Any campaigns launched prior to 1st February 2017 are not eligible for entry. By submitting this information you are confirming that you have permission to share the information and evidence with the judges and relevant individuals as part of the awards. By submitting this information you are giving consent for its use by the IOSH South Wales Branch for the purposes of its awards process. By submitting this nomination you are confirming that the nominee has been made aware and has given their consent to the nomination Once nominations have been submitted, they will be assessed by a panel of judges and a shortlist will then be created. Shortlisted nominees will be contacted prior to the Annual AGM, and winners for each category will be announced during the awards ceremony, which will be held after the Annual AGM. Submission Overview: This category is designed to recognise the implementation of an initiative that has led to significant improvements to the safety and/or health of persons or premises. Entries should demonstrate a clear goal vision, or a link to a long-term plan, or may underpin IOSH priority areas: Musculoskeletal disorders (MSDs) Non-health (incident) related fatalities (falls from height, hit by moving objects, transport safety) Occupational Cancer Rehabilitation – back to work Sustainability and human capital Wellbeing (physical and mental strength Entries should also be linked to the South Wales branch, either by: I. the nominee (who is a member of the Branch), and/or; II. the initiative is linked to an Organisation within the South Wales area.
