As president of the American College of Osteopathic Surgeons, the host organization for the 2016 Annual Clinical Assembly of Osteopathic Surgeons (ACA), I am pleased to extend greetings to all registrants, exhibitors, and guests. Welcome to Phoenix!

This year’s ACA provides the perfect opportunity to participate in educational programs that will help you achieve surgical excellence in this changing healthcare environment, to network with osteopathic surgeons from across the country, to enjoy a few rounds of golf at the Wildfire Golf Club featuring two of the area’s top courses, and to experience this remarkable city.

A special highlight of each ACA, the meeting’s featured speaker sessions bring together attendees from all disciplines to learn about issues influencing healthcare and how to improve their practices. This year’s featured speaker sessions are:

ABOUT THE

ACA OBJECTIVES AND OUTCOMES

The objectives of this meeting are:

• To offer a highly diversified program of continuing medical

education for the specialties of cardiothoracic and general

vascular, general, neurological, plastic and reconstructive,

proctological, and urological surgery;

• To keep surgeons in these specialties, other interested physicians,

and healthcare professionals informed regarding recent

developments and advances in the art and science of surgery;

• To provide a review of selected areas of the attendees’

surgical specialties;

• To discuss pre- and postoperative issues related to surgery;

• To inform attendees of trends and changes in governmental

legislation and regulations; and

• To inform surgical specialists regarding developments related

to surgery in other fields of science, in socioeconomics,

and in education.

The methods that will be used to achieve these objectives are:

• Panels, symposia, and lectures;

• Scientific paper presentations;

• Peer-to-peer discussions;

• Questions from participants and answers from presenters; and

• Scientific exhibits and poster presentations.

At the completion of this meeting, surgeons in attendance will:

• Be more familiar with recent clinical developments in their

surgical specialties;

• Be able to implement knowledge learned into their surgical

practice in order to improve patient care;

• Better understand the indications, techniques, complications,

and outcomes of various surgical procedures; and

• Be more aware of relevant socioeconomic and governmental

issues relating to surgery and patient care.

AOA CME CREDITS AND REQUIREMENTS

You can earn American Osteopathic Association continuing medical education (AOA CME) credit by participating in the educational programs and activities at the 2016 ACA. Instructions on how to access the online CME tracking and program evaluation system are included in your registration packet. It is imperative that you complete the online evaluation and attestation by Dec. 1, 2016. After the program evaluation has been completed, a CME “certificate of attendance” can be printed by the registrant. In accordance with the AOA CME accreditation requirements, you must complete an evaluation of each session attended in order to receive AOA CME credit. Physicians should only claim credit commensurate with the extent of their participation in each ACA educational program and/or activity.

The ACA has requested that the AOA Council on Continuing Medical Education approve this program for a maximum of 29.25 AOA Category 1-A CME credit hours as follows:

An official roster of ACA registrants and the number of CME credits submitted by each registrant will be provided to the AOA. The credits submitted by each registrant will be entered in his or her AOA CME activity report. Any questions regarding the CME requirements, eligible CME credits for this meeting, or problems such as facilities, handouts, or program content may be directed to the ACA staff by calling 800-888-1312, ext. 107.

Unresolved issues regarding the compliance of this meeting with the AOA accreditation requirements may be brought to the attention of the AOA Division of CME by calling 800-621-1773, ext. 8054. Any formal action by the AOA will require a written communication from the attendee.

COMMERCIAL EXHIBITS

The 2016 ACA commercial exhibits are located in the ACA exhibit hall in Grand Saguaro Ballrooms North/South on Level 1 of the JW Marriott Phoenix Desert Ridge Resort and Spa. Booths displaying products and services that you can use in your practice are set up in this area. Company representatives will be available to discuss their products and services with you during the hours listed below. Only registrants with badges are allowed to enter the ACA exhibit hall.

Lunch will be served in the ACA exhibit hall on Thursday, Sept. 22, for all registered ACA attendees from 11:30 a.m. to 1 p.m. Breaks also will be provided in the exhibit hall on Friday from 12 noon to 1:30 p.m. and Saturday from 12 noon to 1 p.m. A complete listing of exhibiting companies and an exhibit hall floor plan are provided on pages 38–39 of this program book.

SCIENTIFIC EXHIBITS AND POSTER PRESENTATIONS

At the ACA exhibit hall grand opening on Thursday, Sept. 22, you also will meet the surgeons who prepared scientific poster presentations for the 2016 ACA. These surgeons will be at their displays in the exhibit hall to discuss their research and findings with you. Plan to be present for this educational opportunity.

Members of the ACA Scientific Exhibits and Poster Sessions Committee will judge the poster displays on Thursday, Sept. 22. ACOS winners will be announced during the ACOS Resident Scientific Research Presentations and Awards Reception on Thursday, Sept. 22, at 5 p.m. in Grand Sonoran Ballrooms C–D on Level 1 of the JW Marriott Phoenix Desert Ridge Resort. See pages 40-46 of this program book for scientific poster listings.

