SYMPOSIUM

»4-seasons orthodontics«Orthodontic therapy for patients of all agesDear Colleagues!

P R A G U E

www.ios-prague.com

Again, we invite you to attend the »14th International Orthodontic Symposium« (IOS) in Prague, November 24 – 26, 2016. This time, the motto will be »The Four Seasons In Orthodontics«. What does that mean? We all know that patients of all ages seek orthodontic treatment — it is long ago that only children and adolescents were the typical patients in our fi eld of medicine. Children, even newborn children may be addressed as the fi rst season of orthodontic pa-tients. The second season is made up by our adolescent patients, and in the third season, our adult patients are found. And according to the success of general dentistry, even highly aged patients live with their own teeth, and as teeth do not stand stable within bone, they also require orthodontic treatment in several cases. This group of patients is our fourth season in orthodontics.

As an exception, I as the president of the IOS-Symposium, was asked by the scientifi c committee to give the pre-congress course of the EAO-Academy and give the "Tiziano-Baccetti-Memorial-Lecture" on the fi rst day of the meeting. Based on my background in craniofacial developmental biology and as an orthodontist in practise, I will do my best to present my experience to address aspects of the right tim-ing and interdisciplinary approaches for patients in all four seasons in orthodontics.

We have invited outstanding speakers who will cover further as-pects of all of these four seasons in orthodontics. And we invite you as the participants, coming from more than 30 nations, to share this international orthodontic congress with its familiar atmosphere, where our young orthodontists come together with the more ad-vanced experts to share the knowledge and to network for ongoing international education.

I am looking forward to seeing you all again in Prague!

»4-seasons orthodontics« Orthodontic therapy for patients of all ages

Long ago only children and adolescents were the typical patients in orthodontics. Today, after general dentistry has become so successful that patients can maintain their teeth lifelong, we as

orthodontists have to accompany them during their whole life. Even highly aged patients live with their own teeth, and as teeth do not stand stable within bone, they require orthodontic treatment in sev-eral cases. Just to catch them and move them back onto the alveolar bone, if retention measures were not successful. Most of the major fi xed prosthetic reconstructions show a better result when the teeth are lined up orthodontically before. So to which extent has orthodon-tics become the main agent in dentistry? Is it true, that well aligned teeth in the adolescent guarantee a long-term maintenance of the dentition during life? And fi nally, which of the early treatment in chil-dren is appropriate and recognised?

‣ This EAO-Academy Pre-Symposium Course will address "The Four Seasons In Orthodontics":

Season 1: Children from 0–10 years of ageSeason 2: Adolescents from 10–18Season 3: Adults from 18–50 Season 4: The best agers, 50+.

We raise questions, review literature, and give an abundance of treatment examples and encourage you to improve networking with your local general dentists.

Molecular medicine in orthodontics — accelerated tooth movement?

In modern orthodontics, the treatment times have signifi cantly shortened. A wise treatment planning, intelligent application of most recent technology, safe anchorage and a good cooperation

of patients contribute to this success. Nevertheless, there is a desire to get even faster results. Our knowledge of molecular signaling and regulation of the biological basis of orthodontic tooth movement, in particular remodelling of bone and other periodontal tissue has in-creased. So, there is much hope to achieve tooth movement even faster.

‣ In this presentation we will present the state of the art and we will discuss, in how much it will change our way of every day orthodontic treatment.

Quintessence of 30 Years of experience with the Bionator

The main objective of functional orthopaedic appliances is to eliminate all factors that may cause a malocclusion: habits, parafunctions, forced bites, habitual mouth-breathing.

Retrospective cephalometric short- and long-term studies of Class II patients treated with the functional device Bionator docu-ment the kind and magnitude of the changes during the growth peri-ods and the following aging. A Class I occlusion is primarily achieved by dentoalveolar compensation during prepuberty and by inherited favorable skeletal growth adaptation during puberty. A comparison of initial and fi nal variables of treated groups with homogeneous untreated groups demonstrate the real treatment effects: skeletal, dento-basal, dento-alveolar and reveals indication and contraindi-cation of a Bionator.

CURRICULUM VITAEAfter Prof. em. Dr. med. dent. Ingrid Rudzki Approbation in 1967, Promotion in 1970 and her Habilitation in 1977 Prof. Rudzki became in 1982 apl. Professur of Ludwig Maximilians University Munich. She passed her orthodontic training under the aegis of Ascher, Rosenstein, Björk and Hasund. Prof. Rudzki performed as offi cial Lecturer 1971/72 (MH Hannover) and 1976–78 at the LMU Munich. From 1974–1991 she run a private practice in Munich. She was elected for a full-time Professorship in 1980 at the University Göttingen and 1991 at the University Munich and led as Head of the Ortho-dontic Department LMU Munich from 1991 to 2008.

