PREVENTIVE CARE At Independent Health, the health and well-being of our members who are expecting, as well as their babies, is very important to us. And we realize that getting an early start to prenatal care is important in helping to reduce any potential risks to mother and child. WHY IS PRENATAL CARE IMPORTANT? Women who see a health care provider early and regularly throughout their pregnancy: • Have healthier babies. • Are less likely to deliver prematurely. • Are less likely to have other serious problems related to pregnancy. To ensure the appropriate prenatal care is provided, Independent Health helps to create an important bridge between the mother and the physician, providing regular contact and support through a variety of methods. Through the Maternity Management Program, we combine live calls, a telephonic information program, text, e-mail and mailed education materials to give the expectant mother a solid foundation that carries them through their pregnancy, postpartum period, and in cases where admission to the neonatal intensive care unit (NICU) is needed. Independent Health’s Maternity Management Program includes the following: ONE-ON-ONE TELEPHONIC CONSULTATION FOR HIGH-RISK PREGNANCIES Independent Health proactively identifies pregnant women who are at high risk for pregnancy-related complications due to age and certain medical conditions, such as hypertension or diabetes. Prenatal case managers provide education and direct members to the appropriate resources, as well as facilitate needed medical care throughout the pregnancy and postpartum period. Case managers in this program are highly trained, experienced obstetrical nurses. These case managers use condition-specific case management tools that provide guidance in assessment, intervention and documentation of key actions needed/taken to fill any gaps in care. Telephonic consultation provides: An assessment addressing medical, behavioral, pharmacy and dental needs. Prenatal and postpartum depression screening and management. Targeted health education material for those with high-risk conditions. Referral for educational and skilled home-care visits through a contracted, certified home health care agency. maternity management program

Independent Health has partnered with the National Healthy Mothers, Healthy Babies Coalition forthe text4baby program, a free mobile information service that provides pregnant women and newmoms with information to help them care for their health and give their babies the best possible start in life.

Members can sign up for the FREE service by texting BABY to 511411 (or BEBE in Spanish). After that, they will receive free SMS text messages each week, timed to their due date or baby’s date of birth.
