INDIVIDUALIZED LIFESTYLE PROGRAM
Program Goals:To reduce the risk of
developing type 2 diabetes through…
• ≥ 7% loss of body weight and maintenance of weight loss
• ≥ 150 minutes per week of physical activity

THE PROGRAM
• Small group – discussion & education
• First - 16 weeks – one hour a week
• Then - one hour a month for the rest of the 12 months
• Classes can be located anywhere!• YOUR place?
