheduledawardremainsunearnedandmustbereturnedtothefederalgovernment.Thefollowingformulaisusedtodeterminethepercentofunearnedaidthatmustbereturnedtothefederalgovernment:
