Delta Fair and WREG News Channel 3 for a special day for ladies at the Delta Fair.

Instructions for Participants

• Two- piece outfi t is suggested to make undressing easier. Participants will be asked to undress from the waist up in a private dressing area. Complete privacy is ensured.

• The participants will be asked to remove all deodorant prior to the exam. An opportunity to remove deodorant is provided in the private dressing room on the mobile unit using baby wipes. Deodorant is available to reapply after the mammogram fi lms are complete.

• Participants should arrive on the mobile at their appointed time. Keeping participants on schedule allows for a full day of 25 participants to be seen in a timely manner. The procedure takes approximately 10-15 minutes.

• Participants should fax their completed paperwork 1 week prior to their appointment to 226-0839. Completed Paperwork consists of: Patient Forms 1 and 2. This paperwork communicates personal history and information required by the Women’s Health Center. Fax the breast history and patient information form to (901) 226-0839. Please write the date/place of the Event in the top right hand corner of each sheet and put to the attention of Lyn Sperling.

• Participants should bring insurance cards + photo I.D. to their appointment. It is recommended that each participant check their insurance coverage to make sure that they: • Have insurance in network with a Baptist facility • Know if a co-pay or deductible may apply to their visit • Are within the correct time frame - at least 365 days from their last mammogram.

• Participants for screening mammograms on mobile unit MUST: 1. Be 35 years or older 2. Have no symptoms (new lumps or masses) 3. Have no personal history of breast cancer 4. Not be pregnant or nursing 5. Have a PHYSICIAN to receive mammography results 6. Have seen your physician within the last 3 years 7. Be able to walk up stairs and stand alone