HEADSHOT LAB

New to the ACA this year is the Headshot Lab, an onsite photography studio where assembly attendees will receive complimentary professional headshots. A high-resolution digital portrait is perfect for use in your business brochures, blogs, website, or on social media sites. An optional exhibit hall badge may be purchased for guests for $100 each.

MEETING ROOM LOCATIONS

ACA educational programs and exhibits will be held at the JW Marriott Phoenix Desert Ridge Resort and Spa unless otherwise indicated in this program book. “The Bottom-line Benefits of Team Leadership Training—Proven Pathway to Minimizing Errors and Maximizing Quality”

New studies have begun validating up to 50 percent reductions in “wrong surgeries” (wrong site, wrong medication, wrong patient, retained object) throughout hospitals and healthcare settings. The engine of change in virtually all of these results is a steadily and universally applied program of Team Leadership Training for the entire medical team involved in any form of surgical intervention at the patient’s bedside, or in the O.R., the cath lab, E.R., or the physician’s office. Going far beyond the basic exposure to Team STEPPS® (strategies and tools to enhance performance and patient safety) and aviation’s Crew Resource Management approach, Team Leadership Training creates proven and professional Collegial Interactive Teams (CITs) but must be built on complete cultural change.

This presentation by John Nance, a pioneer in both aviation and medical safety and quality, lays the groundwork for why Team Leadership Training is such a powerful and certain way to improve the performance of not just surgical interventions, but virtually all medical interactions. The hallmark of a mature CIT is a leader who knows how to bring out the best performance and best participatory ownership in each member of the team. A trained CIT leader knows how to eliminate all communication barriers by making it completely safe for any member to speak up, and by creating and maintaining an atmosphere of mutual respect, constant learning, and non-hierarchical interchange. While these principles, techniques, and methods have never been taught in medical schools, nursing schools, or traditionally used in medicine, the dramatic and positive effects of appropriately inculcating such team leadership go directly to the bottom line, inclusive of significant improvement in morale.

John Nance brings a rich and varied professional background to American healthcare and the cause of patient safety. A lawyer, Air Force and airline pilot, prolific internationally published author, national broadcaster, and professional speaker, he was one of the founding board members of the National Patient Safety Foundation in 1997. He is also the author of the highly acclaimed 2009 book on American healthcare, titled Why Hospitals Should Fly: The Ultimate Flight Plan to Patient Safety and Quality Care.

Theodore F. Classen Memorial Lecture: “Beyond the Scalpel: Soft Skills in the O.R.”

Communication skills, bookmarked by collaborative leadership and high emotional

intelligence, continue to gain prominence in medical literature as the demarcation between the good and the exceptional surgeon. Recognizing the importance of these “soft” skills, the Accreditation Council for Graduate Medical Education (ACGME) added communication and interpersonal skills to the required milestones for competencies of graduating residents in July 2014.

Because of the fast pace, high stakes, and technical “hard-skills” characteristic of the field, the culture of surgery has lagged behind this movement, and the greater part of these efforts have focused on providing strategies for improving the patient-surgeon relationship and communication. Team communication and development have received less attention, despite the Joint Commission’s identification of failures in communication, human factors, and leadership as the most common issues in sentinel events. Complicating the surgeon’s situation further, many O.R.s utilize rotating team members throughout the day. As a result, surgeons today require an exceptional degree of interpersonal flexibility as it has become necessary to simultaneously operate and manage an ever-changing team. This keynote address will focus on the nontechnical skills set of communication, decision-making, and situational awareness in the context of surgical team stress, management, and excellence.

Dr. Annette Brooks is a clinical psychologist specializing in medical psychology and organizational management. She has served in the New Mexico VA Health Care System since 1996 and as an assistant professor in the psychiatry department at the University of New Mexico (UNM) since 1999. While serving in clinical positions including director of pain management and director of medical psychology, she learned the value of change management skills, concentrating on motivational interviewing, human factors, and systems processes in a medical setting. She is a member of the international Motivational Interviewing Network of Trainers and a motivational interviewing consultant for the national Evidence-based Practice Initiative. She has achieved green belt certification through Lean Six Sigma and is working toward her black belt. Dr. Brooks currently works as an organizational consultant for the New Mexico VA Health Care System and with the UNM department of surgery in both resident training and research. In collaboration with the UNM’s surgical residency program, she co-developed a coaching program around early identification of residents struggling with the communication and interpersonal ACGME milestone. Her current interest and research areas are the impact of O.R. stress on adverse events and the implementation of stress recovery education to mitigate these events.