Taking and using photos of your patients — do’s and dont’s

From a legal point of view, you cannot simply take photos of your patients and use them for any purpose you want. This presen-tation outlines the legal framework that governs the taking and

handling of patients’ photos.

‣ In a fi rst step, we will address everything you need to know about obtaining valid consent from your patients to take photos of them.

‣ Secondly, it will be explained to what extend you are allowed to use these photos subsequently (i.e. use them in a lecture or on your website). The presentation will fi nish off with a few practical tips on how to make sure you’re always on the safe side, legally.

CURRICULUM VITAEDr. iur. Philip Radlanski studied law (focused studies: law within the information soci-ety) at the University of Regensburg, Germany, and the University of Sheffi eld, UK. He also holds a master’s degree (LL.M. in Intellectual Property and Information Law) from the King’s College London, UK. While working on his PhD-thesis on “The Concept of Consent within the Reality of Data Protection Law”, Dr. Radlanski was invited to join the New York University Privacy Research Group for three months.

From 2014 to 2015, he worked as a law clerk at the Higher Regional Court of Berlin with stages, amongst others, at the German Federal Film Board, and two international law fi rms in Berlin and Sydney, Australia. He now works as a lawyer in Berlin.

What are the limitations of anterior expansion of the lower arch?

Positioning of the lower incisors used to be one of the funda-mentals in orthodontic treatment planning. At present, to avoid extraction treatment and as a side effect of fi xed class II

treatment, many orthodontic treatments are fi nished with the lower incisors in a more proclined position.

‣ In the presented lecture, possible implications of this treatment approach on stability and on soft tissues as well as possible peri-odontal concerns are discussed by the help of abundant clinical cases seen in daily practise.

This book presents useful tips and strategies on how to inte-

grate the Invisalign system successfully into clinical practice.

The authors review the diagnostic protocols and the bio-

mechanics of aligners before presenting Invisalign treatment

protocols. With the support of accompanying case documen-

tation, discussion of each malocclusion includes information

on the associated symptoms, the rationale behind the selec-

ted treatment approaches, and the various outcomes achie-

ved. The last section of this book deals with the advantages of

the Invisalign system and can help patients and clinicians in

deciding whether this system can provide optimal treatment

outcomes for a particular clinical situation. This is a practical

manual for any clinician interested in the novel treatment

modality of aligner orthodontics.

Leveraging clinical advancements in treatment mechanics — an introduction to the CCO-System

The cliché "…it is not only the bracket system you use, but HOW you use it…" has never been more relevant than now! How many times you have heard of a colleague that changed the bracket

system only to fi nd out it provided no real advantage, but rather a disruption in the offi ce.

‣ This lecture will explain how the CCO System works, how it chang-es and simplifi es your treatment mechanics even in more challenging cases, and why after you know it, it is hard not to fall in love with it!

CURRICULUM VITAEDr. Antonio Secchi is the former Clinical Director of the Department of Orthodontics at the University of Pennsylvania, the same school where he received his DMD, Certifi cate in Orthodontics, and a Master of Science in Oral Biology.

Dr. Secchi’s research interests include, primarily, the relationship between self-ligating appliances, friction and treatment mechanics as well as orthodontics and functional occlusion. He has published in various dental and orthodontic peer review journals. Dr. Secchi wrote the chapter "Contemporary Mechanics Using The Straight Wire Ap-pliance" for the latest edition of the Graber/Vanarsdall/Vig orthodontic textbook. He received the 2005 David C. Hamilton Orthodontic Research Award from the Pennsyl-vania Association of Orthodontists (PAO) and the 2010 Outstanding Teacher Award from the Department of Orthodontics of the University of Pennsylvania. Dr. Secchi is the founder of the "Complete Clinical Orthodontics System" (CCO System™), which he teaches to orthodontists not only in USA but throughout the world. His passion for clinical orthodontics and commitment to education has made him a solicited lecturer at a National and International level. In addition, he maintains an active orthodontic practice in Philadelphia and Devon, PA.

Aesthetics during the treatment without compromises — is it available?

Aesthetics is one of the main goals of orthodontic treatment. Nowadays, due to increasing numbers of adult patients, or-thodontists are faced with the issue of preserving aesthet-

ics during the treatment beside achieving aesthetics as a result of treatment only. Many adults refuse treatment just because of visible elements on the front teeth.

