ALL FORMS TO BE COMPLETED AND SUBMITTED TO LAHD WITHOUT SIGNATURE FROM PLANNING.

– Complete the AHRF (rev. 10/26/09) to the extent possible and submit to LAHD without Planning signature.

– If you have a previously reviewed and signed AHRF and the project has not changed, include this in the package to LAHD.

– Complete any other forms as required by LAHD’s NOFA package and submit to LAHD without Planning signature.

– Provide copies of all relevant zoning information including: B&S Zoning Verification letters, all previous entitlements/ decisions, status of currently filed applications, Q-conditions, Proof of exemption from SPR, certificate of occupancy for existing units, etc.

– Planning will review the AHRF and other forms with LAHD and contact applicants with any questions.

– Appointments will be scheduled on an as needed basis only

Affordable Housing Referral Form

• Purpose

– Initial project review and screening for entitlement need

– Coordination and referral (communication) between City Planning and LAHD, CRA, LA County, and other agencies for funding purposes

– Referral to Planning Public Counter(s) for entitlement filing purposes

– Applicant to complete form as much as possible

– Planning staff to review form and assess need for any entitlements (based on information provided)

– Planning staff to sign and date form as verification of review

– Completed form with Planner’s signature to be submitted to LAHD (or other City/County agency) as required for application purposes.