This keynote session will explore the 100-plus year history of quality and outcomes reporting by surgeons, and how patients have benefited from smart surgical research and rigorous training. The current state of American healthcare and osteopathic principles will be addressed, as well as how healthcare is transforming from a volume-based system to a value-based one. Surgical care will become more reliant on smarter information systems in the future. With the adoption of electronic health records, performance measurements like “meaningful use,” the Merit-based Incentive Payment System, and an orientation toward “value-based” medicine, surgeons must adopt strategies to maximize their ability to demonstrate high-quality care that is effective and efficiently delivered. This session will reveal the challenges of this process as well as opportunities for surgeons to capitalize on their existing qualities and talents through the use of smarter processes and information technology support.

Dr. Rosendale is a board certified general surgeon and an expert in clinical informatics, value-based medicine, and healthcare transformation. He has served as adjunct faculty for the clinical informatics program of Harvard University/Brigham and Women’s Hospital Decision Systems Group, and for the University of California San Diego bioinformatics department. He also served as chief medical information officer for the U.S. Department of Veterans Affairs/Department of Defense Interagency Program Office and as senior physician advisor for clinical informatics in the Veterans Health Administration. He has worked on White House-sponsored activities focused on disaster preparedness and health IT innovation and interoperability. He currently advises Fortune 100 companies on health IT strategies for the future. He is president of the American Osteopathic Association of Medical Informatics and chair of the ACOS Quality Initiatives Committee.

Dr. Atwal is a board certified critical care surgeon in Mount Clemens, Mich., a trauma surgeon in surgical critical care at Sinai-Grace Hospital in Detroit, Mich., and a board certified general surgeon and co-medical director of trauma services at McLaren Macomb Hospital in Mount Clemens. He serves on the Michigan Region 2 North Trauma Advisory Council and on the Michigan State Trauma Advisory Committee, and as physician quality surgical champion for the Michigan Surgical Quality Collaborative at McLaren Macomb. The recipient of numerous teaching awards, he is adjunct clinical preceptor at A. T. Still University Kirksville College of Osteopathic Medicine and clinical assistant professor in osteopathic surgical specialties at Michigan State University College of Osteopathic Medicine. He is chair of the ACOS Residency Evaluation and Standards Committee and serves on the American Osteopathic Association Program and Trainee Review Council.

“ What Happens Today Will Shape Tomorrow”

Presenters: Boyd R. Buser, D.O., and Steven J. Stack, M.D.

American Osteopathic Association President Boyd R. Buser and American Medical Association Immediate Past President Steven J. Stack will address current issues that will have an impact on the future of healthcare and the practice of medicine in the United States. These issues include the Medicare Access and CHIP Reauthorization Act of 2015, graduate medical education, the opioid epidemic, and maintenance of certification and osteopathic continuous certification. Drs. Buser and Stack will discuss the implications of payment and delivery reform on physicians and patients, the importance of physician activism to combat burdensome government regulations at state and federal levels, steps physicians can take to help end the opioid crisis, and the importance of new physician training and lifelong learning and assessment of physicians in practice.

Dr. Buser is the AOA’s 120th president and a board certified physician in family practice and osteopathic manipulative medicine. He is vice president for health affairs and dean at the University of Pikeville Kentucky College of Osteopathic Medicine, where he also serves as professor of family medicine and osteopathic principles and practice. He has held numerous positions in the AOA and has been instrumental in transitioning the osteopathic profession to a single graduate medical education accreditation system. A physician consultant in the AOA Payor Relations Division, he recently served as chair of the AOA Department of Business Affairs and the Bureau of Osteopathic Graduate Medical Education Development. He serves on the boards of the American Association of Colleges of Osteopathic Medicine and the American Academy of Osteopathy, and on the American College of Osteopathic Family Physicians’ Osteopathic Principles and Practices Committee and Practice Management Committee. He is chair of the Appalachian Osteopathic Postgraduate Training Institute Consortium board.

Dr. Stack, the first emergency physician to serve as AMA president, is board certified in emergency medicine and practices in Lexington, Ky. Known for his expertise in health information technology (IT), he has led AMA efforts in this area. He also has served on federal advisory committees for the Office of the National Coordinator for Health Information Technology, including the Health IT Safety Center Task Force, the Health IT Implementation, Usability and Safety Workgroup, and the Interoperability and Health Information Exchange Workgroup. He currently serves on the board of eHealth Initiative, a nonprofit association committed to improving healthcare through the advancement of health IT. He also has made notable contributions in physician licensure, regulation, and assessment. He has served as chair of the Federation of State Medical Boards Maintenance of Licensure (MOL) Implementation Group and as a member of its predecessor, the MOL Advisory Group.