As aesthetic alternatives to labial braces, modern orthodontics offers aligners and lingual technique. Both require compromises from orthodontists. This means going to the same purpose, causing himself and the patient some diffi culties. In the case of aligners — it is about limitations in biomechanics as well as shortcomings of the removable device. In the case of lingual technique compromise is related to the damage of tongue and overall discomfort, caused by braces and archwires on the lingual surface.

Based on this, an alternative technique, implying the absence of labial braces on front teeth, lingual braces and arches on the lingual surface of the laterals can be regarded as attempt to minimise com-promises for the sake of preserving aesthetics during treatment.

CURRICULUM VITAEDr. Hayk Sargsyan graduated from Yerevan State Medical University in 1993, started as a general dentist, soon followed by compilation of the managerial and orthodontic practice. He got the Medical Doctor in 2005 ("Morphological basis of orthodontic tooth movement") and leads currently the multi-profi le dental clinic of Yerevan State Univer-sity. He has extensive experience in orthodontics, has undergone several courses and participated at series of international and national conferences, workshops and semi-nars and his current interest is in cortical anchorage and lingual technique.

Being one of the founders of the Association of Orthodontists of Armenia and currently Vice President of the same alongside affi liation to other professional associations, such as EurAsian Association of Orthodontic, Scientifi c Board of IOS Symposium, Eu-ropean Orthodontic Society, Armenian Dentists Union — constantly contributes to his professional development, enabling application of contemporary knowledge in ortho-dontics in Armenia in addition to support young specialists in the country.

The role of an active self-ligating bracket system in formation of a myodynamic equilibrium

The effect of expansion in application of self-ligating brackets is the subject of debate by many scientists. We proposed a hy-pothesis about a special mechanism of impact between active

self-ligating brackets and soft tissues; thereby it was observed more expansion of dentition with alveolar bone remodelling. A study of biomechanical model of interactive self-ligating bracket in vitro re-vealed, that on dentition alignment stage in round section archwire expansion takes place in the region of the crowns with simultaneous vestibular movement of roots by shifting the center of rotation to-wards crowns, during interaction in the system orthodontic archwire — interactive self-ligating bracket — tooth — soft tissues.

Clinical investigations indicate the stability of results of such an approach in which the shape of the dental arch is determined by the state of the perioral muscles. This phenomenon is particularly impor-tant in the application of the principles of neuromuscular dentistry; where achievement of new muscular balance with relaxation of muscle spasm is carried out before active orthodontic treatment.

CURRICULUM VITAEDr. Hrant Ter-Poghosyan, D.D.S., Ph.D., M.S. graduated in 1993 as a general dentist (Yerevan State Medical Institute) (clinical residency 1997), defended Ph.D. thesis in 1999, started a private orthodontic offi ce, clinic "Alfastom" in Yerevan (2002) and de-fended doctoral thesis in 2010. He is head of the Department of Orthodontics and Pedi-atric Dentistry (YMSU) since 2006, head of the prophylactic center of dento-maxillary diseases since 2002, founder and president of Armenian Orthodontists Association since 2006. He passed various qualifi cation courses in Armenia, Russia, Germany, France and Lithuania and took part in numerous international conferences. He has the permission for engagement in private Russian practise since 2002, offers micro-sem-inars on splint-therapy, neuromuscular dentistry, indirect bonding, computer-related 3-D technology and practice management. He is author of 18 methodical guidelines, 4 rational suggestions, author or co-author of 51 publications and holds patents for 3 inventions. He is a member of: Armenian Dental Association (Since 1996), Russian Association of Orthodontists (Since 1999), World Federation of Orthodontists (Since 2001), Armenian Association of Orthodontists (Since 2006), International Academy of Dentistry (Since 2010), European Association of Orthodontists (Since 2011), and Armenian Association of Sleep Disorders (Since 2013).

Sleep apnea treatment — surgical and orthodontic aspects

Obstructive Sleep Apnea Syndrom (OSAS) is a well-known problem in diagnosis and treatment of different disciplines. Most of the cases occur in patients who are in their fourth

decade of age. In respect to the demographic change of our popula-tion the number is expected to rise. Reduction of obesity and ENT Examination will often be the fi rst step in treatment. A well accepted overnight OSAS treatment option is still CPAP-device to obtain a suf-fi cient oxygenation for the patient. Millions of patients worldwide are applying OSAS treatment systems. In contrast we prefer treatment options, which dont’ interfere as much in the patient’s daily life. These include surgical interventions to eliminate airway obstructions.

These techniques delineate forward movement of the maxillary-mandibular complex to achieve suffi cient airway space, providing undisturbed breathing during the different sleeping periods.

