This programme was very successful last year with lots of interest from General Praconers and Pracce Nurses. The evening will be held at Auckland City Hospital on the evening of Tuesday 31 st May 2016. Auckland DHB would like to invite all General Practitioners and Practice Nurses working in the Auckland District to their next Primary Care Open Evening. This programme was very successful last year with lots of interest from General Practitioners and Practice Nurses. The evening will be held at Auckland City Hospital on the evening of Tuesday 31st May 2016.

Attendees report that they enjoy meeting key hospital management and specialists, and having the opportunity to discuss clinical and local issues with them. The May meeting will take a patient journey perspective examining a number of common clinical issues that arise in older age including polypharmacy, managing falls and frailty. Auckland DHB will also have information available on their new geographically based locality service model, and there will be the opportunity to meet key staff working in your locality.

There will also be site familiarisation tours of the Department; presentations by those working in these areas, and time for Q&A. Safety in Practice (SIP) - An Auckland Regional Initiative The aim of Safety in Practice (SIP) is to enhance the quality improvement capability of General Practices (GPs) within the Auckland region, through a specific focus on patient safety. SIP (adapted from the Scottish Patient Safety Programme in Primary Care) enables practices to learn skills that will allow them to more confidently manage the constant change that is occurring in the health sector.

The 2014/15, the Safety in Practice programme saw a collaborative of 23 general practice groups involved. This has increased to 32 general practices this year.

Improvement Advisors (IAs) from the three DHBs and a Clinical Lead work with PHO staff within practices to provide quality improvement support and facilitation to assist with:

Up-skilling teams in improvement methodology

Identification of current systems, processes and behaviours

Data analysis

Re-design of practice systems and processes

PDSA testing of small-scale change and familiarity with the SIP care bundle audit tools

Practice-based meetings allow reflection on data collected and to identify areas of change to be tested within the practice. The HSQC, RNZCGP, GPNZ and NZNC are strong supporters of SIP.

Planning for Year 3 is underway, with the capacity for 70 practices to come on board.

Practices get a remuneration of $5,400 p.a., and are expected to participate in four learning sessions; attend practice meetings with PHO staff, IAs and the clinical lead; and to submit monthly audit data. They are also expected to test the trigger tool and safety climate survey at their practice.

New Haematuria Clinic Trial - Auckland DHB Auckland DHB Urology service is implementing a trial Haematuria Clinic for three months, for patients who

present with macro-haematuria and a high suspicion of cancer. We aim to see these patients within 14 days of

referral. The trial will initially be for three patient appointments per clinic but will increase to seven over the

coming months.

The purpose is to condense diagnostics and treatment pathway options into one visit for patients and so

improve their time to treatment.

The clinic will be held fortnightly on a Monday at Greenlane Clinic Centre and will include:

Ultrasound imaging Flexible cystoscopy A discussion with the patient regarding treatment options Agreement on a surgical date where appropriate Pre-admission health questionnaire

information and decision support to assist in the daily management of patients. The project continues

to make sound progress with 200 pages of information localised to date.

Changes to National Cervical Screening Programme Reports

PHO Data-match reports now available:

Practices enrolment data is matched electronically with the National Cervical Screening Programme (NCSP) register.

This is provided monthly to PHOs to calculate coverage rates and to provide information to practices.

This report will now replace the monthly overdue reports that practices are used to receiving from the NCSP register.

The report is in a spreadsheet format, which can be used to clinically prioritise overdue women, provide information about women screened elsewhere, and identify women who are not on the register.

To find out more about these reports contact your PHO.

Paper based monthly overdue reports:

These reports are no longer routinely sent to practices by the NCSP register.

Reports can still be requested via the Auckland regional register team.

Paper based quality of smear reports:

The National Cervical Screening Programme will no longer send out quality of smear reports.

Unsatisfactory results have been reduced significantly since the introduction of Liquid Based Cytology

Information about unsatisfactory smears is still recorded on the NCSP register.

A quality of smears report can be requested from the Auckland Regional register team if required for professional development or if self- monitoring is indicated.

CareConnect eReferral Update for Waitemata GPs

The Older Adults, Allied Health and District Nursing services are now electronically reviewing eReferrals. Please use the Older Adults form to refer to community services, including rehabilitation, Outpatients clinic and NASC. Use Mental Health (‘Reason for Referral “Mental Health services for Older People”) when making referrals patients 65 years or older for mental health referrals. Use the Allied Health and District Nursing forms to refer for community services for patients sixteen years and older.

FREE HPV Course Now On-line - A National Initiative

For the latest information about HPV and HPV vaccines, a free HPV update course is now available online.

A one hour online course designed for health professionals who provide advice to parents/caregivers or general public on the HPV immunisation programme, is now available from the IMAC website by clicking here The course is free and you can log in and out at any time, so you can complete this all at once or in smaller sessions as time allows. Additional links and optional reading is included for those interested in extra information.

To complete the course there is a set of multi choice questions to answer, and a certificate is provided.

Influenza Vaccine Offered to Patients Attending Waitemata DHB Dialysis Units

Influenza season is upon us. Due to funding changes by PHARMAC, the Waitemata Renal Service will be offering centre haemodialysis patients the Influvac ® influenza vaccine during May.

Patients who consent will be given Influvac ® 45micrograms in 0.5mL subcutaneously. This will be recorded in the National Immunisation Register (NIR) so that providers in primary care can view this information.