The transition to ICD-10 coding has gone relatively smoothly this year, as Medicare and most payers promised leniency for claims with incorrect or unspecified codes. However, Oct. 1, 2016, ends the year of ICD-10 “light” and brings more than 2,000 new or revised codes. This workshop will address these ICD-10 topics:

• What can you expect in the second year of ICD-10 coding?

• Why is coding for comorbidities essential in today’s

healthcare environment?

• Using unspecified codes—How risky is it, and will these

claims always be denied?

• Using the 7th character in ICD-10 coding—What

constitutes an initial encounter, a subsequent encounter,

and sequelae? When is the 7th character used?

• How should surgical complications be coded?

Programs for profiling and reviewing claims for errors and audits are on the rise. How can you avoid an audit or deal with a record review or repayment demand from Medicare or a private payer? This session will cover audit awareness for surgeons:

What about private payers?

• What key issues lead to an audit? What areas of risk

should you be aware of in E/M coding, surgical coding,

and E/M visits and procedures performed the same day?

respond if you receive one?

• What does a request for records mean, and how should

you respond?

• What happens if the payer demands a repayment?

There are more than 90 American Osteopathic Association (AOA) surgical residency programs in general surgery, general vascular surgery, cardiothoracic surgery, neurological surgery, plastic and reconstructive surgery, surgical critical care, and urological surgery approved by the AOA/ACOS. The annual Osteopathic Surgical Educators Seminar provides program directors and faculty in these residency programs the opportunity to share information and their experiences in teaching osteopathic surgical residents and to discuss common and emerging issues affecting their residents and training programs—including the challenges facing them as they apply for Accreditation Council for Graduate Medical Education (ACGME) accreditation.

The AOA announced on Feb. 26, 2014, that the ACGME, American Association of Colleges of Osteopathic Medicine, and AOA agreed on a memorandum of understanding to create a new “unified accrediting body” for graduate medical education. Effective July 1, 2015, AOA accredited training programs can apply for ACGME accreditation and will be granted “pre-accreditation” status by the ACGME upon receipt of their online application. Programs will maintain pre-accreditation status until they achieve initial ACGME accreditation, withdraw from the application process, or until June 30, 2020, if they have not received full ACGME accreditation. Until AOA programs are fully accredited by the ACGME, programs and program directors must continue to comply with AOA/ACOS standards.

Nearly half of all AOA/ACOS surgical residency programs have applied and received ACGME pre-accreditation status since the July 1, 2015, effective date. What steps should programs take next to achieve “initial” ACGME accreditation and beyond? This year’s Osteopathic Surgical Educators Seminar will focus on initial ACGME accreditation to assist surgical residency programs in reaching this level of accreditation.

More than 50 AOA/ACOS surgical residency programs have applied for ACGME accreditation. Fewer than a dozen programs have received “initial” ACGME accreditation, while several more have received continuing pre-approval status. The goal of the ACOS Residency Evaluation and Standards Committee (RESC) is for programs to receive initial accreditation as expeditiously as possible. ACGME site visit data reveal common reasons that residency programs are not receiving initial accreditation right way—program directors and faculty lack scholarly activities and pub med publication; programs lack a formal evaluation system for faculty and residents; and more. Donna Lamb will address the current ACGME accreditation status of AOA/ACOS programs, and the necessary steps for ensuring that programs receive ACGME initial accreditation.

In accordance with the AOA CME accreditation requirements, you must complete an evaluation of each session attended at the Annual Clinical Assembly of Osteopathic Surgeons (ACA) in order to receive AOA CME credit. Physicians should only claim credit commensurate with the extent of their participation in each ACA educational program and/or activity. CME hours attended at the meeting will be tracked through an online evaluation system. Paid registrants will have access to the system via the American College of Osteopathic Surgeons (ACOS) website using the instructions below.

In order to receive CME credits for all sessions attended at the meeting, registrants must evaluate each lecture attended. The system will calculate the total number of hours attended after the registrant has completed the session evaluation as well as the overall evaluation.

A certificate will be made available to print or email for your records. It will not be mailed to you from the ACOS office.

You must claim your CME credits NO LATER THAN December 1, 2016.

TO CLAIM CME CREDITS FOR THE 2016 ACA:

1. Go to www.facos.org.

2. Click Log On at the top of the screen.

Your default username is the letters “aoa” followed by your AOA number (ex. aoa12345). If your AOA number begins with a “0” please drop the “0.” Your default password is your last name in lower case letters.

3. Select My Profile which will appear at the top screen once you’re logged in.

4. Select Meeting Evaluations

5. Select the link 2016 Annual Clinical Assembly of Osteopathic Surgeons

6. Select your Surgical Discipline

7. Complete the Session Evaluation for each session attended

8. Complete the Overall Evaluation

9. Click the “CME Attestation” button

10. Click “Claim CME Credits”

You can print or email a copy of the CME Certificate for your records. ACOS will submit CME credits to the AOA on your behalf.