In our 20 years of experience we observed remarkable benefi ts for the patients. The procedure has outstanding and lasting benefi ts for the patients: we observed general improvement in patient ac-tivities, decrease of hypertension, regression of diabetes and other benefi cial parameters. Most of the patients where operated in close cooperation with Orthodontists. We will show cases more than ten years postoperative and follow up.

CURRICULUM VITAEProf. Dr. Dr. Bodo Hoffmeister was graduated in 1968, did his medical and dental edu-cation at Christian-Albrechts-University Kiel (CAU), received his DOS in 1975 and his MD in 1977. In 1982 he became specialist in Maxillofacial Surgery and was involved in research of nerve lesions and reconstruction. He got his PhD in 1987 ("Lingual Nerve Lesions in Maxillofacial Surgery"). In 1989 Prof. Hoffmeister became Deputy Chair-man of the Department for Oral and Maxillofacial Surgery, CAU Kiel. After research in Tumor and Reconstructive Surgery he was dedicated for nomination as Associated Prof. of the Department in 1992. Since 1994 he was Chairman of the Department of Maxillofacial Surgery and Facial Plastic Surgery at Charite University Hospital Berlin Campus Benjamin Franklin and since 2008 also Chairman of the Department for Max-illofacial Surgery, Navigation and Robotics there. Due to retirement he left the Charite in March 2016 and moved to Zürich as deputy chairman at Klinik Professor Sailer. Overall his special interest is Plastic and Reconstructive Surgery in respect to the treatment of Facial Deformities, Orthognatic Surgery combined with Aesthetic Surgery.

Neuromuscular Dentistry is a diagnostic and treatment mo-dality that aims to ensure that muscles and TMJ function properly as a result of creating a functional occlusion. It ena-

bles the optimum physiologic position of the jaw to be established to assist in the correction of the underlying causes of craniofacial Tem-poromandibular joint, head and neck pain. Neuromuscular dentistry is also used to determine the optimum physiologic jaw position prior to complex dental restorative procedures, cosmetic dentistry, dental sleep medicine procedures, dentofacial orthopedics and orthodon-tics. It is a treatment modality of dentistry that focuses on correcting the physiologic "misalignment" of the jaw at the Temporomandibular joint (TMJ).

‣ The use of bioelectrical instrumentation such as Computerized Mandibular Scanning (CMS), Trans Cutaneous Electrical Nerve Stim-ulation (TENS) and Surface Electromyography (SE) is described in detail. Clinical cases show the basic procedures of Neuromuscular Orthodontics that describe the road to success.

CURRICULUM VITAEDr. Fabio Savastano graduated in Medicine and Surgery in 1987 cum laude at the Uni-versity of Naples, Italy, achieved his Master in Orthodontics at the University of Padua in 1990 and is adjunct professor at the University Des Les Valls, Andorra. He runs his practice limited to orthodontics and gnathology since 1991 in Albenga. Dr. Savastano is the President of ICNOG, International College of Neuromuscular Orthodontics and Gnathology and member of numerous associations, and has lectured in Brazil, Cana-da, U.A.E., Spain, Bahrain, India and Italy on "Neuromuscular Orthodontics".

Complex orthodontic Aligner Treatment — Diagnosis, Treatment Planning and Therapy

Orthodontics is nowadays an indispensable part from the de-manding treatment concepts of the aesthetic and functional dentistry. By the use of innovative methods and equipment

even complicated teeth misalignments can be corrected. However, orthodontic treatments are often avoided of adolescents and adults for cosmetic reasons, as the majority of patients consider invasive multibracket appliances as unacceptable. Nowadays the orthodontist has several "invisible" treatment options available, one of them is the Invisalign System. After 15 years of experience with the system, we believe that this technology is not only a successful but little invasive treatment option for mild to moderate crowding, gaps in the anterior region and tilted teeth, but also in extensive cases, such as extrac-tions, open and deep bites, dental class II & III treatments and in the therapy of craniomandibular dysfunctions or as part of a combined orthodontic-oral surgical treatment. Expansion of the dental arch, in- and extrusions of individual teeth or groups of teeth as in the deep bite, torque-like movements of front teeth and the correction of the tooth axis have become an integral part of the Invisalign treatment. With Invisalign Teen the technique is also available for children and young people and can be combined with functional orthodontic treatments.

‣ The presentation will show possibilities of the Invisalign System, the treatment planning and gives tips and hints for this treatment technique.

CURRICULUM VITAEDr. Schupp graduated in dentistry 1985 (University of Münster) and continued his stud-ies under the direction of Prof. Dr. U. Ehmer. Collaboration in the orthodontic practice of Dr. D. E. Toll since 1990 followed and a private praxis as orthodontic specialist in Co-logne, Germany. He is certifi ed in Invisalign, Manual Medicine and Osteopathy for Ortho-dontics, foundation member and past president ("German Board of Orthodontics and Orofacial Orthopedics"), board member ("German Society of Aligner Orthodontics"), advisory board member (periodical "Manuelle Medizin") and has published several arti-cles. He wrote the books "Funktionslehre in der Kieferorthopädie", "Kraniomandibuläres und muskuloskelettales System" and "Aligner Orthodontics" and lectured in Europe, America and Asia. He is Visiting Professor at the Capital University, Beijing, China.

3D Diagnostics from Infants to Adulthood — Expanding the orthodontic Spectrum

In modern orthodontics 3D-imaging of the dental, skeletal and soft tissue compartments are available in the daily clinical setting for diagnostics and therapy planning. Moreover the opportunity of

3D- imaging nowadays from infants to adulthood provides further implementation of orthodontics in medicine: In the fi rst year of life a non-invasive 3D-imaging allows an early diagnostic of head and face asymmetries. Thus, the orthodontists’ responsibility could be to counteract a progressive growth of the head and face by orthopae-dic approach already in an early stage of life.

In adolescence 3D-imaging offers a variety of opportunities to get additional therapy-relevant information about dental malocclu-sion, skeletal morphology and soft tissue reaction during compre-hensive orthodontic treatment. At the end of growth in treatment of adults with periodontics or in the treatment of orthognathic patients 3D-imaging permits new perspectives in diagnostics and in plan-ning the biomechanical treatment approach.

‣ The aim of the lecture is to present the application of 3D-imaging in different growth phases in daily orthodontics routine, to underline the expanding of the orthodontic spectrum and to critical analyze the additional value of 3D-imaging.

CURRICULUM VITAEPhilipp Meyer-Marcotty, DDS, PhD is since 2015 Head and chair of the Department of Orthodontics at the University Medical Center Göttingen, Georg-August-University. 2007–2014 he was Senior Physician of the Department of Orthodontics, Medical Fac-ulty, University of Würzburg and 2003–2007 he absolved his postgraduated studies in dentistry and orthodontics (University Munich / Würzburg). Before he was a student at the Dental School (Ludwig Maximilians University Munich), he studied 1995–1996 Jazz music at the conservatory in Hanover. Specifi c focuses of clinical activities are the therapy of complex interdisciplinary adult cases (patients with a severe dysgnathia / patients with severe loss of periodontal attachment / patients with cleft lip and palate and craniofacial anomalies).

Trouble shooting in modern orthodontics

A lot of progress has been achieved recently in modern ortho-dontics which can help to improve and simplify the treatment especially in complicated cases, such as solving anchorage

problems with the application of TAD’s and SUS², solution’s for non-compliance patients, overcoming of treatment-obstacles in aesthetic aspects with the help of various aligners and lingual systems, avoid-ing surgery in adult cases, and management of multiple aplasia, pre-mature molars-loss and teeth retention/ankyloses, as well as recom-mendations to avoid/minimize root resorption and relapse.

‣ This lecture is to demonstrate new and approved methods of modern orthodontics and to show its possibilities and limits. Special focus is put on the teamwork of different disciplines like parodontol-ogy, prosthetic and dysgnathic surgery and on the consideration of the TMJ.

SYMPOSIUM VENUEKaiserstein Palace Malostranské námestí 37, Prague 1

The 14th International Orthodontic Symposium 2016 will take place in the Kaiserstein Palace. The address of the palace is "Malostran-

ské namesti 23/37", Prague 1.An ideal place for meetings from the times of High BaroqueWe are looking forward to hosting you as our guests, and please con-sider our guest book. For more than three centuries, we have had many satisfi ed and famous guests.

The Kaiserstein Palace received a Baroque reconstruction initiated by Sir Kaiserstein but completed by Petr Radecky (1699–1720). In 1859 a monument of marshal Ra-decký was built on the square. Up to 1918 the square was named after him. In 1866, Václav Petzold, a hotel-owner, bought the palace and opened an exclusive restaurant. For the next hundred years it was called "U Petzoldu". The famous opera singer Emmy Destinn, a partner of Enrico Caruso, rented an apartment there (1904–1908). A total reconstruction began under the architects Zdenek Pokorný and Jaroslav Belský in 1977.

In 1981 the palace has been registered as a UNESCO heritage site. In 1997 restitiution procedures were completed and the palace was returned to its original owners.
